My vehicle, a 2023 Lexus RZ 450e, has been out of service for over 30 days due to an unresolved HVAC system failure affecting cabin heating. The issue significantly impacts the safe operation of the vehicle, particularly in cold weather conditions where adequate heat is necessary for driver visibility and occupant safety. Despite the vehicle remaining under the manufacturer’s warranty, the deale
... rship has been unable to provide a diagnosis completion or estimated time of repair. The vehicle was brought in for service on February 20, 2026, and has remained at the dealership since that time. During this period, no clear resolution timeline has been provided, and communication from both the dealership and manufacturer has been limited to general status updates with no substantive progress. The lack of heating functionality raises safety concerns, especially given the vehicle’s reliance on electronic systems for climate control. A loaner vehicle has been provided; however, the extended delay without repair or replacement raises concerns about potential defects in the HVAC or related electrical systems. The inability to resolve the issue within a reasonable timeframe suggests a possible systemic defect that is affecting other vehicles of the same make and model. This complaint is being submitted to document the prolonged repair delay, lack of resolution, and safety implications associated with the loss of heating functionality in the vehicle.Read more

My 2018 Volkswagen Tiguan experienced a failure of the airbag control module caused by water intrusion through the HVAC access area and failed seam seal tape at the cowl support. The dealership confirmed that water leaked through a factory seam, accumulated at the airbag module, and caused the module to fail. The airbag warning light is illuminated, and the system is inoperable. This failure matc
... hes the same defect pattern covered under Volkswagen’s recall campaigns for the Atlas and Atlas Cross Sport (water intrusion leading to airbag control module failure), but my VIN is not included. The root cause and safety implications are identical: water entering through a body seam/HVAC area and compromising the airbag system. This is a safety‑critical issue that disables the airbags and creates a risk of injury in a crash. I am requesting NHTSA review this defect for potential recall expansion. Dealership Findings: • Water present at airbag control module • Water leaking through HVAC access hole • Failed seam seal tape at cowl support • Water accumulation damaging the airbag module • Cowl trim cracked and repaired • Recommended replacement of airbag control module and seam sealing tape • Estimated repair cost: $1,377 + tax Safety Concern: Airbag system is disabled due to water intrusion from a factory seam. This creates a risk of injury or death in the event of a collision.Read more

It was not easy but I was able to pull her out. We did not touch the button or try to move the seat until I arrived back home on Saturday night, 1/31/26. My husband pushed the button and was able to place the 2nd row seat back into place. This was really scarry and with all the sensors the Palisade has, I am not sure how this happened. Component/System Failure: The hybrid/electric propulsion system intermittently loses power while driving. Electric output is progressively reduced to approximately 50%, then 25%, and then fully disabled, even when sufficient battery charge remains. The condition appears to involve the hybrid control/electric drive system. The vehicle is available for inspection upon request. What Happened / Safet
... y Risk: The loss of electric propulsion occurs unpredictably during normal operation, including both highway and city driving. The reduction in available power and sudden loss of electric functionality creates a potential safety concern, particularly when driving in traffic or at higher speeds, where reduced or inconsistent propulsion may impact the ability to maintain speed or respond to driving conditions. Additionally, the vehicle is subject to a high-voltage battery recall involving fire risk, which raises concern regarding safe operation and storage of the vehicle. Reproduction / Dealer Confirmation: The issue has been reproduced and confirmed by the dealership (Johnson Auto Plaza, Brighton, CO). The dealership also confirmed that the same issue was reproduced on another Jeep Wrangler 4xe vehicle on their lot. Inspection by Manufacturer / Others: The vehicle has been inspected by the dealership and escalated to Jeep Engineering. The manufacturer has been unable to diagnose the issue due to the absence of diagnostic codes and has not identified a repair. Warning Lamps / Messages / Symptoms: No consistent warning lamps or diagnostic codes are present when the failure occurs. The loss of electric functionality happens without a predictable pattern and without clear diagnostic indicators. Additional Notes: The issue occurs unpredictably under all normal driving conditions, including: Highway driving at speed City driving at low speed Various battery charge levels (low and high) The dealership has suggested possible driver-related causes; UNsupported by car behaviorRead more

My 2025 Buick Encore GX has experienced an ongoing failure of the rear backup camera system. The issue began intermittently in February 2026, and earlier this month (March) it has progressively worsened. The camera now does not function at all. When shifting into reverse, the display often shows a black screen with a “camera unavailable” warning message. On some occasions, the camera briefly
... functions but displays severe distortion, including abnormal colors, flickering, and warning symbols overlaid on the image. The issue continues to occur intermittently and unpredictably. This failure creates a safety concern because the backup camera is a required safety feature intended to assist with rear visibility while reversing. The malfunction reduces visibility and increases the risk of not seeing objects, vehicles, or pedestrians behind the vehicle. The problem has not yet been inspected by the dealership due to limited appointment availability. I have documented the issue with photos and video showing the malfunction, including distortion and loss of image. Approximately 8 weeks after purchasing this vehicle in March 2025 (5/3/25) the Digital Instrument Cluster (driver information center) and infotainment display went completely black while driving, leaving no visible speed, fuel level, or other critical vehicle information visible to the driver. This raises concern about the reliability of the vehicle’s electronic systems, particularly as the backup camera— another critical safety feature— is now also malfunctioning. This issue has progressed from intermittent to consistent failure.Read more

Component/System: Fuel System – Fuel Filler / Left-Side Radiator and Cooling Fan Assembly (subject to NHTSA Recall 25V536). What happened: On June 15, 2024, I was refueling my 2023 Chevrolet Corvette Z06 at a BP gas station in Phillipsburg, New Jersey. As I removed the fuel pump nozzle from the vehicle, a violent explosion occurred. I was immediately engulfed in a fireball. My clothing caught fi
... re and was burning me. I jumped clear of the flames and extinguished the fire on my clothing. I saw that the vehicle remained fully engulfed in flames with the pump nozzle still lodged in the filler. I ran to the adjacent pump island where a station attendant was attempting but was unable to open a locked fire extinguisher cabinet. I broke the glass with both of my bare hands to remove the fire extinguisher. I sustained a severe laceration to my forehead that was approximately 3 inches long and required plastic surgery of over 50 stitches to close. I then ran to the car that was burning with approximately 10 foot flames and I used the fire extinguisher to put out the fire on the vehicle and pump. I then pulled the pump handle from the fuel filler opening and called the police. The police and fire department arrived and confirmed the fire was extinguished. The flames singed the hair on my head and legs. Component availability: Documentation including photographs, police report, and medical records are available. Safety risk: A fuel-fed explosion at an active gasoline pump created an extreme and immediate risk of death or serious injury to me the station attendants and others. Dealer/manufacturer confirmation: GM subsequently issued NHTSA Recall 25V536, acknowledging the defect — fuel spillage during refueling can contact the left-side radiator cooling fan, which remains active even with the ignition off, causing ignition. This recall directly confirms the defect that caused my incident. Insurance inspection: The police and Geico inspected the vehicle. No warnings at all.Read more

2023 Ram 1500 V6 eTorque, 40,000 miles. Two stalling incidents have occurred under different conditions. Incident 1: While towing approximately 4,000 lbs, the 12V battery began slowly draining while the vehicle was in motion. Battery voltage gradually dropped below 10 volts until the engine shut off completely while driving. No warning lights appeared prior to shutdown. Jump start restored normal
... operation. Stored fault codes retrieved afterward showed 16 simultaneous voltage fault codes across every major module including ECU, ABS, EPS, DDM, PDM, CSWM, IFC, PTS and SCCM — all logging system voltage below allowable range simultaneously, consistent with a complete charging system failure. PCM also logged a U0401 code indicating implausible data received, consistent with the PCM losing stable voltage. Incident 2: After approximately 45 minutes of low-speed driving (5-10 mph) in a Walmart parking lot, the vehicle stalled without warning on a busy road in [XXX] . My wife and two young children were in car seats in the vehicle at the time. The engine cut off completely in live moving traffic with no warning. I was forced to exit the vehicle and jump start it in the middle of traffic while my children remained strapped in their car seats, creating a serious and dangerous situation. Jump start restored normal operation. On the subsequent highway drive home, battery voltage was observed declining from 14V to 13.2V over the course of the drive, suggesting the charging system is unable to maintain adequate voltage output. This is a known recurring issue with the Ram eTorque mild hybrid system. NHTSA has an open preliminary investigation into eTorque stalling on 2022 Ram 1500 models. The MGU, BPCM and PCM are all covered under the 8-year/80,000-mile Federal Emissions Warranty verified via VIN. PCM software updates have never been performed on this vehicle. Component(s) Involved: Electrical System, Visibility/Wiper, Structure: Body: Door: Power Access/Locks. Description of the Defect: The front and rear center console "Command Centers" (touchscreens) have suffered a total electrical failure and are completely black. On the 2025 Escalade, these screens have replaced nearly all physical buttons. Consequently, there is no manual override or physical
... backup for the safety and visibility systems hosted on these modules. The vehicle's Global B (VIP) electronic architecture uses a shared high-speed data bus; a failure in these primary command modules creates a high risk of a "cascading failure" that could spread to the 55-inch pillar-to-pillar main instrument cluster, potentially disabling the speedometer and safety warnings while the vehicle is in motion. Describe the Safety Risk (How this could cause an accident): The total failure of these center consoles creates the following immediate hazards: Loss of Visibility (Defroster/Defogger): There is no physical button to activate the windshield defogger. If the windshield fogs over due to humidity or temperature changes while driving, I have no way to clear it. This creates an immediate "driving blind" situation, which is an extreme safety risk at any speed. Occupant Entrapment & Security: The controls for power door locks, window lockouts, and child safety locks are digital and hosted on these failed screens. In an emergency, I cannot verify or manage the locking status of the vehicle, posing a risk to passengers (especially children in the rear). Driver Distraction & System Instability: The failure of these core modules causes significant system instability. Because these nodes are linked to the main 55-inch driving display via the serial data gateway, an electrical short or data loop in the center consoles could cause the main speedometer and driver assistance alerts to freeze or black out without warning.Read more

For the HYUNDAI Ioniq 5 XRT, the ICCU, fuse and perhaps hybrid battery malfunctioned. We were in a disabled car on a 4 lane highway with traffic: On February 10, 2026, as we drove out of our home we heard a loud pop. As we got out onto a large 4 lane road near our house, the notice “Check Electrical System” came on and the car’s power was drastically reduced. We made a U turn and drove (20

Normally this is an extremely busy road with traffic moving at 40-50 mph and we would have had great difficulty even pulling out to the side -- as there is only marginal side space on this road. Hyundai has confirmed to us that the ICCU and a related fuse failed. They also think the hybrid battery is also involved and they are testing to confirm that. We had no warning at all -- except for the electrical warning that came on as the car's power was reduced. The car had charged normally and had had no warning signals/signs at all.Read more

I am writing to formally report a serious and ongoing safety issue involving the Telematics / Data Communication Module (DCM) installed in Toyota vehicles. This issue presents a clear risk to driver and passenger safety and warrants immediate corrective action. The DCM is responsible for critical telematics and safety systems, including SOS Emergency Assistance, Safety Connect, and Remote Connect
... . Failures of this system are not merely inconveniences—they directly compromise emergency response capabilities and vehicle reliability. Documented Failures and Safety Risks: Severe Battery Drain: Defective DCM units drain the vehicle battery even when the vehicle is off, leaving drivers stranded and unable to start their vehicles—potentially in unsafe or remote locations. Loss of Emergency Services (SOS Failure): Malfunctioning DCM systems disable or impair SOS functionality, preventing drivers from accessing emergency assistance during critical situations. This represents a direct risk of injury or death. Loss of Connected Features: Remote Connect capabilities (remote start, unlock, vehicle monitoring) are rendered inoperable, indicating systemic communication failure. System Errors: Persistent errors such as “Connected Device Not Supported” demonstrate loss of communication between the DCM and the vehicle network. System-Wide Malfunctions: In severe cases, DCM failure can disrupt the infotainment system or broader vehicle functions, effectively “bricking” key systems. Demand for Immediate Action: Given the safety implications—particularly the failure of emergency SOS functionality—I am requesting that Toyota take the following actions without delay: Issue a voluntary safety recall for all affected vehicles, or Provide a cost-free repair or firmware update that fully resolves the DCM defect, and Notify all affected owners proactively of the safety risk and available remedy. If this issue is not addressed promptly, I am prepared to escalate this matter through formalRead more

All trailer connections were securely installed, free from any damage, and had functioned flawlessly for days prior. I was forced to continue to my destination without trailer brakes or trailer lights despite being stuck in a substantially strong thunderstorm with heavy rain and low visibility. When I reached my destination, I called my selling dealership to report the issue. Their only suggestion was to "call and complain" to corporate, and that there was nothing more they could do. This incident with truck occurred on March 20th 2026. I had already called the dealership on March 6th after discovering (via NHTSA site) that my truck had been flagged for the trailer brake control recall. The dealership gave me the same response previously. "We have hundreds of trucks but no parts, call back in four weeks". At that point, I had not received any official documentation of a trailer brake control module recall from Ram. I did not receive my official letter from Ram until March 21st, the day after my incident.Read more

I am writing regarding the diagnosis provided for my vehicle [XXX] . I was informed today that the engine failure is being attributed to a "water hose failure" rather than the engine defects covered under the active safety recall/settlement regarding Connecting Rod Bearing failure. As there were no coolant leaks before or after the vehicle was towed to your dealership. I am formally requesting a
... re-evaluation and more detailed evidence. Given the known history of these engines seizing, I need to ensure this diagnosis is absolutely correct and not a symptom of the internal defect. I request the following information be provided immediately: 1. Evidence of Overheating: Did the vehicle store any specific "overheating" codes in the ECU prior to the engine shutting off? 2. Oil Pan Inspection: Has the oil pan been removed to inspect for metal shavings or debris? If the connecting rod bearings failed (the recall issue), there will likely be metallic debris in the oil, regardless of the condition of the water hose. 3. Bearing Clearance Test (BCT): Was the mandated Bearing Clearance Test performed? If not, I request that this be done, or that a teardown be performed to inspect the rod bearings specifically. 4. Causal Link: Can you provide evidence that the water hose failed causing the engine to seize, rather than the engine seizing causing a pressure spike that damaged the cooling system? I have no transportation to work and school. They would not honor a loaner. This has happened since Decemeber of 2025. I await your written response with the requested photos and technical data. My case # with Hyundai Consumer Affairs # XXX Please provide: All relevant photographs Freeze-frame and ECU data Technician diagnostic notes Results of oil pan inspection and/or BCT (if performed) Any physical evidence supporting the stated cause of failure I request this information be sent in writing at your earliest convenience. I am reporting a serious and unresolved safety defect involving a 2024 Jeep Wagoneer Series II 4x4 (VIN: [XXX] ) with approximately 34,000 miles. The vehicle has experienced repeated power steering warnings and functionality issues, and on multiple occasions (approximately three incidents) has unexpectedly shifted itself into park while actively driving. These incidents have occurred since Fall 2
... 025 and continued in January 2026 and now March 2026, demonstrating the issue is ongoing and recurring. The vehicle was in the dealership for over three weeks between January and February 2026, at which time a repair was confirmed before the most recent occurrence. Despite ongoing repairs, the issue has not been resolved and no permanent repair path has been identified. The vehicle is currently back at the dealership with no confirmed resolution. The most recent incident occurred on 3/18 when the vehicle shifted into park while in motion, requiring a restart in the roadway. Shortly after arriving at my destination, the battery died and the vehicle became inoperable and required towing. The combination of electrical issues, including power steering warnings and a dead battery, along with the vehicle unexpectedly shifting into park, creates a significant and unpredictable loss of drivability and presents a serious safety risk. In approximately August 2023 — approximately seven months after purchase — the vehicle’s battery stalled completely while I was driving a few miles from my house on a surface road. The vehicle lost all drive power without warning. I was fortunate that no vehicle was following me or they might have slammed into the vehicle’s rear if they were unable to suddenly quickly stop. I turned on the

2019 Chevy Blazer. Car would not turn off the electrical components when parked. Put car in park and walked away and there is a warning note that says, "Shift to Park." But the car was in park. Called Chevy and they know about the issue on the 2019-2023 vehicles. They sent me the Technical Service Bulletin. Back in 2020, the car had the shifting harness and button replaced and we just had to have
... it replaced again because it is a consistent issue with the vehicle. Except this time, the dealer charged us $668.40 to fix it! It is a known issue by the company and THEY sent me the service bulletin to get it fixed. It should have been covered by Chevy, or, at least a recall should be instituted. The power to the car wouldn't shut off. We were able to jump the car to get it started again.Read more

At various times key can not be removed from the ignition. I understand this to be a common problem on these trucks, but dealer does not agree.
