My wife was driving our Ioniq 5 on the freeway. She received a message on the dash about the electrical system (probably ICCU failure), and then the car limited itself ("turtle mode") so that the max speed was 20mph. A minute or two later it slowed down even further to 10mph. She was able to get off the freeway safely.

Was trying to unlock the vehicle but it wouldn’t unlock the doors with the remote. Had to crawl through the trunk to access the car. Got a “door latch system service” maintenance error on the screen after the dealership had “fixed” it. This is a huge safety concern as this car is electric and if it does not unlock from the inside, can cause the passengers to be stuck in the car.

The high-pressure fuel pump on my 2018 Honda Accord failed even though the vehicle had previously been repaired under a recall for the same issue. While driving, multiple warning lights illuminated on the dashboard, including the check engine light and other system warnings. The vehicle began running rough and lost power, creating a dangerous driving condition, especially in traffic. This is the
... second occurrence of fuel system failure despite the recall repair being completed. The recurrence of this issue raises serious safety concerns, as sudden power loss or engine malfunction while driving increases the risk of an accident. The vehicle is currently not safe to drive due to the risk of stalling or loss of power. The problem appears to be related to the high-pressure fuel pump system that was previously addressed under recall.Read more

Three separate high-voltage battery section failures within one year, each causing complete loss of propulsion. Two occurred at freeway speeds on [XXX]. FAILURE 1 ([XXX]): Vehicle lost propulsion. Vehicle out of service approximately six months awa

Dealer diagnosed DTC P0AFA, Cell 85 at 1.85V in Battery Section 1 (different section). Section 1 replaced under same Special Coverage. Vehicle returned Jan 28, 2026. FAILURE 3 ([XXX], ~300 miles after Failure 2 repair): Vehicle again lost propulsion on the freeway with check engine light, then progressed to no-start condition. Towed to dealer. Diagnosed Feb 20, 2026: DTC P0AFA, Cell 57 at 1.5V in Battery Section 2 (GM minimum spec 2.5V). Third distinct section failure. Vehicle remains at dealer awaiting GM repair authorization as of filing date. SAFETY CONCERN: Loss of propulsion at freeway speeds is an immediate safety hazard. During the Nov 2025 event, the vehicle lost power on [XXX] at night in active traffic. The progressive failure pattern (three different sections failing sequentially in the same pack, identical DTC) suggests a systemic defect rather than isolated component failures. Prior section replacements have not resolved the underlying problem. Vehicle has been usable approximately 20 total days in the past 12 months. It appears to be either the fuel pump or fuel injectors, at certain times of acceleration the car stops accelerating, the engine light blinks, I have to shut the car off and restart it. It seems like if I slow my car down then try to accelerate, like going through a roundabout or sitting in stop and go traffic really seems to set the problem off. I took it to a Honda dealership a couple years ago
... and they informed me it was fuel injectors but they were not in stock, they assured me the car was fine to drive, I called a couple times to ask if they had gotten the part, they sternly told me they would call me, anyways they never called me and it still happens, there is a recall out for other 2018 Honda Accords that seem to have a similar problem. All my lights are constantly on due to this problem, none of the features on my car work. Sometimes if I reset my battery the lights are fine for a while but the problem still persists. If I "Rev" my car up and gas it pretty hard when I immediately start driving it seems to help.Read more

The vehicle is undriveable due to a high‑voltage enable safety lockout triggered by ECU identity/MAC mismatch after a software/update window; local service instructed me to ‘safely disregard’ active safety alerts contrary to vehicle telemetry, creating a safety risk. Vehicle remains grounded. Safety Concern Vehicle telemetry states HV contactors are blocked to protect the vehicle; service staff advised to disregard active alerts. Inconsistent guidance risks unsafe operation. I grounded the vehicle pending engineering review. Troubleshooting and A/B Tests My home EVSEs successfully charge a different Tesla (Model Y). The subject VIN charges at DC fast charging (Supercharger) and once on a third‑party EVSE, but fails on my two home EVSEs thereafter. This localizes the issue to vehicle‑side AC charge path/charge‑port logic rather than the EVSE. Request OEM to provide CP waveform under load + Toolbox logs. I am submitting this complaint because my vehicle’s driver seat has a dangerous and unpredictable defect that creates an immediate safety hazard. While the vehicle is in motion, the driver’s seat will suddenly and randomly move forward without warning, pushing the driver into the steering wheel and crushing the leg and footwell area. This is not a minor adjustment issue—it is a sudden mechan
... ical movement that physically forces the driver into an unsafe position while actively operating the vehicle. This has occurred at highway speeds and in heavy traffic, which makes the situation extremely alarming. When the seat unexpectedly moves forward, the driver is forced closer to the steering wheel, restricting movement and making it difficult to properly steer, brake, or maintain control of the vehicle. In those moments, the driver’s ability to safely operate the vehicle is compromised. The only way we have been able to stop the seat from continuing to move forward is to perform an emergency maneuver, pull the vehicle over, completely shut the vehicle off, and restart it in order to reset the system. Until the vehicle is fully turned off and restarted, the seat will not return to its normal position. The vehicle has been taken to the dealership twice after the check engine light came on. Each time, we were told the light was being triggered by the “comfort seat module.” However, despite the system indicating an issue with the comfort seat module, the dealership stated they could not find any problems with the seat or the module itself. This situation is extremely frightening and dangerous. A driver being physically forced forward into the steering wheel while traveling at highway speeds creates a serious risk of losing control of the vehicle. This defect puts not only the occupants of the vehicle at risk, but also other motorists and pedestrians sharing the roadway. This issue needs to be investigated before someone is seriously injured or worse. No driver should haveRead more

1. What component or system failed or malfunctioned, and is it available for inspection upon request? The engine and the HVAC systems. Specifically, a persistent "ghost misfire"—where the engine vibrates and shudders without triggering a dashboard warning light—and a total heater failure. The vehicle is currently held at the dealership; all service records (RO# 367641, 367891, 367980, 368179)
... are available. 2. How was your safety or the safety of others put at risk? The "ghost misfire" causes shaking and hesitation during acceleration, creating a risk of losing power or stalling in high-speed traffic. Because the vehicle's diagnostics do not trigger a warning light for this condition, the driver has no advance notice of a potential engine failure. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. Dealership technicians confirmed the misfire on Feb 9 (RO# 367891) and found a defective spark plug gap (.024” vs. .036”). On Feb 25, the service consultant confirmed the "ghost misfire" remained active. The heater failure was confirmed via a 'Heater Support Pump Dry Run' fault code (RO# 367641). 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The vehicle has been inspected by dealership technicians. Formal complaints have also been filed with the California Bureau of Automotive Repair and the California Attorney General. 5.
