Vehicle Safety Defect Report I am submitting this vehicle safety defect report regarding a serious and dangerous failure in a 2021 Nissan Rogue. The vehicle experienced complete electronic shutdowns while driving on the highway. During the most severe incidents, on December 19, 2025 and again on January 14, 2026, the vehicle activated the automatic emergency braking system without external cause,
... shifted into PARK, and shut down completely, leaving the vehicle immobilized in live interstate traffic during heavy rain. The dashboard displayed multiple simultaneous warnings (engine, battery, collision sensors). A repair facility later diagnosed corrosion in the Transmission Control Module (TCM). The module was not replaced, only cleaned, and the failure recurred. This defect presents an extreme risk of collision, serious injury, or death. A vehicle that shuts down and electronically locks while in motion is unsafe to operate. I respectfully request that NHTSA investigate this electronic defect affecting critical safety and control systems. Videos, dashboard images, repair records, and documented evidence are attached. While driving on an interstate highway during heavy rain, my 2021 Nissan Rogue experienced a complete electronic shutdown. Multiple warning lights appeared simultaneously, the automatic emergency braking system activated without cause, and the vehicle suddenly shifted into PARK and shut off, leaving me immobilized in live traffic. This occurred on December 19, 2025, and the same failure happened again on January 14, 2026. The vehicle could not be moved and required towing. A repair facility diagnosed corrosion in the Transmission Control Module (TCM). The module was only cleaned, not replaced, and the failure recurred. This defect creates a severe risk of collision, serious injury, or death. I narrowly avoided being struck by a truck. I am no longer able to safely operate this vehicle.Read more

Smoke set off smoke dector in garrage has a burning smell not plastic like metel and the car has a odor like nail polish on the inside. I am writing to provide an urgent update regarding my Jeep. Per our previous discussions, we had intended to coordinate a service visit to the Concord dealership. However, as of today, Jan 27 2026 at approximately 2:29pm, the vehicle has experienced a catastrophi
... c failure and is no longer safe to operate or store in an enclosed structure. The vehicle currently presents a severe fire hazard. While attempting to move it today, the following occurred: Smoke and Melting Odor: The vehicle began emitting a strong odor of melting components. This was severe enough to trigger the heat and smoke detectors in my garage. Erratic Electrical Behavior: Even with the ignition in the "Off" position and the charging cable disconnected, the vehicle continues to make loud clicking and internal cycling noises. Total System Failure: The dashboard displayed multiple critical errors, including “Unable to detect key fob,” “Service electric charging system,” and “Hybrid System Failure.” Loss of Motive Power: The vehicle entered "Turtle Mode" and repeatedly stalled, rendering it undrivable. Immediate Requirements: Given that this vehicle has already undergone three unsuccessful repair attempts and now poses a direct physical threat to my home and safety, I am requesting immediate instructions on how Chrysler intends to safely remove this vehicle from my property. I have documented the alarms, the error messages, and the sounds via video for your engineering team to review. Because of the fire risk, I will not attempt to start, charge, or drive the vehicle to the Concord location (85 miles away). Please confirm receipt of this safety notification immediately and provide a plan for a tow and a final resolution regarding a buyback, as this vehicle is clearly non-conforming and hazardous. Will not let me upload videos and picturesRead more

I had the ICCU unit recall taken care of on my car. While driving yesterday evening, I had a complete power failure. There were warning lights on the dashboard so I slowed down until it finally totally stopped. The warning lights said check electrical system and a yellow turtle light came on. While waiting for the tow truck all electric stopped including the flashers. It was dark, during rush hour
... and 10 degrees outside. I was left stranded, blocking traffic and with no heat. People were honking their horns and fighting to get around me. The street became one lane. It was very dangerous for me, I was a sitting duck. It also became dangerous for pedestrians trying to walk while cars squeezed around my car. Again, all of this happened in the dark in 10 degree weather. I waited all day to get a diagnosis from the dealer but they were too busy. I was not given a loaner car and have been stranded at home and lost a day of work. I had to have my car towed home last night and waited for a tow again to take it to the dealership. I may lose another day of work tomorrow.Read more

The automatic transmission shift cable, has malfunctioned. The center piece that connects to the gear shift has melted, and prevents the car from starting and shifting gears. The damage was created due to high temperatures from the motor, being that it is very close to it. This should be replaced by the manufacturer.

At least two separate wiring harnesses have failed on my vehicle, affecting the tail lights. My vehicle cannot pass New York State safety inspection because of this issue. Safety of myself and other drivers is at risk, especially at night, since my vehicle is not properly illuminated. Occasionally, one tail light will go out or flicker. I had one trailer harness removed under a safety recall, but
... I don't recall the dealership ever replacing it. Then, a couple of years later, I got a ticket for a tail light being out. Paid the dealership to replace a (possibly second) trailer wiring harness, but they got the wrong one. When they finally got the "right" one, another (possible 3rd), much more expensive harness was found to be shorted due to corrosion, prompting me to look into trading the vehicle in instead of repairing it. All harness issues in question are related to corrosion from water infiltration. The technician working on my vehicle told me that this is a common issue with Santa Cruzes and Tucson's, but only certain vehicles have been recalled (of course, mine was not on the list). My vehicle is only 3 years old. This is all after having the dealership replace a harness under the seat that was causing an issue with the audio, including safety notifications, that started within weeks of owning the vehicle. No warning lights, messages faults, or other symptoms indicated there was an issue. Self diagnostics, as accessed via Hyundai's BlueLink app, has no record of issues since the first wiring harness issues I had when I first bought the vehicle (see below). I have already filed a separate complaint regarding the harness under the seat. This is a new complaint regarding wiring harnesses, unrelated to the first underseat harness complaint.Read more

My 2023 Jeep Grand Cherokee Plug-In Hybrid Electric Vehicle is subject to multiple NHTSA safety recalls involving loss of propulsion and vehicle fire risk. These defects have created ongoing safety hazards and extended loss of use. Safety Recall 73C (NHTSA 25V-576) addresses a Hybrid Control Processor software defect that may cause sudden and unexpected loss of drive power without warning, increa
... sing the risk of a crash. Safety Recall 78C (NHTSA 25V-766) involves potential contamination of the 2.0L engine from the casting process, which may result in catastrophic engine failure, vehicle fire, and unrecoverable loss of drive power. For an extended period, no remedy was available. Safety Recall 68C (NHTSA 25V-741) involves a High Voltage Battery defect that may lead to a vehicle fire. FCA instructed owners not to recharge the vehicle and to park away from structures or other vehicles. I have been unable to safely charge the vehicle for over six months and have been restricted from using my assigned covered parking due to fire risk. Despite multiple dealership visits for recall-related service, the vehicle has not been fully restored to safe, normal operation. No loaner or substitute transportation has been provided during safety-related repairs. These conditions significantly impair the safety, use, and value of the vehicle and create ongoing risk to occupants and surrounding property. I am submitting this complaint to document the safety issues and extended lack of an effective remedy.Read more

ICCU failure after initial recall
Incident date: Dec 18, 2025
My vehicle went in for recall 73B at the end of August 2025. Every since the recall it has been back to the dealer for multiple check engine lights, service hybrid system, abs system, battery heater fail, blind spot warning temporary unavailable, service shifter(would not go into gear), parking brake, stability control light, partial loss of power steering while driving, service electronic throttl
... e control, and service airbag messages. The van has been to the dealership 4 times for these messages. They have replaced components, tried to find loose wires and connectors, replaced the 12V battery, and other fixes. The van will drive normally for a few weeks, and then they come back. On the latest visit (4th since 73B recall) it's been at the dealership for 31 days and counting. The dealership is waiting on Chrysler to figure out a fix for the van. The vehicle has over 50 stored messages. This recall was supposed to monitor the hybrid battery for early indications for a fire, but according to the original recall all batteries are assumed to have the manufacturing defect that leads to spontaneous fires. This van is unsafe, and we never know when it will work correctly. The van operated normally before the recall software was installed.Read more

On Friday evening, my 2025 Hyundai Ioniq 5 was parked at a ferry terminal in central NJ in cold (~32°F), windy conditions near water. At approximately 6:20pm, I remotely activated climate control using the MyHyundai app. When I entered the vehicle around 6:25pm and attempted to start it, the dashboard immediately displayed “Stop vehicle and check power supply” and “Check electrical vehicle
... system.” The MyHyundai app reported diagnostic code PIA9096, which is widely reported by other owners as an ICCU failure. The vehicle became completely inoperable, could not be shifted into neutral, and could not be jump-started. I attempted boosting the 12V battery and AAA also attempted, with no success. Both AAA and Hyundai Roadside Assistance refused to tow the vehicle because it could not be placed in neutral, leaving me stranded in freezing conditions and forced to abandon the vehicle overnight in a public lot. The following morning, I installed a brand new 12V battery, which allowed the vehicle to power on, but the same electrical errors remained. The vehicle was then towed to a Hyundai dealer. After several days, the dealer confirmed diagnostic codes PIA961C, PIA9096, and P056216 and diagnosed failure of the ICCU and high-voltage fuse. The dealer ordered ICCU assembly 36400-1XAD5 and high voltage fuse 375F2-GI040. The dealer cannot provide any ETA for parts and stated the vehicle will be inoperable until repaired. I am informed by many other owners that multi-week or multi-month delays are common for this same failure nationwide. This failure resulted in a sudden, total loss of vehicle function, left me stranded in unsafe conditions, and made the vehicle impossible to move or tow normally. This appears to be a known, systemic defect affecting Hyundai EVs across multiple model years. Vehicle is currently at Hyundai dealer and available for inspection.Read more

Component/System Involved: Engine / Powertrain and Vehicle Electronic Control Systems, including Serial Data Gateway Module Description of Failure: While driving at highway speed (approximately 60 mph), the vehicle experienced a sudden and complete loss of propulsion without warning. Engine power was lost while traveling on an interstate, forcing me to pull onto the shoulder without propulsion. T
... he loss-of-power event occurred more than once during the same trip. The vehicle was towed to an authorized Cadillac dealership for inspection. Dealer diagnosis confirmed internal mechanical engine failure, including metal debris present in the engine oil, requiring full engine replacement. During diagnostic procedures, the dealership also identified electrical communication faults involving the serial data gateway module, including voltage fluctuation and wiring chafing within the vehicle harness. These faults caused loss-of-communication conditions between control modules and required repair. Safety Risk: The loss of propulsion occurred at highway speed with passengers present in the vehicle, creating a serious safety hazard. Sudden power loss on an interstate increased the risk of collision due to the inability to maintain speed or maneuver safely in traffic. Confirmation / Inspection: The failure was confirmed by an authorized Cadillac dealership. The engine was removed and replaced due to internal damage. Electrical communication faults related to the serial data gateway module were also confirmed and repaired. Warning Indicators: No warning lamps or alerts were displayed prior to the loss of propulsion. Communication faults were identified during post-failure diagnostics. Current Status: The vehicle has undergone engine replacement and electronic repairs and is currently under post-repair monitoring due to the severity and safety implications of the failure. Within approximately one minute, the vehicle shut down entirely and lost all propulsion and mobility. I was able to move the vehicle to a safe location only
... because I was not in traffic or on the highway at the time of the failure. Had this occurred at highway speeds or in active traffic, it would likely have resulted in a dangerous situation with a high risk of collision, especially given the severe winter weather, low visibility, and slippery road conditions. The vehicle has approximately 21,000 miles. This is the second failure of the Integrated Charging Control Unit (ICCU) in this vehicle. The vehicle was inspected by an authorized Kia dealer, who has officially diagnosed the failure as a failed ICCU and associated fuse. There were no warning lights or messages prior to the failure. The first indication of a problem was the popping sound, immediately followed by limp mode and then a complete shutdown without further warning. The vehicle is available for inspection upon request. The repeat nature of this failure, lack of advance warning, and sudden loss of propulsion represent a serious safety defect.Read more

Complaint Description: While driving my 2025 Honda Odyssey, the Auto Idle‑Stop system engaged in the middle of an intersection at a traffic light, shutting the engine off as designed. When I released the brake pedal, the engine failed to restart, leaving the vehicle stalled in traffic. The dashboard displayed warning indicators including the battery light, and the only way to move the vehicle wa
... s to turn off the car completely several time and try to manually restart the engine. This issue has occurred multiple times and poses a serious safety risk, especially when it happens in intersections or heavy traffic. I almost got hit by an oncoming car! On each occasion, surrounding traffic was forced to maneuver around my vehicle, creating the potential for a rear‑end or a side collision. this has been happening for a long time now and these instances are occurring almost weekly at this point. They are increasing. I have seen that other 2025 Odyssey owners are reporting the same issue — engine shuts off after Auto Idle‑Stop activates and will not restart — sometimes requiring a full restart and causing dangerous traffic situations on carfax.com. Based on similar NHTSA‑documented complaints and investigations into Honda Auto Idle‑Stop restart failures, this appears consistent with a known defect pattern involving stalled engines and failure to restart after idle‑stop engagement. (lawinfo.com and edmunds.com) The dealership has already replaced my low pressure fuel pump and state that they are fulkly aware of the issue and have not provided a permanent fix, and the problem continues to occur unpredictably. My wife and 4 children are in this car all the time. This defect could easily lead to an accident if the vehicle becomes immobilized in moving traffic. I am reporting this to ensure it is documented as a potential safety defect requiring investigation and mitigation.Read more

I am writing to express my frustration and disappointment regarding the ongoing VSA/ABS module failure in my 2013 Honda Civic Si. This issue, common among Honda vehicles from 2013–2017, has caused significant stress, both financially and emotionally. The ABS module in my vehicle has malfunctioned, affecting the vehicle’s anti-lock braking and stability assist systems. This is a serious safety
... concern, as the failure could impair the car’s ability to brake effectively, especially in adverse driving conditions. The failure of this critical component puts my safety, and the safety of others, at risk.I took my car to two separate service centers, a certified Honda dealer and a Honda tuner shop, both of which confirmed the ABS module failure. They quoted over $2,200 for installation and reprogramming of a new module, which itself costs approximately $1,800. The car has had recurring ABS and VSA warning lights for several weeks prior to the issue becoming critical, and I was not informed by Honda about any recalls or known issues with the module. But this is common. As a loyal Honda customer, I am extremely disappointed that I have to shoulder the cost of what seems to be a widespread design flaw. I am requesting that Honda take responsibility for this issue and assist in covering the repair costs, as this is not an isolated problem but a defect in vehicles from this generation.Read more

I came home from work, (drove 18 miles) and parked, walked towards my building and I heard a noise, as if something turned on under the hood. Like a electrical motor spinning. And it was not the fan because that one had already turned off. I walk back to the car and pop the hood, white smoke starts coming out and in less than 2 minutes a huge flame comes straight out from the passenger side of the
... hood. Called the fire department. Fire was on for 4 minutes, neighbor mechanics came running with fire extinguishers and put out the fire before the firefighters arrived. Too many parts melted and damaged. So far estimate is at $4,000. I do not have the estimate on paper, since at the moment I did not have budget to fix my car. My car was perfectly fine, services got done regularly. Never had issues other than wear and tear. I've had that car for almost 10 years.Read more

The speedometer on the new 2025 Volkswagen Taos undervalues the speed by 5%. For example, if the SUV speedometer says 40 mph the actual speed of the vehicle is 42 mph. This seems to be a known problem on new Taos vehicles but is not being addressed by Volkswagen. They are saying legally a 10% variance is allowed and the vehicle passes electrical tests. This problem has been reported to corporate a
