... as observed to be repeatable, including predictable occurrence while the vehicle was in the built-in Service Mode. After a factory reset, repeated/double boot behavior was also observed. Additional observations: - With the turn signal activated, light leakage was observed in the side repeater camera image (glare/bloom present in the camera view). - With each windshield wiper sweep, the forward camera image was observed to briefly flicker to a purple/green distorted state and then return to normal. - During driving, the on-screen message “Auto Lane Change unavailable” was observed. A separate rear camera symptom has also been observed: intermittent freezing/stale frames (image appears not to update). This rear camera freeze behavior has not yet been captured on video. These conditions have been documented by video/screenshots and reported to the manufacturer/service center for inspection/diagnosis. No crash or injury occurred. The dashboard displays warnings for nearly every safety-related system, and the vehicle disables these features without warning. **Detailed Description of the Problem:** Whenever I start my vehicle, multiple error messages are displayed related to the Lane Keeping Assist System (LKAS), Adaptive Cruise Control (ACC), Collision Mitigation Braking System (CMBS), Road Departure Mitigation, Auto Headlights, and other safety features. These warnings appear together and persist even after restarting the vehicle. Apparently, this issue is widespread among 2018–2020 Honda Odyssey owners, with many reporting the same cluster of system failures. The problem appears to be related to a malfunction in the vehicle’s electrical system, sensors, or control modules. In my case, the failures occur without any external damage, weather event, or driver action that would explain the sudden loss of safety functionality. When these systems fail, the vehicle disables critical safety features such as automatic emergency braking, lane keeping, adaptive cruise control, and automatic headlights. The specific component or system responsible is not yet known, and the vehicle is currently available for inspection. The vehicle experienced a failure of the heater core assembly, which resulted in engine coolant leaking directly into the passenger compartment. ... o repair. The total cost of repairs exceeded $5,300, which I believe is disproportionate and unexpected for a vehicle of this age and mileage. Two separate repair visits were required as it was found after the heater core was replaced the actuator on the driver’s side was damaged by the ongoing leak and required repair. The vehicle has been properly maintained and serviced according to manufacturer recommendations. While this incident did not result in an immediate crash or injury, the nature of the failure raises safety and health concerns, including: •Driver distraction due to coolant fumes and loss of defrost capability •Exposure of occupants to engine coolant within the enclosed passenger compartment •Sudden loss of cabin heating and windshield defogging capability in cold weather Given the age of the vehicle and the severity of the failure, this does not appear to be normal wear and tear. I am concerned that this may represent a design, material, or manufacturing defect that could affect other vehicles of the same model year. The odor was present for many months prior to the failure; however, the local dealership could not examine the issue at the time of a prior oil change due to lack of time on that visit. Cruise control activates without driver input. Voice control activates repeatedly without pressing the steering-wheel button. Intermittent, but significant loss of propulsion power has occurred while driving. The backup camera intermittently remains on while driving forward and backup sensors intermittently fail. GPS location is severely inaccurate, the dynamic road sign display does not function, and the Emergency Call (SOS) system does not function. Car will not connect to the app, leaving many functions unavailable, including the ability to condition the battery before driving in cold conditions. Many of these issues have occurred multiple times over the course of the vehicle's history, but Volkswagen was able to fix many of these issues for a time. Car drains battery. 45,000 miles and has had 3 batteries and have been told it needs another. Dealer says start stop switch, requires a battery of 750 CCA and not the Subaru recommend battery. The recall notice says that the battery cells can develop excessive lithium deposits, which increases electrical resistance and can cause rapid battery heating during Level 3 fast charging. Nissan states it will install software that monitors for “state-of-charge fluctuation” and, if detected, will prevent the vehicle from restarting or recharging in order to avoid a thermal incident. This means the defect inside the battery cells is still present, and the car may disable itself if the defect begins to appear. The recall materials also state there is no warning before overheating occurs. This creates multiple safety concerns: • The underlying battery defect remains uncorrected. • The vehicle can become immobilized (unable to restart or recharge) if the software detects the condition. • Loss of Level 3 charging capability affects the ability to travel safely or plan charging when needed. • A battery fire risk exists during Level 3 charging if the defect is not detected in time. Nissan’s documents state that the software is meant only to prevent the “progression” of a thermal incident, not to repair the defective battery. I am concerned that my vehicle contains a known hazardous defect that has not been physically repaired and that the software-only response is inadequate to ensure safety. I am filing this complaint so NHTSA is aware that the remedy being offered does not address the internal battery defect itself and may leave owners exposed to continued safety and reliability risks.Read more

Mitsubishi Motors has been contacted several times in the past about the ETACS system being faulty. The windshield wipers on this particular car went out in February, 2017, at which time they were contacted and rudely refused culpability. The car has had recall-related services completed: 2 windshield wiper recalls and a corrosion-related recall. I was told by the Mitsubishi tech in Asheville, NC
... that the recall on the wipers did not cover the faulty ETACS, "which is a common repair on 06-11 Mitsubishis." He was more than happy to give me a quote for more than $1,000 to replace the defective ETACS. We've replaced coil springs that snapped in two as I pulled out from my driveway with my children in tow - NOT covered by the recall because the mileage on the vehicle was 3,800 miles OVER Mitsubishi's pre-set mileage limit. I have had numerous problems out of the vehicle: coil springs snapped in two, ETACS failure, (still) non-functioning windshield wipers, corroded/rotted exhaust that cost nearly $1,000 to repair - not REPLACE but to REPAIR -, electrical failures related to headlights/interior lights/Bluetooth/cruise control, and severe cross member corrosion. I have reached out to Mitsubishi Motors several times since purchasing the vehicle, and other than speaking to extremely rude representatives three times by phone, no one has ever responded to me about my concerns. Most concerning right now: The anti-corrosion that was applied by the Mitsubishi dealership should have been a complete replacement of the front and back subframes, as BOTH are completely ROTTEN and CRUMBLING from rust! My daughter had been driving this vehicle with her baby in tow until recently! Repeated failures of the 48v to 12v DC to DC converter. Failure to charge the 12v system results in an complete drain of the 12v battery in under 30 mins while operating the vehicle. The result is an immediate shut down of the vehicle. In my case, while in the middle of a travel lane. Without 12v power the vehicle cannot be shifted into neutral and cannot be moved creating a very unsafe situation.

The service manager accessed the area under the dashboard and restored operation, stating it was likely related to the battery replacement or electrical work performed during the recall. Upon driving the vehicle, I also noticed the steering wheel was not centered when driving straight, and the front end appeared visually misaligned. Less than 24 hours after pickup, while driving at approximately 65 mph on the freeway, the vehicle experienced sudden and violent steering oscillation consistent with death wobble. This required me to immediately pull over for safety. The vehicle had no prior history of death wobble, steering instability, or electrical running board issues before Recall 68C was performed. When the motorcycle has become warm it no longer generates enough voltage at idle to keep the Variable Valve Timing system on the motorcycle functioning properly. 2. Once the Variable Valve Timing system starts to malfunction by turning off and on repeatedly the motorcycle will eventually stall leaving you stuck in the middle of traffic. It is a very well documented issue with these motorcycle and be assured it is dangerous. Harley Davidson is unfortunately profiting on the issue by charging customers upwards of $4000 to $5500 dollars to fix it by upgrading their motorcycles to the latest version of the "fixed" system for the later models. This should absolutely be a recall as it is very dangerous to have your motorcycle randomly stall because of a faulty design of a charging system. Harley Davidson has vast knowledge about this issue but only wants to fleece customers for the profit to fix. Battery warning light went on as I left my house. Since this is an EV, naturally became concerned. Car was towed to Heritage Hyundai, Towson Md and I was later informed by the technician the ICCU went bad and this was a common problem for the Hyundai/Kia EVs which Hyundai/Kia have not fixed. The car is currently at the dealer's service department.

I had a gearbox failure warning and the car went into neutral at highway speed. Power steering failed and I had to manhandle the vehicle off the road into a snow drift to get off the highway.

At random times, usually when the battery is below 40% I will get warnings that say "reduced acceleration drive with car". I've own the car for a year (brand new) and this has happened at least half a dozen times or more.

Notification continued and I ended up getting a stop vehicle and check electrical system with flashing lights. Stopped and cycled off and on the vehicle a few times. Started vehicle and only check electrical system notification was on (no indication not to drive vehicle so I continued driving). Continued driving and heard what I believe was a pop noise and also got the notification to stop vehicle and check system. Pulled over turned vehicle completely off and checked app again. and Still only PA9096 error. Contacted Hyundai roadside. Truck driver contacted me shortly after letting me know they were on the way. Tried to turn vehicle around in parking lot so easier to tow. Vehicle limited to 1 mph. Vehicle died when I got in spot and wouldn't turn back on. As I was driving the car computer showed overheating, brake failure, transmission failure and the car started shaking and the engine died. I had the car towed by triple AAA to a auto repair shop, but the next day when they went to fix the problem could not replicate it.

Since the purchase of my 2022 VW Taos in 2022, there has been a repeated issue of the check engine light coming on at random times. Each time it has happened the code indicates a problem with the fuel evap system. The issue has been fixed multiple times by replacing parts. ... at risk. Being unable to determine an issue with the car and the safety of operating a motor vehicle due to continued negligence by the manufacturer is the opposite of a safe motor vehicle. Additionally, since buying the vehicle there have been repeated issues with the communications module. The module had to be replaced just days after purchasing it. Now, it is not working once again and if there was an emergency, the technology presented when purchasing the car, SOS, information, crash detection, location, etc, is not available to help me. VW presents these as safety features then does nothing to fix their equipment issues that ultimately render these options and functions useless.Read more

Given traffic conditions on I-70, this failure could easily have resulted in a major accident. The vehicle was undrivable and had to be towed to Flatirons Land Rover in Colorado on January 25, 2026. On January 26, 2026, after I had to repeatedly follow up with the dealership, I was informed that the failure was caused by a defective component related to the battery/electrical system. Although the repair is covered under warranty, the required part is on backorder, and I was advised the vehicle will be out of service for approximately six weeks. The dealership stated they cannot provide a loaner vehicle, leaving me without transportation. They said the dcdc converter and I don not understand and they did not explain well A sudden loss of power without warning at highway speeds represents a serious safety defect that puts drivers and the public at risk.
