I was driving on a cold day. When I stopped at a stoplight seeking to make a left turn, the car would not accelerate when I pressed the gas pedal. I looked down and saw the following warning sign "Engine Stopped, Steering Power Low" appear on the dashboard. However, since the car was not in park, it had rolled forward and was close to the being in the nearest lane of oncoming traffic. It was also
... difficult to steer to avoid creeping closer to oncoming traffic. Although it did not cause an accident, it easily could have. I took the car to several mechanics, because the car is under a CarMax warranty. However, none of the diagnostics revealed anything of concern (no weak battery, lack of power steering fluid, etc.). The mechanics said they had heard about this happening, but were unable to pinpoint any reason for it occuring. This seems like a serious flaw.Read more

Subject: Safety Concern: Electronic System/BCM Failure – 2022 Honda CR-V Hybrid Vehicle Information: Year/Make/Model: 2022 Honda CR-V Hybrid VIN: [XXX] ] Approximate Mileage: [45,200] Component: Electrical System / Body Control Module (BCM) Description of Incident: On [04/13/2026], my vehicle experienced a sudden and total failure of the Body Control Module (BCM). The vehicle’s security alarm
... began triggered randomly and could not be silenced or deactivated. Simultaneously, the key fobs became completely unresponsive, preventing me from locking, unlocking, or disarming the vehicle. Upon entering, the dashboard displayed a "Christmas Tree" effect with all warning lights illuminated (ABS, Brake, TPMS, etc.) accompanied by a persistent audible beep. Safety Risk: This failure creates a significant safety hazard. Because the key fob and locking systems failed, I was unable to secure the vehicle. Furthermore, the loss of communication between the BCM and other safety modules (like ABS and Traction Control) puts the driver at risk of losing critical safety features while in motion. Prior History: This issue is identical to the symptoms found in NHTSA Recall 20V771 (affecting 2018–2020 Honda models). It appears the defect persists in the 2022 CR-V Hybrid model year, yet no recall has been issued for this specific VIN range. Requested Action: I am requesting that NHTSA investigate the 2022 Honda CR-V Hybrid for BCM-related electrical failures to determine if a safety recall is necessary to prevent further incidents of driver stranding and safety system malfunctions. When driving on highway, electrical system started beeping and showing various signals, could no longer keep speed or accelerate. Had to put on warning lights and coast to side of road with four small kids in car. Was able to pull off to side and slowly get to safe location by resting vehicle in between events. When fully charged, didn’t seem to have issue so was able to drive to dealer. Thought
... was related to engine switching from electric to gas power. Dealer checked and performed two software updates. On way home from dealer, same problem occurred on highway. Super dangerous to loose all acceleration or same speed on highway! I got a video of second event as I was a passenger and my husband was driving. But as can see in video the car has a charge so the incident is not related to the switch from electric to gas power. Now we are very concerned whether Chrysler has the means to fix this at all and or whether the dealership will be actually able to identify the issue given the car was literally being driven home from the dealer. Known Recall Issue: The failure pattern aligns with known manufacturer defect categories involving: •Engine torque / internal engine failure conditions •Cooling system failure leading to overheating Shortly after I purchased my 2015 Range Rover, I started to have repeated and escalating safety-critical failures involving sudden loss of power, engine malfunction, and vehicle shutdown while dri
... ving at highway speeds. The vehicle has experienced multiple incidents of complete or partial power loss while in motion, resulting in near-collision events. Despite repeated repairs by authorized dealerships, the underlying defect has not been resolved. The failure pattern suggests a systemic mechanical defect involving engine performance and cooling system integrity rather than isolated maintenance issues. The primary safety concern is sudden loss of propulsion while driving at highway speeds, which has occurred on more than one occasion. These incidents present a significant risk of: •Rear-end collisions •Loss of vehicle control •Serious injury or fatality TECHNICAL FAILURE PATTERN Observed progression of failure: •Oil leakage / burning smell •Internal engine degradation symptoms •Cooling system failure and overheating •Electrical instability •Final engine shutdown / seizure events This suggests a systemic mechanical defect affecting engine integrity and thermal management systems. REQUEST FOR INVESTIGATION I respectfully request NHTSA to: 1.Investigate potential safety defects in this vehicle and model year 2.Evaluate whether similar failures are occurring in other vehicles 3.Assess adequacy of existing recall coverage 4.Review risk associated with sudden engine shutdown while driving 5.Determine whether recall expansion or additional safety action is warrantedRead more

I am filing a safety complaint regarding my vehicle due to repeated mechanical failures that have caused it to become unreliable and unsafe. My vehicle has experienced ongoing issues despite multiple repair attempts. Most recently, the vehicle broke down again and left me stranded while I was transporting my mother to a critical cancer appointment and the 1st time had me stranded on the highway
... late at night. This situation created a serious safety concern, as we were left stuck in the vehicle without reliable transportation for the 3rd time. This is not an isolated incident. The vehicle has previously been in the repair shop for an extended period (over 50 days), for a separate issue that has been resolved. Despite these repeated failures and loss of use, the manufacturer, Stellantis, has declined a buyback request. I no longer feel safe operating this vehicle due to its unpredictability and repeated breakdowns. This poses a risk not only to myself and my passengers but potentially to others on the road. I am requesting that this issue be investigated as a potential safety defect.Read more

So was going to wait another two months before calling again. But today (April 2026) one of our drivers accidently backed into a truck, luckily all of our drivers have been hyper cautious so they weren't going very fast so there were no damages to the other vehicle. Not having a fix for the rear view camera system still, has made our drivers feel uneasy when they have to take this van out. It's definitely a huge safety issue, especially since there is no visibility to look out the rear windows. A fix for this would be greatly appreciated.Read more

These events caused driver distraction and a startle effect while the vehicle was in motion, increasing safety risk to vehicle occupants and others on the road. The issue is ongoing intermittent and reproducible during normal driving conditions. The vehicle was presented to an authorized dealer multiple times. A software update was performed in an attempt to correct the issue, but the condition continued. The dealer advised that replacement of infotainment components such as the CMU or display may be required. The vehicle has been inspected by an authorized dealer on multiple service visits. The issue was also reviewed by the manufacturer customer experience case management team which acknowledged understanding of the malfunction but declined repair assistance. No police or insurance inspections have occurred. No warning lights or messages appear before the malfunction. Symptoms occur without warning and include random touchscreen activation, system freezing or rebooting, unintended phone dialing, and unexpected loud audio. On Friday, April 11, 2026 I received an alert on my vehicle that there was a possible issue with the driver's side headlamps. It was night time when this happened and I was very concerned for my safety and the safety of others while driving without proper lighting. The issue has been confirmed by a Genesis Se

It was relayed that moisture gets in the light assembly and shorts out the ILCU, which kills light functions. I have not had the vehicle inspected by the manufacturer, police or insurance representative. I feel that this issue, being known to be a common occurrence, should be a recall item and I am requesting a goodwill repair. I purchased this vehicle used and have had it for nine months. It was out of warranty when I purchased it so I purchased an extended warranty but unfortunately for me it does not include lighting. I am filing a safety complaint regarding an Engine Control Module (ECM) failure in my 2017 Subaru Impreza and the handling of a prior recall repair. At that time, the dealership documented that the ECM calibration was up to date and functioning properly. However, I later experienced a check
... engine light and drivability concerns. Diagnostic records from February 2026 indicate: •Camshaft slow response codes •Persistent sensor issues •A technician recommendation to replace multiple components, including the ECM, stating that codes were “burned into the ECM” The dealership has since attempted to attribute the ECM failure to “engine sludge” and lack of maintenance. Concern with Dealership Explanation: This explanation is inconsistent and raises serious concerns: •Engine sludge is a mechanical lubrication issue and does not directly cause an ECM (electronic control module) to fail or become “burned.” •The ECM is an electronic unit, and failure typically results from electrical defects, internal faults, or manufacturing issues—not oil sludge. •The dealership has not provided technical evidence showing how sludge could damage or “burn” the ECM. •This appears to be an attempt to shift responsibility away from a potential defect or improperly completed recall repair. Safety Concern: The ECM is critical to safe vehicle operation. A failure can result in: •Engine malfunction or reduced power •Unpredictable vehicle behavior •Increased risk while driving If ECM failures are being misdiagnosed or incorrectly attributed to maintenance issues, this could allow serious safety defects to go unaddressed. Requested Action: I request that NHTSA: 1.Investigate whether ECM failures are being improperly attributed to maintenance issues such as engine sludge. 2.Review whether the recall repair (WRE21) was properly completed and whether it is related to thiRead more

The rear subframe wiring harness on my Tesla contains a significant design flaw that creates an unreasonable safety risk. I barely drive the car, and it’s my only vehicle. It’s not safe (lighting issues ongoing, alarms, safety features don’t work, can’t get into the car) and not reliable for travel as charging is hit or miss (at commercial charging stations and home charging station), and the battery life is extremely short, even in perfect conditions using cruise control in eco mode, the car only travels about 130 miles.Read more

The electrical wiring harness connecting to the hybrid system electric fuel pump(s) was found to have significant corrosion and a missing factory-installed protective covering. This is a manufacturing defect. The protective covering was never installed at the factory, leaving the wiring exposed to moisture and corrosion from the time the vehicle was built. COMPONENT THAT FAILED: Electrical wiring
... harness to hybrid electric fuel pump(s). Missing factory protective covering caused corrosion. The failed component is available for inspection upon request. SAFETY RISK: Corroded wiring in a hybrid vehicle electrical system poses serious risks including electrical shorts, potential fire hazard, and sudden loss of hybrid propulsion while driving. The vehicle is currently inoperable and unsafe to operate. CONFIRMED BY DEALER: Yes. The vehicle was taken to an authorized Kia dealership where it remained for approximately 8 months. The dealership and manufacturer inspected the vehicle and confirmed the wiring issue but denied warranty coverage. THIRD-PARTY CONFIRMATION: A third-party warranty representative independently reviewed photographic evidence and confirmed the missing protective covering is a manufacturing defect, not wear-and-tear or owner-caused damage. MANUFACTURER RESPONSE: The manufacturer denied warranty repair, falsely claiming the hybrid component warranty does not transfer to subsequent owners. Federal warranty law requires transfer unless explicitly excluded in the warranty terms. WARNING SIGNS: No warning lamps, messages, or symptoms appeared prior to the failure. The defect was hidden beneath the vehicle and not visible during normal operation or routine maintenance.Read more
