Vehicle: 2019 Acura RDX (Technology Package) I am reporting a serious safety issue involving sudden loss of engine power while driving. On March 14, 2026, I filled my gas tank before traveling approximately 1.5 hours out of town. The vehicle operated normally that day. The following morning, during snowy and icy conditions, while accelerating in traffic, the vehicle suddenly lost power and stall
... ed. Multiple warning lights illuminated on the dashboard (“Christmas tree” effect), and the steering became very stiff, making it difficult to safely maneuver the vehicle out of traffic. I was able to restart the vehicle, but warning lights remained on. The next day, while driving on the highway under normal conditions, the same issue occurred again. While accelerating, the vehicle suddenly lost power and stalled in traffic, creating a dangerous situation. In response, the fuel pump inside the gas tank was replaced; however, the problem persists. The vehicle continues to display warning lights and still loses power when accelerating. I contacted an Acura dealership after seeing reports of a fuel pump recall for similar vehicles, but I was told my VIN was not included. Despite this, the symptoms match known fuel pump-related issues. This defect poses a serious safety risk, especially when driving at highway speeds or in traffic, as sudden loss of power and steering assist can lead to a crash. No injuries have occurred yet, but the issue is ongoing and unresolved. I am requesting investigation into this issue as a potential safety defect.Read more

Shift to Park. Unable to turn the car off even though the car is in park. This was previously fixed in 10/24 and started up in 10/25 a few days after the 1yr warranty. I reached out to the dealership that repaired it in 2024 and was told that I would have to pay for the service as the part is not covered under the powertrain warranty. I reached out to GM for assistance and that was a lost cause. I
... was told that the repairs will only last 1 to 2 yrs, so this appears that it will be a temporary fix. My car has about 55000 miles on it. To know that I may possibly have to continue to get this fixed, I feel like this should be considered a lemon and they should refund me for the cost of the vehicle or give me another car. GM knows that the Shift to Park is an ongoing issue with this year, make, model and have failed to issue a recall.Read more

My 2023 Tesla Model Y suddenly became inaccessible via key card and phone key with no prior warning. The tow operator successfully jump-started the vehicle with a 12V jump and it powered on — suggesting a 12V battery failure, not an HV system failure. Tesla diagnosed water ingress in the HV battery pack and HV harness, citing an isolation resistance fault and open flood port. Their conclusion es
... calated from "water ingress" to "vehicle submerged," with an $11,000 battery replacement recommended and warranty denied. I am disputing this. The vehicle has never been submerged or exposed to flooding. The only physical evidence provided was clean water droplets on a single HV connector — no waterline staining, no sediment, no debris, no pin corrosion. No ingress pathway (seal, gasket, or breach point) was identified. The flood port mechanism was not explained and may activate from pressure changes or seal failure — not exclusively submersion. The vehicle powering on after a 12V jump is inconsistent with catastrophic HV failure from submersion. I believe this may represent a manufacturing defect in the HV battery pack sealing system allowing moisture intrusion under normal conditions. Tesla has classified this as owner-caused damage without sufficient physical evidence. If similar cases exist across 2023 Model Y vehicles, a systemic defect may be present. Safety risk: sudden, complete vehicle shutdown with no warning — potential loss of propulsion while driving. Vehicle is currently at Tesla Service, undisassembled, and available for inspection.Read more

My 2025 Audi RS3 unexpectedly shifts into park and engages the electronic parking brake while the vehicle is in motion at low speeds. This issue has occurred multiple times, has been documented on at least nine occasions, and has undergone four (4) separate service attempts within the first 900 miles. The issue remains unresolved. On multiple occasions, the vehicle became immobilized in active t
... raffic. In one incident, the vehicle shifted into park while making a left-hand turn across a busy intersection, leaving the vehicle stopped in the roadway with oncoming traffic approaching at high speeds. The vehicle would not move and required the driver to exit and perform a full restart before normal operation resumed. In another incident, the vehicle again shifted into park while exiting a parking lot and entering traffic, leaving the vehicle partially in the roadway and creating a hazardous condition for the driver and oncoming traffic. A fault related to the electronic parking brake system was identified and a component was replaced; however, the issue persisted after repair. The dealership has confirmed that the issue can be reproduced but stated that the vehicle is operating as designed. The explanation provided was that shifting body weight while seated and properly restrained may cause the system to behave as if no driver is present. The dealership also indicated that similar behavior could be reproduced on other vehicles during testing in a parking lot environment, rather than under normal driving conditions. Despite this explanation, the behavior results in an unexpected loss of vehicle mobility in traffic, which presents a serious safety risk. There were no consistent warning indicators prior to the events. The vehicle remains available for inspection.Read more

Sudden loss of propulsion at highway speeds - 2023 Escalade. We easily could have been killed. I drive a 2023 Cadillac Escalade that I purchased from Motor Werks Cadillac in [XXX] . It was very expensive. The car has under 33k miles on it, off memory.I informed the dealer I would be taking the car on a family vacation road trip from [XXX] to [XXX] (for spring break), and requested they perform a
... ny necessary preventative service on it to ensure it was in perfect shape to make the trip. They performed the service (basic air filter change etc) and returned the car to me two days before I packed my family [XXX] and [XXX] daughters and wife) into the car and began the journey. After driving less than three hours, the engine completely and suddenly died, without warning, while we were traveling at 75 miles an hour on the highway in the left lane in the middle of rural [XXX] . At the same time all of the electronics in the vehicle died as well. It was an extremely dangerous situation because the road is heavily trafficked by semi trucks, which I was barely able to avoid as I attempted to coast across all the lanes of traffic to get onto the shoulder of the road on the right hand side. Because the car was without propulsion, I was not able to get fully onto the shoulder, but mostly. This left the car in a precarious position because the semi's that we're driving by could easily have hit the car had they not been careful and veered slightly to the left. We easily could have been killed. There's more to the story, like the dealer telling me Cadillac is aware that catastrophic engine failure at highway speeds is a known issue impacting Escalades around the country, and including significant mental trauma that my children experienced by being stranded on the side of the road for several hours in the dark in a car that had no engine, no electronics and no heat. I own a 2023 Ford F-150 Lightning, with 48,200 miles and 2.5 years of ownership. Recently, my truck began displaying a “Powertrain / Reduced Power” warning while driving, and the air conditioning stopped producing cold air. The dealer scanned the vehicle and found DTC P0534 – A/C System Charge Loss. Further diagnostics indicated that multiple vehicle modules may require reprogramming and sof
... tware updates. The dealer stated they need to evacuate and recharge the AC system, add dye, and remove front trunk components to locate the leak. This AC system is part of the high-voltage battery and eDrive thermal management system, meaning that failure of the HVAC system affects battery cooling and powertrain performance. Loss of cabin cooling is a symptom of a thermal system malfunction, which can lead to battery overheating, powertrain derating, and reduced driving performance. The combination of unexpected reduced power and loss of cabin cooling creates a safety risk, particularly during highway driving, merging, or situations requiring acceleration. These failures occurred at relatively low mileage and are associated with the high-voltage EV system, which should be functioning properly to ensure safe vehicle operation. I am submitting this complaint to report a potential safety defect with the 2023 Ford F-150 Lightning affecting the battery thermal management, high-voltage AC system, and eDrive components, which can result in sudden power loss and compromised vehicle safety.Read more

I am submitting this complaint regarding an active fire‑risk recall on my Mercedes‑Benz EQB and MBUSA’s refusal to provide a safety remedy. My vehicle was previously placed under a no‑drive instruction due to the fire‑risk recall. MBUSA attempted a remedy, but that remedy failed, and the safety impairment remains. Despite this, MBUSA Customer Care has declined to confirm or deny a safet

These goodwill measures do not address the underlying safety defect. I have repeatedly asked MBUSA to confirm whether they are denying a safety‑related repurchase under the active fire‑risk recall, the prior no‑drive instruction, the failed remedy attempt, and the ongoing safety impairment. They have refused to answer this question and have stated there is no further escalation within their department. I am requesting NHTSA’s assistance in reviewing this matter, as the manufacturer has not provided a safety remedy and is refusing to address the safety‑related repurchase question. See attached document for complaint. Toyota Motor North America, Inc. has failed or is unable to remedy the safety recall (NHTSA Recall No. 25V-636), issued on 26 September 2025, for my 2021 Toyota Supra within a reasonable period of time. According to the non-dated letter from Toyota, the recall is for a defect in the engine starter relay that may catch fire without warning “during vehicle ope
... ration or when the vehicle is parked and the ignition is off” (Toyota Recall No. 25TA09). The letter further states that “owners are advised to park their vehicles outside and away from structures until the recall remedy is complete” and that “the remedy can be available in the last quarter of 2025.” This defect is serious. For safety I am afraid to drive the vehicle, because it could catch fire without warning. The vehicle is parked outside in the winter weather, instead of in my garage. The promised date for a remedy has passed (last quarter of 2025) with no further information from Toyota. I contacted Toyota via telephone on 3 December 2025 and again on 13 January 2026, without resolution. I sent letters to Toyota Vice Presidents on 15 January 2026 (copy attached), but I have had no response to date. Currently Toyota has not indicated when the necessary repairs will be available, if ever. Please use all of your authority to bring this matter to a satisfactory resolution.
