Subject: Request for Investigation – Instrument Cluster Failure in Ford EcoSport (Similar to Recall 24V140) Vehicle Information: 2020 Ford EcoSport Statement: I am writing to report a safety-related defect regarding the Instrument Panel Cluster (IPC) in my Ford EcoSport. I am currently receiving persistent warnings that the "Instrument Cluster Lamps are malfunctioning" and require service. Th
... is issue renders critical safety information, such as gauges and warning telltales, potentially unreadable or unreliable. Upon taking my vehicle to a certified mechanic, I was advised that the symptoms match Recall 24V140 (Ford Reference 24C01), which addresses an IPC software corruption issue. However, when I contacted Downtown Ford to schedule the repair, I was informed that while they would cover my other open recall (23S64 - Oil Pump Tensioner), they refused to cover the IPC issue because the current recall is only assigned to the Ford Maverick. I am being asked to pay for both the diagnosis and the repair for a defect that Ford has already acknowledged as a safety risk in other models using similar cluster software. I believe this defect extends to the EcoSport line and poses a significant safety risk by failing to display critical vehicle information. I request that the NHTSA investigate whether the scope of Recall 24V140 should be expanded to include the Ford EcoSport.Read more

This car was designed without steering wheel stalks for control of the directionals. In their place it uses push buttons on the steering wheel. It becomes difficult to start the directionals when the steering wheel is turned because the buttons are no longer in a standard position and aren't easily located. This is especially difficult in traffic circles or when making rapid turns where direct
... ionals are needed. In addition if your fingers are kept rested on the buttons the system alarms and tells you they have been in place too long and the buttons stop working. I believe Tesla should offer an after market replacement returning to a much safer and standard steering column stalk which is and has been used on almost all cars for at least 75 years.Read more

I am filing a formal safety complaint regarding my hybrid Jeep vehicle due to ongoing and unresolved safety issues that present a significant risk to myself and others. Since the beginning of ownership, I have experienced repeated and escalating problems with the vehicle, including: * Hybrid system malfunction warnings and persistent red wrench indicator * Aggressive and unsafe gear shifting whi
... le driving * Vehicle stalling unexpectedly * Vehicle intermittently failing to turn on or shut off properly In addition to these critical concerns, my vehicle has been under Recall 68C since October 2025. To date, there has been no available remedy provided. I have been explicitly instructed not to park the vehicle in a garage or near structures and to avoid charging the vehicle due to an increased risk of fire. This situation has created significant hardship and safety concerns. I am unable to safely use or store my vehicle as intended. I have also been informed that potential fire damage related to this issue may not be covered by homeowners insurance, further increasing the risk and liability I am being forced to assume. Despite repeated attempts to address these issues through proper channels, including dealership service visits and manufacturer communication, no resolution has been provided. This vehicle poses a clear safety hazard due to: * Risk of fire associated with the hybrid battery system * Unpredictable stalling and drivetrain behavior while in operation * Lack of manufacturer remedy for an active safety recall I am requesting immediate investigation, accountability, and corrective action to ensure the safety of consumers affected by this issueRead more

The DC/DC converter went out. While driving [XXX], all the warning lights came on, no abs, no front collision warning and all other safety warnings lights flashed. Immediately after the lights came on vehicle completely shut down while driving leaving me stranded in the street in Greenwood, AR. I had my vehicle towed to the dealership. Land Rover was already aware of the DC/DC converter issue but
... told me it was the battery which I paid to have replaced. I drove my vehicle home from the dealership. After arriving home my vehicle would not lock. I contacted the dealership and was advised to bring it back in when they had an appt in 3 weeks. I could not drive my vehicle due to not being able to lock it. While driving the vehicle to the dealership to get repaired because it would not lock, my vehicle shut down this time leaving me stuck in a blind spot on a rural highway (55mph speed limit) with no where to pull over due to deep drainage ditches on the shoulder. For two hours I had to wave at vehicles letting them know there was a dangerous hazard in the road (my vehicle) while waiting for the tow truck. My vehicle was again towed to the dealership, 126 miles away. It was at this time I was advised by the service advisor later confirmed by the General Manager, Land Rover had an ongoing problem with the DC/DC converters. My vehicle has been at the dealership since February 9, 2025 because they do not have the DC/DC converter available. The dealership does not provide loaner vehicles. The malfunction involves the windshield wiper control system, including the rain-sensing activation module and associated control software. The system activates windshield wipers without any driver input under dry and clear conditions. The vehicle remains available for inspection upon request. Safety was compromised due to sudden and uncommanded wiper activation occurring during active driving co
... nditions, including highway and nighttime operation. This results in temporary loss of visibility as the windshield is smeared by debris, requiring driver distraction to manually control or disable the system while operating the vehicle in traffic. Initial complaints regarding this issue were made by [XXX] during dealership service visits; however, those concerns were not consistently documented in the official repair records at the time they were reported and were initially dismissed or not formally recorded by service personnel until later intervention ensured the issue was entered verbatim on the service order. The issue has been reported and addressed during multiple dealership service visits; however, the condition has not been reliably reproduced and permanently corrected by the dealer despite documented repair attempts, including sensor replacement and manufacturer-level software (ODIS) updates. The malfunction has persisted after these interventions. The vehicle has been inspected by an authorized Volkswagen dealership service department. No inspection has been conducted by police, insurance representatives, or independent third parties. No consistent warning lamps or dashboard messages were present prior to the failure. The activation occurs unpredictably and without prior warning indicators. The issue first appeared intermittently at lower mileage and has continued through recent operation. The defect persists after multiple repairs and manufacturer intervention, indicating an uncorrectable electronic system failure causing an unresolved safety risk. I was driving at 60mph. I heard a sudden pop that felt like it came from behind me / under the car. While I glanced to see if I ran over something (I did not), the car suddenly felt like it was about to stall. It was slowing down quickly and dramatically. My safety was in danger as I quickly dropped speeds in heavy traffic on a freeway without warning. I was able to maintain control, and I turned
... my hazards on. I noticed that I was maintaining speed around 35-40 mph (no matter how heavy I hit the accelerator), and I moved to the right lane and continued while wondering if my car would survive all the way home. Luckily I was only about 1 mile from my house at that point, so I proceeded at a low rate of speed and ultimately made it home. It was scary, sudden, and put me and others on the road in danger. A couple of warnings popped up saying "Electrical System issue" and something else that was related to electronics. The vehicle was towed to the dealership that night, and diagnostics and repairs began the following day. The problem has been confirmed by the dealer to be an ICCU failure that needs to be replaced. There were zero warnings prior to the failure--the car was working perfectly up until that moment. Anything is available for inspection, but as I mentioned it's being replaced as we speak so the broken / malfunctioning part wouldn't be there anymore if someone looked at it.Read more

For approximately 3 years, this vehicle has experienced repeated unintended transmission downshifts at freeway speeds without warning or driver input. The vehicle can suddenly and violently downshift at highway speed, creating a serious risk of loss of control, skidding, rear impact, or crash. This defect appears substantially similar to Ford recall 26S28 / NHTSA campaign 26V237, which covers cer
... tain 2015-2017 Ford F-150 vehicles for unexpected downshifts caused by loss of signal between the transmission range sensor and the powertrain control module. Ford reported that this condition can cause the transmission to unexpectedly downshift and create a loss-of-control hazard. (NHTSA) My vehicle appears to use the same Ford-based engine, transmission, and control architecture family and is exhibiting the same dangerous fault pattern. Diagnostic scans show PCM code P0720 and P0722. Freeze-frame data captured the transmission commanded in 3rd gear with turbine shaft speed present at 1041 rpm, but output shaft speed at 0 rpm. This indicates loss of the output shaft speed signal while driving. The transmission tag reads CK4P-7000-FB. The likely fault area is the output shaft speed sensor circuit, internal transmission lead frame, internal harness, connector, or related transmission signal path. We contacted Mobility Ventures, but they have not meaningfully responded or helped resolve the defect. We also contacted a local Ford dealer, and they stated they do not know how to handle or repair this defect on this vehicle. Based on the same dangerous symptom, same Ford-origin driveline family, and same sensor-signal failure pattern, this vehicle should be evaluated for inclusion in, or equivalent treatment consistent with, Ford recall 26S28 / NHTSA 26V237.Read more

On September 11, 2025, Hyundai of New Port Richey (Lithia Motors) executed a Certified Pre-Owned (CPO) contract and delivered a 2023 Hyundai Palisade (VIN: [XXX] act day the manufacturer issued a federal Stop-Sale for Recall #283 (Seatbelt Latch Failure). Internal documents show the CPO sale relied on mechanical inspection data from 34 days prior (August 8, 2025). There is no documentation of a p
... oint-of-sale safety check performed on September 11, resulting in the vehicle being sold despite the active Stop-Sale order. Presumably, because the sale occurred on the exact date Recall #283 was issued, we were excluded from the manufacturer's October safety mailing. We unknowingly operated the vehicle for seven months without knowledge of the seatbelt defect, though we did receive subsequent notices for campaigns 9C9, 9C8, and Recall #292 (Ejection Mitigation). Combined, the active status of Recall #283 and Recall #292 leaves the vehicle with zero safe seating positions. Furthermore, internal records show the vehicle failed a computer diagnostic test ('NG' code) with a severely depleted 11.90V battery prior to purchase. The CPO sale was executed without resolving this documented fault. This correlates to a severe electrical issue where the parked vehicle spontaneously unlocks. Attached documents: "Palisade Sales Paperwork" - Documents the 9/11/25 purchase date. "Palisade Repair Order" - Documents the electrical/battery failure. Most recently, the dealer scheduled a repair appointment for Tuesday of this week. Approximately 30 minutes before the scheduled appointment, the dealer called to cancel, stating that the parts were still not available. A new appointment was scheduled for the following week. As of today, the dealer has confirmed parts are still not available. Symptoms included: interior electronics (infotainment, lights) powered on, but the start button produced no response, the brake pedal would not depress (electronic brake booster inoperative), and the steering wheel remained locked. No error message was displayed. The vehicle was ultimately determined to have a dead 12V battery, the second battery failure on this vehicle, with the previous battery having been replaced under warranty by the dealer in August 2025 (only 8 months prior to this failure). The digital gauge cluster where it shows sensitive information is a blacked out screen. I took it an automotive shop to investigate the issue. ... . This first occured a few months ago and would randomly turn back on, but not the screen is black more than it is on. Luckily I am able to track the fuel gauge (miles to empty) on the infotainment screen.Read more
