A coolant leak from the flow shut‑off valve caused the engine maintenance warning light to remain on for an extended period. This issue is covered under Toyota Customer Support Program 24TE04. The warning light is a safety concern because it indicates an active engine‑related malfunction and can mask other warnings. The defect was first diagnosed by a Toyota dealership at approximately 97,000
... miles, well within the CSP coverage window. The dealership stated the vehicle “did not meet criteria” and refused to perform the repair, even though the symptoms matched those described in the CSP. The issue continued until a second Toyota dealership later confirmed the same defect and replaced the coolant bypass valve. Because the vehicle had since exceeded 100,000 miles, Toyota refused to cover the repair under the CSP, despite the defect being documented earlier within the coverage period. Multiple attempts to resolve the issue with Toyota corporate have resulted in no returned calls or meaningful response. The defect caused prolonged operation with an illuminated engine warning light, which presents a safety risk and indicates a failure to properly address a known issue covered under Toyota’s Customer Support ProgramRead more

I own a 2016 Model S P100D equipped with the Large Drive Unit (LDU). The component that fails is the rotor coolant seal in the rear drive unit motor. Coolant leaks internally into the motor and inverter, causing bearing wear, corrosion, isolation faults, and eventual complete drive unit failure. The affected drive units are still installed and available for inspection upon request. This defect put
... s my safety and the safety of others at risk because it can cause sudden loss of propulsion (especially at highway speeds), power reduction, or total drivetrain shutdown, potentially leading to a crash. Coolant in high-voltage areas also raises electrical fault/fire risks. The problem is widely reproduced and confirmed: Tesla service centers are familiar with it and replace failed units with updated “-U” revisions that include a coolant bypass. Independent EV shops confirm the exact root cause (rotor seal failure) via encoder inspections showing coolant intrusion and through disassembly. This is a documented pattern failure in all 2012–2020 LDU vehicles. No police or insurance inspections have occurred yet, as no crash or total failure has happened. Tesla is aware of the recurring issue through service records on similar vehicles. Early symptoms before full failure typically include rear motor whining/grinding (especially under load), reduced power warnings, “Rear Drive Unit Fault” messages, and visible coolant on the speed sensor. These can appear gradually. My vehicles have not yet experienced total failure, but the defect is known to be inevitable. I am proactively monitoring encoder sensors. Because this is a systemic design defect affecting thousands of vehicles with clear safety implications, I request NHTSA open an investigation and require Tesla to issue a recall for free repairs/replacements on all affected 2012–2020 LDU vehicles.Read more

I own a 2021 Nissan Leaf with an open safety recall (NHTSA Recall 25V-655, Nissan Recall R25C8) issued on September 30, 2025. The recall involves a battery defect that creates a safety risk during DC fast charging (Level 3). As a result, I am unable to use public fast-charging networks such as EVgo. The recall currently has no remedy available and has been unresolved for over 7 months. I do not
... have access to home charging, and without fast charging, the vehicle cannot be used in a practical or reasonable way for daily transportation. This has resulted in a complete loss of use of my vehicle and has impacted my ability to work. I recently purchased this vehicle, and shortly after, it began having serious powertrain issues. When I accelerate, the vehicle intermittently loses power and starts shaking violently. It feels like the engine or transmission is slipping or failing. The RPMs surge and drop rapidly, and the vehicle hesitates, jerks, and sometimes briefly slows down before suddenly accelerating again. This creates
... an unsafe and unpredictable driving condition, especially when merging or driving in traffic. I’m concerned about a loss of propulsion while the vehicle is in motion. My mechanic indicated the issue may involve the torque converter clutch not engaging properly (possible “stuck off” or circuit performance issue), which would explain the shuddering and inconsistent power transfer. Despite how severe this is, there are no warning lights or check engine light, and no diagnostic codes, which raises concern that the failure is not being detected by the vehicle’s onboard systems. I am also experiencing low and fluctuating engine oil levels even after topping it off and a recent oil change, which may indicate an additional engine-related defect. The vehicle has a clean title and no accident history. - Failed system: Powertrain (engine/transmission), suspected torque converter clutch malfunction. Vehicle is available for inspection. - Safety risk: Intermittent loss of power, hesitation, and surging creates a high risk of collision, especially in traffic or at highway speeds. - Confirmation: My independent mechanic has identified as likely a torque converter related issue. - Inspection status: Not yet inspected by manufacturer or insurance. - Warnings/symptoms: No warning lights or codes. Symptoms began shortly after I purchased and have worsened over time.Read more

My vehicle has been garage-kept its entire life except when in the care of a dealership. Corrosion on a recently replaced transmission in a garage-kept vehicle cannot be attributed to environmental exposure or owner neglect. It points directly to a defective part of improper installation at Curry Honda. These failures occurred within months of a new transmission installation with no prior history of issues. I believe this is a safety defect as the vehicle broke down unexpectedly and had to be driven in a compromised condition posing a safety risk.Read more

I am filing this safety complaint on behalf of myself and my wife, [XXX] , the primary driver of our 2021 Ford Escape PHEV, with active under Recall 25SC4 (no remedy available). The vehicle has experienced 8+ sudden complete shutdowns during normal driving since February 2026, always during EV-to-hybrid mode transition, always without prior warning. The only warning is a brief sound like air escap
... ing immediately before shutdown. At shutdown: engine cuts completely, steering wheel locks rigid and cannot be turned, brake pedal locks hard and cannot be depressed, gear selector locks in Drive and cannot be changed. The driver has zero ability to steer, brake, or move the vehicle. Must activate hazards and wait 1-5 minutes for restart while immobilized in an active traffic lane. Incidents occurred: (1) February 2026: multiple times, dealer retained vehicle 5 days, found nothing. (2) Early April 2026: multiple times, including one witnessed by a police officer who directed traffic around immobilized vehicle. Dealer kept 3 days, blamed the original Ford-supplied charger. Replacement did not fix defect. (3) May 1, 2026: 4 times in one day with my wife only in the car, including on the Dolphin [XXX] near Miami Int [XXX] 3:30PM Friday rush hour, with our [XXX] child in the vehicle on thr [XXX] r occasions. Ford's own recall documentation states steering and braking remain functional during shutdown. This is directly contradicted by our firsthand experience. Both are completely inoperable. Dealer on second attempt blamed original Ford-supplied charger — replacement did not resolve defect. This vehicle is subject to active class action [XXX] et al. v. Ford Motor Company (Case XXX). While operating the vehicle on both interstate highways and local roadways, an intermittent malfunction occurs involving the All-Wheel Drive (AWD) system. The instrument panel displays a warning message stating, “Service All-Wheel Drive System.” When this warning appears, the vehicle automatically disengages from AWD and reverts to Two-Wheel Drive (2WD). Once this occurs, the system does not a
... llow the driver to manually return the vehicle to AWD. This issue has occurred on more than three separate occasions. Notably, there have been instances where the warning indicator illuminated immediately upon starting the vehicle, prior to any extended operation. When the malfunction is present, the vehicle exhibits abnormal driving behavior, including a noticeable jerking or hesitation that feels consistent with a transmission-related issue. The failure is unpredictable in nature, with no clear pattern or triggering condition identified. This creates a significant safety concern, particularly when operating the vehicle at highway speeds or in conditions where AWD capability may be necessary for proper traction and control. An appointment has been scheduled with an authorized dealership for diagnostic evaluation; however, at the time of this report, the vehicle has not yet been inspected or repaired, and the issue remains unresolved.Read more

2024 Dodge Durango (SXT Plus, GT Plus, RT trims) Update to my prior complaint regarding a dangerous shifter defect. I went to start the car and it was totally dead. It is used 7 days a week and driven back and forth to work 5 days a week. There was no warning and nothing was left on, just dead. Poor drainage resulting from vehicle design defect. BMW needs to remove or modify the drain outlet to prevent repeat flooding. The warranty and extended warranty never checked for this-- drainholes functionality which results in electronic malfunctioning and safety concerns. 2019 SIB should have enforced extended warranty to investigate and cover this design defect in this vehicle. We are submitting complaint to the office of defects and investigation and requesting a good will repair and replacement of faulty systems and mold abatement which will require replacement of the damaged and mold in the pax and rear trunk areas. I can not give a date as this has been an ongoing concern and I have only recently been able to address. I made an approximation only.Read more

The car has quite a few blind spots that only the backup camera would show you. The rear vision system started to go in and out

They witnessed the camera malfunction with their own eyes and found the recall themselves, but then relayed to me there was nothing they could do because the recall had not been remedied.Read more
