I am writing regarding a very serious and frightening safety incident involving my 2024 Hyundai IONIQ 5. While driving through a busy intersection in California, the vehicle displayed the warning message: “Stop vehicle and check power supply.” Within approximately one minute of that warning appearing, the vehicle completely lost power and became inoperable in the middle of the intersection. Th
... e shutdown occurred without warning and created an extremely dangerous situation. The vehicle’s hazard lights would not function because the car appeared to be entirely without power. Traffic continued moving around me while I was stranded in the intersection, and I genuinely feared that another vehicle could collide with me. I ultimately called 911 because the situation was unsafe and I could not move the vehicle. Police responded and directed traffic until the vehicle could be towed. Hyundai dealership had the vehicle for 22 days and diagnosed it as a faulty ICCU (Integrated Charging Control Unit) which they replaced for no charge. This incident has caused me to lose confidence in the safety and reliability of this vehicle. A complete loss of propulsion in active traffic — especially accompanied by failure of the hazard system — is an unacceptable safety risk. I no longer feel safe driving this vehicle. Hyundai of America would not let me cancel the lease early due to safety concerns.Read more

While traveling at cruising speed (70 MPH) on Interstate 95 South, the vehicle suffered a catastrophic electrical and mechanical failure, resulting in an immediate engine stall. The vehicle lost all propulsion. Due to the sudden loss of power, the vehicle could not safely clear the active lanes of traffic, coasting to a stop near an exit ramp and dangerously blocking highway traffic.Upon attemptin
... g to restart the vehicle to move it out of harm's way, the vehicle emitted a rapid clicking noise and again stalled out. Because this vehicle utilizes an electronic shift-by-wire system, the total power failure permanently locked the transmission into "Park." The vehicle could not be shifted into "Neutral" or "Drive" to be pushed safely off the road. This total vehicle failure created an immediate, life-threatening hazard for the occupants and surrounding motorists on a high-speed interstate, requiring emergency police intervention to manage traffic until a flatbed tow truck arrived.Read more

COMPONENT/SYSTEM THAT FAILED — AVAILABLE FOR INSPECTION: The electrical system failed at the under-hood fuse box / power distribution center (the component sometimes referred to as the TIPM), located near the battery. The vehicle started roughly, shut off within about 3 seconds, would not stay running, and emitted a plastic/electrical burning smell from that area. The vehicle is currently at Oxe
... ndale Jeep in Cottonwood, AZ and is available for inspection upon request. A photo of the damage from service advisor Jason Smith and a dealer pre-service inspection document are also available. HOW SAFETY WAS PUT AT RISK: The vehicle would not run and stranded me in an unfamiliar parking lot in Jerome, AZ. The electrical burning smell created a fire risk. A responding firefighter, a police officer, and online guidance all advised not to restart the vehicle due to the danger of fire. REPRODUCED/CONFIRMED BY DEALER: Not fully. Oxendale's pre-service inspection states they could not perform actual diagnostics due to the mechanical damage and burnt components. The dealer documented the damage but has not reproduced or confirmed a root cause. INSPECTED BY MANUFACTURER, POLICE, INSURANCE, OR OTHERS: A local firefighter and a local police officer observed the burning smell at the scene. The jeep dealership service advisor inspected and documented the damage. The vehicle has not been inspected by the manufacturer or an insurance representative. WARNING LAMPS/MESSAGES/SYMPTOMS BEFORE FAILURE: No prior warning lamps or messages before the incident. Symptoms first appeared at the time of failure on Friday, [XXX]: a rough start, immediate shut-off, and an electrical/plastic burning smell. My 2014 Ford F-150 is exhibiting symptoms consistent with the Ford transmission molded lead frame safety recall 24S37. While driving approximately 55 mph late at night on a five-lane roadway, the vehicle suddenly and violently jerked, downshifted and came to a complete stop becoming stalled out as I was approaching a major traffic intersection. The truck was disabled in traffic and would not resta
... rt for approximately 2-3 minutes, leaving me stranded in an active travel lane and at risk of being struck from behind by another vehicle at high speed. The vehicle has displayed transmission-related symptoms including inability to correctly indicate gear position, harsh shifting, unexpected downshifts into 1st gear while driving, transmission fault warnings, and check engine lights. Initially it was mostly dying while idling and we thought it was the common "Fuse 27" issue, being unaware of the recall. Then the condition reproduced multiple times and symptoms worsened dramatically. Ford dealership inspected the vehicle. According to the technician's written report, the truck was found to be "having transmission problems, unable see what gear the truck is in and shifts to 1st gear while driving" and recommended replacement of the molded lead frame. The dealership confirmed transmission-related diagnostic trouble codes were present and documented symptoms consistent with lead frame failure. Despite the recommendation of the lead frame replacement and documenting symptoms consistent with the safety recall, the repair was not approved. The vehicle continues to experience symptoms associated with the recalled defect and is unsafe to drive away. Dealership confirms software update will not correct problem. I am submitting this complaint as the transmission defect poses a significant safety hazard to me, my children, and other drivers due to unexpected downshifts, loss of power, stalling, and the risk of becoming disabled in traffic. I experienced a safety critical failure in my 2024 Hyundai Ionic 6. While attempting to start the vehicle, the car displayed a red 12 Volt battery warning light, a battery discharge warning, and then refused to enter ready mode. The vehicle would only shift into neutral and was completely undrivable. After sitting for many hours, the vehicle started again, which is consistent with an intermittent
... ICCU/DC-DC converter dropout. This type of failure is known to cause loss of power and has been documented in NHTSA investigation PE23-011 for other Hyundai EV's. The dealership states they cannot replicate the issue and refuses further diagnostics. Hyundai corporate is deferring entirely to the dealership and will not authorize a loaner or confirm reimbursement, leaving me without transportation. They also stated they cannot commit to any repair unless the failure happens again despite the clear safety implications. This failure mode can cause sudden loss of motive power, which poses a serious risk if it occurs at highway speeds. I am reporting this because the vehicle is unsafe to operate, the failure was severe, and Hyundai is refusing to investigate the root cause unless it reoccurs. I am requesting NHTSA review this incident as part of the ongoing concerns regarding ICCU/DC-DC failures in Hyundai E-GMP vehicles.Read more

Issue 1: Cooling Fan Remained On After Engine Shutdown Component or system involved: Engine cooling system and fan control system, including the radiator cooling fan, fan relay, coolant temperature sensor, wiring, or engine control module (ECM). The vehicle is available for inspection upon request. What happened: After turning off the engine, the radiator cooling fan remained running continuously
... for approximately five minutes and would not shut off as expected. No warning light, message, or other dashboard indication appeared before or during the event. This was unusual behavior compared with normal operation. Safety risk: The unexpected fan operation created concern that the cooling system was malfunctioning and that the vehicle could overheat, experience an electrical problem, or become unreliable while driving. A cooling system or electrical failure while in traffic could put me and other drivers at risk. Reproduction or confirmation: The issue occurred several time and was confirmed by a Chevrolet dealer, and was told that my warranty did not cover the issue. I took it to outside mechanic to fix the issue as Chevrolet wanted to charge over $1500 for the issue. No warning lamps, messages, overheating indicators, or other symptoms appeared before the fan remained on. The problem occurred suddenly back in 2025. Issue 2: Ignition Key Gets Stuck and Cannot Be Removed Ignition lock cylinder, ignition switch, transmission shift-interlock system, park-position switch, or steering column lock mechanism. The vehicle is available for inspection upon request. The ignition key intermittently becomes stuck and cannot be removed from the ignition. The problem occurs after parking and turning the vehicle off, and I sometimes need to move the shifter or steering wheel repeatedly before the key can be removed. A stuck ignition key can prevent me from properly shutting down and securing the vehicle. It may also indicate a failure in the ignition or shift-interlock system.Read more

Vehicle was inoperable and over 100F in our driveway, with a flashing "critical high voltage battery fault". My wife injured her hand attempting to get in the vehicle. ... isture. Linkage to Warranted Component: At 85,690 miles, my vehicle is fully covered under Rivian’s 8-year High Voltage Battery Limited Warranty. The critical high-voltage fault and subsequent component failures were directly caused by water entering the electrical/battery system. Proximate Cause via Known Manufacturing Defect: Rivian has an active Technical Service Bulletin (TSB) regarding moisture ingress via the HVAC assembly for 2022–2024 R1S models. The moisture that caused the battery fault is a documented, inherent design defect of the vehicle, not user negligence or external environmental damage. An inherent defect in an out-of-warranty system cannot be used to void coverage for a failure it causes inside a component that is still heavily protected by an active warranty. The photograph provided by the service center does not depict superficial 'moisture in the HVAC' causing external damage. It shows severe fluid ingress and terminal corrosion inside the Woory Industrial High Voltage Cabin Heater assembly. Because this is an internal, high-voltage powertrain component operating directly off the traction battery system, it falls squarely under Rivian's 8-year / 175,000-mile High Voltage System Warranty. The corrosion present on the high-voltage circuit board is a direct result of the factory HVAC water ingress defect acknowledged in Rivian's technical service bulletins for 2022-2024 R1S vehicles. The plastic upper rail glass guide frame on the rear sliding window assembly of my 2023 GMC Sierra developed structural micro-cracks during normal vehicle operation. This manufacturing defect is a known issue tracked internally by the manufacturer under Technical Service Bulletin (TSB) #18-NA-383.This structural failure allows severe water intrusion into the passenger cabin. The leaking water runs
... straight down the inside back wall of the cab, completely flooding and corroding sensitive, factory-installed electronic safety modules mounted directly beneath the leak path. This continuous water intrusion caused the catastrophic electrical failure and corrosion of the video processing control module located behind the rear seat. The sudden failure of this module completely disabled the rearview digital mirror camera and the 360-degree perimeter driver assistance cameras, creating immediate blind spots and an active driving safety hazard while operating the vehicle. The vehicle was inspected and repaired by an authorized dealership, confirming the rear window leak as the direct cause of the electronic destruction. The out-of-pocket cost to replace the water-damaged safety components was $2,747.00. Because the manufacturer refused to cover the replacement of the defective rear glass assembly, the structural leak remains unresolved. This active water intrusion continues to flood the cabin, creating a health hazard via mold growth and guaranteeing that the newly replaced safety cameras will inevitably short-circuit and fail again.Read more

While my wife was operating our 2022 Jeep Wagoneer Series III (5.7L HEMI eTorque, ~44,700 miles) at highway speed, the vehicle suffered a sudden and complete electrical power failure. Electric power steering cut out completely while the vehicle was in motion, creating an immediate safety hazard and nearly causing an accident. The vehicle was disabled and required towing. The dealership (Safford J
... eep) diagnosed fault code P0B32: catastrophic internal hardware and voltage failure of the 48-volt eTorque Hybrid Battery Pack. The 12-volt battery was also destroyed as a consequence of the 48V pack failure — the 12V charges exclusively via a DC-to-DC converter fed by the 48V loop; when the pack failed, the converter lost power and the 12V battery was starved of charge. NOTE: This incident is consistent with the active NHTSA ODI preliminary evaluation of 2022 Jeep Wagoneer and Ram 1500 eTorque electrical failures. FCA/Stellantis has acknowledged these failures are likely caused by an electrical concern. My P0B32 event represents a catastrophic failure of the exact system under investigation, with the additional safety consequence of complete loss of power steering at highway speed. Stellantis Corporate Case #96719164 is open. The vehicle has been repaired (48V battery pack replaced) but Stellantis has declined to address the warranty access barrier created by their diagnostic system architecture. This complaint is in regards to Safety Recall 272 Component Failure and Vehicle Disabling / SOS System Failure On [XXX], at approximately [XXX] PM, I was driving my Genesis vehicle during rush hour when I heard a loud popping sound. Immediately following the sound, the dashboard displayed a critical warning light stating: "Electrical Charging System Fault — Check Electrical System." Because I wa
... s actively driving in heavy rush-hour traffic, this sudden loss of charging system functionality posed an immediate safety hazard. I pressed the in-vehicle "SOS" button to contact Genesis emergency services. The operator, who identified himself as "Eddie," refused to dispatch a tow truck to assist me. Instead, he offered to read a phone number aloud for me to write down. I explained that I was actively operating a malfunctioning vehicle in heavy traffic and could not safely write down information, but he still refused to call for emergency towing. I managed to safely limp the vehicle back to my residence and subsequently arranged for the local dealership, Genesis of Las Vegas, to tow the vehicle. On Friday, June 5, 2026, the dealership confirmed that the vehicle had suffered a failure of the Integrated Charging Control Unit (ICCU), which also destroyed the 12V battery. They informed me that this failure was directly tied to NHTSA Safety Recall 272 (Integrated Charging Control Unit). While the dealership performed the ICCU remedy, they are currently refusing to release my vehicle unless I pay $500.00 for the replacement of the 12V battery. Because the 12V battery was directly damaged as a consequential result of the recalled ICCU defect, it should be covered under the safety recall remedy and vehicle warranty. As of this reporting, the dealer is holding my vehicle hostage over this fee, and Genesis Motor America has failed to resolve the issue despite a formal complaint being filed with them. An independent Toyota dealership diagnosed the vehicle April 27-28, 2026 (approx. 109,000 miles) and found (1) water leaking into the rear hybrid battery area through a sunroof drain "installed incorrectly," c
... ausing a Brake System Control Module low-voltage code, starting issues, a headlight only partially functioning on one side, possible air in the brake system, and a musty HVAC odor; and (2) blind spot monitor fault code C1AC2 (Slave Module Horizontal Axis Misalignment) - the blind spot and rear cross-traffic monitoring systems do not function reliably. At a second visit on May 4, 2026, recalibration was unsuccessful; per Toyota procedure the only remaining option is replacing the right-rear blind spot sensor. The technician wrote the cause is a mystery, "whether it be body damage related or electrical short." Toyota service records show the same C1AC2 code was documented during the selling dealer's own pre-sale inspection, and that in December 2024 another Toyota dealership pulled the rear bumper and found the RR blind spot sensor bracket bent, causing incorrect readings - a recurring safety defect that prior calibrations and repairs have not resolved. Separately, federal safety recall 23V865 (occupant classification sensors; the passenger airbag may not deploy as designed) was open and incomplete when I purchased the vehicle and remains incomplete, and Toyota service campaign 25TD01 (rear traction motor cable corrosion) is also outstanding. The defect has been reproduced and confirmed by two Toyota dealerships, reported to the selling dealer in writing, and reported to Toyota's customer-care center. The vehicle is available for inspection upon request. No crash, fire, or injury.Read more

As I was driving 75 mph down the highway my collision alert flashed all my lights turned off, including my headlights and all interior lights as it was dark time and they flashed back on a couple of times until they finally stayed on. I pulled over and as I came to zero, speed, the vehicle completely shut down. It did start back up, but this is not the first time I get a light on my dash that says
... stability track and then everything resets and I don’t have any lights on at all after that this has been happening off and on I also have three coworkers that have the same make, and year that experienced the same problems ! My situation on the highway was very dangerous. It was pitched dark outside and I lost vision due to my lights going off at 75 mph and my steering wheel also locking up. The next time the vehicle does it I will take a video for proof. I have heard that this problem is very common in these vehicles and believe something should be done because that is a dangerous situation if I was going around a corner and my steering wheel locked up, I could easily flip my car. I have small children that’s why I have the big space but we need safety for our family.Read more

The vehicle lost power rolling from a stop sign, felt like it was trying to shift but wouldn’t then went into limp mode, drove fine the next day. The faulty lead frame is failing, it has no other cause. I refuse to drive and put myself and others in danger to prove it. The dealer said they couldn’t cover it because the code is not covered in the recall, they recommended a new valve body or tra
... nsmission, they didn't even say why. They were able to recreate it wrench light. P2704 is the code they claimed with the 2 leading causes are valve body, they said my fluid was clean and the vehicle has up to date service with no previous sign of wear, or the lead frame going out, the ACTIVELY recalled part. I tried contacting ford directly and they refused assistance beyond “tow it to a different dealer” and then ended the conversation. It is a safety issue that I will not wait to have break if there is clear signs of it happening.Read more

2025 Triumph Speed 400 — Moisture-Triggered Limp Mode and Loss of Turn Signals My 2025 Triumph Speed 400 enters limp mode and displays a check engine light and wrench warning whenever ridden in rain or high humidity. When this occurs, turn signals and hazard lights also stop functioning, leaving me without signaling capability in traffic. Both issues clear only after the bike dries out. The bat
... tery is disconnected and reset has not worked in the past, and continues not to help the issue. At my break-in service (~150 miles ago), I reported the issue. The dealer applied dielectric grease to the speedometer connectors and checked the battery terminals, but the fix was unverified as it didn’t rain until today. On 6/10/26 after riding to work in the rain, the bike entered full limp mode when I tried to leave — power was severely restricted and turn signals and hazards were completely inoperative. The simultaneous loss of power and signaling in traffic is a serious safety hazard with no advance warning to the rider. This is a documented issue across multiple owner forums affecting 2024 and 2025 Speed 400s, and Triumph is aware but has issued no recall or formal remedy. I am requesting NHTSA investigate and compel Triumph to issue a fix.Read more

Highway Incident: While traveling at highway speeds, a sudden evasive braking maneuver caused the truck to drop power. Upon attempting to re-accelerate, the throttle pedal went to the floor with zero response, and the vehicle continued to decelerate on an active highway. Multiple trailing vehicles had to make emergency maneuvers to swerve around the decelerating truck to avoid a high-speed, rear-e
... nd collision. Subsequent Incidents: The vehicle repeatedly forces an uncommanded speed limit of 11 mph while attempting to accelerate from a complete stop or merge from stop signs. This severe lack of predictable acceleration creates an immediate risk of being T-boned or rear-ended by cross-traffic, making the vehicle entirely unsafe to operate in normal traffic conditions. An authorized Ford dealership has confirmed the underlying failure of the hybrid system. They kept the vehicle for approximately one month to replace the entire hybrid drive unit. Following that repair, the vehicle was returned to the dealer a second time, where technicians confirmed a software communication failure between the electric and gas systems, attempting to resolve it by flashing/reprogramming the ECU. The vehicle’s hybrid system and software have been extensively inspected and worked on by manufacturer-certified technicians at an authorized Ford dealership. It has not been inspected by police or insurance, as no physical collision has occurred yet. No warning lamps or messages.Read more
