My name is [XXX] . I own a 2023 Toyota Venza Limited with about 36,000 miles. On [XXX], while driving, we heard a loud popping sound, and the passenger airbag indicator light came on with a red/orange warning symbol. I was sitting, idling in my car when my active headrest restrain system in my passenger seat suddenly went off. I am lucky no one was in my passenger seat and that I wasn't driving at that time. I looked more into it and it looks like the plastic encasing the metal bar which holds the headrest together snapped. Googling online I saw that this is a common issue with dodge, jeep, and Chrysler cars.
... I am now worried my driver side active headrest restraint system could go off while I am driving and cause an accident, as the passenger one definitely flew forward fast when it snapped.Read more

On [XXX], I was parking my 2025 VW ID.4 car in a parking spot in B mode and applying the brake to come to a stop. The car suddenly accelerated forward (of its own will) at maximum speed such that the car ran over a bollard, a sidewalk and into a brick wall causing significant damage. In the space of perhaps 10 feet at most the car went from a stopping position to taking out a brick wall on a comm
... ercial building. This is so incredibly dangerous as someone could have been on the sidewalk and killed. I could have been killed as my airbag never deployed despite charging with great speed into a brick wall! NHTSA must do something to resolve this. VW has overwhelming knowledge of this defect. In June 2025 a class action lawsuit was filed against the VW for this exact reason - link here: [XXX] Per the lawsuit there have been deaths related to this defect. This was never disclosed to me when I bought the car....as a matter of fact, the EV safety features in their sales pitch was one of the overwhelming reasons I bought the car. There are also many blogs related to this issue describing almost exactly what happened to me. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Driving as normal when an alert for a front passenger safety restraint system fault appeared. It went away on its own. I thought it was a software glitch. It would periodically reappear, then go away after a driving session. Last week it persisted between sessions. I learned that if the seat was moved slightly forward or back, then the alert would go away. I scheduled a service with Tesla. Tesla a

Vehicle Information: 2023 Tesla Model 3 VIN: [XXX] Incident Date: [XXX] Location: [XXX] Summary of the Problem: On [XXX], while Autopilot was active, my 2023 Tesla Model 3 suddenly accelerated without driver input. I was pressing the brake pedal, but the vehicle surged forward uncontrollably, struck a fence and a tree, and caused major property damage. Both I and my wife (passenger) required u
... rgent care treatment after the crash. Details / Safety Defect Evidence: •Tesla’s own raw logs show the accelerator spiked to 100% immediately before impact. •Brake pressure was recorded prior to impact, proving the brake pedal was applied. •Despite this, Tesla’s official Vehicle Data Report falsely states “manual brake not applied.” •Autopilot did not disengage when the brake was pressed, contrary to Tesla’s design claims. •The vehicle logged a “Near-Deploy Collision” at ~27 mph, yet the airbags did not deploy. Why This Is a Safety Defect: This incident reflects multiple critical failures: 1.Sudden unintended acceleration. 2.Failure of brake-override safety design. 3.Airbag non-deployment in a crash severe enough to trigger “Near-Deploy.” 4.Inaccurate Tesla reporting that conceals braking input. This defect created an extremely dangerous situation that could easily have resulted in fatalities. I am requesting NHTSA investigate Tesla vehicles for unintended acceleration, brake override failure, and airbag non-deployment. We have contacted dealerships in other states who advised that they've been waiting on these parts for over a year. We are unable to fix the vehicle, and it is unsafe to drive with anyone in the passenger seat. We opened an issue with Hyundai Corporate, but did not receive any resolution. We were told that they would reimburse us for the cost of a rental when we needed to transport additional passengers. They later denied our request because 1) They claim it is not a safety-related issue, and 2) it occurred outside the state where we initially took the vehicle to the dealership since we are on a long-term work assignment and temporarily living in another state.Read more

I own a 2015 Subaru Forester that was involved in the NHTSA recall ID 19V-701 - Subaru safety recall WUM-98 in 2020. My VIN is [XXX] and my dealer is Lancaster County Subaru in Lancaster, PA. For about four months in the end of 2019 until February of 2020 we were unable to use the passenger seat due to the air bag problem which was intermittent. This required either my wife or I to ride in the ba

But our dealer and Subaru of America could not assure us that the air bag would deploy properly if we were in an accident because the system only does a system check when first starting the car, not while driving. Two weeks ago, about [XXX] my airbag issue has returned, again it's intermittent. I opened a case with Subaru of America, Case # [XXX] and they say there is no further recall for this issue. This is the same issue that was resolved by the NHTSA recall ID 19V-70. When it faults an orange SRS image appears on the screen then it shows an orange "i" on the top part of the center screen along with an orange image of the airbag error in the tachometer area and shows the airbag system is off.
