I am filing this complaint regarding an unresolved and potentially dangerous safety defect involving the rearview camera system on my Ford vehicle, which was presented to Wade Ford Auto Dealership and Service in Smyrna, Georgia for a rear camera recall repair. I brought my vehicle, a 2016 Ford Expedition, to Wade Ford for recall service related to the rearview camera. Wade Ford retained my vehic
... le for approximately one week. When I picked up the vehicle on or about [insert pickup date], I discovered that the rear camera issue had not been corrected. The rear camera image remains dark, unclear, visually deficient, and unsafe, creating a continuing hazard when the vehicle is operated in reverse. This is not a cosmetic or convenience issue. A defective rearview camera materially impairs rear visibility and increases the risk of collision, property damage, or injury. Federal safety authorities have recognized rearview camera failures as safety concerns because blank, distorted, inverted, delayed, frozen, or unclear images can reduce the driver’s view behind the vehicle and increase crash risk. NHTSA recall materials for Ford rearview camera defects describe the safety consequence as reduced rear visibility and increased crash risk when the rearview camera displays a distorted, inverted, or blank image. (static.nhtsa.gov) After I raised the unresolved condition, Ford claimed or suggested that the issue may be due to an aftermarket radio allegedly installed in the vehicle. I dispute that explanation. I have not replaced the rear camera, and I do not accept Ford’s unsupported attempt to attribute an unresolved safety recall defect to an aftermarket component without competent inspection findings, diagnostic proof, or written documentation. If Ford or Wade Ford contends that an aftermarket radio is the cause, then I request that they provide a written technical explanation, diagnostic results, photographs, technician notes.Read more

The component or system that failed appears to be the Auto Idle Stop / engine restart system, possibly involving the starter, starter relay, battery/charging system, or engine control software. The vehicle is available for inspection upon request. My safety and the safety of others were put at risk because the vehicle unexpectedly failed to restart and became disabled while stopped at a traffic light, creating a traffic obstruction and risk of being struck from behind or causing other drivers to maneuver around the vehicle. The problem has not yet been reproduced or confirmed by a dealer or independent service center. UNKNOWN whether diagnostic trouble codes were stored. The vehicle/component has not yet been inspected by the manufacturer, police, insurance representatives, or others. I am reporting a critical safety hazard involving my 2021 Nissan Leaf S Plus (approx. 117,000 miles). Since January 2025, the vehicle has exhibited dangerous battery degradation and voltage sag during operation. On my 30-mile highway commute, while driving at normal highway speeds—especially on inclines—the vehicle would suddenly display a low-power warning, and the remaining battery percentag
... e would instantly crash. This caused a sudden, dangerous loss of propulsion in high-speed traffic, creating severe crash risks. I primarily charged at home and rarely used DC fast charging. On May 22, 2026, while actively driving, the dashboard flashed: "Service EV system unable to restart after power off." I pulled over, and upon turning the car off, it completely bricked. The auxiliary electronics power up, but the vehicle permanently refuses to enter "READY" (Drive) mode due to a high-voltage safety lock. The vehicle was towed to an authorized Nissan dealer. The certified technician's diagnostic report confirmed that the high-voltage traction battery has completely failed and is internally damaged. Crucially, this vehicle is currently under an active, unremedied safety recall (NHTSA Recall 25V655 / Nissan R25C8) due to excessive lithium deposits that cause severe battery degradation, sudden power loss, and fire risks. The physical failure of my battery is a direct manifestation of this exact manufacturing defect. Nissan’s software-only "interim remedy" did not fix the hardware defect; it simply bricked my car after exposing my family to highway propulsion loss. Nissan must be held legally responsible for replacing these defective, recalled battery packs regardless of mileage, as they present a severe, systemic risk of catastrophic failure and traffic accidents.Read more

Attempts to Resolve I requested: trade-in/lease option, buyback assistance, rental coverage, and help shipping the vehicle to Texas after relocating. All were denied. Impact I am without a vehicle, still making payments, incurring extra transportation costs, and may have to pay to ship the car. Resolution Requested Rental reimbursement, goodwill compensation, shipping assistance, or buyback/trade-in consideration. Conclusion While not a warranty issue, the prolonged delay and lack of support reflect poor customer care. I am seeking a fair resolution.Read more

SUBJECT: Formal Safety Defect Complaint: 2016 Subaru Outback – Manufacturing Nondisclosure of Latent "Day 1" Design Flaw Causing Ignition Key Entrapment, Uncommanded Steering Column Lockout, and Severe Family Safety Risk I am writing to formally report a severe, unaddressed safety defect and a deceptive corporate practice by Subaru of America, Inc. affecting my 2016 Subaru Outback. This vehicle
... suffers from a "Day 1" latent engineering flaw that causes the physical ignition key to become permanently trapped in the ignition cylinder, triggering an uncommanded, rigid locking of the steering column assembly. Subaru has possessed documented knowledge of this failure for years, cataloging it under Technical Service Bulletin TSB 16-112-18R. Rather than fulfilling its statutory obligation to issue a comprehensive federal safety recall, Subaru deployed a voluntary, time-bound 8-year warranty extension. Subaru conveniently did not notify me of this warranty extension. By doing so, the manufacturer unconscionably ignored this defect, placing its customers at direct peril. They capitalized on asymmetric information to shift both the financial burden and the severe physical safety risks of a known factory flaw onto my family, the moment an arbitrary time threshold expired. My family and I have been at active, continuous risk in this vehicle since the day we purchased it due to an engineering error that was present when the car rolled off the assembly line. The root cause of this failure is a fundamental engineering defect within the Continuously Variable Transmission (CVT) shifter assembly, specifically, the premature degradation of the electrical contacts within the select lever park-range switch.Read more

Safety risk: Active Electronic Stability co
... ntrol (ESC) faults and "service start-stop" dashboard warnings are present. This defect causes severe voltage fluctuations and sudden heavy steering loss while driving, creating an immediate crash hazard and spontaneous under hood fire risk while parked. Dealer history & warranty abuse: certified jeep dealerships have repeatedly failed to fix the root cause. Elm Grove Jeep (Wheeling WV) misdiagnosed the system and repeatedly billed my third-party warranty for thousands of dollars to fix cascading mechanical damage instead of the underlying defect. Parts replaced is 4 consecutive sets of rear shocks, front shocks, a steering clock spring and a drag link pitman arm boot. when elm grove failed to resolve it Stellantis corporate instructed me to take it to another dealer for a star case to be opened there. I took it to Steubenville Jeep (Ohio) with an active ASC light and failed rear shocks and was told "couldn't replicate" the issue and dismissed it. A few weeks ago, the service "start-stop" safety light triggered. Stellantis had corporate knowledge of this ongoing defect for months via prior state Attorney General case but refused to cooperate and waited knowing AG wouldn't keep it open past a certain number of days and replied a day outside of the window. They are leaving an active fire hazard on the road, omitting my VIN from the database rollout, and depleting my personal warranty's financial limits. The camera has been replaced by the local Ram dealer (Mullahey Jeep/Dodge/Ram in Paso Robles, CA) at least once under warranty, but the problem recurred
... . It has been there at other times for the issue, but the problem had rectified itself by the time I got it there. After researching this on the internet the problem appears to be in the backup camera wiring harness in the tailgate, possibly in the electrical connectors. Many people complain about the issue on various forums, and there are quite a few videos on You Tube with homegrown fixes which I have tried, but they are a temporary at best. The best temporary remedy I have found is to open and then slam the tailgate shut. The problem appears to be quite common. I informed the dealer about the wiring harness possibly being the source of the problem the last time I brought it in a few years ago, but they seemed to ignore my advice. I know that there have been safety recalls of RAM pickups for backup camera issues from model years 2019 onwards, so why not other affected years? The problem most recently recurred today as I was backing out of my driveway. It has occurred numerous times over the years. RAM appears to have no permanent fix for this safety hazard. Luckily, I have not yet had an accident, but no one should have to wait for an accident, injury, or a fatality to order a recall. I hope my complaint does not fall on deaf ears as it has with RAM. My BMW's Head-Up Display (HUD) and Speed Limit Information system displays incorrect speed limits that do not match posted roadway signs. The HUD frequently shows "LTM" or an incorrect speed such as "25 mph" instead of the actual posted speed limit. This is a serious safety concern. Drivers rely on HUD speed limit information while driving. Inaccurate readings can lead to unsafe driving decisions,
... unintentional speeding, traffic violations, and increased accident risk. The vehicle was taken to an authorized BMW dealership for repair and kept for approximately one week. I was informed on June 4, 2026, and again on June 8, 2026, that the issue had been resolved. After each attempt, the defect remained completely unchanged. The dealership repeatedly misrepresented the repair as complete when it was not. I previously owned a BMW X5 for three years with no HUD or speed limit display issues, confirming this is a defect and not normal system behavior. The vehicle was purchased new in November 2025 and remains under warranty. Despite multiple failed repair attempts, the defect persists and continues to compromise safe vehicle operation.
