I purchased the 2022 Lexus RX 350 used a couple of months ago. While driving down the highway the instrument panel went blank. I was terrified. I continued to drive not knowing what speed I was driving. The instrument panel went panel came back minutes later. Another incident occurred when the car was parked in my driveway. I turned on the engine and the instrument panel was blank. No numbers
... and no information on the instrument panel. As I drove down the street slowly the instrument panel returned to normal. A third incident occurred when occurred as we were driving on the highway. The light on the instrument panel began to darken. My husband was driving and I was in the passenger seat I was able to access the light app on the dash and restore the light to the original setting. This car is not on the recall list. It should be because this is a matter of life or death. The car had 59000 miles on it when we bought it so it is not under warranty. I contacted Lexus and they refused any help until I take it to a Lexus dealer and have it diagnosed. I don’t drive it daily because I have other vehicles but I would like to be able to drive it and have full use of the instrument panel. The problem repeats itself.Read more

I am reporting a serious and ongoing safety defect involving my 2024 Volvo XC90. The vehicle has experienced intermittent unintended acceleration, intermittent loss of safety and alert systems, and false obstacle detection that triggers unexpected braking or steering input. These events occur randomly and without warning, and are not tied to a specific driver, key, driving condition, or user beha
... vior. At times, the vehicle accelerates on its own without driver input. At other times, the vehicle detects objects that are not present, causing the car to suddenly stop while reversing or swerve to avoid perceived obstacles that are not actually in the roadway. These events occur unpredictably and create a risk of collision with surrounding vehicles, pedestrians, or objects. During these occurrences, safety systems and alerts may become unavailable or inoperable, leaving the driver without audible or visual warnings. Audio alerts and system feedback have also stopped functioning during these events, further increasing the safety risk. The vehicle has been brought in for service approximately 8–9 times for various concerns, including these safety issues. Despite repeated service visits, the problems remain unresolved. Restarting the vehicle sometimes restores functionality, but the defects continue to recur unpredictably. This vehicle is used to transport young children, and these failures present a significant risk of serious injury or death. The unpredictability of the unintended acceleration, false braking/steering, and loss of safety systems makes the vehicle unsafe to operate. I am submitting this complaint due to concern for my family’s safety and the safety of others on the road, and because the manufacturer has not resolved the defect after repeated attempts. These issues have occurred since we received the car in September 2024Read more

SEE FULL COMPLAINT NOTES IN ATTACHED PDF; SUMMARY HERE (you do not provide sufficient space to provide all info you request) I own a 2019 Ram 1500 4WD Limited (not eTorque). On multiple occasions, the vehicle has unexpectedly shifted out of Drive into Neutral while driving. When this occurs, a warning chime dings and a message on display directs me to “Shift to Park”. The vehicle immediately l
... oses forward power (because it has shifted itself to Neutral), until I come to a complete stop, shift to Park, and then shift back to Drive to move forward. Usually the engine stays on, but on at least one occasion the engine turned off and I had to restart. This has happened at least five times total, including three times within the past month. The events have occurred while decelerating to a stop, braking but not stopping, and accelerating from a slow down. Each time, I experienced a sudden and unexpected loss of propulsion, requiring me to stop the vehicle in traffic, shift fully into Park, and then reselect Drive before the vehicle would move again. These incidents have occurred in active traffic situations and have placed me and others at risk of a collision due to the sudden loss of power (SEE DETAILS IN PDF). I previously brought the vehicle to a dealer for this issue after it had just occurred (~1 year ago, before the recent increase in frequency); however, the dealer stated that the vehicle did not throw a code, or no code was stored. They could not replicate the problem and no repair was made. There are at least 55 NHTSA complaints filed that appear the same as my issue; none appear to have been resolved by dealers or RAM. This appears to be a systemic safety defect involving the powertrain and electronic shifting system. The unexpected loss of propulsion in traffic presents a serious risk of injury or death. I am submitting this complaint due to the ongoing and unresolved safety concern. I am scared to drive my family in my vehicle until it is known to be fixed.Read more

My car has a problem This will be the 3rd visit to the Chevy dealership and the 6th time in a garage. After a certain amount of time the battery gauge will move to the highest point, the warning lights of ABs, traction control, stabilitrak, trailer brake light, battery light (first time on 1/27/2026 for battery light), loss of drivers assist on 1/26/2026, tire pressure light, car dies coming to a stop sign. I can’t turn it back on for at least a min. I’m stuck inside with zero power, can’t unlock the doors to get out to direct traffic, open windows or put on 4 ways so I don’t get smacked in the middle of the road. After that minute I heard a click (no key in ignition, not trying to start) and then the dash lights started coming back on again and I started it. The battery did not restart the time on the radio screen so I’m assuming I didn’t die fully. The radio screen always goes black first, then the heat goes in and out, the warning lights come on and it dies. Takes a minute or so to be able to start it again and the lights aren’t on anymore but will continue to do the same shit after awhile of driving and now daily. I’m afraid to drive it anywhere since almost getting stuck inside and plowed at an interaction. I have reached out to Chevy about this problem and was referred to take it back to the dealership again, and to reach back out as soon as it’s there to open a case on the issue.Ive had it, this has been ongoing for a year, and I have to keep paying for things that doesn’t fix the issue and it’s getting worse.Read more

The vehicle was subject to an active manufacturer safety recall involving the oil pressure control system and diagnostic trouble code P06DA, which requires inspection and replacement of the Engine Room Sub-Harness if the condition is present. The defect was confirmed by an authorized dealer, DTC P06DA was verified, and the Engine Room Sub-Harness was replaced in accordance with the recall remedy
... instructions. However, the manufacturer failed to fully remedy the recall, as the consumer was required to pay a significant portion of the repair cost out-of-pocket. While the manufacturer provided partial reimbursement, the remaining balance was charged to the vehicle owner despite the repair being performed solely to address a safety recall condition. Requiring consumer payment to obtain a mandated safety recall repair does not constitute a complete or timely remedy under recall obligations. This complaint is submitted to document the manufacturer’s failure to fully comply with safety recall requirements and to seek appropriate oversight and corrective action.Read more

This complaint is being filed as an update to prior NHTSA complaints regarding the same vehicle and Recall 78C. The issues described below remain unresolved. My vehicle is subject to Recall 78C, which warns of a defect that may result in catastrophic engine failure due to an engine block casting issue. The recall was issued approximately 90 days ago, and the status remains listed as “remedy unde
... r development,” with no repair available and no timeline provided. Since the recall was issued, I have contacted Stellantis three separate times in writing requesting: confirmation whether the vehicle is considered safe to operate while the recall remains open, and transportation accommodation while no remedy exists. None of these emails have received a response. I also contacted two authorized Jeep dealerships seeking transportation assistance. Both initially stated they do not provide loaner vehicles and are not required to follow corporate directives. Both later stated loaner vehicles do exist but were unavailable and subject to a waitlist; however, neither dealership took my information or placed me on a waitlist, nor provided any follow-up. No meaningful transportation accommodation has been provided. I have been advised only that rental reimbursement may be available, which does not prevent continued operation of the vehicle. As a result, I am being forced to continue driving a vehicle under an active safety recall that explicitly warns of possible catastrophic engine failure, without a remedy, without a timeline, and without alternative transportation. This complaint is submitted to document the prolonged lack of a remedy, manufacturer non-response to repeated safety inquiries, failure to provide interim accommodation, and continued requirement that I operate the recalled vehicle.Read more

I am submitting a vehicle defect complaint pursuant to 49 U.S.C. Chapter 301 regarding a potential safety related defect involving engine, electrical, and powertrain systems. The vehicle experienced early engine and electrical malfunction at relatively low mileage, requiring replacement of the engine wiring harness and removal and reinstallation of the engine and transaxle assembly. These are majo
... r powertrain and electrical repairs not reasonably expected on a nearly new vehicle and raise concerns regarding vehicle reliability and safe operation. The vehicle displayed a Malfunction Indicator Light warning indicating an engine and emissions system malfunction, with warnings that continued operation could cause additional damage. Such warnings and failures create a risk of reduced power, drivability issues, or further system failure while operating the vehicle. Insurance repair documentation reflects the selection of a recycled LKQ drivetrain related component, including a recycled engine, transaxle, or CVT related component, which raises additional concerns about durability and safety when installed in a nearly new vehicle. Early powertrain and electrical failures involving engine management and emissions systems may implicate compliance with federal motor vehicle safety standards and warrant review under NHTSA defect monitoring authority. I am requesting that NHTSA log, track, and evaluate this complaint for defect trend analysis, potential noncompliance, or further investigation under 49 U.S.C. Chapter 301 to determine whether similar failures are occurring in other vehicles of the same make and model.Read more

The 12 volt battery was not charged by the voltage regulation system in our car. Horns (high and low) have been replaced 5 times under a warranty. This is a serious safety hazard. Vehicle: Kia Carnival (Model Years 2022–2024) Issue: Repeated horn failure / electrical malfunction Owners of 2022–2024 Kia Carnival vehicles are experiencing repeated horn failures that present a safety concern. Reported symptoms include complete loss of horn function, intermittent operation,
... delayed response, and in some cases random horn or alarm activation without driver input. When the failure occurs, both the high-pitch and low-pitch horns are often found to be inoperative. Many owners report that dealerships replace both horns, sometimes multiple times, with some vehicles requiring second or third replacements. Kia has acknowledged this issue through a Technical Service Bulletin (ELE-263), which instructs dealers to replace both horns when one fails; however, the bulletin does not identify a permanent root cause or corrective design change. As a result, the issue frequently recurs after replacement. A non-functional or intermittent horn significantly reduces the driver’s ability to warn other road users and avoid collisions, creating a clear safety risk. Given the recurring nature of the failure across multiple model years, this appears to be a systemic defect rather than isolated component wear.Read more

Incident date: Nov 7, 2025
While driving at normal city speeds, the vehicle suddenly displayed a warning message stating “Stop vehicle and check power supply,” accompanied by a red battery warning indicator. Shortly after the warning appeared, the vehicle entered a reduced-power or “limp” mode, with severely limited acceleration and speed. This created a safety concern as the vehicle could not maintain normal traffi
... c flow. The high-voltage traction battery state of charge was normal at the time, indicating the issue was not related to battery depletion. Prior to the failure, there were no unusual driving conditions, accidents, or modifications to the vehicle. Ambient temperature was cool but within normal operating conditions. Due to the loss of power and safety risk, the vehicle was not driven further and was towed to an authorized Hyundai dealership for inspection. The problem was confirmed by the dealer, who diagnosed a failure of the Integrated Charging Control Unit (ICCU), which manages charging and power conversion between the high-voltage system and the 12-volt electrical system. The dealer determined that the ICCU requires full replacement. The vehicle remains at the dealership awaiting parts and is available for inspection. The failure resulted in sudden loss of drivability without prior meaningful warning, posing a potential hazard to the driver and surrounding traffic.Read more
