Incident date: Aug 1, 2025
The ICCU (integrated charging control unit) failed July 6, 2025 when the car had 6,800 miles on it. I was driving it on the road at the time and it suddenly would not go faster than 20 miles per hour. It was in the shop for weeks to repair. The replacement ICCU just failed again on January 31, 2026 at a mileage of 15,380. The car is inoperable and cannot be driven.

Onboard AC charging has failed 3 times in 4 months. Each time the GHCA has been replaced with a new charger. When the GHCA shorts out it only impacts AC charging leaving everything else working normally. DC charging is not impacted and works normally. Shortly after getting the vehicle back, while traveling with my family, I received a dashboard warning: “Stop safely – electrical fault.” Immediately after this warning, the vehicle lost power and died in the street with my family in the car, creating a major safety hazard and risk of being hit by traffic. The vehicle had to be towed. I am currently stranded in NY (away from home in MA) and the vehicle is now at a dealer here. It has been difficult to even get it seen by a technician, and we are supposed to return home tomorrow (Sunday). I have learned many other owners are experiencing similar “Stop safely” failures and it appears related to the P400 DC/DC converter. There have also been incidents where the steering wheel would not turn, and the only way to restore function was disconnecting the negative battery terminal to reset the vehicle computer, which is extremely dangerous. Additionally, the heated steering wheel overheated and burned a hole through the steering wheel and burned my hand while driving. The dealer replaced it under warranty, but the repair notes suggested the damage caused the heater issue, when the damage occurred because of the overheating/burn. This vehicle has had repeated safety-related defects involving the cooling system, electrical system, steering, and heated steering wheel, including sudden shutdown in the roadway with passengers. In addition, the second row seatbelt has been stuck and it won’t go back in. The third row seats often times try to disengage and come down. Anytime I hit a small bump or go over a speed bump even very slow. My Bluetooth goes off-line as well as the Navigation and then it comes back online within 3 to 5 minutes. Carr seems to have had a number of odd issues, including the way the engine has sounded as well as the electrical system is not engaging properly. I feel highly unsafe continuing to drive this vehicle and until all recalls and full proper analysis is conducted to remedy these issues I would want Either a full refund of the cash value that I purchased my vehicle for, a brand new vehicle at no additional charge, or a loaner vehicle until all of these issues are remedied, and then a full analysis to ensure that it meets the standards. My vehicle was inspected by a independent service center that confirmed that Ford's electrical power steering is not safe because it can disable while driving, however I called to see if Ford could inspect the issue complimentary, they declined and said it would cost. Not knowing that as a consumer, that is extremely terrifying to know that while driving I'm at a high risk of an accident. I also called Ford to see about a recall because I was informed Ford had multiple recalls for their electrical power steering in most of their Ford Escapes, but there was none for mine. The Ford service worker i spoke to, infact, volunteered to inform me that all Ford Escapes from 2008-2012 should be recalled due to the electrical power steering issues. Once I got home turned my vehicle off and the next morning started my vehicle up to show my husband the power steering issue, 24 hrs later a Warning came on saying power steering assist fault. With that warning coming on after the fact, 24 hours later, without that warning could have ended fatal. This is not a situation that should be handled lightly because it can end fatal, and with no warning that these vehicles power steering can disable while driving there's no telling how many accidents have ended fatal already because of this very issue. This report documents two separate safety incidents involving the same vehicle that occurred on different dates. Approximately one month prior to the most recent incident, while driving my young children to school, the vehicle experienced a sudden and severe loss of braking effectiveness while in motion. ... required to press the brake pedal fully to the floor to slow the vehicle, and even then braking response was significantly reduced and not functioning as designed. The engine remained running and the vehicle could still accelerate. After stopping and restarting the vehicle, braking function temporarily returned. On a later date, while again transporting my children, the vehicle experienced a complete loss of power while driving. The vehicle abruptly shut down while in motion. I was unable to accelerate and experienced loss of power assisted braking. The vehicle only came to a stop after coasting. Following this shutdown, a burning or melted plastic odor was present. The vehicle was later towed and inspected by a dealership. The battery was replaced; however, this did not resolve the issue. After the battery replacement, the vehicle later hesitated to start again. The exact cause of these failures remains unknown. These incidents occurred without warning and represent serious safety critical failures involving braking and propulsion systems. Both incidents placed my children, myself, and other motorists at risk. Had either event occurred on a highway or heavily trafficked roadway, there would have been little to no ability to safely slow or stop the vehicle, creating a significant risk of collision or serious injury. The air conditioning system in my 2020 Honda Civic EX has a refrigerant leak at the evaporator assembly, causing very weak or almost no cooling even when set to the lowest temperature settings. This happens consistently, including in moderate to hot outside temperatures, making the cabin uncomfortably warm during drives as it could get pretty hot in inland southern California. This issue creates a safety risk because reduced visibility from window fogging in humid or rainy conditions (due to lack of proper dehumidification and cooling) can impair safe driving. In extreme heat, it also increases driver fatigue and distraction from discomfort. The problem was confirmed by a Honda dealership through diagnostic testing, which identified the evaporator leaking refrigerant. They previously repaired a related condenser leak under warranty, but the cooling issue persisted and was later traced to the evaporator. The vehicle has been inspected by the dealership (manufacturer-authorized) on multiple occasions for this A/C concern. No specific warning lamps or messages appeared on the dashboard related to the A/C system. This appears to be part of a broader pattern reported by owners of similar 2016-2021 Honda Civics, where condenser failures (sometimes covered by extended warranty) are followed by evaporator leaks, possibly related to the vehicle's use of R-1234yf refrigerant and system pressures. I recently received partial goodwill assistance from the manufacturer toward repair costs, but the issue highlights a recurring defect in the A/C system that affects cooling reliability. The car sputters when fuel is added but will drive. Went to get it checked out. Ford said it was spark plugs and a canister purge valve stuck. Got it looked at for a second opinion and they said the plugs were fine and the note saying the canister purge valve was stuck, but the canister purge valve was operating fine. I’m thinking there’s a bad PCM, TCM or BCM in this model and it just hasn’t been mentioned yet. Historically, Ford says it’s the spark plugs often and other issues and take several times to fix the issue. Last time it has a shut off problem, they tried two different things before replacing a solenoid somewhere and it lasted for a while up until now. My BRAND NEW car is suffering an ICCU issue. I first noticed the screens going completely black while driving. I then noticed that I can’t charge at all on Lvl 1 and Lvl2 chargers (AC), but I went to the dealership and the helped me charge on lvl 3 DC fast charging. This is an incredibly dangerous problem, if this results in my 12V battery dying, I could be stranded while driving.

These issues occur during normal driving and present potential safety concerns by distracting the driver, limiting access to vehicle information, and raising concerns regarding braking reliability and electrical system stability. I previously filed a formal complaint directly with Volkswagen regarding these defects and have received no meaningful response, resolution, or follow-up. I also brought the vehicle to an authorized Volkswagen dealership for inspection. Despite review, no permanent repair was completed and the same defects continue to occur. I was not provided a clear explanation, corrective action, or assurance that the vehicle is safe for continued operation. I have documentation showing that these issues are not isolated to my vehicle. Identical brake noises and infotainment system blackouts have been widely reported by other Volkswagen owners across social media platforms and Reddit community groups, suggesting a broader or systemic defect. In addition, I personally possess videos and photographs documenting the brake noises, infotainment blackouts, warning lights, and electrical irregularities affecting my vehicle. This evidence is available upon request, and I am prepared to provide follow-up information as needed. Given the repeated failures, lack of manufacturer response, and corroborating reports from other owners, I am requesting that this complaint be formally recorded and evaluated by the NHTSA.Read more

This submission supplements my prior NHTSA ODI complaint regarding the same vehicle and loss-of-motive-power condition. On Nov 8, 2025, the vehicle experienced a loss of propulsion at highway speed, creating an immediate safety risk. The vehicle could not maintain speed and required evasive action with no propulsion to avoid traffic hazards. Following the incident, the vehicle was inspected and
... stored diagnostic trouble codes P0A3F-00 and P0A43-00 were retrieved, relating to drive motor position sensor / resolver signal loss or degradation. These fault codes are associated with loss of propulsion and indicate a propulsion control signal-path fault, rather than a condition resolved by software calibration alone. The vehicle remains available for inspection and is out of service at a dealership due to unresolved safety concerns. Since my prior ODI complaint, I provided formal written notice to the manufacturer describing the incident, the retrieved fault codes, and the safety implications, and requested a written response. Receipt was confirmed by USPS certified mail on January 6, 2026. No written response was provided, and repeated attempts to contact the manufacturer through customer service channels were not returned. The manufacturer authorized only a recall firmware update and declined to authorize or perform the intermittent wiring and resolver diagnostics identified in its diagnostic procedures for these fault codes. The firmware update does not inspect, repair, or replace the resolver, wiring, or related hardware indicated by the stored codes, and does not determine whether the underlying propulsion fault has been corrected. As a result, the vehicle has been out of service for more than two months, and the risk of another loss-of-propulsion event remains unresolved. This update documents the manufacturer’s non-response after notice and reliance on a software-only remedy that does not address the indicated propulsion system fault.Read more

Eventually car would drive a short distance then hard brake, slip out of gear, not be able to drive. This was repeated 4 times before finally being able to drive without failure. -Audi inspected, could not reproduce the issue. It occurs very randomly, so it’s difficult to reproduce on demand. -No fault codes were found or stored -Dealer said without being able to reproduce it there was nothing to be done. I asked for TAC case to be opened for assistance. They strongly refused but eventually agreed. Case was opened but immediately rejected (within minutes). Later they said it was rejected for missing info, including location of incidents, which they never asked me for and I could have provided. Sudden random very hard braking and going into park while accelerating, then not being able to drive, with no warnings or fault codes, is a safety hazard.This has happened many times over a year.
