Component/System: The fire department determined the cause of the fire to be accidental due to an electrical malfunction within the vehicle. The most extensive damage was located in the engine compartment, though damage spread to the passenger compartment as well. The vehicle is available on request. The vehicle caught fire while parked on a street. No injuries or displacements occured. However,
... the vehicle became "well involved" with fire, creating an active emergency that required emergent fire department response and suppression. Has the problem been reproduced or confirmed by a dealer or independent service center? Since the vehicle receive so much damage, it would only be the emergency response and origin determination by Lubbock Fire Rescue. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The vehicle was inspected on-scene by Lubbock Fire Rescue (Unit E7). The fire department noted no signs of vandalism or tampering, concluding the cause was accidental. The police was also on the scene as well. Were there any warning messages? The owner stated they had driven the vehicle around 20:30 (8:30 PM) the previous evening and it had been parked since. The fire did not break out until approximately 05:13 AM the following morning. There wasn’t any warning lamps or unusual symptoms l present during or prior to that final drive. Summary: On May 28, 2026 the 2018 Nissan Rogue internally combusted. Subsequent inspection of the vehicle revealed fire damage in both the passenger compartment and the engine compartment, with the most severe and extensive damage concentrated in the engine bay. Fire officials found no evidence of tampering or vandalism. Without any warning or physical impact, the central infotainment screen completely blanked out and suffered an irreversible hardware failure. This screen acts as the mandatory display terminal for the vehicle's rear-visibility backup camera, as well as the audio generation module for the vehicle's driver alerts. Because of this component failure, the vehicle is in direct non-compliance with Feder
... al Motor Vehicle Safety Standards (FMVSS): 1.FMVSS 111 (Rear Visibility): The vehicle completely fails to display a rear-view image when shifted into reverse, creating an immediate blind-spot hazard for pedestrians. 2. FMVSS 101 (Controls and Displays): All critical audible safety alerts, pedestrian alerts, and driver warning chimes have been entirely silenced. The manufacturer (General Motors) and the authorized franchise dealership (F.H. Dailey Chevrolet) have acknowledged the failure and have the part on backorder, but refuse to fully cover the safety restoration under warranty or provide a temporary loaner vehicle. This forces the primary driver, a [XXX] senior citizen, to choose between total isolation or operating a hazardous, federally non-compliant vehicle. This appears to be a systemic manufacturing or design defect inherently tied to the internal HMI module architecture on the late-model Chevrolet Bolt EV platform, rather than individual wear and tear. Severe, chronic rainwater intrusion through the vehicle's front cowl and body seams, resulting in catastrophic flooding of the passenger-side cabin floorboards. This issue occurs even when all sunroof drains are completely clear and functional. Toyota has issued Technical Service Bulletin 0055-24 acknowledging body seam and cowl leaks for this vehicle type. The passenger-side floorboard houses cri
... tical electrical wire harnesses and sits adjacent to the Supplemental Restraint System (SRS/Airbag) control module. Chronic standing water on the floorboard presents an immediate risk of electrical short-circuits, potentially causing sudden vehicle stalling, electrical component fires, or the uncommanded deployment/failure of the vehicle's airbag safety systems. Furthermore, the thick foam insulation beneath the carpet remains saturated, creating an immediate, toxic mold hazard within an enclosed passenger cabin, severely impacting occupant health and air quality. The manufacturer and dealership have left the vehicle unrepaired for over 30 days, refusing to remediate this safety hazard.Read more

High voltage battery pack, Power Inverter Module (PIM), and HV charging system. PIM replaced twice (p/n 68381781AD, 8/4/25 and 8/18/25) after being found melted with arcing damage at HV connections. Battery pack has active BPCM fault codes and a recorded cell voltage excursion. Vehicle available for inspection. Vehicle lost all motive power without warning at highway speed on multiple occasions,
... requiring the driver to pull over. This occurred on a platform subject to recall 24V-536 (HV battery fire risk). A melted inverter and arcing at HV connections were found on a vehicle still being driven daily. BPCM data shows Cell 79 recorded a voltage excursion to 1,526 mV — less than half minimum safe operating voltage — stored in permanent memory. Yes. Lawless Inc. (Woburn, MA), authorized Chrysler dealer, confirmed fault codes PA121-00 and P0A43-00 on 8/4/25 and replaced the PIM. Vehicle returned 8/18/25 with same failure — dealer confirmed arcing damage and melted inverter. Three visits total; defect unresolved after two repair attempts on the same component. All three visits performed by authorized Chrysler dealer under 10-year/100,000-mile HV battery warranty. Stellantis warranty claim opened each visit. No police or insurance involvement. EHICLE: 2023 Kia Sorento, VIN [XXX] , currently 48,000 miles. Vehicle has never been in a collision. DEFECT: MASTER POWER WINDOW SWITCH / WIRING HARNESS — OCCUPANT EGRESS HAZARD The driver's master power window switch fails intermittently, causing simultaneous loss of all window operation and electric door lock function. When this occurs, occupants cannot operate any window or unlock any door el
... ectrically until the condition resets, typically by opening and closing the driver's door. The condition was first reported to Arapahoe Kia at 24,859 miles on 11/08/2024 (RO 208018) and again at 25,050 miles on 11/15/2024 (RO 208449). On the 11/15/2024 visit, the dealership confirmed the defect on customer-supplied video and performed a full diagnostic including pin tension testing on connector D01, power, ground, B-CAN continuity testing, and a wiggle test of the harness. The dealership's written conclusion on the repair order: "we've determined that the harness is probably not the issue and it will need a new master switch." The master switch was not ordered or installed, and the vehicle was returned with the defect unresolved. The condition continues to occur intermittently as of the current 48,000-mile reading. SAFETY IMPLICATION: This defect creates an occupant entrapment and egress hazard. In a fire, submersion, or crash event where the mechanical door release is inaccessible, inoperative, or where occupants are incapacitated, simultaneous loss of window operation and electric door lock function eliminates secondary egress options. The defect is intermittent and unpredictable. The vehicle remains within the 5-year / 60,000-mile basic warranty period. ... I was outside of warranty and “There is an internal processor issue with your autopilot system that will prevent updates from installing. The only solution is replacing the computer on April 9, 2026, 12:22,” I had had other computer related issues I brought up during regular service visits but weren’t recorded and I was told software updates would fix such as: delayed rearview camera upon startup, navigation volume resetting, headlights staying on for extended periods after exit, and lockout issues. I asked Tesla to document the specific failure codes which they never sent. They said “Tesla diagnostics are showing the primary Autopilot computer is not booting properly, which is preventing cameras, software updates, and related systems from functioning. Because the computer is not initializing, a firmware reinstall will not complete successfully. At this point, replacement of the computer is the required repair to restore normal functionality.” My car had a safety recall during its service appointment that they could not install software for unless I paid $3200 to replace computer. I had total loss of rearview camera (FMVSS 111 violation) and loss of Active Safety Systems (AEB/FCW), but Tesla refused to repair a federally mandated safety defect if I didn’t pay the full repair cost.Read more

I am writing to report an unresolved high-voltage safety defect involving the Motor Power Inverter Module on my 2019 Honda Clarity Plug-In Hybrid. The vehicle's high-voltage hybrid propulsion inverter is emitting a severe, high-pitched mechanical squealing noise during acceleration and regenerative braking. The high-voltage inverter serves as the foundational link regulating current between the lit
... hium-ion battery and the electric drive motor. A failing inverter module presents an immediate, unreasonable safety hazard. Internal failure, thermal runaway, or component degradation within the inverter stack creates an imminent risk of sudden high-voltage isolation fault. This will result in an immediate vehicle stall and a total loss of propulsion while driving at highway speeds, leaving the vehicle stranded in traffic with no power to accelerate out of harm's way, creating a severe crash hazard.Furthermore, an authorized Honda dealership—Visalia Honda (Groppetti Automotive) in California—has failed to properly diagnose, isolate, or resolve this high-voltage drivetrain defect despite the vehicle being brought in specifically for this issue. Despite the component being legally covered under California’s mandated 15-year/150,000-mile extended emissions and hybrid warranty, the dealership has taken no constructive corrective action, allowing a known, physically degraded high-voltage component to remain active on public roadways.Federal intervention and a formal technical investigation into Honda's clarity hybrid propulsion inverter systems are required to address this vehicle stalling hazard before an electrical failure results in a highway collision.Read more
