My 2021 Volvo XC90 Recharge has experienced multiple hybrid system failures under Volvo's factory 8-year/100,000-mile hybrid warranty, with fewer than 45,000 miles on the odometer. The high-voltage battery modules have failed multiple times, directly related to recall R10312 (NHTSA 25V-179), which identifies a supplier process deviation in LG battery modules creating a risk of internal short circu
... it and thermal runaway — an acknowledged fire risk. Critically, my vehicle has suffered two actual battery module failures, not preemptive recall replacements. This places my vehicle in a substantially higher risk category than others in the recall population with no prior symptoms. A vehicle that has already experienced the exact failure mode identified in a safety recall presents a materially greater ongoing safety risk. Additionally, my vehicle suffered a complete ERAD (Electric Rear Axle Drive) failure — a known defect that Volvo acknowledged by releasing a redesigned ERAD2 unit, confirming awareness of this systemic issue. These repeated failures left my vehicle un-drivable and parked at the dealership for more than 6 months, with replacement parts repeatedly placed on backorder, causing severe and prolonged loss of use. I filed a formal complaint with Volvo Customer Care requesting a vehicle buyback. After months of inadequate communication, Volvo declined to buy back the vehicle or offer reasonable trade-in accommodation. Instead, Volvo offered a nominal monetary concession contingent on signing an NDA, which I declined as insufficient given the documented safety risk, repeated failures of a recalled component, extended loss of use, nor out of pocket car rental expenses. I am filing this complaint to ensure my vehicle's actual failures are part of the NHTSA record for R10312/25V-179, and to request NHTSA evaluate whether Volvo's response to owners who have experienced actual failures — rather than preemptive recall repairs — warrants further regulatory scrutiny.Read more

I am reporting a recurring electrical and mechanical system failure on a 2024 Lexus RX 350h Luxury. On [XXX], the vehicle suffered a total power loss that rendered the electronic door latches unresponsive to all inputs. Because the occupants were a [XXX] and a [XXX] strapped into car seats, they were physically unable to reach the interior manual overrides. Critically, the mechanical backup key
... also failed to unlock the vehicle from the exterior, preventing immediate rescue. This created a life-threatening entrapment scenario. The doors only responded after multiple minutes of repeated manual attempts. Once entry was gained, the vehicle would not start and the headlights began blinking rapidly, indicating a total system communication failure. I had to use jumper cables to restore power. This is the second occurrence of this catastrophic failure, documented on Repair Order [XXX] at Lexus of Westminster. The failure of both electronic and mechanical failsafes constitutes an extreme occupant safety hazard. My jeep will randomly not let me back up as it thinks there is something behind it and automatically apply the emergency brake. I brought it to dealership when it was still under warranty for this issue and they could not figure it out. My jeep will randomly stop as it’s driving, it will still rev when I push the pedal but it will not move in reverse or drive. The automatic shut off says it n
... eeds to be serviced and does not work. The automatic emergency braking comes on randomly if I stop. I constantly get alerts that my blind spots are blocked. I’ve cleaned sensors as the dealership has recommended which doesn’t make a difference. My lift gate will only work intermittently. Sometimes it shuts on my kids. My screen only works sometimes. Which is dangerous when I don’t have access to back up camera. I currently owe $31,000 still on this vehicle and I have 5 kids. I really need this vehicle fixed as I cannot afford to get rid of it. It has been unreliable since I’ve owned it and I’m really disappointed in this Wagoneer. It’s very expensive to insure and it’s had nothing but issues. The tow bills are becoming expensive as well.Read more

1) The electric power steering system failed. The dashboard displayed the message “Service Power Steering Now,” and the steering assist stopped working, making the steering extremely stiff. The vehicle also lost reverse gear, where the engine revs but the vehicle does not move when shifted into reverse. The vehicle is still in my possession and available for inspection upon request. 2) When th
... e power steering assist failed, the steering wheel became extremely difficult to turn, which made the vehicle harder to control. This created a safety risk when attempting to turn or maneuver the vehicle in traffic or parking areas. Additionally, the inability to use reverse gear could create a hazardous situation if the driver cannot back out of traffic, parking spaces, or other areas. 3) The problem has not yet been inspected or confirmed by a dealership or independent service center. 4) No, the vehicle has not been inspected by the manufacturer, police, insurance representatives, or other parties. 5) The dashboard displayed the message “Service Power Steering Now,” and the steering assist stopped working. The problem appeared suddenly when the vehicle was started. The steering immediately became extremely stiff, and the warning message appeared at that time. This failure created a serious safety hazard because the steering assist stopped working without warning, making the vehicle extremely difficult to control. A sudden loss of steering assist can increase the risk of a crash, especially when turning, parking, or driving in traffic. I am concerned that other drivers could experience the same sudden steering failure. The vehicle displayed the message “Service Power Steering Now,” and the steering immediately became extremely stiff. The vehicle is currently undriveable in reverse and difficult to steer safely.Read more

The rear camera comes “loose” and does not register all the time. It seems to be a simple electric issue; possibly an electrical short. I believe it can be easily “fixed” (temporarily) by opening the trunk and running fingers over the camera, it should not happen. I purchased my Malibu new and seldomly drive, approximately 25k miles. I maintain proper maintenance and ensure cleanliness to

Chevy dealerships do not acknowledge the issue and expect the car owners to repair the entire electrical system. Definitely should be a recall item!Read more

Date: 3/7/2026 I am submitting this complaint regarding a potentially serious safety issue with my 2025 Ford Escape Plug-in Hybrid manufactured by Ford Motor Company. My vehicle began displaying multiple warning messages including “High Voltage Battery Warning,” “Powertrain Malfunction,” and “Service Engine Soon.” These warnings appeared while the vehicle was charging using a Level 2
... charger, which raises significant concerns about the safety and reliability of the vehicle’s high-voltage electrical system. After these warnings appeared, I brought the vehicle to an authorized Ford dealership for diagnosis and repair. The vehicle has now been in the repair shop for approximately 30 days, reportedly waiting on parts related to the high-voltage battery or powertrain system. This situation raises several concerns: The possibility of a defect in the high-voltage battery system. The potential for sudden loss of power or drivetrain failure. The risk associated with charging a vehicle that displays high-voltage battery warnings. Extended vehicle downtime due to the lack of available replacement parts. Given the critical safety implications of high-voltage battery systems in plug-in hybrid vehicles, I respectfully request that the National Highway Traffic Safety Administration investigate whether this issue represents a broader defect affecting other vehicles of the same model.Read more

Audi A6s have had massive issues with the door actuators. There are times when you go open the passenger side doors (both front and rear) and the doors will not open as there is either an electrical, software, or device specific issue. It requires 2-3 pulls of the door latch to open the front and rear passenger doors. The issues keep appearing a month or two after the dealership supposedly fixes t

I have asked them to push this up to Audi corporate and have received no response. Audi will also not offer a lemon buyout for these door issues. I believe this needs to be heavily reviewed as if you search the Audi forum websites online, this is a constant problem for the 2018-2023 A6s.Read more

1. Electrical System Failure: While traveling on highway, alternator failed suddenly within 2 minutes of “service soon” and “check electrical system” warnings. Caused complete loss of motive power, electronic parking brake locked up while driving, loss of power steering, all gauges failed, transmission shifted into park while in motion. Battery was damaged (“fried”). Engine continued
... running for several minutes after stop without ability to shut off ignition. Incident documented by Ohio State Highway Patrol. 2. Seat Belt Issue: Driver-side seat belt interlock sticks in unlocked position, creating risk of failure to lock during a crash. 3. Electronic Parking Brake: Intermittently engages while vehicle is moving downhill to a stop (mostly at or below 5 mph). Parking brake light flashes three times then turns solid while in motion. Ford TSB indicates vehicle not eligible for related recall. 4. Infotainment System: Intermittently freezes, requiring forced reboot. Preceded by audio glitches, laggy dash computerized systems. 5. Transmission: Severe clunk and physical jolt when decelerating from ~30 mph to 8 mph. Also occurs during acceleration, particularly upshifts from 2nd to 3rd and 3rd to 4th gears, more pronounced when turning. 6. 4WD System: Intermittently fails to disengage from 4WD HI to 2WD HI. “Failure to shift” message appears several minutes after command to return to 2WD HI. 7. Front Differential/Axle: Previously, faulty front differential housing fractured due to system malfunction, causing intermittent loss of front driveline engagement in 4WD LO (250-500ms delay then re-engagement under throttle). Ford replaced front differential and axle under warranty after vehicle out of service for 2.5 months. Component Involved: Visibility/Backup Device, Electrical System, Forward Collision Avoidance Description of Incident: The Android 12-based infotainment system in this vehicle is suffering from a progressive hardware/software failure that creates a severe, multi-layered safety hazard. CRITICAL DRIVER DISTRACTION: The system has become largely non-responsive, requiring me to repeatedly press o
... n-screen buttons to perform basic functions. This forces me to divert my eyes and manual attention away from the road for periods far exceeding 2 seconds. This "visual-manual distraction" is a direct result of the system's inability to process commands, making safe operation impossible while attempting to use vehicle controls. VISIBILITY FAILURE (FMVSS 111): The failure impacts the digital rearview mirror and the safety camera feeds. These cameras frequently glitch, lag, or freeze, depriving me of necessary rear and surround visibility while moving. This is a violation of federal safety standards for rearview visibility. SYSTEMIC ELECTRICAL FAILURE: This "slowly dying" system is also interfering with the vehicle's communication bus, causing the power door locks to malfunction. This creates an entrapment or security risk. I have reported these issues to General Motors/OnStar. Despite their awareness of these "electrical gremlins" and the existence of limited service updates (like Program #N242435631), no permanent fix has been provided. This is a systemic safety defect that requires a mandatory recall to prevent accidents caused by driver distraction and visibility loss. The system needs updating to Android 14. It is Android 12 now. This is the fix. Gm want me to pay $250 for a hour for a macanic to update it. Gm was updating over the air, but it was causing the battery on the vehicle's to prematurely die under warranty. It was affecting their bottom line.Read more

I own a 2024 Jeep Wrangler Rubicon 4xe (VIN [XXX]) that is subject to a federally mandated safety recall involving the high-voltage battery system. I attempted to have the recall performed by an authorized Jeep/Stellantis dealership, Carson Chrysler Jeep Dodge Ram in Carson City, Nevada. During communications with the dealership’s Service Manager, I was informed that the dealership would not pe
... rform the recall unless I signed dealership-specific authorization documents and agreed to their internal policies. The dealership further stated that if their evaluation determined the hybrid battery required replacement, they would retain (“quarantine”) the vehicle for an extended period and would not provide alternative transportation beyond a limited initial period, requiring me to pay for a rental thereafter. I objected to being required to pay transportation costs caused by a manufacturer safety recall and to signing waivers as a condition of recall service. I made clear that I was requesting that the recall be performed. The dealership then refused further service and removed me as a customer, stating that because I would not agree to their conditions they would not perform the recall and recommending I find another dealer. I subsequently sent written notice to Stellantis / Jeep corporate headquarters explaining the situation and requesting assistance to complete the recall. Stellantis did not respond to either of my written notices. As a result, my vehicle remains subject to an unresolved safety recall despite my good-faith efforts to have the recall performed by an authorized dealer. "The vehicle is experiencing an intermittent, loud popping/crackling noise coming from the speakers and infotainment system. This is a known symptom of a failure in the MOST (Media Oriented Systems Transport) bus network, specifically related to defective FAKRA connectors and coaxial cabling. This issue was previously the subject of NHTSA Recall #23V-431 for 2018–2023 models, but the defect pers
... ists in this 2024 model." Safety Risk: "When the MOST bus network loses connection for more than 20 seconds, the infotainment screen freezes or goes black, causing a total failure of the rearview (backup) camera image. This failure occurs unpredictably while the vehicle is in reverse, significantly increasing the risk of a back-over accident or collision. Additionally, the loud, sudden popping noises are a severe driver distraction." Dealer/Manufacturer Response: "The Honda dealership confirmed the issue is consistent with the known MOST bus failure (TSB 23-047). However, because the vehicle has 49,000 miles, both the dealership and Honda Corporate (Case # [Insert your number if you have one]) have denied a goodwill repair, despite this being a documented safety defect that has been the subject of prior federal recallsRead more

2021 Silverado tailgates often stop working due to a faulty, corroded electronic switch caused by water intrusion. A major 2020-2024 GM recall exists for this, which can cause unexpected opening. Check for corrosion in the tailgate switch/connector, a pinched wiring harness, or check for a blown fuse. This is the 2nd time my truck is at ganley Chevy in Cleveland Ohio. They already fixed 1 ro
