While traveling on [XXX] in Van Buren, Arkansas, I was involved in a hit-and-run accident. The impact caused significant front-end damage and resulted in physical injuries. The vehicle’s dashboard/electrical system indicated AIRBAG:ERROR, and the police report reflects that airbag deployed yet we have photo evidence that the airbag did not deploy. However, photo evidence clearly shows that no ai
... rbags deployed for any occupants. The airbags remained completely intact following the crash. The vehicle is a certified pre-owned 2019 Volkswagen Jetta that was still under warranty at the time. This raises serious safety concerns regarding the accuracy of the airbag deployment system and the failure of the airbags to function during a serious collision. The vehicle is being held for preservation at the co-part yard. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I was involved in a fatal head-on collision with a pedestrian while driving my Kia EV6. What component or system failed or malfunctioned, and is it available for inspection upon request? Forward Collision-Avoidance Assist : The FCA system appears to have completely failed. It did not detect the pedestrian, did not provide any audible or visual warnings, and did not initiate automatic emergency
... braking prior to the collision. Airbag System: The system failed to deploy despite a severe head-on collision. Regarding availability for inspection: The car was impounded by the SDPD and was inspected by both the police and State Farm before being sold to SF after being rendered a total loss. How was your safety or the safety of others put at risk? My Safety: The failure of the airbags to deploy put me at significant risk of serious injury or death in the severe head-on collision. The Safety of Others: While the primary consequence of the FCA failure was the tragic impact with the pedestrian, the malfunction of a safety system designed to prevent or mitigate collisions inherently puts all potential road users at risk. Had the FCA functioned correctly, the collision might have been avoided entirely or the impact speed significantly reduced, potentially altering the outcome for the pedestrian. Has the problem been reproduced or confirmed by a dealer or independent service center? No not to my knowledge. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? As mentioned above, the car was impounded by the SDPD and was fully inspected by both the police and State Farm. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? None.Read more

Side Blind Zone Alert (SZBA) system continues to be an issue with this vehicle. Sensing module(s) on inner left or right side of bumper cover fail due to water intrusion. When sensing module fails, "Service Side Detection System" message will appear on driver information center on instrument cluster. General Motors has previously issued a special coverage "N172097060", which is available on NHT
... SA website. As indicated in the special coverage document, if the issue is ignored, "the condition could result in the corruption of LS GMLAN that may result in the loss of some driver interface functions such as cluster, infotainment, displays, chime, and passenger presence detection (passenger airbag could be suppressed). Within approximately 30 days of the "Service Side Detection System" message, while driving, instrument cluster speedometer and tachometer went dead, and airbag warning system notification, anti-theft system, tire pressure monitoring system all illuminated. In addition to no speedometer reading, vehicle ignition/security interlock was causing vehicle to run rough. Vehicle would not immediately restart after shutting vehicle off. This is the 4th time a SBZA sensing module has failed on this vehicle. First failure was replaced under factory warranty, second failure was replaced under special coverage N172097060, third failure was replaced out-of-pocket. Contacted General Motors/Chevrolet, and they are not interested in providing continued coverage of the issue, which is a known issue, and likely a design defect, as the design was changed for later model years.Read more

Hello, I am writing a request to supplement 19N01 on Ford models as I had a near accident that almost happened. First, as I was driving, my mileage odometer was glitching showing dashes "------", also notice that my check engine light came on and as a result the airbag off warning was showing as well. As a result of all this, seems that the vehicle malfunction as my vehicle was in "limp mode" whil
... e all of this was happening and when pressing the gas no acceleration was happening (mind you that was all happening in heavy traffic). After moving steadily and avoiding heavy traffic, I parked my vehicle to the side and turn it on and off. This seems to restart the system however my check engine was still on. After further research on this issue, I see that this is a known issue that happens with the model of my vehicle. After calling the dealership with my issue, they state it is a customer program rather than a recall. I beg NHTSA to please supplement this known issue and instead of 10 years, please use the 150K mileage. I cannot be driving with this unsafe issue as it was a scary issue that happen during traffic.Read more

Dear NHTSA Safety Team, I am submitting this complaint to bring attention to an unresolved safety recall issue on my 2021 Toyota Camry SE – SEDAN 4DR (VIN: [XXX] ), which I purchased used in July 2023. The dealership that sold me the vehicle failed to inform me of the open recall, and I later learned that it involves the airbag system—a critical safety component. After checking my VIN, I con
... firmed that the recall is officially registered as: - NHTSA Recall Number: 23V865 - Manufacturer Recall Number: 23TB15 - Date Issued: December 20, 2023 - Recall Status: Recall Incomplete – Remedy Not Yet Available According to the recall summary, the issue involves potentially defective Occupant Classification System (OCS) sensors in the front passenger seat, which may have been improperly manufactured. This defect can result in the airbag failing to deploy as designed in certain crashes, thereby increasing the risk of injury to passengers. Toyota has not yet made a remedy available and only estimates a solution might arrive by Quarter 3 of 2024, which remains uncertain due to parts availability. In the meantime, I have been unable to refinance my vehicle due to this unresolved recall, and this situation has directly impacted my financial health. This is not only a severe safety risk but also a major consumer protection concern. I urge NHTSA to take swift action to ensure that Toyota expedites the availability of the remedy and prevents further financial and safety consequences for affected consumers. Thank you for your attention to this matter. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

My wife was driving the car on March 7th 2025. Before she leaves the house, she had her front defroster on and the windshield was clear before she leaves the house. 5 minutes later, the front windshield became so foggy to the point she could not see anything in front of her and the front defroster was still on. The car was taken to a body shop and was inspected by the body shop and I believe a representative from my insurance company went to inspect the car at the body shop. I am submitting this complaint to report a significant safety system failure in my 2025 Cadillac XT4 during an accident that occurred on April 12, 2025, in Dallas, Texas. During the incident: •The Anti-lock Braking System (ABS) did not activate, and I was unable to stop in time to avoid a collision. •Neither the driver nor passenger airbags deployed, despite sufficient impact that should have
... triggered deployment. •OnStar Automatic Crash Response did not activate, nor was I contacted by any emergency response service through the vehicle. None of the vehicle’s safety systems functioned during this event, despite being standard factory-installed features. My XT4 had no warning lights or diagnostic messages indicating any malfunction prior to the crash. This failure closely resembles previous complaints noted in NHTSA filings (e.g., ABS sensor failures, OnStar module failures, and the 2025 XT4 passenger airbag recall). I am extremely concerned that this may point to a broader defect within the model line’s safety infrastructure, including collision sensors, electronic communication systems, or airbag deployment logic. I respectfully request that NHTSA investigate whether a defect exists in the braking or airbag systems, particularly where no safety features activate in a crash. I would be willing to provide repair records, crash photos, and allow the vehicle to be inspected if needed. Thank you for your attention and for helping to ensure driver safety through proper oversight.Read more

My 2020 Tesla Model 3 has displayed a "Front Passenger Safety Restraint System Fault - Service is required" error since June 2023. The issue relates to the spiral cable assembly in my 2008 Toyota RAV4, which falls under Safety Recall 14V-168. My pregnant wife is parking her car at her work when she accidentally step on the gas pedal instead of the brakes. She panicked and she drove all the way to the wall. Insurance deemed it totaled due to frame damage. I also complained
... to Toyota. They hired a software engineer to investigate. Just received official letter from Toyota there weee no defects in their safety system. The issues are the airbags not being deployed at the time of impact and pre collision auto braking non existent. The impact on the wall according to the people inside the nursing she works felt like an earthquake inside. But our main concern is the auto brake feature did not function. This accident could have been worse. Good thing seatbelt was not an inch lower from her chest preventing injury to our unborn child. The impact could have been worse if it was made of wood and could have injured other people in the nursing home. The auto brake feature should have worked because there were many obstacles at her front including first the bushes and eventually the wall. Their response letter is frustrating for us because they are claiming there were no problems in the car. But our concern is their safety system is not reliable compare to other car companies. Kia and Honda safety system is more reactive based from my driving experience. I think there are lapses in their system.Read more
