The vehicle has experienced repeated failures of the integrated telematics system, including loss of GPS location accuracy, loss of data connectivity, navigation malfunction, and intermittent failure of voice command and steering wheel controls. On one occasion, the vehicle experienced a complete display blackout while driving, during which both the infotainment screen and digital instrument clus
... ter went entirely black. The condition has occurred repeatedly and persisted despite multiple repair attempts. The vehicle has been out of service for approximately 82 cumulative days for diagnosis and repair of these issues. An authorized Chevrolet dealership ultimately diagnosed a failure of the telematics control module, which was replaced under warranty. Following this repair, the same data connectivity failure recurred and was documented with photo and video evidence. The vehicle was returned to the dealership for further diagnosis. The failure is intermittent and may not be present during every inspection. The telematics system supports navigation, driver information displays, and emergency communication features, including transmission of vehicle location and system data. Loss of GPS and connectivity may impair the vehicle’s ability to provide accurate location information during an emergency. The prior display blackout resulted in loss of driver information while the vehicle was in motion. The condition has been reported multiple times to authorized Chevrolet dealerships. Diagnostic procedures and programming were performed, and the telematics module replacement confirms a system-level failure. Recurrence after component replacement indicates the condition has not been permanently resolved. Prior symptoms included loss of GPS signal, incorrect location, loss of connectivity, navigation failure, and intermittent driver assistance alerts.Read more

The issues lies in the Hybrid systems electric only function. When climbing grades, the vehicle experiences severe power loss which can be felt by the driver and is displayed by the vehicle on the dashboard information cluster. Power output from the hybrid system drops to or below 50% output even when pressing the accelerator to maximum. This issue occurs almost every time climbing a steep grade
... regardless of exterior temps. After losing speed and power on a steep grade while working the accelerator to or near maximum application the engine is activated abruptly without warning and without the proper hybrid warm up sequence and causes startling shifting by the powertrain and jerks the vehicle. Vehicle: MINI Cooper Clubman (Model R55) VIN: [XXX] Recall: NHTSA 23V-337 Dealer: Flow MINI of Raleigh, NC 1. COMPONENT FAILED: The Junction Box Electronics (JBE) failed due to moisture/water intrusion, producing electrical fault codes across all core systems including airbags, brakes, and a JBE communication fault. The vehicle is located at Flow MINI of Raleigh and is available for inspection.
... 2. SAFETY RISK: Recall 23V-337 identifies that moisture intrusion into the JBE can lead to corrosion, short circuit, and in rare cases a thermal event (fire). My vehicle's carpet is wet, confirming active water intrusion into the footwell where the JBE is installed. Airbag and brake faults appear exclusively when it rains or when the vehicle is washed, directly and unambiguously linking all symptoms to water intrusion. The vehicle is currently unsafe to operate. 3. DEALER CONFIRMATION: Flow MINI of Raleigh confirmed moisture/water intrusion is causing the JBE faults — directly reproducing the failure during a dealer-performed wash. The dealer declined the recall repair solely because [XXX] is not listed among covered states. 4. MANUFACTURER RESPONSE: MINI USA denied coverage exclusively on the basis that North Carolina is not included in the recall's geographic region. The defect itself has not been disputed. 5. BASIS FOR COMPLAINT: The recall's geographic restriction was established based on road salt usage. North Carolina roads do receive road salt treatment during winter weather events. The recall documentation (RCRIT-23V337-7125.pdf, Page 4, Item 12) explicitly names the JBE as a covered component. I request that NHTSA: (1) treat this as urgent given confirmed airbag and brake faults, (2) review whether NC should be included in the recall's geographic scope, and (3) compel MINI USA to perform the recall repair on VIN [XXX] at no cost. I have had numerous safety warning messages display upon startup and also while driving. The messages are usually combined with a cold ambient temperature and do not display during warm temperatures. The FCA dealership has attempted to fix this issue on at least six different occasions so far without any change. To date, FCA has replaced the original battery, they have updated, flashed the ECU mod
... ule, performed NHTSA Recall B4C for flashing the cluster, and most recently they replaced the ORC module. FCA indicated that NHTSA-25V882 was not applicable to my VIN despite my VIN production date in April '25; within your suspect period. Original issue remains, FCA STAR case generated however cannot replicate error condition for FCA engineers. The next scheduled visit to FCA is to replace a telematics module for possible link to issue with the Uconnect5 system. This safety issue was first noted before 800 miles and continues through 2768 miles thus far. As this issue pertains directly to a vital Air Bag sensor issue and the inability to call for a SOS assistance, I expect the NHTSA to raise this concern with FCA USA and to determine the cause of this safety defect and issue a new Recall Notification for repair and/or replacement of affected parts.Read more

On March 20, 2026, while my 2022 Audi Q4 e-tron (VIN: [XXX] ) was parked in my private garage at approximately 98% battery charge, I attempted to start the vehicle and experienced a complete electrical system failure. All electronic systems failed simultaneously, including: brake system (Brake System Failure warning displayed), parking brake sensor, traffic/charging system, power windows, and side
... view mirrors. The vehicle could not be placed into neutral. AAA was dispatched and the vehicle required a jump-start to the 12-volt auxiliary system before it could be moved. The vehicle was towed to Audi Beverly Hills for inspection. The dealership diagnosed the fault as: Control Module Main Processor Performance, Symptom Code 20079, per Technical Service Bulletin TSB 2074484/3. Per that TSB, the dealer was instructed by Audi of America to create a PSS (Pending Service Solution) record, erase the fault memory, and release the vehicle to me with no repair performed. The stated guidance from Audi of America to the dealer was to inform me that this is "an extremely infrequent fault" that is "being reviewed." No fix exists. No timeline for a fix has been provided. The vehicle was released to me with the underlying defect unresolved. This is a known, manufacturer-acknowledged defect with no current remedy. As a fully electric vehicle, the Q4 e-tron's braking and all safety systems are electronically dependent. A recurrence of this fault while driving presents an unreasonable safety risk. I am requesting NHTSA investigate whether TSB XXX /3 and Symptom Code 20079 warrant a formal safety recall. My 2020 Lincoln Aviator Grand Touring Plug‑In Hybrid has experienced multiple high‑voltage powertrain failures that create an immediate safety hazard. The vehicle has twice lost propulsion while driving, displaying “Powertrain Malfunction / Reduced Power” and “Service Engine Soon” before shutting down completely. During the first failure, the vehicle stopped in active traffic and would
... not restart; a police officer stopped to assist and called a tow truck because the disabled vehicle posed a roadway hazard. In addition, the digital instrument cluster has repeatedly gone completely black while the vehicle is in motion, eliminating all visibility of speed, gear position, warning indicators, and hybrid system status. During the most recent blackout, an [XXX] child was in the vehicle with me, significantly increasing the danger. These failures are accompanied by hybrid‑system fault code P0A1E (Starter/Generator Control Module), a pink charging‑port indicator signaling a charging fault, and irregular hybrid‑battery behavior (range not updating, battery not depleting). The vehicle has been inspected four times by authorized Lincoln/Ford dealers and has been out of service more than 34 cumulative days. Although the P0A1E code and symptoms were documented, the dealers repeatedly stated they were “unable to verify” the concern and returned the vehicle unrepaired. Required high‑voltage diagnostics were not completed. The problem is ongoing, intermittent, and has not been corrected. Sudden propulsion loss, dashboard blackout, and high‑voltage control failures put occupants and other motorists at significant risk. 2021 Hybrid CR-V - within the last 10 months the vehicle has twice lost engine power while driving on expressways with just enough time to pull off the road. In the same period of time, at least 6 times, when initially starting the engine, the engine runs roughly and will not accellerate. So far, this has happened in cold or cool weather but not extremely cold weather. In an effort to restart th
... e system, we then shut the system off, wait a few minutes and try restarting. Occaisionally, the engine will start normally after this "reset". More often, if the "reset" doesn't work, we shut off the heat/cooling system (to keep the system from calling for the engine power) and restart the system in EV Mode. The car will begin moving in EV Mode. The engine then starts and runs normally when accellerating beyond the EV system's capacity to accelerate. We understand this problem does not register in the car's computer as a "fault" once the car's engine operation returns to normal. Therefore, the dealers are unable to diagnose the problem. We have read of many other people having the same experience without any satisfactory correction from the dealerships or mechanics. Because this is an intermitant and unpredictable failure, and because the engine and system operates normally for long periods between incidents, the error must be computer/electrical and not related to the fuel pump. We have also replaced the aged engine battery and cleaned the cables. This did not correct the problem. Any assitance in resolving this issue is appreciated. We are concerned that the system will fail completely at the most inopportune time, leaving someone stranded on the highway or being a road hazard on a highway.Read more

I am the owner of a 2026 Hyundai Palisade, purchased new less than 60 days ago. This vehicle is currently subject to a safety recall involving the 2nd and 3rd row power seat assemblies, which reportedly lack sufficient anti-pinch protection during certain seat adjustment functions, including automatic power-folding (stow) and the one-touch tilt-and-slide (“walk-in”) feature. We experienced th
... is defect firsthand in our vehicle. During operation of the second-row seat adjustment feature, the seat continued moving without adequate resistance detection or automatic stop, indicating a failure of the anti-pinch protection system. This created a hazardous condition where an occupant could become trapped or injured by the moving seat mechanism. This issue is especially concerning because these seat functions are located in areas of the vehicle intended for passengers and children. We have an infant who will be occupying these seating areas, and this defect presents a direct safety risk. Since purchasing the vehicle, I have also become aware that this defect has been linked to multiple injuries and the death of a young child. Additionally, Hyundai has issued a stop-sale on affected vehicles, yet current owners are still expected to operate them without a confirmed repair available. At this time, no remedy has been completed on my vehicle, and I do not believe it is safe to operate given the nature of the defect and its location within the passenger seating area. I was told to ask the dealership for a loaner, which the dealership does not have. Additionally, Hyundai said they would cover $60/day in rental fees. A similarly-sized SUV in our area rents at $210/day - a payment discrepancy we cannot afford.Read more

Water began accumulating in the front footwell area of the vehicle, resulting in a soaked carpet and persistent musty odor. The condition developed during normal operation of the air conditioning system, without any external water intrusion (no flooding or open windows). The issue appears to be caused by a failure of the A/C condensate drain system, which is not properly draining water outside th
... e vehicle. Instead, condensation is leaking into the cabin. This creates a risk of mold growth and potential damage to electrical components located beneath the floor, including wiring harnesses and control modules. The problem has been reproducible, especially when the air conditioning is in use. There were no warning lights or alerts prior to noticing the issue. Research indicates that BMW has identified this exact defect in Service Information Bulletin SIB 64 04 16, which states that the condensate drain hose may be improperly installed, kinked, blocked, or damaged during installation, leading to water ingress into the footwell area. This suggests the issue is due to a known design or manufacturing defect rather than normal wear and tear. This condition poses a safety concern due to possible electrical malfunction and reduced vehicle reliability. The affected components are available for inspection upon request.Read more

I am filing a complaint regarding my 2022 Acura MDX Type S with approximately 70,700 miles. The vehicle developed a suspension system warning, and upon inspection by an Acura dealership, I was informed that the suspension stroke sensor and floor wiring harness are corroded and require replacement. I was quoted approximately $12,000 for the repair. This failure is extremely concerning because the v
... ehicle is only a few years old and this type of corrosion affecting a major wiring harness should not occur under normal operating conditions. The affected components are part of the suspension system, which directly impacts vehicle handling and safety. Due to the failure, the suspension system may not function properly, potentially affecting ride height, stability, and overall control of the vehicle. This raises safety concerns, especially at highway speeds or during emergency maneuvers. There was no prior warning of water intrusion or external damage that would reasonably explain this level of corrosion. This appears to be a premature failure of a critical electrical component. Acura has only offered minimal goodwill assistance, leaving the majority of this extremely costly repair as the responsibility of the owner. I am concerned that this may be a broader issue affecting other vehicles and that it could pose a safety risk if not addressed. I am requesting that this issue be investigated. Thank you.Read more

I am reporting a recurring safety-critical failure. On two separate occasions, the vehicle has experienced a total electrical and mechanical shutdown while in operation. During these events, the 12V power system fails completely, turning off all instrumentation and, most critically, the electronic power brake assist. This results in a 'hard pedal' condition where the vehicle is nearly impossible t
... o stop without extreme physical force. The first instance occurred at road speeds, forcing an emergency maneuver into a turning lane to avoid traffic. The second instance occurred while reversing, causing the vehicle to lose all braking and propulsion and roll into a busy road resulting in a near-miss collision. The vehicle is currently unsafe to drive as the shutdowns are intermittent and occur without warning.Read more

I was driving on the highway when all of a sudden my gas pedal stopped propelling the car and the engine seemed to be off. I looked at the dash board and saw that the car was indicating it was off and that I needed to step on the brake and hit the ignition button to turn it back on. I barely had control of the car but was able to pull off to the side of the road. I then had to wait several minutes
