The electrical system in my 2023 Mitsubishi Outlander has repeatedly malfunctioned since September 2023. The primary failure involves the dashboard completely shutting down while the vehicle is in motion, resulting in the loss of critical driving information such as speed and system warnings. The issue is intermittent but ongoing and may be available for inspection upon request. This failure creat
... es a serious safety risk because I am unable to see essential driving information while operating the vehicle, placing both myself and my family at risk. The vehicle has been taken to authorized Mitsubishi dealerships multiple times for these issues. Repairs have been performed during these visits; however, none have permanently resolved the problem. The issue temporarily stops after repairs but eventually returns and continues to recur. The problem has been reproduced multiple times. The vehicle has been inspected by authorized Mitsubishi service departments on multiple occasions, but the underlying issue has not been properly diagnosed or permanently repaired. Prior to failure, there have been repeated battery-related issues and sensor malfunctions. The vehicle has required at least three battery replacements since purchase. These symptoms often occur before the dashboard shuts down. In addition to the electrical issue, the vehicle has also experienced fuel pump concerns, which have persisted despite repair attempts. This ongoing and recurring defect makes the vehicle unsafe to operate and remains unresolved.Read more

The first time this happened I was pulling into a parking lot and all service lights started flickering and it said to turn turn off car immediately as I was driving, I pulled into the nearest parking lot and it just stopped driving. The second time, which was Friday, April 17, 2026 I was driving down a busy road and all lights started flashing, the big NO ASB brakes flashing and I had no brakes.
... It was terrifying. Thankfully I ended up going down a hill onto flat ground and was able to pull into a gas station and get stopped by the curb. I immediately turned off the car and called the dealership. This happens without any warning at all, just when I am driving. This car is completely UNSAFE and unreliable. I needed to drive 188 miles on Saturday and when I picked the car up from the dealership the service manager told me NOT to take it on the trip, after being told the updates were completed. The issue has not been reliably reproduced, but the vehicle has been taken to the dealership twice for the same concern. After the most recent visit, the service manager advised me not to drive the vehicle due to safety concerns, indicating the issue may still be present and unresolved. The vehicle has been inspected by an authorized dealership on two separate occasions. It has not yet been inspected by the manufacturer directly, police, or insurance representatives. No warning lights, messages, or prior symptoms appeared before either incident. The loss of braking occurred suddenly and without any advance indication. The failure involves the vehicle’s braking system and potentially the electrical/software systems. In addition to the braking issue, when the dealership connected a diagnostic device to the vehicle, it incorrectly registered the odometer reading as approximately 77,000 miles, while the actual mileage is approximately 4,801 miles. The failure involves the vehicle’s braking system and potentially the electrical/software systems. In addition to the braking issue,Read more

Randomly, and without warning, the instruments, gauges, and displays will switch into a language I do not speak or understand (Italian). Coinciding with the switch to Italian language, cluster and radio display settings revert back to default settings, auto door locks and paddle shifter becomes inoperative (with no menu to enable), pedestrian emergency brake (peb) lamp or forward collision warning
... (fcw) lamp turns on with no way to disable, units of measurement including speed, temperature, etc. all change, and many other random settings change as well. ***This is clearly a dangerous situation because of all the functions and gauges that change, and the fact it is difficult if not impossible to recover from this due to the language of the setting menus, and the complexity of discovering and navigating through all these changes. I have taken it to the dealer twice, and they say there is nothing they can do and to wait for a possible software update, but none has come and it has been over a year. When this happens, even if one is able to wade through all the settings menus and change setting back, some items cannot be reset (like the FCA off lamp). According to various Jeep owner forums, some dealers have been able to reset something and temporarily correct the issue, but they charge for this service, and the issue returns randomly again. Here is an example forum discussion, indicating many if not all vehicles are affected: [XXX] Many people on the forum believe this issue did not start until a software update a year or so ago. This is a well known and widespread problem, and there is even a DOT Bulletin on the issue: dot.report Bulletins Because this is dangerous (for settings/gauges/units to no longer be useful while driving), and it occurs randomly, I strongly believe this issue should be a manufacturer recall. ... o my home The vehicle was actively charging at the time of the incident. Safety Concern The failure presents a significant safety risk, including: Potential for fire due to sustained electrical arcing Possible high-voltage backfeed from the vehicle’s onboard charger (OBC) onto the AC input lines Failure of protective devices (surge suppression components destroyed) Risk of similar incidents occurring in other vehicles The damage pattern suggests a high-energy electrical fault localized between L1 and L2 without ground involvement. Current Status Vehicle has been out of service since February 21, 2026 Subaru has had possession of the vehicle since February 24, 2026 No determination or repair has been provided after several weeks Requested Action I request that NHTSA: Investigate potential onboard charger (OBC) failure modes Evaluate risk of electrical backfeed or uncontrolled fault conditions during charging Determine whether this represents a broader safety defect affecting other vehiclesRead more

On April 17th 2024 my vehicle received a recall update 68C regarding the plug-in hybrid electric vehicle battery. Two days later, I was driving on a major freeway in Southern California, noticed that the EV battery was not being depleted as it remained at 100%. Shortly thereafter, a check engine light came on, and the power was severely restricted. Within a few minutes, the vehicle completely lost
... power in the middle of the freeway and I was able to get over to the right shoulder. At this point, the vehicle had no power and I could not turn the vehicle off either regarding the electronics. I had the vehicle towed to the dealership in San Diego, and by the time it was at the dealership, the vehicle was in full lockout mode and had zero battery power left and could not even lock the doors. This caused a significant safety hazard as I was with my [XXX] child and the vehicle just completely lost power on the freeway. The vehicle is now at the dealership and they are giving me a very hard time saying that there is nothing wrong with it, but I have read that there have been many problems with this recall update. The dealership told me that the problem was a bad 12 V battery and that I needed to pay to replace that and I argued with them that that was not the problem and they are not addressing the major issue with a complete power loss on the freeway after a required software update that has known problems. Blind spot monitoring system consistently malfunctions, illuminating the driver mirror warning light when no other vehicles are present. It occurs more frequently after dark, which flickers the bright light continuously, or leaves it on constantly, creating a significant distraction and mistrust of the systems reliability. Dealer had no recommendation for fix.
Incident date: Aug 1, 2024
The following systems and features have been repeatedly failing in my vehicle, posing a safety hazard: 1) The side view mirrors change orientation automatically while driving. 2) The blind spot warning is inconsistent and is not trustworthy. 3) The infotainment display turns off erratically while crossing critical intersections where I am supposed to turn. This is extremely distracting and pose
... s a severe safety risk. 4) The infotainment system’s audio volume, audio source and car play connection turns off randomly. This is extremely distracting and poses a severe accident risk. 5) The air conditioning strength and temperature changes automatically with the fan blowing extremely cold or extremely hot air. 6) The windshield wiper automatic speeds are mis-calibrated and gradation between the various levels is non-linear and erratic. This forces the driver to frequently adjust the wiper speed while driving, posing a distraction and accident risk. 7) The automatic high beam feature doesn’t work, or works erratically. This ambiguity causes distraction and dangerous night time driving situations. 8) With manual operation of high and low beams, there is a significant delay while switching from one to another, posing risk while night time driving. 9) The windshield cleaning feature with dispersal of the fluid works erratically. It works some of the time and doesn’t work other times. This ambiguity poses a severe risk while driving due to unclear windshield. 10) The tire pressure sensor displays inaccurate inflation pressure at times. This ambiguity can be catastrophic given the vehicle size and California’s mountain road driving under slick and snowy conditions. 11) The GPS directions displayed on phone and infotainment system are inaccurate and inconsistent, causing massive confusion while exiting. This is a new vehicle from 2024 and has been inspected and serviced at the dealership diligently. I have video evidence for some of the problems.Read more

We think there’s an issue with the abs module and wheel speed sensor because we started having a problem where the brakes wouldn’t work or would stutter. The only thing that made it stop was to unplug the abs but then other parts of the car stopped working
... like the speedometer and the sensors around the car. So we got the abs replaced but the new one started doing the same thing. We did some research and it sounds like the same issue Hyundai is having with the palisade. The incident that made us get it replaced to begin with was were driving down the road and we got to a traffic light but when we tried to brake they just didn’t work so we drove through the red light luckily the one person who was supposed to be going saw us and didn’t pull out in front of us so we didn’t get hit. We haven’t taken it to the dealership yet but my sister who teaches automotive repair at the community college is the one who diagnosed it so she said we should report it here so maybe it won’t happen to anyone else. Before we had the brake issue we were having problems with basically everything on the dash not working randomly. We’d be driving down the road and the speedometer and the outside sensors and the gas gauge and the traction control all would go out at the same time it’s the same stuff that went out when we had the abs unplugged so it seems that might be related.Read more

As soon as we were able to this morning, my husband brought it to the Honda Service to be checked out. We have to heard from them yet other that $230 to diagnose the problem. We were very lucky there were not others close to us when it happened. This could have been a very serious wreck if we were in traffic or going an even higher speed on the interstate(which we had been doing earlier).Read more

2018 VW Tiguan. The fuel injector harness failed. There is a NHTSA letter stating 2018-2019 Tiguans had this issue and there’s an extended warranty up to 120,000 miles. My Tiguan is at 91,000 miles, but VW is saying the warranty isn’t assigned to my VIN. So there’s a warranty for this exact issue and year, but my vehicle isn’t covered. This warranty obviously needs to be extended by build

April 3, 2026: my 2025 Subaru Outback Premium (VIN [XXX] , 2,052 miles) suffered simultaneous failure of multiple safety-critical systems due to corrosion on a CAN bus pin in the driver door wiring harness (Subaru part #XXX) at its dash harness connection, caused by water intrusion into the cabin. Trip start: EyeSight, collision avoidance, blind spot, and lane assist all offline; BRAKE indicator i
... lluminated with no brake engagement. Minutes later, airbag warning activated. Dealer service advisor described the dash as "lit up like a Christmas tree." While reversing through a 3-point turn on a commercial drive used by heavy trucks, I LOST STEERING at single-digit speed. 60 seconds further would have placed me on an inclined high-traffic road near a freeway interchange. Vehicle was towed by Subaru from the failure location. Confirmed by authorized dealer Driveway Subaru of Moon Township (Invoice #680646, warranty claim). Subaru SSM4 scan recorded 17 unique DTCs across 10 control systems, including C1460 Brake Booster Abnormality; U1131 Lost Comm Power Steering Control Module; U0151 Restraints Control Module; U0122/U1122 VDC; U0126 Steering Angle Sensor; plus multiple EyeSight faults. Dealer documented moisture on both A-pillars, driver kick panel, standing water in cupholders and center console. Prior intermittent symptom: passenger seatbelt alarm on empty seat, two occasions in prior months, consistent with past-status U0151 on scan. Dealer cleaned the corroded pin but did NOT identify or seal the original water ingress point. Vehicle remains in dealer possession. Subaru of America Customer Advocacy Case #XXX-XXXhas declined repurchase/replacement, stating vehicle is "operating safely as designed." I dispute this conclusion: the ingress source is unremediated and recurrence risk has not been addressed. Repair invoice, full DTC scan, SOA correspondence, and owner statement available on request. I was involved in a collision. The front driver's side hit another car, causing a lot of damage. The surprise was the fire. There is no reason for it to have caught for the way it did. The door was jammed, so resorted to unhooking the soft top, while I caught fire. I opened the top and jumped off the top, crawling away with burns and cuts before the entire engine exploded. My 2019 Kia Stinger GT2 with approximately 25,000 miles has been out of service since February 12, 2026 for diagnosis and repair of a system issue. The vehicle has remained at the dealership for over two months. Multiple repair attempts have been made, including escalation to Kia corporate engineering. Despite these efforts, the issue has not been clearly diagnosed and no permanent repair has bee
... n completed. The vehicle is currently unusable, and the inability to identify the root cause raises concerns that the issue could result in unpredictable or recurring system behavior. Given the reliance on electronic and computer-controlled systems, there is concern about safe and reliable operation if the vehicle is returned without a confirmed fix. The manufacturer has not provided a clear resolution or timeline.Read more

I am submitting a formal safety complaint regarding a failure of the power tailgate system on my 2023 Honda Odyssey. The issue involves water intrusion into the liftgate control system, which has resulted in corrosion and subsequent failure of the power tailgate. This malfunction presents both a safety concern and a significant reliability issue, particularly for a vehicle of this age and mileage
... . What is most concerning is that this does not appear to be an isolated incident. There is a documented history of similar failures in Honda Odyssey vehicles, including prior class action litigation related to tailgate and electrical system defects. Despite this, the issue does not appear to have been adequately addressed or corrected in newer models, including the 2023 version. My vehicle has approximately 30,500 miles and is only slightly outside of the warranty period. I was explicitly assured at the time of purchase that the vehicle was reliable and that the manufacturer would stand behind it. However, I have since been informed that this repair will not be covered, despite the apparent design flaw and known history of the issue. A power tailgate failure is not merely an inconvenience—it can pose safety risks, especially when transporting passengers, children, or cargo. A malfunctioning liftgate can fail to open or close properly, potentially causing injury or limiting access in urgent situations. I respectfully request that NHTSA investigate this matter further, as it appears to be part of a broader pattern affecting Honda Odyssey vehicles. Consumers should not be responsible for costly repairs stemming from known defects that have not been properly resolved. Thank you for your time and attention to this matter. I am happy to provide any additional documentation or details if needed. Sincerely, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Low beam headlight bulbs fail consistently every 90-120 days. I've replaced them approximately 20 times in 9 years. This is a common failure across this vehicle line, as noted in many online forums. This is a safety hazard.

The vehicle’s instrument cluster appears to be a non-original unit that may not be correct for the model year. The cluster is responsible for displaying the odometer and communicating with other vehicle systems. After purchase, I observed that diagnostic scan tools could not consistently access or communicate with multiple modules, and an experienced mechanic indicated the instrument cluster may
... be incompatible with the vehicle’s configuration. Because the odometer reading is displayed through this component, there is concern that the mileage may not be independently verifiable. The vehicle otherwise operates, but diagnostic communication inconsistencies raise questions about the reliability of system-level data. Additionally, there does not appear to be any service documentation indicating that the instrument cluster or odometer system was replaced, repaired, or recalibrated. This creates uncertainty regarding how the current mileage reading was established. The inability to fully verify mileage and system data may affect maintenance accuracy, resale valuation, and overall confidence in vehicle information.Read more
