(NHTSA Tracking Number 11726105) I am updating my existing complaint regarding my 2024 Hyundai Tucson (NHTSA Tracking Number 11726105) for an ongoing safety concern involving an intermittent warning chime while driving. This issue began shortly after I purchased the vehicle in Nov 2025 and has been reported to the dealership multiple times over several months. The warning chime occurs randomly wh
... ile driving without any visible warning message or dashboard alert. This creates uncertainty because I do not know what the vehicle is trying to warn me about, which is distracting and creates a safety concern while driving. The dealership previously replaced both front seatbelt buckle assemblies after identifying intermittent high resistance as a possible cause. However, after that repair was completed, the warning chime occurred again while I was driving to the dealership for a follow-up visit. This confirmed that the issue was not fully resolved. I immediately returned the vehicle to the dealership, and they are now performing further diagnosis. I was advised that if the issue reoccurred, the next possible areas for diagnosis could involve the instrument cluster or the control module, both of which are safety-related systems connected to warning alerts and seatbelt functions. In addition, during this same visit, the dealership confirmed there is an actual oil leak requiring repair. This is significant because when I first raised this concern after an outside inspection in December 2025, I was told it was likely leftover residue from a prior oil change. The dealership has now confirmed it is a real leak and kept the vehicle for repair while providing a loaner vehicle. Because this vehicle was sold as Certified Pre-Owned and these safety and mechanical concerns continue shortly after purchase, I remain concerned about the reliability and safety of the vehicle.Read more

My vehicle is subject to open safety Recall 67C involving the cooling fan module and related fuse protection in the engine compartment. The recall has been open since October 2025. The manufacturer and dealer have confirmed that the remedy/parts are still not available and that there is no ETA for repair. At this time, I have not experienced an obvious operating failure with the vehicle itself.
... However, the open safety recall remains unresolved, and because no remedy exists yet, the recall cannot be completed. This is my sole work vehicle, and the unresolved recall is preventing lawful completion of the import/registration process in Canada and preventing me from using the vehicle for work. The problem has been confirmed by the manufacturer and an authorized dealer as an open recall with no available remedy at this time. I am reporting that the manufacturer has failed to provide a timely remedy for this safety recall.Read more

I was pulling out of driveway, it is a steep driveway, I have kept vehicle in 4WD, I then backed up to turn around, the truck “lurched” and “jerked” forward abruptly. To Whom It May Concern, I am submitting a formal safety complaint regarding my Kia Sorento and a recurring brake light failure that poses a serious risk to my safety and the safety of my children. After every single oil change, without exception, my brake lights stop functioning properly. This has happened repeatedly and consistently. Most recently, immediately after an oil change, both brake li
... ghts went out. I replaced the bulbs, but the issue did not resolve. Instead, the lights are now stuck on continuously, which strongly suggests a failure related to the brake light switch or associated electrical components—not normal wear and tear. This pattern is extremely concerning. The fact that the failure occurs every time the vehicle is serviced indicates either a defect in the design or a component that is highly susceptible to failure during routine maintenance. Brake lights are a fundamental safety feature. When they fail, other drivers are not alerted when I am slowing down or stopping, putting me, my children, and everyone around me at risk of a serious rear-end collision. I understand that brake light switch failures have been a known issue in Kia vehicles in the past, including recalls for similar defects. Given the repeated nature of this problem and the safety implications, I do not believe I should be responsible for the cost of repairs for what appears to be a known or recurring defect. I am requesting that this issue be formally investigated and that Kia be required to repair or replace the faulty brake light switch and any related components at no cost to me. Additionally, I ask that this matter be reviewed to determine whether further action, including a broader recall or service campaign, is necessary to protect other drivers. This is an ongoing and dangerous issue that has now occurred multiple times. I should not have to worry that a routine oil change will leave my vehicle unsafe to drive. My 2024 Nissan Frontier has experienced repeated intermittent shut off and no-start failures. The problem began in late 2024 and continued through January 2025, throughout 2025, 2026, and is still occurring. First Incident- volume kept rising on its own. Would turn it down, it would go right back up. Would turn the radio off, it would turn on by itself and would keep rising until full blast. This time the vehicle would accelerate
... by itself. No cruise control was on, vehicle would keep going faster and faster. First time this happened the vehicle needed to be towed, cops were involved. I finally was able to stop the vehicle at a red light (where it eventually broke down and would not start). Extended warranty covered the tow. There was no check engine light on. Replaced the battery as I was told the errors were pointing back to low voltage. After battery was replaced, maybe three weeks later the vehicle did the same thing on the highway again. Car speed kept increasing on its own, no cruise control or pedal input. Vehicle kept speeding. I took this vehicle to Moon Township, PA Nissan Cochran dealership and they could not replicate the issue. The extended warranty company for this vehicle said they can’t do anything since the dealership says no check engine light is on. This is a report of a Failed Remedy for Safety Recall N321 (Recall #19V293). Records confirm that this vehicle had the recall repair performed on 12/09/2020. That repair has now failed. A certified technician at the dealership has confirmed via video that the windshield bond has detached, leading to significant water intrusion. This flooding has ruined safety-related electronic modules (cameras/au

The manufacturer is admitting the failure but is refusing to provide a 100% covered remedy for this failed safety recall, offering only a 40% 'goodwill' contribution. The vehicle is currently dismantled and unsafe to drive. A failed windshield bond compromises the structural integrity of the vehicle in a rollover and prevents the proper deployment of the passenger airbag. Safety Defect: Data Communication Module (DCM) Failure Affecting Hands-Free Microphone and Safety Connect / SOS Emergency System Vehicle: 2019 Toyota RAV4 Hybrid LE The DCM (which controls both the CarPlay/Bluetooth hands-free microphone and the Safety Connect SOS emergency button) suddenly failed without any warning or accident. Symptoms: - No one can hear me during CarPlay or Bluetooth calls

This is a known recurring issue documented in Toyota TSBs T-SB-0112-20 and T-SB-0089-21. Toyota of Hackensack diagnosed it and quoted $1,300 for DCM replacement/update while charging me $200 for diagnosis. They did not disclose during the multipoint inspection that this is a documented safety-related DCM fault. I performed the manufacturer-recommended 12V battery disconnect reset (standard TSB procedure), which fully restored the microphone and returned the SOS light to solid green. However, this is only a temporary fix for a known defect. This failure compromises hands-free calling while driving and the emergency SOS system — both critical safety features. It seems that when the outside temperature is very warm/hot, when turning the car on or when idling at a traffic light or driving at very low speeds, on occasion the car will abruptly and without warning, shift itself into Park and the emergency brake activates. I took the vehicle in for repair on 10/23/25 and was told there was a "thermostat internal failure" and that the "thermostat assembly" w
... as replaced. I was just recently traveling in Florida ( reside in NY) and the exact same problems occurred with my vehicle which I can only assume was due to the warmer temperatures that my vehicle has not been exposed to since the last repair as I reside in New York. I feel it should be worth mentioning that when turning OFF the auto start/stop button, the problems occurred less frequently but STILL happened on occasion. I've also had 3 other times when the "check engine" light came on. I was told that the first 2 were related to software updates with the transmission. On the other occasion, a "valve body" was replaced. I'm unsure if any of these were related. I've also had to replace the auxiliary battery but I don't think that's related so I didn't upload that file. Although I'm taking the vehicle to be repaired today, I took it in on 04/14/26 because the check engine light came on while in Florida when the car shifted itself into Park and the Emergency Brake was activated. I took my car in to Tomball Hyundai due to intermittent failure of the headlight switch and the wiper switch when both of these features are set to the ‘Auto’ setting. These are very intermittent issues, but as luck would have it, it was sprinkling on my drive to the dealership on [XXX] and I was able to capture the wiper issue via video on my phone which I shared with the service advisor. The
... n, fortunately, 2 days later, the service technician was able to verify the intermittent headlight failure. I was aware of a TSB (TXXH) which is meant to address this problem, however, Hyundai issued a superseded TSB a few months after the original, in which they deleted the wiper switch from the service. The 2015 Genesis multifunction switch is sold as a single integrated unit (P/N 93400-3M031), but Hyundai’s TSB 20-BE-009H-1 mandates a "hack" repair that leaves the failing wiper switch in place. It is my assertion that when Hyundai settled a class action lawsuit regarding this switch, they agreed to extend the warranty on the entirety of the Multifunction Switch Assembly (NOT just the Headlight portion of the switch) for 15 years and Unlimited mileage. The Hyundai National Customer Service Case Manager (Roobika, 714-912-2118 National Consumer Affairs) told me that my request of having the complete switch replaced was denied due to the vehicle being out of warranty and having 73,481 miles. *to be clear here - Hyundai HAS agreed to replace the Headlight switch only - their denial centers on the Wiper switch. Eager to find a solution, I suggested that I could cost-share. This was declined as well. I was only given the option of buying the whole switch myself and allowing them to provide the labor to install it. Furthermore, It is technically illogical to perform a warranty repair by disassembling an integrated unit to replace one half while ignoring the failure of the other half—especially when the labor required to access the part is identical. SUPPLEMENTAL TO NHTSA COMPLAINT #11713668. This is a supplemental filing regarding the 2020 Volvo XC90 T8 Inscription, VIN [XXX] . The hybrid battery cell module failed in December 2025, disabling the vehicle's hybrid propulsion system, all-wheel drive, and electric-only mode. As of this filing, the vehicle has been without hybrid functionality for over four months. The replacement part remains u
... navailable. Since the original complaint, the owner initiated civil proceedings against Volvo Car USA LLC and Volvo Car North America LLC in the State Court of Gwinnett County, Georgia (Case No. [XXX]). Through that litigation, the following facts have been established as deemed admissions under Georgia law, which neither Defendant contested: 1.This vehicle is excluded from Recall 25V-179, yet experienced a catastrophic hybrid battery cell module failure. 2.Battery failures in 2019-2022 XC90 T8 vehicles are not exclusively caused by the "LG supplier process deviation" identified in Recall 25V-179. 3.Volvo is aware of battery failures in 2019-2022 XC90 T8 vehicles outside the Recall 25V-179 population. 4.Volvo possesses internal engineering data showing the battery defect affects vehicles outside the population reported to NHTSA in Recall 25V-179. 5.Volvo has not issued a recall for the specific failure mode affecting this vehicle. 6.Expanding Recall 25V-179 to cover the full 2019-2022 XC90 T8 population would cost Volvo tens of millions of dollars. These facts were established against both Volvo entities through separate sets of Requests for Admission that went unanswered within the statutory deadline. The owner requests that NHTSA evaluate whether Recall 25V-179 is adequate to protect the full population of affected vehicles. Supporting documentation, including copies of the deemed admissions and service records, is being submitted by mail to the Office of Defect Investigation. To Whom It May Concern, I am submitting this formal complaint regarding a malfunction in the air conditioning system of my 2024 Chevrolet Trax. Specifically, the evaporator and/or condenser has failed, resulting in inadequate cooling performance and potential system leakage. This issue is more than a matter of comfort—it presents a significant safety concern. Operating the vehicle in high temper
... atures, particularly in Texas, creates hazardous conditions inside the cabin. Excessive heat can lead to driver fatigue, dehydration, and reduced concentration. Additionally, the malfunctioning A/C system affects humidity control, which may cause windshield fogging and reduced visibility while driving. The problem has been consistently reproducible during normal vehicle operation. Symptoms include poor cooling performance, inconsistent air temperature, and possible unusual odors or sounds when the A/C system is in use. Notably, no warning lights or dashboard messages appeared prior to the failure, making it difficult to detect the issue early. At this time, the vehicle is available for inspection and can be evaluated by a dealership, certified technician, or manufacturer representative upon request. I request a thorough inspection to confirm the failure and determine whether this is due to a manufacturing or design defect. If necessary, I am also willing to have the vehicle reviewed by independent service professionals or relevant authorities. Given that this issue has occurred in a nearly new vehicle, it raises concerns about reliability, quality control, and potential widespread defects affecting other vehicles of the same model. I urge prompt investigation and corrective action to ensure driver safety and product integrity. Please advise on the next steps for inspection, repair, and resolution of this matter. I was driving my car home from work and many warning lights lit up on my car dashboard (brake problem, engine problem, uphill start, steering problem) etc.. I got off the freeway and my car dashboard lit up and notified me “to pull over to a safe location because my car needle (temperature) was on high. I pulled over to a safe location and opened the hood and noticed that the antifreeze coolant
... compartment was empty) my car could have caught on fire because there was no coolant inside the compartment. After I got home and filled the car with coolant I placed cardboard under the car engine to see if the coolant would leak. There are no signs of leakage. I took my car to my mechanic and he told me that he couldn’t fix the problem and suggested I call Honda USA customer service. The mechanic told me that the problem is with the “head gasket” and that the coolant does leak into other parts of the engine. He didn’t charge me anything for the inspection and added coolant to my car. I called Honda USA customer service and they told me that there are “no recalls” regarding coolant leaks and head gasket problems. I googled problems with “Honda 2016 civics and learned that there is a problem that exists and it’s very costly to fix. I also learned that Honda was sued in Canada and Honda settled with customers regarding this problem. Honda USA simply told me to take my car to a Honda dealer and ask for an “inspection” I asked if they would cover the cost and they didn’t not say they would pay for the inspection. This is a manufacture problem that occurs after a Honda Civic has gone past 70,0000 miles. People still complain about this issue and wonder where the coolant in the car is leaking and where is the coolant going into since the coolant is not visibly leaking. The coolant is making its way within the engine and causing more problems in my car. I have never had a problem like this one. This is the second time that the coolant inside my car leaks into the engine.Read more

Since the day of delivery, this 2025 Audi S5 has experienced daily electronic and safety system failures. At least once per day, on startup, the vehicle throws 10 or more warning codes disabling safety-critical systems including blind spot monitoring, lane departure warning, forward collision avoidance, rear traffic alert, and adaptive lighting. These failures occur without warning and persist for
... varying durations before clearing on their own. Additionally, on 5 occasions the adaptive cruise control refused to activate, displaying a false "door open" warning with all doors and trunk confirmed closed. On 4 occasions the power trunk failed to latch using the electronic controls. The 360-degree parking camera system intermittently stops rendering surroundings, displaying only the vehicle graphic with no camera feed. The vehicle was brought to the selling dealer for service. After one hour, the dealer stated no issue was found. A battery diagnostic was requested based on a documented case of an identical 2025 S5 with the same symptoms traced to a faulty 12V battery. The dealer declined to cover the diagnostic under warranty. The service manager later acknowledged these are known platform issues and confirmed a software update is expected from the manufacturer in July 2026. The vehicle already has the latest available software update (TSB 2079690, KD2, version 03.10.00/C) installed and the problems persist. The dealer has confirmed there is no current fix available. These failures put the driver and other road users at risk, as safety systems are non-functional during the error state with no indication of when they will restore.Read more

The car will all of a sudden have light started flashing and then it will stop going.

Hybrid battery system, still don't have heat after 2 years of car ownership. Not to mention the hybrid battery failure rate, I've driven several times and the vehicle will just have no power. I bring it to the dealership almost monthly and they always have some excuse. The vehicle's lane assist is also a hazard. I'll just be driving down highway and it'll just jerk the wheel randomly.

My 2024 Honda Odyssey EX-L has a recurring Auto Idle Stop failure. About 1–2 times per week, when stopped at a light and Auto Idle Stop engages, the engine fails to restart properly when I release the brake. The engine sputters or shuts down, the battery warning light comes on, and the vehicle shifts itself into Park. I then have to hold the start button for several seconds to restart the engine
... . This creates a safety risk because the vehicle becomes disabled in active traffic when the light turns green. The vehicle was previously taken to a Honda dealer for this issue, and the battery was replaced, but the problem continues.Read more

I purchased the vehicle with approximately 56,000 miles. Within 2–3 weeks after purchase a “traction battery fault” warning appeared. I took it to a dealership, which said it was a battery module fault. They replaced defective battery cell under warranty (repair value ~$7,000–8,000). During the same initial period and continuing after the battery repair, the vehicle exhibited intermittent
... AC charging failures. It would begin Level 2 charging sessions and then terminate them prematurely. This includes both the Level 2 charger I installed new at my home upon purchasing the car, and commercial Level 2 chargers. The home charger would repeatedly show a fault that the vehicle was ending the sessions. Over the following months, the condition progressively worsened from intermittent to near-total loss of AC charging capability. The vehicle can still DC fast charge. Dealer diagnosed the failure as a defective BCCM requiring replacement at a cost of approximately $6,700. This is a known and documented issue: Jaguar has issued TSB SSM75883 (NHTSA Document MC-10227452-0001) addressing I-PACE vehicles unable to charge with BCCM fault code U3000-49, and TSB SFCC Jul20_11 (NHTSA Document MC-10178140-0001) for the 2020 I-PACE specifically. The vehicle cannot be charged at home or at any AC charging station, rendering it effectively inoperable for normal daily use.Read more
