The central locking / electronic door lock system on my 2019 Audi A6 intermittently malfunctions, sometimes with a long beep sound and an error message appearing on the screen in front of my steering wheel. I attempted to enter the vehicle quickly for safety, but the central locking system malfunctioned. As I tried to unlock and open the door, the vehicle appeared to lock again and the door would not open. I was temporarily locked out of the vehicle during a potentially unsafe situation. The door eventually opened only after I pushed the door inward with force, locked and unlocked the vehicle again, and tried the handle again. This created a very concerning safety risk because I could not immediately access my vehicle when I needed to get inside quickly as the individual approached me. The problem has not yet been fully reproduced or confirmed by a dealer to my knowledge. I previously mentioned the central locking issue during service and asked whether there was a recall. I was told there was no recall and that a lock replacement would cost approximately $850. The issue I have been experiencing appears consistent with Audi Technical Service Bulletin XXX/5, which applies to 2019 to 2024 Audi A6 models and describes central locking / door lock concerns and related fault codes. Because the defect is intermittent, it may be difficult to reproduce, but it creates a safety concern because the driver may be unable to access the vehicle when needed. Cabin temperatures rapidly became dangerous for an infant. My husband was eventually able to remotely unlock the vehicle using the Tesla app, narrowly avoiding forced entry by emergency responders. The vehicle failed to detect the phone key inside the cabin and improperly initiated locking while a vulnerable child occupant was present. This created a serious risk of heat injury or death. The vehicle also failed to prevent the lockout despite an infant occupant being seated in the second row, raising concerns about whether rear-seat occupancy detection and child safety protections functioned properly during the event. The selling Chevrolet dealer replaced the K212 pushbutton circuit board and reprogrammed the HPCM2 module. Second failure occurred at approximately 54,969 miles in March 2026, only about 9,300 miles after the first repair. Same diagnostic code family, including the identical U18D2 code, plus U0076, U18C6, and U240E. The same dealer replaced the same K212 pushbutton circuit board a second time. The repair was processed under manufacturer warranty. Safety risk: when the failure occurs, communication with the transmission range selector is lost intermittently and without warning, leaving the vehicle unable to be reliably shifted into Drive, Reverse, or Park. This creates a clear hazard in traffic, parking lots, and on inclines. The failure mode is sudden and the warning lamps appear only after the malfunction has begun. The component is located beneath the center cup holder area. There is no visible waterproofing or moisture isolation around the circuit board, despite its location directly under a known liquid exposure zone. The date of repair for both items was April 15, 2026, as I was unable to make time for an immediate appointment. I received the car back on May 5, 2026, and noticed the front collision error light was turned on, the parking collision mode worked only intermittently, and I had difficulty one time the next day with my door opening (was stuck partially latched/open and my gas cap would not open). The door and gas cap worked after the incident, but, due to both repair shops' lack of quality work, I'm skeptical that the initial problem was remediated. I would like to request both the Fix Auto Downtown Los Angeles and Yorba Linda locations be investigated for returning my vehicle in an unsafe condition. I have previously taken the vehicle to Kia for recalls, including recalls related to engine issues. Based on my research and the symptoms of my vehicle, I believe the recall involving the piston oil rings may be related to the camshaft and crankshaft issues now occurring. I am concerned this may represent an additional safety-related defect or recall issue that has not yet been fully recognized by Kia. ... come glossy, crack and have broken off into pieces and the shine/gloss from the dashboard causes blinding while driving due to the reflection of the sun on the dashboard. These issues have been going on for a period of over 10 years and I had multiple accidents due to these numerous issues. I have reported these to Lexus corporate on multiple occasions, including in 2016 and 2017. However, Lexus has refused to fix them stating that they are aware of these issues and had the ZLJ, ZLD and ZLA warranty programs but that they had expired in 2017. However, I later found out that none of those programs had expired at the time I called Lexus's corporate office. One of their programs, the ZLD warranty program, was apparently extended until 2021, however, Lexus failed to inform most vehicle owners of this. These issues are a safety issue. They have caused my car to malfunction for over 10 years and to have accidents. Many Lexus owners have also reported about these issues in Lexus owner forums on the internet and have stated that they have had accidents due to these issues, but Lexus failed to properly inform customers of the extended warranty and refused to fix these cars. With these issues continuing and becoming worse over the years, they are no longer a warranty issue, but a safety issue for drivers. My 2021 Tesla Model Y HW3 experienced complete failure of all cameras, GPS, Automatic Emergency Braking, and Adaptive Cruise Control following the installation of software update 2026.8.6.1 — the OTA remedy update issued by Tesla for NHTSA Recall 26V283000. ... ervice advisor confirmed in writing: "During the update, the computer failed to reboot properly. We attempted to perform a 12V reset but unfortunately it is still stuck in the boot loop state." The Autopilot ECU (Primary/Turbo A) is stuck in a permanent boot loop and cannot be recovered by software means. Safety impact: All cameras including rearview camera are completely non-functional. Automatic Emergency Braking and all ADAS features are disabled. The vehicle cannot safely detect obstacles or pedestrians. GPS navigation is frozen. Tesla has confirmed the computer requires replacement but is classifying it as a customer-pay repair citing expired warranty, despite the failure occurring during Tesla's own recall remedy software deployment. This vehicle meets every technical criterion of Recall 26V283000 — HW3 hardware, 2021 Model Y, software version 2026.8.6.1, complete camera failure — but does not appear in the recall VIN database.
