Vehicle shut off while driving. Luckily we were in right lane and we just drifted into breakdown lane with no power steering or power braking. These cars are unsafe 2500-4000 lb missiles going down the roadway and the government is continuing to allow them on the roadway with serious life jeopardizing known defects. The dealers point at Jeep, Jeep points to dealers, both point to Stelantis and it
... an endless run around. All we want is out of the lease and nobody will help. We can’t drive the car as it’s unsafe. Our dealer is trying to help and gave us a loaner car (of much less value) but we are paying a lease on a much higher cost car while driving a low value loaner, meanwhile Stelantis just ignores the request to be let out of the lease without paying the full lease value on turn in. That’s over $13,000 if we turn it in now. Stelantis says work with your dealer. The dealer says we can’t do anything until they come out with a fix. We just want out of the lease. These cars are unsafe, by their own admission, and should not be on the roadways.Read more

I am reporting a serious safety concern involving my Tesla Model Y. While driving, the vehicle’s main center screen (MCU) suddenly failed and entered a continuous reboot loop. This resulted in the loss of critical driving information and controls, including navigation, camera display, and vehicle system status. The issue occurred without any warning and nearly caused a serious accident, as I wa
... s unable to safely monitor essential driving information. This failure made the vehicle extremely unsafe to operate. The problem appears to be related to a known MCU system failure affecting some Tesla vehicles. I am concerned that this issue may present a broader safety risk for other drivers. My driver side headlight assembly stopped working. The headlight assembly includes the driving lights, high beams, and turn signals. I am open to having this product inspected upon request. This impacts the safety of me and others due to decreased visibility and the inability to appropriately indicate when I am turning. The dealer has not reviewed this yet, but I have a scheduled appointment on Fr
... iday, April 24. Vehicle has not been inspected by any third-party, improving manufacturer, police, insurance, or other representatives. There were no warning, lamps, messages, other symptoms of the problem prior to failure. The failure first appeared at around 55,000 miles, which was 5000 miles beyond my warranty. I believe that Tesla is pushing out updates that intentionally cause light assemblies and other high priced items to stop working after the warranty. there are multiple instances people replacing the same fixture after their warranty period has expired. Tesla charges $2000 to replace one light fixture. I believe this is fraudulent and corrupt business practices by Tesla.Read more

My Jeep Wrangler 4xe now has controlled/limited electric propulsion while driving in electric only mode after 68c recall was performed. With no warning while driving in electric only mode, my vehicle will not accelerate without accelerator press almost to the floor to activate the internal gas engine. This behavior on a cold run starts out fine, but as the vehicle(electrical system) warms, the l
... imitations come on strong can catch the driver completely off guard if needing to accelerate quickly to merge into traffic or enter freeway on ramps. This erratic behavior is both dangerous to the driver along with other drivers on the road. My dealer has driven the vehicle on a short(in town) test drive and concluded the vehicle drives as intended by the manufacture. No warning are lamps visible, and as outside temps rise, this issue seems to be more apparent. This electrical behavior started the day I received my vehicle from the dealer after 68c recall was performed. The software that is now installed on these 4xe vehicles seem to be limiting/cutting the electrical output of the onboard high voltage battery to limit or eliminate heat stress on the internal battery known to catch fire and burn under heavy load. This creates a safety hazard to owners who drive these vehicles that basically stall out(no acceleration/dead pedal) without warning given to the operator prior to the software limiting the battery output to the vehicle drive train at will. When this occurs the electrical boost gauge on the 4xe dash will not rise above 25% no matter how far down you push the accelerator pedal, where normally(when not limited by software) the gauge can reach 100% during the electrical acceleration process.Read more

We bought it to travel around the country we retire. What a nightmare. We have a granddaughter who lives with us and goes camping with us most of the time. We paid over $100,000 for this van and cannot use it. We are requesting that we return the van and get all our money back. I am filing a safety complaint regarding a sudden and complete loss of power event while driving my 2021 Land Rover Discovery. The vehicle experienced a DC-DC converter failure with no warning, resulting in a sudden shutdown. The engine lost power, critical systems went offline, and I was unable to shift the vehicle into neutral. The vehicle became immobilized in active traffic, creating a dangero
... us situation. This incident occurred without warning and left me with no ability to safely move the vehicle out of traffic or rely on normal emergency functions. This presents a serious safety hazard to both the driver and others on the road. The vehicle has been at the dealership since mid-January 2026. The dealer has stated that the DC-DC failure caused additional component failures. However, no root cause has been identified beyond “internal failure,” and I have not been provided confirmation that the system has been properly tested or that the issue will not recur. I am concerned that this failure can cause cascading electrical issues, including loss of the 12V system, impacting multiple critical vehicle functions at once. The lack of a clear diagnosis and verified repair raises concerns about reliability and safety. Given the sudden loss of power, inability to move the vehicle, and absence of a confirmed root cause or validated repair, I am formally requesting that the National Highway Traffic Safety Administration open a defect investigation into the MHEV/DC-DC system. This failure creates a significant safety risk, including vehicles becoming immobilized in active traffic without warning. The risk of recurrence without a defined cause is especially concerning and may indicate a broader issue.Read more

On April 1st I had my windshield replaced due to a crack in the upper central portion of the windshield. When the glass place took the windshield off my car they found a burnt section in the headliner. The glass place reported it immediately to the Volkswagon dealership and asked the device person at the dealership to come look at the burnt headliner before they placed the glass back. The service
... person at the dealer did go and look at the car so the glass people finished installing the windshield but took pictures prior to installing the new glass. The dealership then reach out to me and asked to bring the car in ASAP. The car was dropped off a few days later for the service department at the dealership to look into the burnt headliner. After the dealer has had the car for 10 days VW Corp is refusing to conduct a full investigation to determine the issue of this burnt area. Concerned this car is a fire risk and unsafe due to wire issues or overheating elements around the windshield. The risk and issue is completely hidden in the car and not easily known about or identified during an event. Could put my house in jeopardy if fire starts while in my garage. Manufacture/dealership has inspected the car. Insurance is determining if they will inspect the vehicle or not. No warning lights or any indication this area was hot or hot even to burn the headliner.Read more

I need to open a safety case for my vehicle. I am concerned with my vehicle. Car was having trouble starting on 4/16/2026 while parked at home giving a "full accessory power active" error. I called Ford and I was told it could be a battery issue. Triple A was called and help was dispatched. The Triple A representative did not jump start the battery and instead gave directions to power up the car
... back up with no electrical current necessary. Car started after trouble shooting it and it was tested through the night just like Triple A directed. The following day on 4/17/26 the car had no issue starting and all was normal. While testing the vehicle it started to produce a strong burning rubbery smell. I opened the hood and there was a leaking sound followed by a light smoke and leakage near the battery. A video was taken if needed. The smell was extremelly strong and I am reporting this safety concern as the VIN associated with the vehicle currently has an unrepaired recall with no available remedy yet by FORD - 25V467. The safety risk associated with this recall number is fire under hood due to possible injector crack that could lead to fuel accumulating on top of the engine. I want this issue recorded as I am concerned with the safety of my family.Read more

Issue: Intermittent infotainment failure – MOST network connection lost I am reporting a recurring electrical failure involving the infotainment system in my 2019 Honda Odyssey. The system intermittently loses all audio and displays a “MOST network connection lost” message. At times, the screen also goes black and reboots. This issue has occurred repeatedly over time, especially after exte
... nded driving (e.g., long trips). The audio will stop working completely, and the system may or may not recover after restarting the vehicle. Multiple repair attempts have been made, including replacement rear entertainment system. Despite these repairs, the issue continues to occur. Based on the error message and symptoms, this appears to be a failure within the MOST communication network, potentially involving the amplifier or related components. This creates a significant distraction while driving, as the system becomes nonfunctional and unpredictable. I am unable to access hands free phone capability and navigation features. I am also unable to access the heating and cooling features in the car when the screen is black. This has been happening since about one week after I purchased the vehicle in 2021. I am concerned that this may represent a broader defect affecting multiple vehicles.Read more

Faulty panel wiring but everything works on shore and truck power. Called Grand Design (case #00996054) and was told it was a bad LiFEPO4 battery . I open the distribution panel and the schematic shows I should have 2 red wires (one to converter and the other to the battery) but I only have 1. Solution: Issue was in the original wiring from the factory. I called GD again and tech noticed the red
... wire was there but incorrectly connected to the DC fuse panel side (essentially bypassing the converter) as "battery disconnect." For my sanity I spent $165, not reimburseable per GD, and had a RV service tech come out and confirm the wiring was now correct and was told once I got a new battery everything will work as intended. I purchased new smart LiFePO4 and now all 12V equipment works as it should both on and off shore power. In conclusion, I've had no battery power once both the OEM and replacement LiFePO4 batteries went dead because they were "NEVER/EVER" being charged since the batteries were "NOT CONNECTED". This meant the battery disconnect never functioned from the beginning and when the RV battery disconnect is switched to "off," it severs the connection between the battery and the RV's 12-volt DC system and therefore the battery will not power appliances or charge when connected to shore power. By connecting an RV battery directly to the 12V DC panel (bypassing the converter) means the battery becomes the sole power source for all 12-volt appliances. Issues: Limited power duration--12V items will run until the battery drains, which can happen in a few hours/days depending on usage. No automatic charging--The battery will not be recharged, as the converter is not in the loop. Once the battery dies, you will lose all 12V power. Safety risk--The battery is likely connected to the panel without a battery disconnect switch or proper fuses, creating a potential fire hazard if a short occurs.Read more

Subject Urgent Complaint – Recall Repair Delay for 2022 Jeep Wrangler I am writing to formally express my dissatisfaction with the handling of a recall repair for my 2022 Jeep Wrangler, specifically involving the electric battery system that powers the vehicle’s electric motor. 2020 Chevy Blazer RS 3.6L V6: having intermittent issues with ‘check engine’ light activating and giving code P0128 - indicating “Coolant Thermostat (Coolant Temperature Below Thermostat Regulating Temperature)”. Symptoms include: - Temp gauge drops to reading “C” (cold), - AC blows warm/hot air, - Dash displays message, “Steering Assist is Reduced, Drive with Care”, - Radiato
... r (electric) fans run full speed. It seems that the computer is interpreting the engine coolant temperature sensor drop as a ‘stuck open’ thermostat, thus the indicated code. However, at the same time it also appears to assume a bad coolant temp sensor, and failsafes to disable the AC and set the radiator fans to max to avoid potential overheating. The problem with this is that this can occur after driving for some period that the engine is suitably warm, then I can pull off the road, disconnect the battery for a few minutes, and everything goes back to normal operation. A broken thermostat would not behave this way. * This happened in Mar 2025 - after disconnecting the battery about 5-6 times over about a 1wk period the code cleared and we were able to pass inspection. The issue did not reoccur until about the same time this year. It has been recurring periodically since. * After reading numerous postings on this issue, with numerous different vehicles that have this engine and computer, I have no faith that the dealership will provide a solution other than charging me to replace the thermostat, and ending up with the same problem a few weeks later.Read more

Incident Detail: In April 2026, my 2022 Toyota RAV4 Prime (VIN: [XXX] ) suffered a critical failure where the charging cable became physically locked in the vehicle’s high-voltage (HV) charging port. I transported the vehicle to Ken Garff Cheyenne (authorized Toyota dealer), where management allowed non-certified, non-technical staff to perform unauthorized physical interventions on the HV port.
... No manufacturer-certified technician was provided for a safe diagnostic. Safety Hazard and Immediate Consequence: Following these unauthorized interventions, my wife (an educated automotive technician) determined the vehicle was in a hazardous state, risking thermal runaway or electrical fire. This hazard was so severe that the owner of the property where I was staying in Cheyenne—a [XXX] dependent senior—was too frightened to allow the vehicle to remain on the premises. Due to this immediate safety risk and the dealership's refusal to coordinate a safe tow, I had to have the vehicle towed to a neutral facility to protect the life and property of a vulnerable widow. Legal and Technical Context: I am a permanent resident of San Diego, California, and this vehicle is covered under the California Song-Beverly Act. The vehicle is also subject to Federal Recall 23V478 (DC-DC Converter Fire Risk). The dealership’s unauthorized tampering with the HV system, combined with ambient temperatures in Cheyenne often falling below the 41°F safety threshold for this system, created an unmitigated fire risk that the manufacturer has failed to address. Request: I am seeking a federal investigation into the dealer’s failure to follow High-Voltage safety protocols. I demand that Toyota Motor North America provide a Field Technical Specialist (FTS) to perform a safe diagnostic at the neutral facility where the car is now located, as the dealership’s actions rendered the vehicle a public safety hazard. The driver was unable to remove the child from under the seat nor able to physically lift the seat, but the buttons on the front passenger seat on the lower right side, did work in order to lift the seat back into an upright position and unpinned the childs legs. The driver took it and to a dealership to get it looked at and they failed to reproduce the issue. The local dealership did look at the vehicle, but failed to reproduce the issue. There were no warning lights to indicate this was going to occur. The local dealership suggested this was a user error, and that something must have damaged the components of the seat to activate an auto recline, which is still problematic. The dealership said due to the owner unplugging the power to the seat that the issue could not occur again, but they did not solve the mystery or issue of how it actually occurred. The child was restrained in a car seat, and the seat was pushed back in the middle section of the vehicle as far back as it can go. The front seat was still able to auto recline somehow into the child’s car seat and pin them down.Read more

The rear Large Drive Unit (LDU) has coolant intrusion due to a known design defect in the internal rotor seal. Coolant from the stator cooling system is leaking past the seal into the motor, degrading the winding insulation. An independent inspection by The Electrified Garage (Orlando, FL) on 04/08/2026 confirmed moisture at the rotor sensor (yellow condition) and degraded insulation resistance me
... asured at 3.0 megohms (should be hundreds of megohms on a healthy unit). The vehicle requires a complete drive unit replacement at an estimated cost of $7,000. This is a well-documented design defect affecting 2012-2016 Model S vehicles. Tesla has acknowledged the issue by redesigning the seals in later production units and performing "coolant deletes" on remanufactured replacement units, but has not issued a recall or extended warranty coverage. The vehicle currently has 53,542 miles and no prior drivetrain warnings, yet the drive unit is failing due to a manufacturing/design defect, not owner abuse or normal wear.Read more

A software update (version 5.0.5) was complete on my 2024 Volvo XC90 at the end of March. The software was marketed in emails to have an enhanced infotainment display to support a safer experience. Since the upgrade, I have experienced the opposite of enhanced safety. My infotainment screen and dash displays randomly go black and it restarts itself several times a day while driving. In addition
... to that, the blinkers malfunction when this happens. The most important safety issue is navigation outage on the displays. The inability to rely on the Google maps navigation within Volvo, or within CarPlay, results in my having to use my phone to input and view directions, taking my eyes off the road. It also disables to cameras when I need them. On 3/31 I emailed my sales rep asking if he'd heard of others having this complaint, he did not appear to be aware and asked me to make an appt with service center. I brought the car to Volvo on 4/2 where the service check-in employees did mention they were seeing many cars coming in with the same concerns. The technician was not able to confirm the issue but said they would contact Corporate and let me know of a response, which was 15 days ago. According to online searches, multiple other Volvo drivers are experiencing the same. Since we have become so dependent on the driver assistance features, and this issue is now occurring, I have become a nervous driver in a brand of car that is marketed as one of the safest vehicles in the world. I get worried that my BLISS or lane assistance or other driver assisted features will malfunction as well. No notifications from Volvo have been received thus far. I have not contacted the Corporate offices of Volvo yet. Please take this safety issue seriously.Read more

Component Involved: Hybrid Propulsion System High Voltage Battery / EV Battery Electrical System Power Train Incident Summary: I am reporting a serious and repeated safety defect involving my 2015 Hyundai Sonata Hybrid. This vehicle has experienced three separate incidents where warning messages related to the EV/hybrid battery system appeared, followed by a sudden loss of forward propulsion. Duri
... ng these events, the vehicle would not move properly or lost the ability to accelerate safely. This defect creates a major safety hazard because it can occur while driving, increasing the risk of being rear-ended, stranded in traffic, collision, injury, or property damage. I transport my small children in this vehicle and no longer feel it is safe. Detailed Timeline: Initial Incident: Vehicle displayed warning messages involving the EV/hybrid battery system and lost the ability to move forward properly. Vehicle had to be shut off and restarted. Independent Mechanic Inspection: Vehicle was inspected by an independent mechanic who advised the hybrid battery system likely needed repair or replacement and recommended service through Hyundai dealership. Dealer Visit #1 – Stew Hansen Hyundai (3/18/2026): Vehicle was taken to authorized Hyundai dealer for diagnosis. Dealer stated they could not duplicate the issue and no repair was made. Second Failure: After release from dealer, same dangerous loss-of-power condition occurred again, resulting in towing and additional inconvenience. Dealer Visit #2 – Stew Hansen Hyundai (4/8/2026): Vehicle returned to dealer for same issue. Dealer performed software update and stated this should correct problem. Dealer advised vehicle had been driven approximately 40 miles and was safe to release. Third Failure (4/12/2026 approx.): Only two days after picking vehicle up on 4/10/2026, same warning messages and propulsion failure occurred again. Safety Concern: This repeated loss of propulsion without warning is extremely dangerous.Read more

The contact owns a 2019 GMC Sierra 1500. The contact stated that while driving approximately 45 MPH, the accelerator pedal was depressed; however, the vehicle lost automotive power and stalled. Several unknown warning lights were illuminated when the vehicle stalled. The contact was unable to restart the vehicle, and the vehicle was towed to the residence. The contact became aware that the 400 AMP
... fuse was blown. The contact replaced the fuse and replaced the starter, however during a test drive, the vehicle stalled. The vehicle was taken back to the residence, and a 400-AMP fuse was replaced a second time. The contact also replaced the starter, the alternator and replaced the ground cables, and during another test drive, the vehicle stalled. The vehicle was taken back to the residence and the contact determined that the engine had seized. The vehicle was not repaired. The manufacturer was informed of the failure. The contact researched online and related the failure to NHTSA Campaign Number: 25V274000 (Engine and Engine Cooling); however, the VIN was not included in the recall. The failure mileage was approximately 159,000.Read more

The component that failed/malfunctioned is the instrument cluster and it is available for inspection upon request. The instrument cluster which displays the RPMs, miles, gas gauge, vehicle temperature, speedometer and other pertinent information while operating the vehicle has started to go out. If the entire panel goes completely out there is no way to see any of this information which could als
... o lead to odometer fraud as well as other notifications that are displayed on there going unnoticed. I have taken the vehicle to the dealership to see what would need to be done. They stated the entire instrument cluster would need to be replaced. I feel like this should not already be happening on a not even 2 year old car. There was no warning or any other symptoms of the problem prior to this happening in February of 2026. The cost to replace the entire panel would be over $1200 and after speaking with one of the service technicians at the dealership he stated that this was not the first vehicle he has had in with the same issue. Nissan needs to address this issue because if you can't even get a new vehicle to last 2 years or 45,000 miles before it has a malfunction like this then what are we doing.Read more

This is a report of a recurring, life-safety defect involving unintended vehicle movement in a 2024 Ford Expedition. The vehicle has experienced two separate incidents where it was placed in 'Park,' remained stationary for 13-30 seconds, and then began driving itself backward without driver intervention or throttle input. Incident 1: Occurred on March 29, 2026 and I have this one on video. Incid
... ent 2: Occurred on April 8, 2026. The video clearly shows the vehicle remaining stationary and in 'Park' for 13 seconds before the unintended movement begins. This delay proves the movement is not a 'start-up rocking' sensation or 'normal clutch drag,' but a delayed mechanical or electronic failure of the park actuator or transmission system. I presented the vehicle for service at a Ford dealership on April 17, 2026. Despite my report of a delayed-onset rollaway, the dealership documented the issue as a 'normal and desired characteristic' of the 10-speed transmission. They characterized the event as a minor 'rocking' sensation that occurs immediately, which is factually contradicted by my video evidence showing a 13-second delay. This leaves me with an unsafe vehicle prone to unintended movement. I am requesting a formal investigation into a potential park-engagement failure.Read more
