The vehicle had safety and mechanical problems shortly after purchase, even with low mileage. These problems started early and continued over time despite repairs and maintenance. Before the incident, the vehicle showed signs of malfunction, incl

I believe the vehicle had defects from the beginning that affected safety. While reversing into a parking space, the parking sensor system repeatedly indicated an obstacle on the left side of the vehicle even though no object was present. The system displayed a red warning on the left side and automatically applied the brakes very suddenly, causing the vehicle to jerk to a stop. I exited the vehicle to check the area and confirmed there was nothing on the ground or near
... the vehicle that would explain the warning. The surface was flat and clear. Because the system continued to detect a phantom obstacle on the left side, it repeatedly interfered with maneuvering the vehicle while reversing. During this situation the vehicle ultimately made contact with a pole behind the car, which I did not initially realize because the braking intervention was so abrupt. The parking sensor was damaged in the impact. The vehicle was inspected by a BMW dealership, and they stated diagnostics showed no malfunction prior to the collision, only that the sensor malfunctioned after the impact. However, this was not the first time the vehicle’s parking system has falsely detected an obstacle and locked the brakes while reversing, which raises concerns about the reliability of the parking-assist automatic braking feature. The sudden braking and false obstacle detection create a potential safety risk, particularly if the system unexpectedly stops the vehicle or interferes with maneuvering in tight spaces. I am reporting this incident so it can be investigated as a possible defect in the parking sensor or automatic braking system.Read more

While traveling from upstate New York, the vehicle displayed an engine overheating warning shortly after startup while the engine was still cold. Because I was away from home, I stayed overnight and the next morning took the vehicle to a Kia dealership for inspection. The dealership inspected the vehicle and diagnosed a problem with the exhaust heat recovery system pipe. According to the service
... department, coolant is leaking internally through this component and is being burned in the exhaust system. The cooling system was pressure tested and held pressure externally, indicating the coolant loss is occurring internally through the exhaust heat recovery system rather than from an external leak. This condition can cause coolant loss and overheating while driving. If the engine overheats while the vehicle is in operation, it could create a safety risk due to sudden loss of power or engine damage. The dealership confirmed the diagnosis after inspection and testing and provided a repair estimate to replace the affected exhaust heat recovery system component. The overheating warning message was the first symptom observed prior to diagnosis. The problem was confirmed by the dealership after inspection and testing. This issue may indicate a defect in the exhaust heat recovery or thermal management system used in this hybrid vehicle, as coolant is entering the exhaust stream internally rather than leaking externally. If this type of failure occurs in other vehicles, it could lead to overheating while driving and potential engine damage.Read more

This vehicle experiences multiple recurring safety-critical system failures while driving, creating a dangerous operating condition for both the driver and the public. 1. Dashboard / Instrument Cluster Failure While Driving While the vehicle is in motion, the dashboard display intermittently shuts off completely. When this occurs, I cannot see my speed, warning lights, battery status, or any othe
... r gauges. This creates a severe safety risk because I am effectively driving without any instrument information. The issue occurs while actively operating the vehicle on public roads. 2. Navigation / GPS Location Failure The vehicle’s built-in mapping system frequently shows my location several states away from my actual position. Even though the location is incorrect, the system continues to provide turn-by-turn navigation instructions based on the incorrect location. If these directions were followed, it could cause a driver to make unsafe maneuvers or become distracted while attempting to correct the navigation error. 3. False Automatic Emergency Braking Activation The automatic braking system frequently activates even when there are no vehicles, pedestrians, or obstacles present. The vehicle abruptly slows or brakes on open roads with nothing nearby. This creates a significant safety hazard because it could cause a rear-end collision if another vehicle is following. Repair History / Ongoing Issue: This vehicle has been taken in for repair multiple times for these issues. Despite attempted repairs, the problems continue to occur. The recurring nature of these failures indicates a systemic defect that has not been corrected. Safety Concern: These failures create multiple hazardous driving conditions: Loss of dashboard information while driving Incorrect navigation location and directions Unintended automatic braking events Allowing this vehicle to remain in service with these unresolved defects places both the driver and the public at risk. My 2010 Volkswagen CC became completely inoperable because of the active fuel pump control module recall (NHTSA 17V509). The vehicle stalled and nearly caused an accident. The check engine light came on just before I noticed a burning smell coming from the backseat of my car immediately after smelling that the car suddenly stalled out in the middle of a busy intersection and the engine would not s
... tart back up. I had to push it to a gas station parking lot across 3 lanes of traffic. I lifted up the backseat and found that the fuel control module was overheated and melting on top of the fuel pump so I carefully and properly disconnected it from the vehicle. The next day I had the vehicle towed to Lokey Volkswagen in Clearwater, FL. Today 3/14/2026 They did the recall repair and told me my vehicle is safe to drive. Today 3/14/2026 I drove the car 1 mile from the dealership when the exact same thing happened again the car stalled out, and I had to carefully maneuver to the shoulder of hwy us 19 s narrowly missing vehicles in the other lanes. The vehicle was then towed back to lokey Volkswagen. It is pretty clear that the issue was not resolved, and that this fuel control module failing may have affected other parts of the fuel system/wiring and or the fuel control module failed again. I have pictures of the fuel control module, and I gave the initial melted/failed fuel control module to the service department and they took it. The dealership service department is stating that this may be to do something other than the recall repair and that I may be liable for the diagnostic and towing charges. This is a very serious safety issue that almost cost me to get in accidents on the highway twice. I feel that they (Volkswagen) needs to be held responsible for rectification and to find out if there is a connection between this recall and the continuing failure of the vehicle to operate safely.Read more

The high-voltage battery on my 2022 Nissan Leaf SL+ is subject to Recall NHTSA ID: 25V-655. The recall notice states that the battery cells can develop excessive lithium deposits, which increases electrical resistance and can cause rapid battery heating during Level 3 fast charging. Nissan instructs owners not to use Level 3 charging at all until a “remedy” is available. The issue is that the
... remedy described by Nissan is not an actual repair of the battery defect and has been significantly delayed, with no remedy provided for the loss of use of my vehicle, due to the dangers of the battery, which is still covered by warranty. Nissan states it will install software that monitors for “state-of-charge fluctuation” and, if detected, will prevent the vehicle from restarting or recharging in order to avoid a thermal incident. This means the defect inside the battery cells is still present, and the car may disable itself if the defect begins to appear. The recall materials also state there is no warning before overheating occurs. This creates multiple safety concerns: • The underlying battery defect remains uncorrected. • The vehicle can become immobilized (unable to restart or recharge) if the software detects the condition. • Loss of Level 3 charging capability affects the ability to travel safely or plan charging when needed. • A battery fire risk exists during Level 3 charging if the defect is not detected in time. Nissan’s documents state that the software is meant only to prevent the “progression” of a thermal incident, not to repair the defective battery. I am concerned that my vehicle contains a known hazardous defect that has not been physically repaired and that the software-only response is inadequate to ensure safety. I am filing this complaint so NHTSA is aware that the remedy being offered does not address the internal battery defect itself and may leave owners exposed to continued safety and reliability risks.Read more

I am reporting a safety concern involving a 2018 Dodge Journey SE (VIN: [XXX] ) related to a failure in the vehicle’s emissions control system, specifically the catalytic converter and oxygen sensors. On January 13, 2026, the vehicle began producing a strong sulfur or “rotten egg” odor while driving, and shortly afterward the check engine light illuminated. Diagnostic testing revealed troubl
... e code P0420 (Catalyst System Efficiency Below Threshold – Bank 1). This code indicates that the catalytic converter is not properly converting exhaust gases, which can result in increased emissions and potential exposure to exhaust fumes. The vehicle was inspected by an authorized Chrysler, Dodge, Jeep, and Ram dealership in Maurice, Louisiana, which confirmed that the catalytic converter and oxygen sensors are failing and must be replaced. The vehicle remains available for inspection upon request. This vehicle was purchased for our growing family and is regularly used to transport our children, including a [XXX] infant and a [XXX] child. Since the emissions system issue began, occupants have noticed persistent exhaust-like fumes and strong sulfur odors while the vehicle is operating. Our infant has experienced recurring respiratory issues, and our [XXX] began experiencing respiratory symptoms after we purchased this vehicle. While we cannot definitively determine the cause, we are concerned that exposure to exhaust fumes from the malfunctioning emissions system may be contributing to these health issues. In addition, adult occupants have reported feeling unusually tired or fatigued after longer car rides in the vehicle, which raises additional concerns about potential exposure to exhaust gases inside the cabin. Because catalytic converter failures can allow harmful exhaust gases such as carbon monoxide and other pollutants to pass through the exhaust system improperly, we believe this issue presents a potential safety risk to vehicle occupants, p INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 URead more

My 2025 Hyundai Ioniq 5 has experienced repeated catastrophic 12-volt battery failures following an ICCU (Integrated Charging Control Unit) failure in November 2025. The ICCU failure caused the vehicle to lose power in traffic and required towing. The ICCU was replaced but the 12-volt battery was not replaced. Since that repair, the vehicle has experienced repeated electrical failures. The 12-vo
... lt battery dropped to approximately 3.5 volts on one occasion and approximately 1.72 volts on another. These voltages were measured and documented on video using multiple multimeters before any jump start or charging. At these voltage levels the vehicle cannot unlock doors, power systems, or operate normally. The vehicle required towing by AAA. The dealership has repeatedly recharged the battery before testing and then claimed the battery “tests good,” refusing to replace it under warranty. Numerous owners online report similar failures where ICCU problems lead to repeated 12-volt battery collapse and vehicle immobilization. Because loss of 12-volt power can disable critical vehicle systems and leave the vehicle stranded or stopped in traffic, I believe this represents a potential safety defect. I don't have the repair reports from the dealer scanned in but I can if you want them. ICCU is known failure and recall but for some reason our car doesn't appear on any recall list. 12V battery that drops below 6V is deemed unsafe and bad and 3 times (including this time) it's been below 6v but they don't believe it's a bad battery.Read more
