I am writing to formally notify the National Highway Traffic Safety Administration (NHTSA) that Mercedes-Benz USA has failed to remedy a known safety defect within a reasonable amount of time, pursuant to 49 U.S.C. § 30120. This notification is submitted in relation to the recall issued under the National Traffic and Motor Vehicle Safety Act regarding the high-voltage battery cells in the 2023 EQ
... B. The manufacturer has acknowledged that an internal short circuit within the battery cells may lead to a vehicle fire while driving or parked. Despite this, a timely and effective remedy has not been provided, leaving me to manage the following severe burdens: • Safety and Psychological Distress: Every time I operate this vehicle, I experience significant anxiety. My primary concern is the safety of my grandchildren; I am constantly haunted by the question of whether I could safely extricate them from the vehicle should it ignite while in motion. • Property and Liability Concerns: I am currently forced to park the vehicle away from other cars and structures to mitigate the risk of a conflagration. I am deeply concerned about my legal and financial obligations should the vehicle ignite and cause damage to third-party property or result in personal injury. • Diminished Performance: The vehicle currently exhibits a decreased mileage range, failing to meet the performance specifications promised at the time of lease. • Breach of Lease Terms: I am required to make full monthly lease payments as per my agreement. However, Mercedes-Benz is failing to uphold their portion of the contract by providing a vehicle that is safe, functional, and fit for its intended purpose. I request that the NHTSA investigate Mercedes-Benz’s delay in providing a definitive remedy and hold the manufacturer accountable for the loss of use and the inherent dangers posed by this defect. A car that "potentially could ignite into flames" at any moment is not a "remedied" vehicle under the law.Read more

Defect Description for NHTSA Complaint Intermittent but recurring failure of the Uconnect infotainment system and rearview backup camera on a 2025 Ram 1500. The defect occurs at vehicle startup and when shifting the transmission into reverse. Symptoms include: (1) the center console display freezes and becomes unresponsive; (2) the rearview backup camera fails to display an image when reverse is s
... elected, eliminating rear visibility required under FMVSS 111; (3) audio output cuts out completely; and (4) a loud electronic buzzing noise is frequently emitted from the dash or speakers during the failure. This is a safety-related defect because the rearview camera is inoperative at the exact moment it is federally required to function — when the driver shifts into reverse — creating a rear visibility hazard and increasing the risk of a backover incident involving pedestrians, children, or obstacles. The vehicle has been taken to an authorized Ram dealer multiple times for this issue. The dealer previously replaced the center console display/head unit under warranty, but the defect has returned and continues to occur, indicating the root cause was not the display itself. The persistence of the failure after a hardware replacement suggests a systemic issue — likely related to Uconnect software, the CAN bus communication network, a wiring harness or ground fault, or another vehicle module — rather than an isolated component failure. No crash, injury, or fire has resulted, but the loss of the backup camera at the moment of reverse engagement presents an ongoing safety risk during every use of the vehicle.Read more

The forward-facing camera system, Automatic Emergency Braking (AEB), and Autopilot/cruise control have repeatedly failed on a 2023 Tesla Model Y with approximately 22,000 miles. The AEB system becomes unavailable and the Autopilot disengages without warning, including at highway speeds of 70 mph and city speeds of 30 mph. The active safety systems are functional less than 10% of the time. The sudd
... en, unexpected loss of active safety systems at speed presents a serious risk of accident and injury. The problem has been reproduced and confirmed by an authorized Tesla service center on three separate occasions. Repairs attempted include replacement of the triple forward-facing camera assembly (twice) and replacement of the car computer (HW3) with a firmware update. Despite these repairs, the defect recurred within days of each service visit. A fourth service appointment is now scheduled for the same issue. Warning alerts appeared on the vehicle display prior to each failure, specifically "Camera Blocked" and "Automatic Emergency Braking Unavailable." These alerts were first detected in August 2025 and have recurred through January 2026. All three repair attempts were covered under the manufacturer's Basic Vehicle Limited Warranty, confirming acknowledgment of a manufacturing defect. The vehicle and components have been inspected by the manufacturer's authorized service technicians. The vehicle is available for inspection upon request.Read more

The first problem began at approximately 5,000 miles. After refueling, the vehicle’s transmission would slip severely. The slipping continued to occur intermittently after that point. I brought the vehicle to a dealership Initially, I was told that nothing was wrong with the vehicle. After further testing, the dealership identified a transmission issue related to Technical Service Bulletin 25-NA
... -211 and stated that the repair was completed. At approximately 8,000 miles, an additional issue occurred where the check engine light illuminated and a warning appeared indicating that the brakes had stopped working. This was extremely concerning and created a serious safety risk. Following the transmission repair, a new and more dangerous issue developed. The vehicle now fails to accelerate properly from a complete stop. When attempting to accelerate, it becomes stuck at approximately 1 mph for around 45 seconds before finally moving. This happens intermittently and creates a high risk of collision when entering traffic or crossing intersections. These issues—transmission slipping, brake warning alerts, and failure to accelerate—pose a serious safety hazard to myself and others on the road. The vehicle has been taken in for service, but the problems have not been fully resolved. I am requesting that this issue be investigated as a potential defect affecting this vehicle.Read more

This vehicle has died 4 times in one week. when this happens, multiple lights have flashed in the dash board, including the "eye sight" warning. Then, it would not restart but continued to "turn over" when the ignition button was pushed. One instance was on a busy street at a stop light, another at a busy intersection in the inner left turn lane, the other 2 instances were in parking lots. The fir
... st 2 times the vehicle was able to be jump started and was not taken to the dealership. The 3rd, it was jump started and taken to the dealership. They said the battery was slightly low, but had sufficient charge. They changed the fob battery, and topped off the car battery. That was all. The next day was the episode where it died in the turn lane. I had it towed to the dealership. This time they changed the battery to a more high capacity battery, and admitted that these vehicles have a battery put in them at the manufacturer that is not strong enough for the systems it runs. I even found a service bulletin about it online, #TSB 07-218-23R. This should be a recall item, and they should have changed this battery at my first visit! A vehicle suddenly dying on a busy roadway puts those inside at risk, as well as everyone around them. Very disappointed in this 2023 Subaru Forester Premium.Read more

2018 Volvo XC90, VIN [XXX] , 54,618 miles. Original owner. All service performed exclusively at Volvo Cars West Houston (same dealer, all three failures). At 12,017 miles (August 2020), the dealer installed the evaporator resistor per TJ 34648.1.2, Part No. 32241228, under VST Operation 98734-2 — meaning Volvo Corporate has documentation of this vehicle's evaporator vulnerability on file since 2020. At 15,930 miles (March 2021), the AC compressor failed and was replaced under warranty, consistent with refrigerant starvation from the same underlying evaporator leak. The evaporator has now failed completely at 54,618 miles despite Volvo's own corrective measure being applied at 12,017 miles. The current failure (April 2026) includes a confirmed front evaporator refrigerant leak and AC discharge hose leak, documented by dealer borescope inspection showing corrosion on the evaporator fins and visual evidence of refrigerant oil residue at the discharge hose fitting. Dealer repair estimate is $4,406. This is a documented pattern failure across 2016-2018 Volvo XC90 models. This is a supplemental report to my previous filing regarding a 2023 VW ID.4 battery fire. On May 28, 2025, while being operated, the vehicle suffered a catastrophic thermal event. White smoke emanated from the front compartment followed by active flames. The vehicle was a total loss. Critical Safety Finding: A subsequent forensic examination conducted on October 29, 2025 (File 27739-01) by a cer
... tified fire investigator confirmed that after the initial fire was suppressed, the vehicle reportedly caught fire an additional time while being held at a tow/storage facility. Component Failure: The fire originated in the front compartment area. The failure is consistent with the "thermal runaway" risks identified in Recall 26V030, involving defective battery cell electrodes. Safety Risk: The multiple ignition nature of this failure poses a lethal risk to occupants, emergency responders, and tow/salvage facility operators. The fact that the vehicle reignited twice in storage, long after the initial suppression, suggests that current safety protocols or the scope of the existing recall may not fully address the volatility of this specific battery defect. Inspection Status: The vehicle was inspected by the manufacturer’s engineers and an independent forensic investigator. However, the manufacturer and the insurer are currently withholding the Photographic Report and Technical Field Report from the consumer. These records are vital to understanding the severity of the "multiple -ignition" event and should be subpoenaed or requested by NHTSA investigators to ensure the recall remedy is sufficient to prevent secondary fires in storage environments.Read more

Incident Description: I purchased a 2016 Hyundai Veloster (VIN: KMHTC6ADXGU255844) in May 2025. Shortly after purchase, the vehicle began exhibiting serious mechanical issues, including engine knocking, excessive oil consumption, stalling, misfiring, and loss of power. Despite these ongoing issues, the vehicle is listed as having Recall 198 completed. However, the symptoms associated with that rec
... all continued and progressively worsened. The vehicle ultimately stalled in traffic while I was making a turn, resulting in an accident. In a separate incident, after exiting the vehicle and believing it to be in PARK, the vehicle moved unexpectedly and ran over me, causing serious physical injuries including fractures. Prior to these incidents, the vehicle had ongoing mechanical issues that were not properly resolved. The vehicle also required repeated jump-starts and became unreliable to operate. Additionally, there are discrepancies in the service records. The vehicle was towed to a dealership on or about June 8, 2025 and remained there until approximately July 15, 2025. However, the documentation provided reflects only a limited number of service days and consolidates multiple visits into a single summary. Due to these inconsistencies and the continued presence of failure symptoms, I am unable to verify that recall-related repairs were properly performed. These incidents raise serious concerns about engine failure and possible unintended vehicle movement, both of which present significant safety risks.Read more

In April 2025, I purchased a new vehicle from AutoNation Chrysler Jeep Dodge Ram North Richland Hills as a retirement gift to myself. After years of driving gas-powered vehicles, I decided to transition to an electric vehicle and purchased a Jeep Wagoneer S. While the vehicle is visually impressive, my ownership experience has been nothing short of a nightmare. Since the purchase, the vehicle has
... experienced ongoing and severe mechanical issues. It has spent more time at the dealership than in my garage. I have worked with multiple service departments and communicated with several representatives from Stellantis, repeatedly warning that this vehicle poses a serious safety risk and could result in injury or worse. The primary issue is extremely dangerous: the vehicle will unexpectedly surge forward with force and then abruptly lose all battery power. Despite being repaired approximately 8 to 10 times, the problem continues to recur. Multiple technicians, engineers, and even an investigator have attempted to diagnose and fix the issue, all without success. A few weeks ago, I experienced what I believe could have been a fatal incident. At approximately 5:15 a.m., while driving on the highway near the 121 and 635 merge in Grapevine, the vehicle failed completely. It was still dark, and traffic was moving at speeds exceeding 80 mph. My disabled vehicle was left blocking active lanes of traffic while other drivers swerved and honked to avoid me. Fearing for my life. This has happened so many times, but this time the failure could have been deadly. Following that incident, the vehicle was towed back to the dealership, where it has remained for approximately five weeks. Despite everything that has occurred, I was later contacted by a representative from Chrysler who offered $1,000 as a “goodwill” gesture for the whiplash I sustained during the incident. I made it clear that I was seeking a full buyback of the vehicle, including reimbursement of my investment. That reRead more

The 2022 ioniq 5 was bought used with 43k MI and within 2k miles of driving in 50-85F climate and 7kwh l2 charging I witnessed the orange check EV light flash while I was driving on the highway. I exited and powrr cycled the car and the EV light remained. The next morning the orange check EV light was gone. I went to the dealership for a diagnostic and they said they could not replicate the error
... code or had found any history of the error code coming up without proving to me or showing me any substantial evidence that they tested for anything at the Escondido Hyundai dealership in California. I escalated the issue with Hyundai corporate and was assigned a case manager that ultimately gave me the same answer that they could not replicate any codes and that I should take the car back and drive normally. After extensive research I found that the orange check ev light is a more dangerous sign than the red check ev light because it indicates that the ICCU unit is about to fail. Research shows that due to thermal stress the ICCU will inevitably fail after the orange check AV light comes on. Further research shows that Hyundai had officially stated a 1% failure rate for the ICCU unit in all of their EVs built on the e-gmp cars to date but some projections are at 10% failure rate or higher and that Hyundai Kia is downplaying the issue similarly to how they tried to do the same with the Theta 2 GDI engines that they had been sued and were responsible to pay over $300 or so million dollars as well as $24 million to a employee whistleblower a few years ago. I highly suggest that the nhtsa or other organizations that have the power to force Hyundai Kia to do the right thing so that no further lives are at risk from these ICCU units that have flawed designs that have not been fixed, only remedied via software update or replaced with refurbished units at the most with inevitable failure as thermal stress is experienced.Read more

The vehicle's Telematics Control Box (TCB), located beneath the roof-mounted shark fin antenna, failed due to water intrusion and internal corrosion. The failure produced the following fault codes confirmed by BMW of Norwood, MA: B7F341 (backup battery hardware defective), B7F33C (internal ECU error), and B7F327 (microphone short circuit to ground), along with cascading faults across multiple othe
... r vehicle modules. Symptoms included a persistent 'Emergency Call System Malfunction' warning on the instrument cluster, loss of GPS/navigation accuracy, and loss of odometer display. The shark fin antenna base seal — a foam tape and adhesive system — had likely deteriorated after approximately 6-7 years of service, consistent with a pattern of failures reported by numerous other owners of 2017-2023 BMW X1, X3, X4, X5, X6, X7, M440i, M550i, 330, 340i, and 750i vehicles. BMW dealer quoted $3,300 for repair. An independent BMW specialist quoted $3,500. BMW North America declined to cover the repair as the vehicle is out of warranty, despite acknowledging the issue as known. This failure mode is the subject of active class action litigation (Craft v. BMW of North America LLC, Case No. 1:24-cv-06826, U.S. District Court for the District of New Jersey) and an ongoing investigation by Sauder Schelkopf LLC on behalf of affected owners. SAFETY CONCERN: The failure disables the Intelligent Emergency Call system, which automatically contacts emergency services in the event of a serious accident. Additionally, the persistent warning obscures the instrument cluster including the odometer display, impairing normal vehicle operation. Water intrusion reaching vehicle electronics represents a broader safety risk if it propagates to safety-critical modules. ACTION REQUESTED: Formal investigation into the adequacy of the shark fin antenna sealing design across affected model years, and consideration of a recall or extended warranty to cover TCB replacement costs for affected owners.Read more

What component or system failed or malfunctioned, and is it available for inspection upon request? The Powertrain/Engine Control System. The vehicle suffers from a total loss of motive power while driving in traffic. It is available for inspection. How was your safety or the safety of others put at risk? The vehicle unexpectedly enters "Limp Mode" while driving at various speeds in active traffic.
... When this happens, the accelerator pedal becomes completely unresponsive, forcing the vehicle to a crawl or a dead stop regardless of the flow of traffic around it. This creates a life-threatening risk of rear-end collisions and high-speed accidents, as the car cannot maintain speed or move to safety. I am forced to operate a vehicle that effectively "shuts down" while I am in the middle of the road. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. Multiple independent mechanics have confirmed the stalling and electrical instability. Despite replacing the crankshaft sensor and both batteries, the sudden loss of power in traffic persists. Shops have stated the issue is a manufacturer-level failure they cannot fix. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? It has been inspected by independent service centers. The manufacturer has not inspected it yet, as it is extremely hard to duplicate unless it is driven for a certain time at certain temperatures. These failures occur while the vehicle is in motion and create a safety risk due to loss of system visibility, inability to control vehicle functions, and unexpected vehicle behavior. The vehicle was initially taken to an independent repair shop due to these issues, where I was informed of Recall 97GF (wiring harness issue), which is associated with the same electrical failures.
