In November 2022, I started getting a dashboard "shift to park" on my 2019 Acadia after I had parked and shut the vehicle off. At the time I took it into service to have it looked at/repaired, they told me there would probably be a recall on this issue,as it seemed to be a frequent problem, and I would get a refund for the repair. That recall never happened, and now my car is once again displaying
... the same issue. The dealership will not cover the repair, and I will be responsible, as it is out of their 2 year warranty window. I can't just ignore this issue and keep driving, as with the "shift to park" display, there is a constant alarm that doesn't stop unless the park switch recognizes the car is in park, even when it is shut off. I feel this issue needs to be placed under a recall, and I should not have to pay AGAIN for it to be repaired.Read more

I own a 2022 Audi e-tron Sportback that experienced multiple serious issues involving the high-voltage battery system. During service at Audi South Atlanta, the dealership documented diagnostic trouble code P29FF00 – Hybrid/EV Battery Thermal Runaway Detected. This code is related to battery overheating and potential thermal runaway, which is a critical safety condition in electric vehicles. D
... espite identifying this safety-related fault, the dealership did not perform any repair or replace any components. Instead, they cleared the fault codes and returned the vehicle to me stating it is operating as designed. Prior to and during this issue, the vehicle experienced: •Severe range reduction (approximately 60 miles at 60% charge) •Charging interruptions and warning messages •Regenerative braking malfunction •Abnormal energy consumption •Mechanical drag and braking irregularities These issues are consistent with a high-voltage battery or power system defect. The dealership’s decision to reset the fault code without addressing the underlying cause raises serious safety concerns. A thermal runaway-related fault should not be cleared and ignored, as it may indicate a risk of battery failure or fire. I am concerned that this vehicle may be unsafe to operate and that the defect was not properly diagnosed or repaired.Read more

I am writing to formally document a recurring and dangerous safety defect involving the B&O audio system. On multiple occasions, the vehicle has emitted an ear-shattering popping noise followed by high-decibel electrical static. This issue is not merely a "speaker glitch"; it is a violent acoustic event that persists even after the ignition is turned off. Critical Safety Concerns: Driver Incapac
... itation: The volume is loud enough to cause immediate disorientation and potential hearing damage. If this occurs at highway speeds, the risk of a collision is extreme due to the "startle response" and inability to hear sirens or road cues. Electrical Instability: The static is accompanied by a sensation of electrical discharge throughout the cabin. To stop the noise, I am forced to manually disconnect the battery—which itself emits static popping during the process. Repeated Component Failure: The vehicle has already exhausted two batteries and requires a third. I suspect a faulty amplifier is creating a parasitic draw and low-voltage condition, triggering this catastrophic system failure. Lack of Professional Response: Despite SSM 51663 addressing this exact issue, dealerships have refused to perform the software/hardware fix due to the "sporadic" nature of the problem. Furthermore, when I contacted Ford Customer Service, my concerns regarding a potential crash were met with derision. I am requesting an immediate escalation, a formal inspection of the B&O amplifier, and the application of the relevant TSB/SSM software updates. I no longer feel safe operating this vehicle, and I am documenting this for the purpose of a Lemon Law evaluation and an NHTSA safety report.Read more

Component: Electrical System / Forward Collision-Avoidance Assist / Speedometer Incident Summary: I am reporting a catastrophic and uncorrected safety defect in my 2021 Kia Sorento (VIN: [XXX] ). Pre-Accident Attempts: Before April 2024, I repeatedly sought diagnosis for instrument cluster 'glitches.' The Kia dealer misdiagnosed this as a battery failure, charging me out-of-pocket for a repl
... acement that did not resolve the issue. Collision (April 22, 2024): While driving, the instrument cluster suffered a total system crash into 'Factory Mode,' losing all telemetry. Simultaneously, the vehicle's Forward Collision-Avoidance Assist (FCA) failed to engage. Despite the vehicle being equipped with automatic emergency braking designed to detect and mitigate collisions, the system provided no warning and no autonomous braking prior to striking a deer. Post-Accident Failures: Following the crash, Geico Insurance funded a wiring harness replacement at a Kia facility (backed by a Geico Lifetime Warranty). However, the glitches and 'Factory Mode' errors have returned, proving the vehicle's core safety systems remain compromised. Safety Risk: This vehicle is non-compliant with FMVSS 101 and 127. The failure of both the speedometer and the autonomous emergency braking creates an extreme risk of further injury or death. Kia Corporate has failed to address this documented accident history. However, during the time my son was stuck inside with the over 95 degree weather, the had no safety mechanism in place to open it physically like a traditional key fob would. It is very disheartening knowing my son could have potentially lost his life over something so simple that could be resolved with a physical keyhole on the outside like almost every traditional vehicle has. Since this event the issue has occurred twice more on two brand new batteries on no incline, and I now fully disable the auto-stop feature before driving to avoid an accident. The car starts properly with no battery damage if started from the full ignition off position, even if auto-stop originally stopped the engine, pointing to a failure of the auto-stop system specifically. We own a 2024 Cadillac Lyriq EV. We began experiencing safety concerns related to battery performance and overall vehicle reliability during the active bumper-to-bumper warranty period. These concerns were reported to an authorized GM dealer, Napleton Cadillac, prior to warranty expiration. Service records at Napleton Cadillac document the history of these complaints. The vehicle has been unable
... to process required over-the-air (OTA) software updates through GM's intended automated update process. During a dealer visit on February 17, 2026, at GM's direction, the Napleton Cadillac service technician confirmed that the vehicle had multiple open notices and that our reported concerns are consistent with known issues affecting the 2024 Cadillac Lyriq across multiple units — indicating this is not an isolated occurrence. The dealer advised that correcting the update failure would require approximately five hours of manual labor at an estimated cost of $1,250 — a cost GM has declined to cover, despite the fact that the OTA update process is the manufacturer's intended method for delivering safety-critical corrections and the failure to update is a vehicle deficiency, not a customer-caused condition. Due to these unresolved safety concerns, we have not been driving the vehicle. We do not have confidence that the vehicle is operating safely or as designed. We formally requested a repurchase from General Motors through the EV Concierge program (Case No. 87486271). That request was denied on March 14, 2026, citing two rationales: (1) the vehicle is out of warranty, and (2) system updates do not qualify for repurchase. We believe these rationales fail to account for the fact that the original safety complaints were filed during the warranty period and that the root issue is battery and system safety — not a routine software preference. We are filing this complaint to create a formal federal safety record and to request that NHTSA evaluate an investigation or recall.Read more

The failure involves the instrument cluster (driver information display) on my 2017 Volvo XC90. This component is responsible for displaying speed, warning indicators, and all critical driving information. The failed component remains installed and is available for inspection. The issue began while driving on public roads. The display intermittently showed distorted, “barcode-like” patterns
... that would appear suddenly, disappear, and then return. The condition progressively worsened over time. The failure is now consistent and reproducible. When starting the vehicle, the cluster may appear normal for a few seconds before becoming severely distorted or completely unreadable. Now it is nonfunctional from the moment of ignition. This creates a significant safety hazard because I cannot view speed, warning lights, gear position, or any system alerts. The initial failures occurred while the vehicle was in motion, which increased the risk at the time of occurrence. The vehicle is not safely operable in this condition. The vehicle has approximately 24,000 miles, has never been in an accident, and has been properly maintained. There were no external factors that could explain the failure. The only prior symptoms were the intermittent display distortions described above. An authorized Volvo retailer inspected the vehicle and diagnosed the instrument cluster as failed, recommending replacement. No repair has been completed. Volvo Car USA declined goodwill assistance despite the extremely low mileage and safety‑critical nature of the failure. I am aware of similar instrument cluster failures reported by other 2016–2017 Volvo XC90 owners, suggesting this may be part of a broader pattern.Read more

While on/idol, the 3rd row seat (left/drivers side) folded down on my 9 year old niece. Myself, mother, 6 year old daughter, sister in law, brother and 12 year old niece all witnessed this happen as we were loading into the car after playing at the park (I had the car turned on). Her head got pinned to the headrest of the 2nd row captain seat. The buttons did not respond as they usually/supposed t
... o do to stop and reverse the folding action. The seat continued to press against her skull. After realizing the buttons in the cargo were not going to work, I had an idea and ran to the 2nd row door and pressed the captain seat button to slide it forward (my child was in that captain seat in a rear facing carseat). It wasn't able to move much but enough that the trapped girl was able to force her head out and her mom pulled her the rest of the way out to release her. It was very traumatizing. That 3rd row seat remained in that almost closed position as the buttons still would not function so we drove home. Eventually I tried the buttons in the again and it unfolded the seat. I have tested these buttons over and over since then to figure out what happened. Every time I test them, each button allows me to either release it and the action stops, or press the button again and the action stops. I do not know why it wouldn't respond to doing that same that day. Prior to that incident, sometime between getting the car on 7/31 and mid-November, I had an empty unbuckled backless booster car seat in the third row right/passenger side as a backup for daughters friends to use, and without remembering it was there, I pressed the button to fold down the 3rd row right side chair and it kept stopping and auto reversing, several times before I realized oh ya, the booster is back there. So when the innocent happened with my niece, it came as a big surprise because that booster weighs nothing compared to a 9 year old. It has not happened againRead more

The vehicle experienced a major electrical system malfunction involving multiple critical components. The issue began with the Volvo On Call system light remaining illuminated continuously, which suggested a possible communication or battery-related fault. Shortly after, additional systems failed, including the turn signal system, windshield wipers, and high beam headlights. These components are c
... urrently non-functional and can be made available for inspection upon request. The failure significantly impacted vehicle safety. The loss of turn signals made it impossible to properly indicate lane changes or turns to other drivers, increasing the risk of collisions. The failure of the windshield wipers created a serious hazard in adverse weather conditions, reducing visibility. Additionally, the loss of high beam headlights limited nighttime driving visibility. While daytime running lights and hazard lights remain functional, they are not sufficient substitutes for the failed systems. The problem has been consistent and reproducible. Each time the vehicle is operated, the same warning messages appear, including “Turn Indicator System Malfunction” and “Wiper Failure – Service Required,” and the affected systems do not respond. At this time, the issue has not yet been formally confirmed by a dealer or independent service center, but diagnostics codes (including communication-related fault codes) have been observed. The vehicle has not yet been inspected by the manufacturer, law enforcement, or insurance representatives. However, based on the symptoms, the issue appears to involve a failure in the vehicle’s central electronic or communication control systems. Prior to the complete failure, warning signs were present. The Volvo On Call light remained on persistently, indicating a potential underlying electrical or module communication issue. This warning appeared before the loss of the turn signals, wipers, and high beams, suggesting a progressive system failure.Read more
