As I was driving down the interstate doing about 75 miles per hour the car lost all power without a warning. It was at night in the middle of no where. I almost had a bad accident but I was able to veer off the road before the car brakes locked. I was unable to use safety flashers.

We saw an opening in between the stopped vehicles and crossed over from right to left lane into the grassy median, hit a ditch, and did a sharp turn into opposing traffic, almost hitting another vehicle as they moved out of way, and stopped the vehicle on the opposite side of the road. When the vehicle hit the ditch crossing the median, the right front tire popped and helped in stopping the vehicle. Four family members including myself were in the vehicle and there were several cars already stopped in the lanes. This incident caused great safety concerns for my family and the vehicles around us. Had we hit the cars while crossing over, it would have caused a chain reaction of "hits" as the cars were already lined up and stopped. We brought the car to the Jeep dealership and they did confirm that the internal valve body failure and TCM fault contributed to improper communication within the transmission. They did replace the TCM and valve body assembly, reprogrammed the module and road tested the vehicle. The auto stop/start warning light came on after the incident. They performed a battery test for the main and auxiliary, and both failed load test and read at low voltages. Both batteries were replaced and fault codes cleared. Upon receiving the vehicle, I tested the cruise control component and it has been working for the past three months now.Read more

This is a very known issue that has been reoccurring with GMC Acadia. The torque converter slipping. That cost alone is over $7.000 for the transmission. I am still making monthly payments for my 2021 Acadia. With rising cost above the board GMC could have resolved this issue before releasing this vehicle to the public for purchase. After doing some grocery shopping for about 20–30 minutes, I returned to my vehicle, put it in drive, pressed the accelerator, and received no response for approximately 3 to 4 seconds. On Monday, May 11, I went to the Carvana Chrysler Dodge Jeep Ram location in [XXX] , where I was informed by the Service Manager that they would likely need to keep the vehicle for over a month to troubleshoot the issue with Chrysler before they could attempt a resolution, since the entire electrical system is effectively locked down and controlled by FCA electrical engineers. The contact was informed that the repair was not covered under the recall. In addition, the contact was informed that the unknown recall only covered the seal. The contact was informed that the recall did not cover the replacement of the gateway control modular. The manufacturer was made aware of the failure and a case was filed. The recall notice states that the battery cells can develop excessive lithium deposits, which increases electrical resistance and can cause rapid battery heating during Level 3 fast charging. Nissan states it will install software that monitors for “state-of-charge fluctuation” and, if detected, will prevent the vehicle from restarting or recharging in order to avoid a thermal incident. This means the defect inside the battery cells is still present, and the car may disable itself if the defect begins to appear. The recall materials also state there is no warning before overheating occurs. This creates multiple safety concerns: • The underlying battery defect remains uncorrected. • The vehicle can become immobilized (unable to restart or recharge) if the software detects the condition. • Loss of Level 3 charging capability affects the ability to travel safely or plan charging when needed. • A battery fire risk exists during Level 3 charging if the defect is not detected in time. Nissan’s documents state that the software is meant only to prevent the “progression” of a thermal incident, not to repair the defective battery. I am concerned that my vehicle contains a known hazardous defect that has not been physically repaired and that the software-only response is inadequate to ensure safety. I am filing this complaint so NHTSA is aware that the remedy being offered does not address the internal battery defect itself and may leave owners exposed to continued safety and reliability risks.Read more

I immediately attempted to apply the brakes in order to avoid a collision, but I was unable to stop the vehicle in time, resulting in a minor impact with the vehicle in front of me. I believe there may have been a mechanical or electronic malfunction related to the vehicle’s acceleration, braking, or automatic assistance system. I cannot identify with certainty the exact component that failed; therefore, the specific cause is UNKNOWN at this time. The vehicle remains available for inspection upon request. The situation placed both my safety and the safety of the other driver at risk, as the vehicle moved unexpectedly despite previously being stopped with the brake hold system engaged. At this time, the issue has not been reproduced or confirmed by a dealership or independent repair shop. I am also not aware of any technical inspection performed by the manufacturer. However, the incident was reported to the appropriate insurance company. Before the incident, I did not observe any warning lights, dashboard messages, or obvious signs of a mechanical or electronic problem. The sudden acceleration occurred unexpectedly and without prior warning. The collision was minor. The airbags did not deploy, and neither I nor the other driver suffered physical injuries. However, both vehicles sustained property damage, especially my vehicle, which suffered visible front-end damage because several exterior components are made of low-resistance plastic material.Read more

The disconnection occurred during front-end service work at that visit, which included front fascia inspection and removal. The technician did not reconnect the PWS speaker before returning the vehi

The disconnection was not discovered until the second service visit (Invoice XXX, Tesla Palo Alto, 9/18/2025), at which point Tesla's own invoice documented: "The issue was caused by a disconnected front pedestrian warning speaker. Technician reconnected the front pedestrian warning speaker to resolve the issue." The [XXX] s therefore operated with a disconnected PWS for approximately 4 weeks — from August 22 to September 18, 2025 — without my knowledge. During this entire period, the vehicle was not emitting the required audible low-speed alert. The PWS emits an audible alert at low speeds to warn pedestrians, cyclists, and visually impaired individuals of the vehicle's approach. This system is required under FMVSS 141 (Minimum Sound Requirements for Hybrid and Electric Vehicles). This is a service-inflicted deactivation of a federally-required pedestrian safety system. The deactivation was caused by Tesla's Palo Alto service center (4180 El Camino Real, Palo Alto, CA 94306) during routine warranty repair work. On one occasion, when I attempted to drive forward, the vehicle moved backward instead. The vehicle displays repeated drive system and engine warnings and becomes unreliable during operation. I have children and regularly transport them in this vehicle, and I do not believe this vehicle is safe to drive. The vehicle is still under factory warranty, but I have been unable to obtain timely warranty service. The Volkswagen dealership in Jackson, Mississippi advised they have severe staffing limitations and limited diagnostic availability, and I was told I need to take the vehicle to another state for service. I was also told by Volkswagen customer service that towing assistance may be available under warranty, but when connected with towing assistance I was advised coverage was only for a very limited distance 6 miles! I was quoted approximately $1,900 to tow the vehicle to the available Volkswagen service location in Louisiana. Because the vehicle is not safe or reliable to drive, I have also been forced to pay out of pocket for a rental vehicle while attempting to obtain warranty repairs. VW refused to help me with a loaner or rental. ... ver, it failed at the turn lane while waiting for the light to turn. We turned it back on, bought a new battery, had it installed. On 12/29/25, my husband was driving. He turned into the intersection with a traffic light. As he was making the turn, the car simply quit. No warning. The car was dead, and blocking traffic! He pressed the start button to turn off the car, then again to turn it on. He was able to complete the turn and parked on the side of the road. By this time it had a "check engine" light on. He then drove slowly the few blocks back to our house. On 12/30/2025, the car was towed to a local mechanic. He said it threw the code PODE7, "almost certainly for a failed High Voltage battery." It was still under warranty, and we had the car towed to the dealership. After keeping it for 2 or 3 weeks, they confirmed the code and that the control panel needed to be replaced. We finally picked up the car (w new control panel) on 2/20/2026. In the meanwhile, we looked up Hyundai control panel repairs on a reddit forum. We found lots of chatter about it. Turns out it's a recognized problem in Sonata and Ioniq EV and PHEV's! To the Office of Customer Relations and Vehicle Safety: I am writing to formally document a serious safety concern regarding my 2000 BMW 328 Ci, which is currently subject to the second airbag recall in this vehicle's history (Takata NADI recall 21V-401). The vehicle is exhibiting diagnostic fault codes in the DME (Digital Motor Electronics) that indicate possible electronic interference with the engine control and safety systems.
Diagnostic Evidence — May 7, 2026 On May 7, 2026, a professional BMW diagnostic scan (iSCAN S/N 70EB2) revealed the following active fault codes in the DME: 1. P1622 (MAP cooling activation fault) — Present. This code indicates a communication fault between the DME and the EWS (immobilizer) system. In my research, this code is consistent with RF interference on the DME/EWS communication line.
2. P0304 (misfire cylinder 4) — Present and continuous. The misfire on cylinder 4 is persistent and does not respond to component replacement.
3. P0301 (misfire cylinder 1) — Present and intermittent. The misfire on cylinder 1 is also persistent but less frequent.
Component Swapping Test — Critical Finding The technician performed a component swapping test: * Spark plugs and ignition coils from cylinder 1 were moved to cylinder 3
* Spark plugs and ignition coils from cylinder 4 were moved to cylinder 6
* Result: The misfires remained on cylinders 1 and 4. They did not move to cylinders 3 or 6.

When we searched online, it is clear this is a common problem with this car, but there have been no recalls or TSB issued. This is a safety issue because we need to disconnect the battery and leave the car unlocked when we leave the vehicle. This has not been inspected by any service center. Based on our research, Acura will recommend replacing the smart power control unit at a cost of up to $1500 or more. We are hoping that others have reported this as well.Read more

... o realize that I was rolling towards my opening garage door so I slammed on the brakes before any damage occurred. This was one of two scenarios where I experienced this issue. Another time I experienced the issue happened when I pulled into a supermarket parking lot. I clicked the Park brake and proceeded to undo my seatbelt and exited my ID4. I felt the vehicle slowly roll away but then I heard the parking brake engage (it was the ID4's safety feature to engage the parking brake when it detects that the driver has left the seat). Thankfully the ID4 had barely touched the curb so no damage happened. These incidents cause me to now click the parking brake multiple times to ensure that the vehicle has actually engaged the parking brake. I believe this might be a button issue that happens once in a blue moon because every time I try to recreate the scenario by clicking the button lightly, it had always properly engaged. I will inform my local dealer the next time this vehicle is due for maintenance but until this issue is fixed, I will spam the park button.Read more

... . The contact sustained pain to the abdomen for a couple days and sought medical treatment at an urgent care facility. The contact was informed by a doctor that she had suffered a hematoma to her abdomen and was informed that the soreness would last approximately 4-6 weeks. The contact initially reached out to her auto insurance who then referred her to a dealer or the manufacturer. The vehicle was towed to a dealer where she was informed that there were no recalls and was given an estimate for the repair. The contact then reached out to an independent mechanic who linked the contact's failure to NHTSA Campaign Number: 06V459000 (Electrical System). The contact then brought a print out of the recall campaign to another dealer where she was informed that the vehicle was not included in the recall. The manufacturer was not notified of the failure. These failures impact both brake light operation and cruise control functionality. The safety implications are extremely serious. Motorcyclists may unknowingly operate their motorcycles without functioning brake lights, significantly increasing the risk of rear-end collisions. Owners may be completely unaware their brake lights are not functioning. In my case, I only discovered the defect because the cruise control stopped working. Had that not occurred, I may have continued riding without realizing my brake light system had failed. I also found reports indicating that other owners filed complaints with NHTSA and were subsequently contacted regarding dealership repairs facilitated through KTM. This suggests the issue may already be known within the manufacturer and regulatory system. Given the repeated reports across multiple KTM models and years, I respectfully request that NHTSA investigate this defect and issue a safety recall . We contacted GM customer relations and after speaking with them multiple times they approved a partial adjustment on the bill for approximately $150. I explained that according to their campaign notice and ongoing issues with this component we wanted a full refund and would not accept a partial payment of 1/3 our cost. When I called the dealer to address this, the representative told me they are “not responsible for the car after this long.” The defect existed over a year before I even purchased the vehicle.
