Vehicle experienced multiple safety-related failures at approximately 500 miles. The failures included complete instrument cluster loss (no speedometer or gauges), unpredictable windshield wiper operation, headlight flickering, complete loss of brake lights and turn signals, rear liftgate malfunction, and airbag/seatbelt warning indicators. These issues created unsafe operating conditions, includ
... ing inability to monitor vehicle speed, loss of rear signaling and brake indication to other drivers, and uncertainty regarding airbag system functionality. This presented a risk to both the driver and surrounding traffic. The failures occurred intermittently and in combination, indicating a potential systemic electrical issue rather than a single isolated component failure. The vehicle has been taken to an authorized Mazda dealership and is currently under inspection and repair. The dealership has indicated the instrument cluster requires replacement and is currently on backorder. Full root cause has not been confirmed. Warning indicators, including airbag and seatbelt system alerts, were observed prior to and during the failures. Additional symptoms included erratic system behavior across multiple vehicle functions. Vehicle remains at dealership with no confirmed repair completion date. Component is available for inspection upon request through the servicing dealership.Read more

I am submitting this complaint regarding a serious and ongoing safety issue with my 2017 Honda CR‑V equipped with the 1.5L turbocharged engine. While driving, the vehicle experienced sudden and unexpected power loss, creating a dangerous situation, particularly while in traffic. During this incident, the vehicle also began emitting visible smoke, indicating a serious mechanical or engine-relate
... d failure. At the same time, the dashboard and instrument panel malfunctioned or failed, limiting my ability to receive warnings or diagnose the issue while operating the vehicle. Over the past 23 months, I have brought this vehicle to three different Honda dealerships due to warning lights, abnormal behavior, and ongoing concerns related to performance and reliability. Despite multiple service visits, the dealerships were unable to properly identify or resolve the underlying issue. Temporary explanations or repairs were provided, but the problem persisted. It was only during the most recent dealership visit in the last week that the issue was finally properly identified, after months of unresolved complaints and warning indicators. By that time, the vehicle had already experienced a serious failure involving power loss, smoke, and dashboard malfunction. The prolonged inability to correctly diagnose and repair this vehicle, combined with the sudden power loss and system failures, created a significant safety hazard. This failure could have resulted in a crash and placed vehicle occupants and others at risk. I believe this issue may be related to a broader defect affecting Honda CR‑V vehicles equipped with the 1.5L turbo engine. I am requesting that NHTSA investigate this matter due to the severe safety implications and the pattern of unresolved warnings over an extended period.Read more

The contact owns a 2010 Hyundai Santa Fe. The contact stated that while driving at an undisclosed speed, the vehicle stalled. No warning lights were illuminated. The vehicle failed to immediately start, but started after several attempts. The vehicle was taken to an independent mechanic, who replaced the starter, oil filter, and engine filter, and changed the oil. On a hot day, there was a loud so
... und coming from the vehicle, and it was noticed that the unoccupied vehicle was attempting to start independently. The contact unlocked the vehicle and opened the vehicle door, put the keys in the ignition, and continued to crank for approximately four minutes. The accelerator pedal was pumped, and the vehicle stopped. There was smoke coming from the vehicle, and the vehicle eventually stopped. There were flames coming from the vehicle. The contact called 911, an independent mechanic, her father, and the fire department. The Fire Department was able to extinguish the fire and disconnect the battery. The mechanic determined that the engine cover was burned. There were no reported injuries. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The contact indicated that the failure was related to NHTSA Campaign Number: 11E040000 (Equipment). The insurance company referred the contact to the NHTSA Hotline for assistance. I am reporting a repeated safety-related issue involving my 2021 Jeep Cherokee Overland. The vehicle has experienced ongoing engine misfires and overheating, which create unsafe driving conditions. The issue presents as follows: -Engine begins to misfire, causing rough operation and shaking. -The check engine light flashes intermittently indicating active misfire. -Vehicle performance becomes
... inconsistent, with concern for loss of power while driving. -Vehicle experiencing coolant loss/overheating, concern for engine failure. These conditions create a serious safety risk, as the vehicle may lose power unexpectedly or become inoperable while driving. Multiple repair attempts have been made, including replacement of ignition components (coils and spark plugs) and other related service. The issue continues to recur, and the root cause has not been resolved. Despite repeated attempts to resolve the issue through the dealership and manufacturer, the problem persists and the vehicle remains unreliable. The manufacturer has not provided a resolution despite multiple attempts, and communication is unsuccessful I am submitting this complaint due to ongoing safety concerns and a lack of resolution.Read more

RE: Safety Concern – Instrument Cluster Failure Consistent with NHTSA Recall 24V-652 Dear Sir or Madam: Regarding our 2019 jeep wrangler vin# Milage of 74602.9 on 3-22-26 We are writing to report a safety-related defect involving a 2019 Jeep vehicle exhibiting instrument cluster malfunction consistent with conditions described in NHTSA Recall No. 24V-652 (FCA US LLC). Although this vehicle is no
... t currently included within the recall’s identified VIN population, it is exhibiting substantially similar symptoms, including: Intermittent loss of speedometer function Tachometer irregularities Intermittent or complete failure of the instrument cluster display Recall 24V-652 identifies a defect in the instrument panel cluster (IPC) which may result in loss of critical driver information. As recognized by the National Highway Traffic Safety Administration, such failures may increase the risk of a crash due to the driver’s inability to accurately monitor vehicle speed and operating conditions. Given the similarity between the observed condition and the defect described in the recall, this raises a concern that the issue may extend beyond the currently defined VIN range. Accordingly, we request: A formal review of this vehicle for potential inclusion within Recall 24V-652 or any related service campaign; and Appropriate corrective action consistent with applicable safety and warranty obligations. We will also be submitting this matter to the National Highway Traffic Safety Administration (NHTSA) for further review. Please advise regarding next steps. The high-voltage battery system in my 2023 Mercedes-Benz EQB 250+ is subject to NHTSA Recall 26V-073, the third recall issued for the same battery cell defect. The manufacturer has acknowledged that the two prior recall remedies (25V-050 and 25V-894, both software-based Battery Management System updates) were "failed remedies" that did not eliminate the risk of internal short circuit and th
... ermal event (fire). The manufacturer has instructed all affected owners to park the vehicle outdoors and away from any structure and to limit charging to 80% state of charge until a full battery pack replacement can be performed. The battery replacement parts are not expected to be available until approximately December 2026 — roughly 9 months from the date of this complaint. These restrictions create a significant safety risk and loss of use for my household. I have a newborn infant and no viable outdoor parking option. The vehicle cannot be safely parked in my garage per the manufacturer's own instructions due to fire risk. My spouse cannot safely park away from our residence and transport an infant on foot. The vehicle was previously brought to an authorized Mercedes-Benz dealership for prior recall-related service. The underlying battery cell defect — attributed by the manufacturer to deviations in the supplier (Farasis Energy) production process — has not been resolved despite multiple repair attempts. The high-voltage battery is available for inspection upon request. No warning lamps or messages appeared prior to the recall notification. The defect is an internal cell-level issue that may cause thermal runaway without warning, whether the vehicle is parked, charging, or in operation. Two confirmed fire incidents among affected vehicles in the United States have been reported to NHTSA. I am requesting that this complaint be documented to support the record of this recurring, unresolved safety defect affecting approximately 11Read more

THIS IS MY 3RD COMPLAINT (Ref: Previous Reports). THE VEHICLE IS NOW A TOTAL LOSS. 1. UNRESOLVED ENGINE/STEERING (Prior Complaints): This 2013 Malibu has a documented history of sudden engine stalling and total loss of power steering at highway speeds. Despite reporting this twice to the NHTSA and to GM, the manufacturer failed to provide a remedy. 2. RECENT SYSTEMIC COLLAPSE: Prior to the car bec
... oming completely undrivable in my driveway, the instrument cluster suffered a total blackout (no speedometer/indicators). This matches GM TSB MC-10181662-9999, proving a known design defect in the electrical system/BCM. 3. RECALL DISCREPANCY: In addition to these engine/steering failures, a mandatory federal recall for the Rear Suspension Toe Link (20V764), which I was notified of in 2020, has 'vanished' from the system without being performed. 4. DENIAL OF SERVICE: The dealer is demanding a $1,900 diagnostic fee to even look at these documented, life-threatening safety defects, including the free federal recall. This is a violation of the Maryland Implied Warranty of Merchantability. On January 6, 2026, the check engine light illuminated, followed by a total instrument cluster blackout and electrical failure. The vehicle has been undrivable in my driveway since this total system collapse. This follows years of stalling and steering issues that the manufacturer failed to remedy."Read more

The vehicle's horn becomes completely inaudible during heavy rain, particularly during downpours. This is a serious safety concern as the horn is a critical safety device for alerting other drivers and pedestrians. The issue has occurred repeatedly since the vehicle had approximately 7,060 miles on it and continues at the current mileage of 28,617 miles.\n\nThe issue was first reported to the manu
... facturer's service center on February 20, 2025, at 7,060 miles. The service center documented the concern as the horn not working after rain. A Low Voltage Circuit Integrity Check and miscellaneous labor were performed (Invoice #3000S0012397168). However, the manufacturer's service center stated the horn was \"working as designed\" at the time of inspection and advised the owner to monitor the issue and record video. The problem was not resolved.\n\nThe issue has been reported to the manufacturer's service center on multiple subsequent occasions, but they have been unable to reproduce it because the horn functions normally in dry conditions. The defect only manifests during rain and downpours, making it difficult to demonstrate on demand at a service center.\n\nThis is a significant safety hazard. During heavy rain, when visibility is already reduced and driving conditions are more dangerous, the horn -- which is one of the most basic and essential safety devices on any vehicle -- is rendered completely nonfunctional. The owner feels powerless in dangerous situations when the horn cannot be heard. The horn is required safety equipment under FMVSS 571.108, and a horn that fails during adverse weather conditions represents a serious defect.Read more

I am writing to report a serious safety concern involving my 2024 Chevrolet Silverado that resulted in a child being locked inside the vehicle with the key fob present inside the car. On March 25 2026, after parking and turning off the vehicle, I exited the driver’s seat with my key fob inside my purse, which remained in the front of the vehicle. I then walked around to remove my daughter from
... the back seat. During that time, she climbed into the front seat and pressed the interior door lock button, locking all doors. My daughter has autism and is not able to follow instructions or unlock the vehicle on command. As a result, she was locked inside the vehicle with the key fob present inside, and I was unable to gain access from the outside. The vehicle did not prevent the lock from engaging, nor did it allow me to open the door despite the key being inside the vehicle. This situation required emergency assistance (911) to safely remove her from the vehicle. I understand that many vehicles have “lockout prevention” features; however, in this case, the system did not account for a vulnerable occupant inside the vehicle. Allowing the vehicle to be locked from the interior while the key fob is inside—and without any external override—creates a significant child safety risk. I am requesting that Chevrolet review this design behavior as a potential safety issue, specifically: - Why the vehicle allows interior locking when a key fob is detected inside - Why there is no external override to unlock the vehicle when the key is present - Whether a software update or safety feature enhancement can address this risk This is not just a convenience issue—it is a safety hazard that can lead to dangerous outcomes, especially for children or individuals with disabilities. Please confirm receipt of this concern and advise whether this issue has been previously identified or is under review.Read more

I am filing this complaint regarding an unresolved safety recall (Recall 78C) on my 2024 Jeep Grand Cherokee 4xe, which has now been open for approximately 139 days without a remedy. This recall involves engine manufacturing contamination that may lead to catastrophic engine failure, loss of propulsion, and potential fire risk. Despite the seriousness of the recall, Stellantis has failed to provid
... e a remedy and has given conflicting and unclear guidance regarding safe operation. At one point, I was instructed to operate the vehicle using the gasoline engine only. I was later told Stellantis cannot provide technical guidance and to contact the dealership. The dealership has stated they have no clear direction from Stellantis. As a result, I have no clear understanding of whether it is safe to operate the vehicle, use electric mode, or rely solely on the gasoline engine. Restricting use to the gasoline engine also prevents use of the plug-in hybrid system, meaning the vehicle cannot be used for its intended purpose, resulting in increased fuel costs over several months. I have also incurred out-of-pocket rental car expenses during recall service. Stellantis indicated reimbursement would be available but has not provided a process or responded to follow-ups. Given the nature of the recall and lack of guidance, I cannot determine if this vehicle is safe to transport my newborn child. At this point, I am concerned with both the unresolved defect and the manufacturer’s failure to provide consistent safety direction, communication, or a path toward resolution.Read more

On March 5, 2026, while I and my 2 passengers were traveling on the 405 Freeway at approximately 40–50 mph, the vehicle suffered a catastrophic total loss of power. The engine shut off and all electronic systems failed simultaneously, including the instrument cluster and all interior lighting. Most critically, the hazard lights were inoperable, leaving the vehicle invisible to high-speed traffic
... during the failure. Since I was traveling at highway speeds I was able to safely glide the vehicle to the center shoulder. After approximately 10–15 minutes, the dashboard, infotainment, and climate control systems began flickering intermittently before power temporarily returned. Upon attempting to re-enter traffic to reach an exit, the vehicle stalled a second time while in motion. I returned to the shoulder again. After another 10 minutes the vehicle started and I was eventually able to exit the freeway and reach my residence. The following morning, the vehicle was completely unresponsive and would not start. It was towed to Land Rover Santa Monica dealership on March 6, 2026, where it currently remains for diagnosis and repair. This incident on the 405 freeway marks the third occurrence of a total loss of propulsion and electrical failure while the vehicle was in motion. Despite previous repair attempts, the defect persists, rendering the vehicle fundamentally unsafe for operation. I am gravely concerned for my personal safety, as well as the safety of other motorists on public streets and highways. A vehicle that spontaneously stalls at high speeds—while simultaneously losing the ability to activate hazard lights—presents an unacceptable risk of a high-speed rear-end collision. Given that this is a recurring, catastrophic safety failure, I am requesting an immediate and permanent resolution, as the vehicle in its current state is a liability to public safety and unsafe to be driven by me or anyone.Read more

The vehicle has now been out of service for approximately 71 days. Most recently, the authorized dealership confirmed that the required parts to complete the repair are unavailable, there is no estimated timeline for parts availability, and the repair cannot be completed at this time. As a result, there is currently no repair plan in place. This situation raises safety concerns due to the loss of a critical safety feature (rear visibility via the backup camera) and the presence of unresolved electrical system damage. Damaged or compromised wiring may create the potential for electrical malfunction, system instability, or other hazards if the vehicle were to be operated. The inability to repair the vehicle, combined with the lack of available parts and absence of any repair timeline, raises concerns about the overall safety and reliability of the vehicle. The vehicle remains at the dealership and is available for inspection.Read more

The vehicle developed a leak in the AC condensate drain. The leak is directly above the safety restraint control module. The module got wet deactivating the vehicle and causing havoc with the airbag system. Many electrical systems turned off such as the power to the steering wheel buttons( the left blinker, the right blinker, and the horn). Luckily this happened while my vehicle was parked but i
... f this happened while someone was accelerating or driving on the highway they would lose control of the vehicle. I spoke to a Tesla Service Tech about the concern and he informed me that this AC leak happens very often in the 2021 and newer Tesla Model S vehicles and that Tesla is aware that the AC drain leaks onto the Restraint Contol Module but has not created a technical service bulletin or a recall for the safety issue.Read more

I bought the 2007 Honda CRV in 2022. Approximately 1 month ago my keys were locked inside. 3 reputable locksmiths could not unlock any doors. My extra fob or extra key will not unlock any doors. The passenger door had been making a ticking then grinding noise when it locks itself. All 3 locksmiths said this a serious safety problem and the actuator is at fault. The only way to get in was to retrie
... ve my set of keys out of the wedged door. I haven't received any recalls on actuators but there was a class action in 2015-2016 (which I was not the owner then) that stated ALL 2007, 2008 and a few 2009 Honda CRVs were to have the actuators replaced at Honda expense. I'm afraid that if my disabled son is inside and it locks itself, we won't be able to get him out. Please assist me in this matter.
