While driving normally at highway speed on I-880 South, the braking system failed to decelerate the vehicle. When the brake pedal was pressed firmly, there was no normal slowing—only a sliding sensation, as if the wheels locked and the car moved forward on ice. This led to a collision with a stopped vehicle ahead. The following day, the Mazda App displayed a red alert message: “Brake system ma
... lfunction. Braking effectiveness may be decreased.” The vehicle’s airbags deployed very forcefully, causing bruising to both driver and passenger, even though the impact was not severe. Three days before the incident, the vehicle had a scheduled maintenance and recall service at a Mazda dealer, including a brake inspection and air-bag recall repair. The vehicle was declared a total loss by the insurance company and is currently stored at Copart Hayward, CA. The problem has not been inspected by Mazda yet, but a formal safety review is requested. The issue posed serious safety risks due to total loss of braking control and unexpected air-bag deployment.Read more

To Whom It May Concern, I am writing to formally report repeated safety system failures in my Hyundai vehicle that have resulted in significant injury and ongoing hardship. The first incident occurred in 2021, when my vehicle was struck from the front by another driver who fled the scene. Despite the impact causing visible front-end damage, the airbags did not deploy, and the seatbelt did not en
... gage properly. The vehicle was repaired by an authorized Hyundai dealership, yet at no point was I informed that the vehicle’s safety systems might be compromised or that it should not be driven. The second and more severe incident occurred in December 2024, when my vehicle was rear-ended by a commercial truck and subsequently pushed into four other vehicles. Once again, neither the airbags deployed nor the seatbelt engaged, resulting in multiple bodily injuries for which I am still undergoing treatment. After reporting this second failure to Hyundai, I was informed that the airbags and seatbelts may not activate when the vehicle is struck from behind. However, this explanation does not address the fact that the same safety failures occurred in the 2021 front-end collision as well. I believe these repeated malfunctions indicate a serious defect in the vehicle’s safety restraint and airbag systems that poses an ongoing danger to drivers and passengers. I am requesting a full investigation into these safety system failures, including whether the dealership’s repair process or Hyundai’s design or manufacturing contributed to these malfunctions. Due to these failures, I have experienced significant physical injury, emotional trauma, and ongoing medical issues, and I am seeking appropriate accountability and resolution.Read more

The issue has not been reproduced or confirmed as a known defect by the dealer or manufacturer. There were no warning lights, error messages, or other symptoms prior to the deployment. Description of the Issue: The airbag (SRS) warning light on my 2013 Hyundai Elantra GT Hatchback intermittently turns on and off and flickers when turning the steering wheel, indicating a potential fault in the spiral cable assembly (clock spring). Hyundai issued a Clock Spring Extended Warranty (TSB #17-BE-004-1) for certain vehicles, but my vehicle is not covered. A complete failure of the cloc
... k spring may prevent the driver’s airbag from deploying in a crash, creating a serious safety risk. The airbag warning light first appeared on [XXX]. The issue occurs during normal driving, particularly when turning the steering wheel. Safety Concern: If the clock spring fails completely, the driver’s airbag may not deploy during a collision, significantly increasing the risk of injury or death. Requested Action: I request that NHTSA investigate the intermittent airbag warning light issue in 2013 Hyundai Elantra GT vehicles, focusing on potential clock spring failures, and consider a safety recall or other remedy to ensure that driver airbags deploy reliably. Additional Information: The vehicle is currently located in Ottawa, ON, Canada. Photos of the dashboard with the airbag light illuminated can be provided if requested. There was also a delayed airbag deployment from the back of the driver seat that only ripped the seat but never deployed fully. The smell of fire / burning and debris was all that happened from the driver seat. I requested a crash report from Tesla which outlines the speed and airbag deployment. The report says that knee level airbags deployed but they never did. I crossed referenced crash test speed data and various complaints about Tesla airbags. The data reviewed seems to indicate that I experienced failed equipment which is currently under review. I’ve also reviewed various sources of information about 7 2024 Model 3 driver side airbag known issues, where Tesla only recalled one vehicle. I’ve been told that other recalls for this issue are u see evaluation and to keep checking my vin for details. I share this because serious injuries can be caused by this failure, especially when hit by an SUV. The vehicle was totaled, not drivable and towed from the scene.
