Incident 1: While driving my 2024 Mazda CX-90 PHEV at low-to-moderate speed, the vehicle suddenly and completely locked up without any driver input. There was no gradual slowing or normal braking behavior. Instead, the vehicle went immediately from moving to an abrupt, forceful stop, as if maximum braking was applied instantly. There were no warning lights, alerts, or audible signals before or dur
... ing the event, and there were no obstacles or vehicles in front of me. Incident 2 (approximately 6 months later): A second, nearly identical event occurred while I was accelerating or cruising. The vehicle again immediately locked up and came to a sudden stop with no warning and no driver input. There was no progressive braking or deceleration. The stop was instantaneous and extremely forceful, causing me to lurch forward into the steering wheel. There were no hazards or objects present that would justify emergency braking. Additional Details: Both events involved an immediate, full-force stop with no transition or gradual braking. There were no dashboard alerts, warning sounds, or messages during either incident. The behavior did not match any known system alerts (such as door open or limp mode). The dealership was unable to reproduce the issue and found no diagnostic trouble codes. This creates a serious safety risk, especially the risk of being rear-ended during normal driving.Read more

I am writing to formally document a serious, recurring safety defect in ou 2026 Hyundai Tucson Hybrid, purchased less than a month ago. The vehicle has now exhibited multiple distinct dashboard warning faults related to the electronic shifter system, has left me stranded on more than one occasion, and has been sitting at Schomp Hyundai in [XXX] , for the second time, for over one week, with no res
... olution. The dealership has been unable — or unwilling — to properly diagnose the issue despite photographic documentation of the active fault warnings and the existence of Hyundai-issued Technical Service Bulletins directly addressing this problem. This letter serves as formal written notice pursuant to Colorado's Lemon Law (C.R.S. § 42-10-101 et seq. Since taking delivery, I have experienced multiple occurrences of two distinct dashboard fault warnings on this vehicle. Both have been photographically documented with date-stamped images: • "Shifter System Malfunction! Service Immediately" — The vehicle became inoperable and could not shift gears, leaving me stranded. "PARK Malfunction. Engage parking brake when parking vehicle." — A secondary fault indicating failure of the electronic park system, creating a serious safety hazard. These faults are not related to any collision or external damage. They are pre-existing manufacturing defects that began occurring in the first week of purchase. Notably, this is a known and documented issue affecting Hyundai Tucson Hybrid models. Hyundai has issued Technical Service Bulletins TSB #21-AT-007H and TSB #21-AT-009H. I drove the vehicle approximately 8 times (including to and from the repair center) and have had the issue multiple times.I am formally requesting the following immediate actions: • Direct the dealership to perform a complete GDS historical DTC scan per TSB #21-AT-007H and #21-AT-009H and provide full written results. This vehicle is subject to NHTSA Recall 25V-655 involving the high-voltage traction battery. The recall restricts the vehicle from using DC fast charging (Level 3), which significantly limits the safe and practical operation of the vehicle for its intended use. My household must routinely complete 300+ mile one-way trips on a weekly basis. Without DC fast charging capability, the vehicle cannot r
... eliably complete these trips, creating a risk of being stranded or unable to safely reach destinations. Reliance on Level 2 charging requires extended delays of 6+ hours, which makes the vehicle impractical for its intended use and inconsistent with reasonable consumer expectations for normal operation. I contacted Nissan North America Consumer Affairs and was directed to work with the local dealer. I then contacted Dick Hannah Nissan in Gladstone, OR, who redirected me back to Consumer Affairs. As a result, no party is taking responsibility and no remedy or interim solution has been provided. The manufacturer has issued a safety recall but is unable to provide a repair, timeline, or mitigation. The vehicle’s primary propulsion system is effectively restricted, and the vehicle cannot be used as intended for necessary travel. I have requested a loaner or comparable replacement vehicle while awaiting a remedy, but this has not been provided. This unresolved recall creates an ongoing safety and usability issue, and the manufacturer has failed to provide a reasonable interim solution.Read more

RF Module lost connection to CAN Bus and caused the vehicle to shutoff while driving. Symptoms include messages about key fob not within range and TPMS not able to communicate with sensors. RF Module was replaced however the issue returned. Other symptoms include parasitic draw caused by RF Module that in turn causes the system to report low voltage while driving. Vehicle has left me stranded on m
... ore than one occasion due to RF Module failures. These situations where preceded by no starts and shifter errors. Most of these issues can temporarily be resolved by disconnecting the main battery for extended periods. Dealer or service center has not reproduced the issue. Dealership inspected the RF module during installion of a new RF module after a water leak. The issue first appeared after the dealership repaired the "backlite" window, which was leaking on to the RF module. I suspect this might be related to system updates installed during the window installation service call. All of this started in early 2024.Read more

I am the original owner of a 2020 Jeep Wrangler. My instrument cluster has completely failed. I know that there is an active recall for this exact issue (30B) but my VIN number is not included. This is clearly a safety defect as my cluster is non-functional, I do not have a speedometer or warning indicators.i have no idea how much or how little gas I have, if my tire pressure is low…I am the ori
... ginal owner, as stated ,and this is not normal wear. I am also aware that there is an open investigation into this recall and I believe my Jeep should be included in the recall. Prior to the cluster going out, there was not any warning. I started the jeep and it was working and as I was driving it went off. It never came back on. We checked connections and fuses, it’s not that. My husband has addressed it with two separate dealerships and was told there was nothing they could do and offered no suggestions. I went today and spoke with a new dealership and they put me in contact with Jeep Customer Care. I have begun the process but will most likely still be paying out of pocket with money, I frankly do not have. This is a safety issue and should be taken care of by Jeep, and is being covered for some by the recall.Read more

Their own official inspection report R/O 101930 dated March 28, 2026 confirms: ‘REMOVED OIL PAN AND INSPECTED FOR BEARING DEBRIS. FOUND SMALL AMOUNT OF METAL SILVER COLORED.’ The report also documents my reported intermittent power loss and references two photos of the debris taken during inspection. Despite finding documented metal debris consistent with recall R25B2 bearing failure, Infiniti Roseville is denying engine replacement claiming silver colored debris does not qualify. Nissan’s published recall criteria makes no such color distinction — it states metal debris qualifies for engine repair or replacement at no charge. Warning symptoms began December 5, 2025 including headlight flickering, high pitched engine whistling, VDC warning light January 25, 2026 — all documented with timestamped photos and video. I am reporting multiple safety-related issues involving my 2016 Chrysler 200 S. On 02/06/2024, after an oil change at a dealership, the underbody skid plate bolts were not reinstalled. While driving, the skid plate fell and I lost control of the vehicle, creating a serious safety hazard. No warning indicators were present prior to this failure. The issue was identified immediately after leaving th

In July 2022, following a battery replacement by the same dealership, the vehicle repeatedly lost power and stalled, leaving me stranded multiple times. I paid for diagnostic testing, but there is no documentation confirming proper diagnosis or repair. I was later advised incorrect hardware had been installed. On 02/27/2026, I was told the check engine light was on and diagnostic codes were cleared without repair. There is no documentation of the codes or repairs performed. I was also informed on 11/19/2022 that parts were ordered for a safety recall. From that date until 02/27/2026, I was repeatedly told parts were unavailable. I scheduled repair and dropped off the vehicle on 03/13/2026. I received notice that repair had begun, but later discovered the vehicle had not been serviced at all. No explanation was provided. Additionally, service records are inaccurate. A vehicle history report shows service on 03/17/2026 at a dealership I have never visited. The vehicle was not serviced anywhere on that date. The power liftgate on my 2024 Toyota Sequoia intermittently fails to close using the automatic function. When pressing the close button (either on the liftgate or key fob), the system beeps twice and does not initiate closing, requiring manual force to shut the liftgate. Additionally, the pinch sensor appears to malfunction, falsely detecting an obstruction when none is present. This creates an i
... nconsistent and unreliable operation of the liftgate system. This issue presents a safety concern because the liftgate may not properly secure while driving, increasing the risk of it opening unexpectedly. Furthermore, the failure of the pinch sensor system raises concerns about potential injury, especially to children or passengers, if the sensor does not function correctly during operation. The issue occurs intermittently and has been difficult for the dealership to permanently resolve. This appears to be a broader defect rather than a one-time failure.Read more

I am reporting a persistent and escalating electrical safety defect in a 2021 Ford Explorer. On February 28, 2022 (at approximately 10,542 miles), I formally reported a strong burning metal smell and the engine would be loud and idle high, there was abnormal noises and unusual electrical changes to the dealership (West Herr Ford). The dealer service advisor documented no problem found and told me
... not to worry about it. Over time, this unresolved electrical short and metal smell has resulted in a total system failure. In February 2026, the vehicle suddenly suffered a complete ‘black screen’ (APIM/infortainment failure) and a no-start condition. Most dangerously, the battery suddenly drains within days, which can lead to total loss of vehicle power while in motion, disabling power steering, braking assists and you can’t even put the car in neutral when this happens. I am gravely concerned that this unaddressed metal burning smoking/ electrical short issue poses a significant potential fire risk to myself, my passengers and other drivers. The same dealership has now had this car in there possession for 32 days and admits that what has happened is what happens while they have it and that they cannot find the root cause. Yet the previous service advisor from 2022, and the current service advisors ( gave the car back on 2/21/2026 and 3/6/2026 saying their was no problem) allowed the vehicle to remain on the public roads despite documented fire hazard symptoms.Read more

The vehicle (2025 Kia Carnival Hybrid) has suffered from a systemic parasitic 12V battery drain since the first month of ownership. The first occurrence was July 20, 2025, requiring a jump-start from Kia Roadside Assistance. On 02/26/2026, the vehicle was presented at Moritz Kia Alliance for the same issue. The dealer diagnosed the 12V battery as defective (Code R04) and replaced it under warrant

I had to utilize Kia Roadside Assistance for a jump-start and drive the vehicle directly to the dealer without turning the engine off to ensure it could be serviced. This marked the second official repair attempt for this recurring defect. The vehicle has remained in dealer possession since March 10 (21 consecutive days). On March 11, only one day after drop-off, the Service Director threatened that the vehicle would be "impounded" after 60 days, despite the vehicle being the subject of an active Texas DMV Lemon Law Case (#26-0012638.CAF). On March 26, a critical safety failure occurred: during a start-up attempt, the Integrated Power Brake (IPB) system malfunctioned, causing the brake pedal to pulsate and physically "kick back" with significant force against my foot. I have serious concerns that this braking failure could occur while in motion, posing an unreasonable risk of high-speed collision, serious injury, or death. No root-cause diagnosis has been made. The unexpected loss of propulsion significantly increased the risk of a collision. The nature of the failure appears to be consistent with **internal engine component failure (such as connecting rod bearing failure)**, which is not typical for a properly maintained modern engine at this mileage. Importantly, my vehicle was manufactured in **November 2016**, placing it within an early production period for this engine. There is evidence from technical service information and consumer reports suggesting a **pattern of premature internal engine failures** in this engine family, particularly involving **rod bearing wear leading to catastrophic failure**. This raises concern that the issue may represent a **systemic defect** rather than normal wear and tear. I am concerned that other vehicles with this engine may experience similar failures **without sufficient warning**, potentially resulting in dangerous driving situations, especially at highway speeds. I am requesting that NHTSA: • Review this incident as a **potential safety-related defect** • Evaluate whether similar complaints exist involving the **3.3L Kia Lambda V6 engine** • Determine if an **investigation or recall** is warranted I am able to provide supporting documentation, including maintenance records, diagnostic findings, and repair estimates upon request.Read more

At 31,000 mile my horn is no longer working along with my steering wheel controls on the left side of the wheel. Upon researching online it seems these truck have a common issue with clock spring fail. This is concerning because not only does the clock spring provide power to the horn and steering wheel controls, it could also effect the airbag function.

When going over bumps such as speed bumps or bumps in the road there has been a clunking noise and sometimes a grind, for a few seconds. With this the ABS light also comes on. Then goes off. This has went on for the past 6-7months. Its not every time,it is very random. It will do it and not again for a while. On 3/16/26 the SVC FWD warning light came on while driving. After restarting it it take
... s about 30seconds and it comes back on and stays on until the vehicle is shut off. There has been a couple times the vehicle has hesitated to go forward when the gas was pressed. I have taken it to an auto shop and they found code: C14A7-97 for PTU failure. They recommended to contact Jeep since there is a recall for that year. When the dealer checked my VIN it does not fall under the recall. I am afraid of driving it as I don't want it to stall on me or cause an accident as I usually have my young child with me. The shop said it would be around $6000 to repair. If not under the recall.Read more

The engine of vehicle repeatedly failed, causing the vehicle to stall while driving, which posed a serious risk to my safety and others on the road. The vehicle had approximately 65,000 miles and was maintained routinely and on schedule. The failure first occurred before November 18, 2025, when I reported stalling issues to Kia authorized service & parts. The vehicle was returned as “fixed” on
... December 12, 2025, but the engine stalled again during normal operation. The vehicle was returned to Kia authorized service & parts multiple times (December 12, December 18, December 23, January 9, January 12, 2026, and March 4, 2026), with repeated assurances that it had been repaired. Each time, the car continued to stall, including during a test drive at Subaru, demonstrating that the failure persisted despite repeated dealer intervention. The stalling created a significant hazard, as the car could lose power while on public roads, including highways, potentially leading to accidents. There were no consistent warning lights or messages prior to the engine stalling; the first symptom was intermittent loss of power while driving. The failure has been reproduced and observed by multiple service personnel at Kia authorized service & parts and by Subaru during the trade-in test drive. The component(s) causing the failure were inspected by Kia authorized service & parts staff, but no effective repair was made. Documentation of repair orders and dealer inspections is available for review.Read more

I am reporting a safety defect involving my 2016 Jaguar XJ. The driver-side door cannot be opened from either the inside or the outside. This failure occurred without warning and has made the vehicle unsafe to operate. This issue presents a serious safety risk. If an emergency occurs, such as a crash, fire, or other urgent situation, the driver may be unable to exit the vehicle quickly. Being trap
... ped inside the vehicle due to a non-functioning door creates a dangerous condition and increases the risk of injury. The vehicle has been used under normal conditions, and there were no prior indications that the door latch, lock, or electronic mechanism would fail. A complete loss of access to the driver door is not expected and suggests a potential defect in the door latch or locking system. This problem may not be isolated and could affect other vehicles of the same make and model. I am requesting that this issue be investigated as a potential safety defect that may warrant further review or recall. A vehicle door that cannot be opened from inside or outside compromises basic safety and emergency exit requirements.Read more

Nissan Recall R25C8 (NHTSA ID: 25V655), Incomplete, remedy not available Primary Issue: High-voltage battery fire risk during Level 3 (DCFC) charging due to excessive lithium deposits. I have been told by Nissan not to use Level 3 fast charging. This has significantly impaired the utility of my vehicle due to: - Limited Range/Utility: Without fast charging, I can't use the car for trips beyond i
... ts immediate single-charge radius, basically turning a long-range EV into a local-only car. The proposed software may limit charging or prevent the vehicle from restarting if it detects a dangerous condition, rather than permanently fixing the defect. - Time Loss: I can't charge at home unless (only overnight & if weather permits) I run a cable out of my apartment window, across the grass & sidewalk for a trickle charge (Level 1). Otherwise I have to drive to a Level 2 charger & wait many hours to charge up. This is significantly longer, makes it impossible to quickly 'top off' as advertised. For comparison: L3 (DCFC) gets me at least 15% in about 15 minutes (60% per/hour); L2 only gets 11% per/hour. L1 only gets 2% per/hour. Without DCFC this car is almost useless. - Diminished Value: Nissan sold me a $38,000 car & I'm concerned that a software-only 'fix' that may throttle charging speeds or simply disable the car if it detects heat will permanently reduce the vehicle's market value. I actually expected to keep this car for the rest of my life, but the CHAdeMO system (which appears is becoming un-supported) that also lacks a cooling system, makes that very unlikely. - Impact to income due to part-time employment that requires travel, which the LEAF was capable of doing with DCFC stops. Now, I need to rent a vehicle for these events. Purchased Cherry Hill Nissan March 11, 2022 2/3/2026: Nissan Service appointment. They are still unable to fix the recall issue, whether via software or hardware. 3/18/2026: I received another safety recall notice. Still no solution.Read more

I am reporting a safety concern involving a 2019 Chevrolet Malibu. The vehicle was previously involved in accidents prior to my purchase and had known mechanical issues, including a gear shifting problem that was not properly repaired. In February 2024, while the vehicle was insured under Ocean Harbor Casualty Insurance Company, it sustained a significant front-end collision impacting the driver
... side. The impact was severe enough to cause structural damage, including a dent to the vehicle frame. Despite the severity of the collision, the airbags did not deploy at all. Based on the extent of the damage, airbag deployment would reasonably be expected. Prior to this incident, the vehicle had ongoing issues following repairs, including system errors and concerns that repairs may not have been properly completed by a certified facility. This raises concern that the airbag system may have been defective, improperly repaired, or compromised prior to the accident. I was not informed of any airbag or safety-related defects at the time of purchase from HGreg. I am submitting this complaint due to concerns of a potential safety defect involving failure of the airbag system during a serious collision.Read more
