I used a fire extinguisher to extinguish the fire and then disconnected the vehicle’s battery. No injuries occurred. The vehicle was not restarted and was subsequently towed to an authorized Toyota dealership for inspection. Visible damage is present in the engine compartment, including the main fuse/relay box and associated electrical components. The dash display displayed a message " precollision system malfunction" and "secondary brake collision malfunction". While operating my 2024 Kia EV6, the vehicle began displaying repeated warnings related to the power system and 12-volt battery. The instrument cluster first showed a “Power limited” message, followed shortly by a “Stop vehicle and check power supply” warning. ... r full charge. Shortly thereafter, the 12-volt battery became fully depleted, resulting in a complete loss of low-voltage electrical power. I was unable to electronically unlock the doors, and the vehicle could not be started or shifted into gear. The vehicle became immobilized and inoperable, requiring towing. This failure occurred without warning and created a safety concern, as loss of power could potentially occur while driving or leave occupants stranded and unable to access the vehicle. The vehicle had previously been serviced at a Kia dealership for known power system concerns related to the Integrated Charging Control Unit (ICCU). During that visit, the dealership applied a firmware update only and did not replace the ICCU hardware. The current failure occurred after that firmware update, suggesting the issue was not resolved. This incident represents a loss of motive power and loss of vehicle electrical systems, which could present a serious safety risk if it occurs while the vehicle is in motion or in unsafe environments. The inability to unlock doors or start the vehicle due to complete 12-volt battery depletion is particularly concerning. When roadside assistance arrived to tow the vehicle to the dealership, the vehicle’s 12-volt battery was completely depleted, preventing normal recovery procedures. A portable jump starter was temporarily connected, but the vehicle would not remain powered on long enough to complete the steps required to place the vehicle into neutral or tow mode.Read more

Given that I rely on this truck for work and frequently travel long distances between New Jersey and Texas, this behavior makes the vehicle unsafe and unreliable for its intended use. The truck has already undergone repeated diagnostics, module replacements, software updates, wiring repairs, and battery replacement at authorized Ram dealerships. Despite these efforts, the issue persists. At this point, I have lost confidence in the safety and reliability of this vehicle and do not feel comfortable continuing to drive or own it. I have acted in good faith and allowed multiple opportunities to repair the vehicle. The ongoing shutdowns while driving demonstrate that the defect has not been corrected within a reasonable number of attempts. I'm reporting this because a vehicle fire is a significant safety concern and I'm probably not the first to have had this happen. This failure is a safety hazard as it will quickly disable the vehicle and strand the occupants wherever they be at the time of failure. (highway, back roads, day or night). At one point, the dealership bypassed or cleared the check‑engine indicator without resolving the underlying problem. After this, the vehicle began performing noticeably differently, including a significant drop in fuel efficiency—going from approximately 18 MPG to around 14 MPG. In addition to the fuel system defects, the vehicle has had persistent battery issues. The battery drains unexpectedly and frequently dies without warning. Even after replacing the battery, the new battery lasted only about three months before the same drainage problems began again. The vehicle will randomly shut down or stop functioning correctly until another battery replacement is performed. These issues raise serious concerns about the safety, reliability, and overall drivability of the vehicle. The repeated failures of both the fuel system and electrical/battery system pose potential safety hazards while driving and have made the vehicle unreliable for normal use. ... Electrified GV70, which experienced a sudden “Check Electric Vehicle System” warning consistent with a failure of the Integrated Charging Control Unit (ICCU). Incident Summary On February 5, 2026, at approximately 4,480 miles, the vehicle displayed a “Check Electric Vehicle System” alert and was no longer safe to operate. Due to the risk of propulsion loss and electrical system malfunction, the vehicle required flatbed towing to an authorized Genesis dealership for evaluation. A dealership service advisor subsequently informed me that the vehicle has been diagnosed with an ICCU failure, a component responsible for charging the 12-volt battery and maintaining critical electrical systems. Failure of this component can result in loss of drivability, propulsion limitations, or vehicle shutdown, creating a significant roadway safety hazard. Safety Concern Failures of the ICCU have been widely reported across Hyundai, Kia, and Genesis electric vehicles and have been the subject of prior recalls due to the potential for unexpected loss of motive power while driving. Despite prior recall actions, failures continue to occur, including in my vehicle. Current Status The vehicle remains out of service pending repair, and no written repair completion timeline has been provided. The failure rendered the vehicle unusable and required emergency towing. The p0101 code has been going on and off of this car since purchase (3 years) the MAF sensor has been replaced twice and the check engine light comes back on after just a few days each time. This poses a safety risk because the car stops at stop lights and turns off, the car shakes at idle, the car refuses to accelerate risking pulling out in front of another car without being able to accelerate.

Unintended Deployment: At highway speeds, the unlatched flap opens/flaps due to aerodynamics, creating a visual obstruction and distraction for the driver and traffic. 3. Moisture Ingress: Failure to seal the port exposes the high-voltage interface to pressurized rain/road spray. Per NHTSA 19V434 (Recall 93E8) regarding moisture ingress in this system, this failure represents a recurrence of a known safety risk. Evidence of Defect: Audi has iterated through three versions of this part (4KE810001, B, and C), confirming an inherent design flaw. This component is specifically identified in Service Campaign 93H6 for proactive replacement. Despite this, and the failure occurring immediately after recall service 93V2 on October 1st 2025, Audi Burlingame denied a 'goodwill' repair and quoted $1,100+ for a part already internally flagged as defective. ... ons exposed especially if one didn’t know the mold was present. *The vehicle has been repaired at VW dealership in Missoula, MT. Service tech couldn’t figure where the water was coming from. Finally did research to find an Audi TSB 2059792/5 with the cause and remedy. *Recently Purchased this car and later found the seat not working. The VW dealership/service found the electrical corrosion and mold. Mold was not visible until the seat was moved. Mold was extensive throughout the passenger side under the seat - spread and growing under the carpet, insulation and in the electrical well in the floor board. Pictures attached - tech write up also available upon request. No warning lights were present. ** Audi put out a TSB (Tech Service Bulletin) in 2023 and repaired the cars under warranty - assume this was a wide spread issue for a TSB to be issued and for Audi to manufacture parts to fix the problem. Mold can have serious long term consequences to anyone exposed. I believe Audi should have done a recall for safety reasons. I just bought the car on 1/10/26 used with under 40,000 miles. Carfax or other research did not turn up a recall. I have sent a letter to Audi of America detailing the problem, referencing the TSB and asking them to cover the cost of repair as they had covered the cost on vehicles under warranty. This car is just out of the 5 year warranty but has only 40,000 miles on it. No recall was issued that I can find. Any help you render would be appreciated. Approximately a year ago I started having trouble with the shifting in my transmission. Had the fluid changed thinking this may be the problem. My engine light continued to stay on and has been on ever since. Periodically the truck will down shift or not shift into another gear without the rpms going above 3000.

The vehicle was such in a state that the hazard lights were not operational and I had no way of getting any heat. (It was 20F with a wind chill down to 14F.) It also happened in a bad part of town, meaning that I did not want to leave the vehicle unattended for a tow truck. This also occurred 60mi from home. Reproduced: The dealership it was towed to is not open until Monday, February 9, 2026 but the tow's rescue battery made the system run enough to report the Diagnostic Trouble Code of "P1A9096". Doing a quick search online for that shows a number of cases with the same problem. While waiting for the tow, I tried to start the car a number of times to no avail. Inspected: Not yet, see "Reproduced" section above. Any other signs/symptoms? Nothing until the "pop" itself. on April 7, 2025, our 2024 Kia EV6 was towed away from our driveway after an ICCU failure the day before. The tow truck driver attempted to jumpstart it with a battery eliminator and he could not. He could not get it shifted into neutral and had to pull it up on the ramp with the back tires stationary, sliding up the ramp. ... , it was towed away again because of a 12 V battery failure. If this first computer failure had happened while we were on the highway, we would have lost power in traffic. We were told that the replacement ICCU part which we received was a different model number than the first one, but I have heard on Facebook discussion groups that it is identical and just as likely to have a failure. Our Kia EV6 is a 2024 model, an extended battery version Wind AWD. Both the initial incident on April 6 and the second incident on April 21 happened just outside my garage after just backing the car out of the garage going maybe 5 mph. For some reason, the system will not let me upload a video of the first time it was towed up the ramp, only the second time when it was taken away for the 12 V battery failure, but it also would not drive in that condition. In addition to the rough idle, the vehicle has experienced intermittent electrical issues, including warning lights, erratic behavior of vehicle systems, and inconsistent performance. Similar complaints are widely reported by other Ram 2500 owners online, including rough idle, engine surging, stalling, and electrical failures. Why This Is a Safety Concern!!!!! This defect creates a serious safety risk. The rough idle causes violent shaking and unpredictable engine behavior, raising concern that the vehicle may stall while stopped or when maneuvering in traffic. Vehicle: 2016 Ford F-150, 5.0L V8, 6R80 automatic transmission Mileage at time of failure: Approximately 118,000 miles Approximate date of occurrence: Late 2025 (Late October to Early November) While driving at speeds above approximately 40 mph, the transmission would randomly and unexpectedly downshift, causing a severe and sudden jerking motion accompanied by extremely high engine RPMs. After s
... everal seconds, the transmission would violently upshift back into the original gear. Most occurrences involved the transmission downshifting one or two gears; however, on multiple occasions it downshifted from 6th gear directly to 2nd gear while traveling at highway speeds. This caused the rear wheels to momentarily lock up, creating a dangerous loss of traction and loss of vehicle control. These events occurred frequently, including during short trips, and were not related to driver input, braking, or road conditions. During these incidents, the check engine light and wrench warning light would illuminate. Diagnostic trouble code P0720 (Output Shaft Speed Sensor Circuit) was consistently stored. This pointed to a failure within the transmission molded lead frame, which houses the output shaft speed sensor and related circuitry. Due to the severity of the symptoms and the known issues with the Ford 6R80 transmission molded lead frame, I replaced the molded lead frame myself. After replacement, the erratic downshifting, wheel lockup, warning lights, and fault codes were completely resolved. The vehicle has operated normally since the repair. Ford has a documented history of issues related to molded lead frame failures in 6R80 transmissions, including erratic shifting and loss of speed sensor data. This failure presented a significant safety risk, particularly at highway speeds, due to sudden and uncontrollable deceleration and momentary wheel lockup.Read more

The sudden loss of power created an extremely dangerous situation for myself, my passengers, and surrounding traffic. After the engine failure, the vehicle could not be placed into neutral, preventing it from being moved out of traffic if it had stopped in an active lane. This design issue significantly increased the risk of a collision. I was stranded on the roadside for several hours in cold conditions awaiting a tow. The failure was later confirmed by an authorized GMC dealer, who diagnosed the engine as failed and replaced the entire engine assembly. The vehicle and replaced components should be available for inspection through the dealership. The manufacturer had previously issued a recall for this vehicle related to engine failure, and I had requested a buyback prior to this incident due to safety concerns. That request was denied unless the vehicle experienced a failure. There were no warning lamps, messages, or symptoms prior to the failure. The only warning message appeared after the engine failed, indicating an engine overheating or “engine hot” condition, which occurred after power was already lost. Prior to this incident, the vehicle exhibited no warning signs, including no pulling, steering issues, warning lights, or diagnostic codes. We immediately pulled over and had the vehicle towed to a mechanic who inspected the truck but could not replicate the issue. Although the vehicle was later driven approximately 120 miles home and short distances locally without recurrence, we believe it remains unsafe due to the unpredictable nature of this failure. A vehicle should never abruptly and forcefully change lanes without driver input. We have also experienced ongoing issues with the key becoming stuck in the ignition and intermittent starting problems, raising concerns about potential electrical or mechanical defects. This incident involved two uncommanded lane deviations under normal driving conditions with no identified cause or warnings. Due to the risk of serious injury or fatal collision, this remains an unresolved safety concern requiring investigation. We are requesting that NHTSA review this complaint, evaluate whether similar incidents indicate a pattern or defect involving the steering, transmission, or electronic control systems in the 2016 Ford F-150, and take appropriate action to protect public safety.
