Purchased a 2020 Cadillac Escalade Suburban about three months ago from GMC dealership at 8155 Interstate 35, San Antonio, Texas 78224. This vehicle had about 50,000 miles when we bought it. We were not driving the Escalade everyday, we use it occasionally and about a month after we purchased it the battery went dead. Before the battery went completely dead we were on the road one day and we rol
... led down the window and it would not roll back up, then the back window popped open and it would not shut back down. GMC dealership we bought it from came to our home and used a charger to recharge the battery. We took it to the dealership service department and dealership determined it was a bad battery, they gave us a new battery. On May 4, we attempted to use the Escalade and once more the battery was dead. Service person from GMC dealership came back to our home to charge the new battery but it would not start at all this time. We no longer feel safe driving this vehicle and want to turn it back in as a lemon and get our money back.Read more

What component or system failed or malfunctioned, and is it available for inspection upon request? The flexible brake hose on my 2015 Ford Edge failed/broke shortly after I purchased the vehicle. The vehicle also began experiencing engine performance issues, including a rough idle and rough driving condition after the check engine light came on. The vehicle and failed brake hose should be availabl
... e for inspection upon request. How was your safety or the safety of others put at risk? The broken flexible brake hose created a serious safety risk because it could affect the vehicle’s braking ability and increase the risk of losing control or being unable to stop safely. The rough engine performance also caused concerns about the reliability and safe operation of the vehicle while driving. Has the problem been reproduced or confirmed by a dealer or independent service center? The issue has been observed and experienced consistently while driving the vehicle. The brake hose damage and rough engine performance were noticeable after the vehicle was purchased. (If a mechanic has looked at it, you can add: “An independent mechanic/dealership confirmed the brake hose was damaged.”) Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? I have taken it to the dealership and they said nothing about it but I got it diagnosed at autozone and the code came up right away. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Approximately 2–3 weeks after purchasing the vehicle, the check engine light came on and the vehicle began running roughly. Around the same time, issues related to the broken flexible brake hose were discovered. Symptoms included rough driving performance and concerns with the braking systemRead more

Intermittently I will receive a dashboard message reading “shift to park” after I park the vehicle and do shift to park. This is a known issue with this vehicle across several years identified as “a known issue caused by a faulty switch in the shifter assembly failing to detect when the car is in Park”. GM has identified and found a solution to this problem by replacing the entire shifter

My 2019 Dodge Durango R/T began experiencing drivetrain and warning system issues after collision/body repairs were completed by a repair facility. The original repair work involved suspension/body-related repairs and did not originally include documented transfer case or internal drivetrain damage. At the time I picked the vehicle up from the repair facility, the tire pressure monitoring system
... warning light was already illuminated. I notified the repair facility and was told to drive the vehicle for approximately 10 minutes and the warning would clear itself. I have video documentation related to this interaction. After the repairs, additional problems began appearing including: * Service AWD warning * Traction control warning indicators * Ongoing drivetrain concerns * Diagnosis involving transfer case/internal damage and burnt fluid The vehicle was later reported to have transfer case clutch failure and internal drivetrain-related issues despite the original repair scope not documenting drivetrain damage. My concern is that the post-repair warnings and drivetrain symptoms may indicate improper inspection, repair, reassembly, fluid servicing, calibration, or unresolved damage related to the repair process. The vehicle has been inspected by the repair facility and diagnostic information was later provided identifying drivetrain/transfer case issues.Read more

May 7, 2026 National Highway Traffic Safety Administration 1200 New Jersey Avenue SE Washington, DC 20590 RE: Safety Defect Complaint – Brake System / Vehicle Control Concerns 2020 Chevrolet Malibu Dear NHTSA, I am submitting a safety complaint regarding persistent braking and vehicle control issues involving my 2020 Chevrolet Malibu. Prior to December 31, 2025, the vehicle exhibited jerk
... ing during braking, all dashboard gauges go to the left, warning lights, reduced power (20-25mph), and inconsistent braking behavior. This situation happens without warning, at any given time, even in the flow of traffic. An independent inspection indicated the brake system was in proper condition, suggesting a more complex underlying safety issue. The vehicle was taken to Reed-Lallier Chevrolet for diagnosis and remained there for approximately five months. During that time, I was not provided documentation confirming completion of a brake system diagnostic or a clear resolution path. Due to the extended delay without diagnosis, I no longer feel safe operating the vehicle. I am also concerned that prolonged inactivity without proper repair may have worsened the condition and increased risk of failure. Given the nature of the symptoms involving braking and vehicle control, I request review for potential safety-related investigation. Vehicle Information: 2020 Chevrolet Malibu VIN: Mileage:Read more

The accident occurred while I was driving south on Highway 38 at approximately 45 mph. Another driver pulled forward into my path and did not appear to see my vehicle. The impact caused significant front-end damage to

I am concerned that the airbag system, crash sensors, or related safety components may have failed to activate properly during the collision. Due to the speed of travel and force of impact, I believe the lack of airbag deployment placed my safety at risk and could have resulted in serious injury. The vehicle is available for inspection upon request. At this time, I am requesting an investigation into why the airbags did not deploy during the crash.Read more

Upon taking my 2021 Rav4 Hybrid in for the rear traction motor cable corrosion recall (25TD01) on 5/7/2026, I was told there are no parts, no estimated arrival time, and therefore not even a rough estimate for a timeframe of repair. I was told that no one would inspect or evaluate the part in question to indicate if the car was safe to drive. I expressed my extreme concern given the recall include
... s risk of power failure, even while driving. I received inconsistent, avoidant answers and an absolute denial to remedy the situation in any way. I was told the dealership is "Dolan" and not "Toyota". I again expressed my concerns about safety and the dealerships' denial to, at minimum inspect the recalled part and/or housing of the part. Emily, the Service manager stated that Toyota does not "allow" loaner vehicles for this particular recall. She could not provide any sources for this information. To summarize; the dealer with not provide an inspection of the recalled part and/or housing, or give an estimated timeline for repair given I am #125 on a waitlist. There is a complete lack of regard for mine or my children's safety as the dealer will not provide any assessment of how bad the damage could be. They suggested that I continue to drive the vehicle, acknowledging all of the safety hazards that could occur. I am terrified of the possibility of having an accident with my kids in the car due to this recalled part failing, not having a vehicle to get to and from work, and the potential additional costs that this unaddressed recall issue could cause. The issue involves the vehicle’s drivetrain and hybrid/electronic control system, where the car suddenly jerked forward in stop-and-go traffic on the freeway as if it had been struck from behind. Immediately following this event, the vehicle would not accelerate beyond approximately 40 mph, indicating a loss of normal power. This created a significant safety risk, as the sudden jerking could hav
... e led to a rear-end collision, and the inability to accelerate properly made it difficult to keep up with traffic on the freeway. I was barely able to safely pull off the freeway while surrounding traffic continued at higher speeds, increasing the risk to myself and other drivers. The problem appeared to resolve after turning the vehicle off and then back on; however, this raises concern about intermittent failure. The vehicle was taken to a dealership for inspection, but they were unable to reproduce or identify the issue. The vehicle remains available for inspection upon request. The vehicle has been inspected by a dealer but not by the manufacturer, police, or insurance representatives. Prior to the failure, there were no warning lights, messages, or noticeable symptoms indicating an issue. Based on similar reported experiences, this event may be consistent with a “limp mode” condition or a malfunction in the transition between the electric motor and gasoline engine, potentially involving the transmission or hybrid control system.Read more

The vehicle experienced hard steering and intermittent loss of power steering assist while driving. The failed component was diagnosed by an independent repair facility as the electric power steering rack and pinion assembly and steering module. The repair shop replaced the rack and pinion assembly, reset steering codes, performed a wheel alignment, and road tested the vehicle. The steering fai

The condition was confirmed by an independent repair shop. The manufacturer has not inspected the vehicle. The removed component may no longer be available for inspection because the repair has already been completed. Prior to the repair, the vehicle experienced intermittent hard steering and/or loss of steering assist. No specific warning lamps or messages are recalled at this time. This vehicle is a 2013 Ram 1500 and experienced symptoms similar to known electric power steering issues affecting similar Ram 1500 vehicles, although this VIN was not included in the recall population.Read more

2019 Subaru Impreza has experienced intermittent and now persistent failures involving the factory EyeSight driver assistance system. Prior to complete failure, the EyeSight system would randomly disable and re-enable while driving. Adaptive cruise control and related safety features became unreliable and would intermittently stop functioning. The issue worsened over time and eventually resulted
... in repeated “EyeSight Off” warnings and multiple system fault messages. Vehicle diagnostic scans showed repeated U1235 “Lost communication with EyeSight” faults across multiple modules, including ABS, steering, transmission, and instrument cluster systems. Additional communication and airbag indicator-related faults were also present. The windshield was recently replaced due to a crack located near the EyeSight camera area. Subaru service personnel stated that EyeSight systems are extremely sensitive to windshield specifications and calibration requirements, and that aftermarket windshields may prevent proper operation or calibration of the system. Subaru also indicated that EyeSight calibration equipment is highly specialized and expensive, and that repair costs may not be practical relative to the vehicle value. Despite attempts to reset the vehicle systems and inspect the issue, the EyeSight system continues to malfunction and remains disabled. Because EyeSight controls important driver assistance and collision-avoidance features including adaptive cruise control, forward collision warning, lane departure warning, and automatic emergency braking, this issue creates significant safety concerns. The intermittent nature of the failure, repeated communication faults, and inability to reliably restore system operation raise concerns regarding the long-term reliability and serviceability of Subaru EyeSight systems.Read more

On April 17, 2026, my 2026 Mercedes-AMG GT Coupe experienced a sudden and complete electrical failure while operating on a public roadway in Texas. Police officers responded to protect the vehicle and occupants due to the dangerous roadway location. Due to the low ride height/body configuration of the vehicle, a specialized tow truck was required, causing the disabled vehicle to remain exposed in traffic for more than five hours. The vehicle was finally removed around 11:00 PM and transported to a Mercedes-Benz service center. Mercedes-Benz service documentation confirmed: Internal failure involving the 12V battery system Consequential overload/failure of the 48V system Multiple diagnostic trouble codes Vehicle shutdown conditions The vehicle and components are available for inspection upon request. The condition was confirmed and inspected by Mercedes-Benz technicians and police officers at the scene. Prior to the total failure, dashboard warning messages and malfunction alerts had appeared. The headlights and dashboard lose power intermittently while driving at highway speeds, specifically triggered by using the turn signal. This is ongoing since January when I started having to take it for a recalls.More things kept happening. I kept having to have more repairs. Every time I went, I told them about this and it just keeps going on. Today I took a huge list of things they should check
... to see why it’s doing this. This is the following list that I gave them -2017 Kia Forte - Service Request: •Issue: Headlights/dash icons flicker specifically when using turn signals. •Issue: Average MPG keeps resetting (Computer Brownout). •Mandatory Checks: 1Multi-Function Switch (internal short). 2Manual Voltage Drop Test on the G01 Ground Point. 3Alternator Ripple Test (check for "dirty" power). Last night on my way home from my daughter’s house which is a 50 minute drive, it was in the dark pouring rain and no longer than I had got started going home, I used the turn signal and my lights flickered a little which that’s what they’ve done before. Then the icon inside the car for headlights flashed on and off, lights inside got very dim and then my headlights got so dim that I had to follow other cars that had LED headlights down an interstate at 65 mph or less safely just to get to my exit to get home for over 45 minutes. I have mentioned this to them repeatedly, but I gave this list to them this morning. I called a little bit ago just to check on things and I was told “well we’ve been driving it for over an hour and we can’t seem to get it to act up”. What component or system failed or malfunctioned, and is it available for inspection upon request? The vehicle appears to have experienced a failure or malfunction involving the electrical power system, charging system, 12 volt battery system, vehicle communication modules, or related components. Based on the recent Land Rover recall I found online, I am concerned that the DC to DC converter or re

The vehicle is currently with Jaguar Land Rover Houston Central and should be available for inspection upon request. The vehicle has not been returned to me, and the dealer has indicated that the vehicle was not communicating with their diagnostic computer and was not initially showing fault codes because the modules were not communicating. They have told me that my car is not part of the recall. How was your safety or the safety of others put at risk? My safety and the safety of others were put at risk because the SUV experienced a complete shutdown while I was driving on two separate occasions. The first event occurred Thursday afternoon at approximately 4:32 PM while I was driving on Highway 610. The vehicle suddenly lost power while in motion. I was able to glide off the highway and park on the side of the road, but this could have caused a crash or created a serious hazard because it occurred on a major roadway. The second event occurred Friday morning around 7:20 AM while I was driving on South Rice Avenue to take my daughter to school. The vehicle shut off again and left me stranded. On April 16, 2026, within minutes of being released from a certified dealership, my 2019 Mercedes-Benz GLC 300 suffered a sudden and total loss of propulsion while traveling at high speed on the Belt Parkway (NY). Despite depressing the accelerator, the vehicle failed to maintain speed and stalled in active traffic. This created an immediate life-safety hazard, forcing me to navigate across multip
... le lanes without engine power to avoid a high-speed rear-end collision. The vehicle was released to me by Mercedes-Benz of Brooklyn on April 16, 2026, following a repair for which I paid $2,134.94. The dealership diagnosed the issue as a wiring harness/sensor fault and formally certified the vehicle as "Found OK" and safe for operation. The catastrophic failure occurred only 22 minutes after I departed the dealership lot. The vehicle had to be towed back to the facility immediately. Upon the vehicle being towed back on April 16, the dealership admitted via recorded voicemail that the initial $2,134.94 repair was a misdiagnosis. They subsequently issued a new diagnosis for a "Valve Body" failure and a new quote of $6,424. The dealership's Service Director has since admitted the vehicle suffers from concurrent transmission and engine failures, rendering the car a liability. This represents a critical failure of the powertrain and electronic control systems. Releasing a vehicle that suffers a total power loss on a highway immediately after a "certified" repair is gross negligence. I am reporting this to NHTSA as a catastrophic failure of a safety-critical component (Transmission/Valve Body) that resulted in a "dead-stick" situation on a major New York thoroughfare, posing a severe risk to myself and other motorists. The vehicle remains at the dealership. Mercedes-Benz USA has offered only a $2,000 "goodwill credit," which does not address the safety defects or the initial misdiagnosis. No recall has been initiated for this batch.Read more

Dealership's resolution was to reduce the safety settings from highest to medium strength. Told did not have to sign anything, could just leave. Drive test now undocumented. --- Reached out to Mopar, was told they aren't the right department. Reached out to Stellantis/Jeep customer service, created case number, told to go to another dealership, if they can't find problem, then I could reach back out to Jeep for a more advanced repair. --- Any vehicle that offers various levels of safety features should be able to use any of the settings in order to maintain highest level of safety on the road for others and themselves. Having random alerts go on and off while on a safety setting meant for the highest protection is potentially dangerous for all.Read more

I am reporting a systemic safety fraud involving three critical points:1. INHERENT DEFECT & CRASH LINK: This vehicle has a pathological history of 6 transmission fluid changes starting from 308 miles and multiple alignment failures. There is strong suspicion of inherent defects in both the POWERTRAIN and REAR DRIVETRAIN/STEERING systems. These defects likely contributed to the previous owner's 5-s
... ide collision in May 2022. The record suggests a "Sudden Loss of Motive Power" and "Control Instability" risk existed from day one.2. RECALL MALPRACTICE (#23V-844): Despite the massive structural damage from the crash (bent hinges and frame misalignment), the dealer (Planet Honda) certified the fire-safety recall as complete in Dec 2024. Installing electrical recall parts on a compromised chassis violates federal safety standards and creates an unaddressed fire hazard.3. FRAUDULENT CPO RE-SALE: The dealer and manufacturer conspired to "launder" this known-accident vehicle back into the market via the CPO program, concealing the lethal combination of mechanical failure and structural instability from the next victim. The vehicle is currently mothballed due to these hazards. This matter is under active investigation by the (AID)Colorado Department of Revenue, Auto Industry Division (Case #[[XXX]). I petition NHTSA to audit the i-HDS diagnostic logs from the Dec 2024 recall to investigate this safety compliance fraud. 2024 Ford F-150 Platinum PowerBoost Hybrid. Diagnostic codes for power steering loss and CAN bus communication faults documented at 337 miles, well within the 3-year/36,000-mile new vehicle warranty. The vehicle has been at a Ford dealer since March 10, 2026 for symptoms including the truck randomly turning off while driving and loss of power steering communication. As of this report, the truck h
... as been at the dealer 58 days without resolution. Documented diagnostic findings include: Lost Communication with PSCM (U0131), SCP/J1850 Communications Bus Fault (U1028), permanent code P1A42 Propulsion System Status Signal Performance, hybrid-specific High Voltage System Interlock U3523-00, Steering Position Sensor P060A, no communication with Power Steering Control Module, intermittent PCM communication during self-tests, BCMC fuse 69 with no power, and circuit CBB69 with no voltage. A Ford Field Service Engineer was dispatched April 23, 2026. On April 24, 2026 the dealer located damaged wiring inside a harness behind the BCMC bracket. Prior to that, the dealer escalated through R&I of multiple modules (PSCM, PCM, BCMC, Gateway Module access) over 45 days without locating the actual cause. The vehicle is currently subject to NHTSA recall 26V104 / Ford 26C10 (Trailer Module loss of communication at startup) issued 3/18/2026, status Recall Incomplete. The recall describes the same general failure mode (module communication loss at startup) the customer has been experiencing. Power steering loss while driving is a serious safety concern, particularly given the vehicle's hybrid drivetrain and CAN-bus-dependent safety systems.Read more

On May 2, 2026, ten days after purchasing the vehicle, the driver‑side seatbelt locking mechanism failed while I was driving. The vehicle was inspected the next day at CarMax Tuscaloosa. Their mechanic confirmed two separate safety defects: 1. The driver‑side seatbelt mechanism requires replacement. 2. The motor mounts are broken/loose and require replacement. The mechanic explained that debri
... s was found inside the seatbelt mechanism, and the seatbelt needed full replacement. He also confirmed that the motor mounts were broken/loose, which can cause excessive engine movement, vibration, loss of power, or engine shifting during braking or acceleration. These conditions increase the risk of stalling or sudden shutdown, which is consistent with what occurred during my incident. The failed components are: • The driver‑side seatbelt locking mechanism • The motor mounts/engine mounting system Both components remain on the vehicle. The seatbelt failure left me unrestrained while driving. The vehicle then shifted into Park and shut off in the middle of traffic, creating a risk of being struck by other vehicles or losing control. The broken motor mounts increase the risk of engine movement, stalling, or shutdown while driving, which could lead to loss of steering control or sudden deceleration in traffic. Yes. A CarMax mechanic at the Tuscaloosa, AL location confirmed both defects: As mentioned above The vehicle was inspected by CarMax Tuscaloosa. Jaguar Birmingham declined inspection because they do not service Jaguars older than 2018. An independent Jaguar specialist declined inspection due to prior non‑payment issues with CarMax. No manufacturer, police, or insurance inspection has occurred. There were no warning lamps or messages prior to the failure. The seatbelt malfunction occurred suddenly. The engine shutdown occurred immediately after the seatbelt unlatched while braking. The vehicle remains unsafe to drive due to the unreliable seatbelt, broken motor mounts.Read more

The contact owns a 2014 Ford Escape. The contact stated that the vehicle failed to start after multiple attempts without warning. The contact attempted to jumpstart the vehicle using multiple methods but to no avail. Due to the failure, the vehicle was towed to a dealer where they discovered that the insulation on the wire that runs from a gearbox in the back of the vehicle to the front driver sea
... t had melted at various parts of the wire. The contact was informed that the wiring harness needed to be replaced; however, the wiring harness was no longer in production. The contact with the help of a friend had the vehicle towed back to his home where the contact replaced the wire himself. Despite the repair, the failure persisted and the vehicle was towed to another dealer where they inspected the contact's work where they discovered that a fuse was not fully connected; once the dealer fully connected the fuse, the vehicle resumed normal operation. The manufacturer was notified of the failure and was informed that there were no recalls on the vehicle. The vehicle was repaired. The failure mileage was 97,544.Read more

The vehicle began experiencing repeated Autopilot/FSD failures while driving. Symptoms included sudden Autopilot disengagements, cruise control becoming unavailable, and loss of road visualization/navigation display functionality. The issue progressively worsened over time. The vehicle was first brought to Tesla service in 2025 for the same overheating-related concern. At that time, Tesla identi
... fied a coolant-related issue, topped off the coolant system, and returned the vehicle to service. However, the problem continued and gradually became more frequent and severe afterward. During the most recent service visit, Tesla diagnosed the HW3 Autopilot computer as overheating due to a heat transfer failure between the Autopilot board and cooling cold plate. Tesla stated that heat generated by the board could no longer be efficiently transferred to the coolant system, causing the Autopilot computer to overheat. Tesla further advised that continued operation without repair could eventually impact additional vehicle functions, including the center display screen shutting off due to overheating-related failures. Given the loss of driver-assistance functionality and potential impact to core vehicle systems, I am submitting this report so the issue is documented as a potential thermal management or hardware reliability concern involving the HW3 Autopilot computer system.Read more
