Sensor on passenger side airbag stays off at times when there is a passenger in the seat. The notification on the dash and above the GPS screen notifies the driver that the airbag is off. The driver's side active restraint headrest deployed unexpectedly while parked. The vehicle has never been in an accident. Upon inspection, I can see where there are plastic retaining holes that that had a metal retaining pin in it cracked. The metal pin was still in its clip. The vehicle is drivable and available for inspection. The vehicle was parked and unoccupied when the head restraint went
... off. No injuries were incurred. However in order to drive the vehicle now, the seat back must be adjusted back making the driving position awkward. Neither the vehicle nor the component has been inspected by anyone at this time. There were no warning lamps or messages presented prior to the issue. I did recently notice that the back plastic cover on the headrest was askew and figured it had been bumped loose by a rear rider at some time. However, it now seems that this was showing the weakening and deformation of the plastic retaining parts that were holding the metal pin.Read more

Vehicle Diagnostic Summary – Trouble Codes Explained 1. P0562 – System Voltage Low The vehicle’s electrical system is running at low voltage. This could be due to a weak battery, a failing alternator, or poor electrical connections. It may cause various electronic issues and unexpected shutdowns if not fixed. 2. P1005 – Fuel Level Sensor Control Module: Too Many Resets The fuel level sen
... sor module has been reset too many times, likely due to electrical glitches. This may cause the fuel gauge to be inaccurate or unstable. 3. P14C0 / P14CD – Fuel Pump Driver Control Module Signal Error There is a configuration signal error from the fuel pump control module. This could lead to fuel delivery problems, starting issues, or engine stalling if it worsens. 4. U0151 – Lost Communication with Airbag Control Module (SRS) The system lost contact with the airbag diagnostic module. This may affect airbag deployment during a crash, making it a serious safety concern.Read more

The curtain airbags deployed without any collision or impact. This suggests a failure of the airbag sensors. The vehicle and all components, including the deployed airbags and related sensors, are available for inspection upon request. The sudden airbag deployment created a significant safety hazard. If this had occurred while driving at higher speeds, it could have caused me to lose control of th
... e vehicle. The force of the deployment itself poses a risk of injury and can cause burns, bruises, or hearing damage. Myself and the passenger experienced these injuries. Other drivers could have been endangered if my vehicle reacted unexpectedly due to the deployment. The manufacturer inspected the EDR system which claimed that there was collision. There was no collision and no physical damage on the vehicle to suggest otherwise. This suggests a fault with the EDR system, sensors, and safety components of the vehicle. The vehicle is currently under inspection by insurance representatives. A few months into initially driving my vehicle, a warning lamp stating "RESTRAINT INDICATOR LAMP FAULT" was appearing. In March, there was dent and paint damage on the vehicle caused by a driveway gate, unrelated to the recent airbag deployment. These damages and warning lamp were fixed by an insurance certified shop. A report from this repair shop will be provided upon request. It cannot be attached below due to significant file size. There were no indication or warnings suggesting further fault with the airbags or any other safety restraint from March to the July, when the incident occurred.Read more

Purchased used 2019 RDX from Acura dealer in June 2025. Within the first 30 days of owning the RDX we noticed the Airbag Sensor for the front passenger seat would not recognize my 90 lb daughter seating in the seat and the air bag sensor would indicate the seat was empty, which would result in the airbag not deploying if we were in an accident. I took video of this happening and took my RDX into d
... ealership for repair as we were still within the 30 day period for issues. The Acura dealership insisted they could not replicate the problem so it was fine. I again referred to the video and said we have seen proof of the issue. After much back and forth and the service manager telling me his large dog sometimes doesn't set off the sensor he agreed to call American Honda to send out an engineer to test. The engineer after running tests said seat is calibrated correctly no issue to fix, even if we wanted to pay to fix it ourselves they wouldn't fix the seat sensor because there is no issue to correct. I don't understand how a car that advertises a safety feature for anyone that weighs over 65lbs can expect me to continue to drive this vehicle when it has a proven faulty sensor. Obviously how they test the sensor is faulty as well. This is a huge safety issue for the 2019 RDX and a disappointing and shocking lack of response from Acura. We are now working to return the car to the dealership and I am sure we will have to fight to not pay extra costs that will occur with them buying it back.Read more

Front passenger airbag does not turn on and/stay on when my adult wife is seated (5'7" 100lbs). Moving the seat up/down and/or back/forth sometimes allows the system to activate but it then turns off later on while driving. The Subaru owner's manual instructs the owner that when this occurs the passenger should sit in the back and Subaru Service should be obtained. Subaru Service has inspected t
... he car and indicated the system is functioning properly, and there is nothing to repair. Afterwards, calling the Subaru Customer Advocate they indicated that they expect the system to turn on for my wife, and that there is no weight minimum for the air bag. They said that I should return to the service center with her so that they could witness the issue. A return appointment was made and completed today. Subaru Service center staff witnessed the passenger air back system ignoring my wife in the seat. Their response was comprised of confusion as to why we were there followed by the judgement that the system was working as designed (based on the previous visit) so there is no problem to be fixed. It appears that the Subaru air bag system does not protect all adult passengers. Also, the owner's manual instructing the driver to tell the passenger to sit in the back when this occurs, seems to indicate that this is not a normal situation and needs to be repaired.Read more

Title: Possible Defect – Uneven Airbag Deployment Description: On July 20, 2025, around 5:30 PM, i was driving a 2021 Volvo XC90 on a main road when another vehicle suddenly entered from the right side without stopping at a stop sign. I was unable to avoid a collision and my vehicle T-boned the other car (front impact on my vehicle). During the crash, I believe the front airbag system did not
... deploy properly or deployed unevenly. I felt like the left side of the airbag (from the steering wheel) was full of air but not the right side (that side was flat when it deployed), and I believe I hit my head on the steering wheel. I experienced severe injuries to my head, face, and right eye, including commotio retinae, lens displacement, corneal abrasion, corneal edema, phacodonesis, iridodonesis, and concussion. I am concerned that the Supplemental Restraint System (SRS) malfunctioned, contributing to the severity of my injuries. No modifications have been made to the airbag system. I request that NHTSA investigate whether there is a defect in the airbag deployment of the 2021 Volvo XC90, particularly for asymmetric or partial deployment. The automatic emergency braking (AEB) system also did not activate despite the imminent collision. There were no warning lamps, messages, or unusual symptoms prior to the crash. The issue has not yet been reproduced or confirmed by a dealer or service center, but The police was at the scene. My insurance company has inspected the vehicle. I have also reported this to the customer care team at Volvo.Read more

The rear camera on my Tesla intermittently fails when shifting into reverse. Additionally, the rear trunk will not open using the in-car release button, mobile app, or key fob. A warning appears stating the function will return on the next drive, but I am often forced to use the emergency release to access the trunk. I believe this may be related to a prior recall involving the rear trunk harness,
... but when checking my VIN, no active recalls appear. Tesla service also reports no recalls on file. I am also receiving an intermittent fault message regarding the front passenger safety restraint system, stating that service is required. This is a serious concern, as a malfunctioning restraint system may fail to protect occupants in the event of a crash. Tesla has given me an estimate of $225.00 to address the safety restraint fault and $495.27 to repair the trunk/rear camera issues, with a total cost of $739.60 including taxes. I believe that safety-related repairs, especially those involving airbag and restraint systems, should be covered by the manufacturer and not passed on to the vehicle owner when not due to misuse or damage. The passenger restraint fault poses a direct safety hazard in a crash. The rear camera issue, while less severe, still impacts safety, as the camera provides a wider and clearer view than mirrors alone. I am asking NHTSA to review whether these issues are safety defects and whether they may be connected to known problems or recalls not applied to my vehicle.Read more

On [XXX], the vehicle was involved in a complex multi-impact accident. It was struck by a vehicle on the passenger (right) side and, simultaneously, by another vehicle on the driver's (left) side. (Police Report). During this sequence of collisions, and despite the magnitude of the impact, I must express my extreme concern that the left side airbag (driver's side) did not deploy. This failure caus
... ed Mr. [XXX] , the undersigned, to suffer a neck injury as a direct consequence of the incident. Even more seriously, the right-side passenger, Ms. [XXX] , sufferedsevere internal injuries, including three small intestine perforations, according to medical reports. These injuries resulted from improper pressure or malfunction of the seatbelt for that seat. These combined failures, the non-deployment of an essential airbag in impact, and the serious internal injuries caused by the seatbelt represent unacceptable deficiencies in the vehicle's safety systems. We believe that these injuries could have been avoided or mitigated if the safety components had functioned properly, as expected in an accident situation of this magnitude. Therefore, I am at a loss to understand: 1. Why the left side airbag did not deploy during the impact. 2. Why the right passenger seatbelt caused such serious internal injuries, including the three diagnosed intestinal perforations. We also require that concrete and immediate action be taken to address and compensate for the serious injuries suffered by the vehicle's occupants, as this has caused psychological and work- related harm.
