On September 2, 2025, my 2025 BMW X5 xDrive50e was involved in a head-on collision with another vehicle. Despite the severity of the impact, the following safety systems failed to deploy: Front airbags Driver seatbelt pretensioner BMW Assist / automatic emergency notification Witnesses reported that my vehicle briefly went airborne before landing with the rear wheels up on a curb. I suffered ne
... ck injuries, and my vehicle sustained significant frontal damage. BMW later downloaded the Event Data Recorder (EDR) and confirmed: A frontal non-deployment event Three fault codes in the airbag control module (ACSM) post-crash The crash algorithm activated and reset ΔV reached approx. 12.4 mph longitudinal Forward Collision Mitigation system issued an acute warning 0.5 seconds before impact BMW states the “impulse was not sufficient” to trigger restraint deployment BMW has refused to provide: The deployment thresholds (ΔV criteria) for airbags or pre-tensioners Any engineering explanation for why restraint systems did not activate Any technical rationale for the ACSM fault codes Any escalation beyond Customer Relations All I have been given is an unsigned summary letter and the raw EDR file. BMW Customer Relations stated that engineering is “not customer-facing” and that no additional information will be provided. This leaves serious concerns that: The restraint systems did not function properly in a real-world impact. BMW is refusing to disclose the engineering basis for its conclusions. Consumers may be at risk if this is a repeatable failure mode. I am requesting NHTSA review this incident as a potential defect in the airbag, pretensioner, and/or crash-sensing system in the 2025 BMW X5. I received a safety alert on my car screen: Front passenger safety restraint system issue. Service is required. The front passenger safety restraint system fault typically involves a malfunctioning occupant weight sensor in the passenger seat, which controls whether the airbag activates or deactivates EVGOER. This is a serious safety concern because the airbag may be disabled at that seat locatio
... n in the event of a crash. The issue is often caused by loose, broken, or unplugged harnesses or connectors under the seat, particularly related to the Occupant Classification System (OCS) Tesla Owners OnlineDigital EV Doctor. Many owners report this happens because wires under the seat get crimped over time due to Tesla's "easy entry" feature that automatically moves seats. Since this a widespread Tesla Model 3 problem, I believe this should be a recall.Read more

I was involved in an accident on the interstate traveling at about 80 mph and I hit a guard rail. Because of this water intrusion, the following safety systems have malfunctioned or failed: • ABS and DSC (stability control) warnings • Chassis stabilization warning • Frontal collision and pedestrian warning malfunction • Airbag system warning • Loss of audible warning tones • MOST bus communication failure • Electrical modules intermittently shutting down or throwing codes These
... failures affect braking, stability control, crash avoidance, and driver alerts, creating a significant safety risk while driving. This is not normal wear or maintenance. Many BMW owners have reported the same issue: clogged or poorly designed sunroof drains that route water directly into areas containing critical electronics. Once water reaches these modules, it causes electrical shorts and safety system failures that require expensive repairs. BMW dealerships typically diagnose this as “water damage,” but the location of the drains and the placement of the electronics indicate a design flaw that allows a simple drain issue to disable major safety systems. I am filing this complaint because the water intrusion should not be able to cause total failure of ABS, DSC, collision warning, and airbag-related systems. This is a safety-related design defect, not wear and tear. I request NHTSA review and investigate BMW sunroof drain failures and their impact on essential vehicle safety functions.Read more

On October 8, 2025 My cousin, [XXX] , and I were on highway Mile 113, 65 North and were about to exit when an ambulance exited onto the highway ahead causing the traffic to suddenly stop. As the traffic stopped, I hit my brake, which usually jolts when I hit it but it didn't this time, the brake didn't feel the same, the brake was soft and it felt like my car wasn't stopping. The warning red light
... that usually goes off didn't and I hit a Jeep at a speed of about 55 to 60 mph yet none of my airbags diployed. We smelled fire and saw some smoke come in from the vents but couldn't see over the hood that had been bent up, both of our seatbealts had been stretched out so that when we took them off, we could not put them back on, and the adjuster I had on my seatbealt to keep it off my shoulder completely broke off. I pull over as far as I could safely since I couldn't see. The driver of the Jeep got out of her car and inspected it, she came back to the passenger side of my car and said that she was in a rental, there was no damage and couldn't afford to turn in accident in so she had to leave, and left the scene. Both my cousin and I were feeling pain in our chest so when 1st responder came, he called for an ambulance. My car was towed and we were both taken to Eskenazi ER where my cousin was treated and released the same day. I was admitted into the hospital ICU and had emergency cervical fusion spinal cord surgery on October 9, 2025 and wasn't released until October 17, 2025 with ongoing treatment for therapy and loss of mobility of my right arm which is due to nerve damage and abdomen bruising, I am still recovering and have ongoing treatments for my spine. I noticed the alarm didn't sound when it should have times before but since nothing was said about any problem when I took my car in for service in September, I didn't think that there would be an issue like this. A police report was made but I do not have a copy yet. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I had a passenger seat belt light on and took it to Jim Coleman Infiniti in Bethesda,MD. They then were going to charge me a diagnostic fee to fix it, but instead proceeded to perform a zero point seat reset and got the error code B00A0 and then the passenger air bag light came on. They subsequently said that the vehicle was not covered under any recall or warranty and said that the seat needed t
... o be replaced and informed me that it would be around $8000 at that time. Now it must be more. Looking at the recall campaign 16V244000 my vehicle does not qualify since it is one model year later, but I have seen others have the same issue that I have with the 2017 Q50. There is infact one in Florida that has a similar experience as mine. Talking to the dealer and Infiniti neither wanted to do a goodwill jesters and would only reduce the cost by a few thousand dollars. That still meant that I would need to put out more than $4000 to get a safety issue that should be covered. The symptoms and error codes are exactly the same. So I ask that you extend the recall campaign to the 2017 model year as well for the Q50. It is a safety hazard that will be present on the streets. The dealer also refuses to do the module reset saying that it will fail again. Pleas ask Infiniti to cover the cost of ensuring this critical safety for the passenger side is resolved for all model years that have the error code from the recall.Read more

I purchased a used 2019 GMC Acadia from Grieco Honda around November 20, 2023. They showed me a CarFax report indicating the vehicle had never been in an accident. Immediately after the purchase, I reported several issues and attempted to return the car under their 30-day warranty. However, the salesperson convinced me not to return it, assuring me that they would address all of the problems. Fro
... m the beginning, I complained that the car shook violently when driving between 45–60 mph, especially during acceleration. The vehicle took an unusually long time to heat up, and the brakes were loud and squeaky. A few months later, after driving over a large speed bump, the airbag warning light turned on. During my next oil change, I mentioned this issue, along with a problem with the window sensor. They dismissed my concerns and told me to take the car to GMC across the street because they lacked the tools to diagnose it. At GMC Hurd, I was told the diagnostic service would cost $160 and that I would need to leave the vehicle for the day. At the time, I couldn’t leave the car because I needed it to pick up my children. Now, in November 2025, my inspection is due. I brought the car back to GMC, and they informed me that the vehicle had been in a major accident prior to my ownership. They discovered that the airbags had previously deployed and were stuffed back into the driver’s seat and sewn shut. I have never been in an accident with this car. I reported this to Grieco since they were the ones who inspected the vehicle in November 2023 and passed it. I believe they managed to get the car through inspection without properly or thoroughly examining it before selling it to me. They have dismissed my concerns and told me there is nothing they can do. This is outrageous, as the airbags would not have deployed in an emergency, putting my safety at serious risk.Read more

Vehicle: 2014 BMW X1 xDrive28i Incident Date: 11/21/2025 Location: Interstate highway in Wisconsin Description of the Issue: My husband was driving our 2014 BMW X1 xDrive28i when he was involved in a front-end collision at approximately 40 mph. He struck the rear of another vehicle with the front passenger-side portion of the BMW. Despite the force and angle of the crash, neither the driver airba
... g nor the passenger airbag deployed. Immediately after impact, heavy smoke began coming through the dashboard vents within seconds, and the vehicle caught fire within approximately one minute. The entire front of the vehicle was destroyed by the fire. The airbags never deployed at any point, including during the fire. We believe this represents a serious failure of the airbag system or the crash sensors. A collision at this speed and angle, followed by catastrophic fire, should have triggered airbag deployment. This creates a significant safety risk for the driver and any passengers. What Happened to the Occupant: The driver experienced impact to the chest and upper body that likely would have been mitigated by airbag deployment. He required medical evaluation. Why We Are Reporting: We want to notify NHTSA of this potential airbag system failure, as it may indicate a defect affecting additional BMW X1 vehicles. We are requesting that the incident be reviewed to determine whether: The frontal crash sensors malfunctioned The airbag control module failed Wiring or electrical issues prevented deployment A previously unknown defect exists in this model This was a dangerous failure of a critical safety system. There was no prior airbag warning light or malfunction indicator before the crash. The vehicle is a total loss and is currently with our insurance company. We will request the airbag control module (ACM) and crash data from the insurance inspection. The vehicle has not previously been involved in an airbag deployment. The vehicle had up-to-date maintenance.Read more

Incident date: Mar 7, 2025
What component failed and is it available for inspection? The front driver-side roof seam on my 2022 Toyota Tundra developed corrosion under the paint and eventually opened enough for rainwater to enter the cabin. Water drips directly into the A-pillar area onto the side curtain airbag wiring, squib connector, and padded harness. It also reaches the dashboard and electrical buttons near the steeri
... ng wheel during rain. The roof seam, A-pillar wiring, interior rust staining, and all affected components remain available for inspection. How was safety put at risk? The A-pillar contains the SRS curtain airbag system, its wiring, and connectors that should never be exposed to moisture. Continuous water intrusion over many months can cause corrosion, increased resistance, short circuits, or damage to the airbag inflator/squib circuit, which may lead to unintended deployment or failure to deploy in a crash. Water reaching the dashboard controls and electrical switches could cause intermittent electrical faults or malfunction of driver controls. These issues present a significant safety risk because the vehicle’s airbag and electrical systems may not function as designed. Has the problem been reproduced or confirmed? Yes. The Toyota dealer has repeatedly observed the active leak, interior rust streaks, and bubbling/corrosion along the roof seam. The water intrusion occurs every time it rains. Has the vehicle been inspected? Yes. Toyota has inspected the truck multiple times, including by a Field Technical Specialist. However, Toyota has been unable to provide complete documentation or photos from the FTS inspection, and the underlying safety issue remains unresolved. No inspections have been done by police or insurance. Warning lamps or messages? No warning lamps appeared. The first symptom was water leaking into the cabin around March 2024, followed by rust streaking inside the A-pillar and exterior bubbling. The issue continues during rainfall.Read more

1. What component or system failed or malfunctioned, and is it available for inspection upon request? Multiple safety systems failed simultaneously, including: SRS Airbag System Pre-Collision System Vehicle Stability Control (VSC/TRAC) Advanced Ultrasonic/Sonar System The engine is also subject to Safety Recall 25TA14 (potential loss of motive power). The vehicle is currently located at Crown Toyo

I was driving on the 60 freeway with my two small children when all of these safety systems failed at the same time. The truck also began pulling to the right and hesitating during acceleration. With the airbag system, pre-collision system, and stability control disabled, our safety was seriously at risk, especially at freeway speeds. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. Crown Toyota advised me not to drive the vehicle and instructed me to have it towed immediately as an emergency. They confirmed the warnings and safety alerts. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? The vehicle was inspected by Crown Toyota service technicians. They noted the safety system malfunctions and the active engine recall. Toyota Corporate has opened a repurchase case: #251118002114. 5. Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear? Yes. The RV currently is sitting at MB of Modesto where it can be inspected. The issue, when we attempted to rotate the rotatable front passenger seat, and an Airbag error was displayed on the dash and the vehicle was disabled, would not start. When we rotated the seat back we assumed it took pressure off of the wiring harness and the error disappeared and the vehicle started. We tried to rotate the
... seat back to its secure position but wires had become tangled under the seat and we could not secure the seat. We wanted to drive the RV to MB of Modesto but were concerned the airbag might deploy and disable the RV while we were driving. We decided to have the RV towed to Mercedes of Modesto. We discovered there were three separate recalls on this issue, MB Mfg #VS3DREHSIK, NHTSA Mfg. recall # 20V-322 and CA Bureau of Auto Repair # 20250701. Mercedes states that the recalls do not cover our specific chaise but the problem is exactly as described on the NHTSA recall. MB of Modesto inspected the RV and generated a Repair Order (RO) which is attached along with the NHTSA recall. The RO seems to imply that when Tiffin added the RV house they made changes under the seat that caused the problem thus the problem was not Mercedes. We contacted Tiffin and they said the problem was not theirs. Our Good Sam Extended would not cover it because it is a wiring harness issue. Seems like this is a MB and Tiffin wiring defect and the problem should be included in the recall.Read more

The emergency call (eCall) system activated and connected the contact with an operator, who then called an ambulance and the police on the contact's behalf. A police report was filed. Both the contact and her husband, who was occupying the front passenger's side seat, were transported to the Emergency Room by ambulance. The contact sustained Hematomas on both legs, and the contact's legs were swollen. The contact was admitted for 5 days and was discharged to a rehabilitation center. The contact's husband did not sustain any serious injuries. The vehicle insurance company towed the vehicle to a tow yard. The dealer was notified of the failure and referred the contact to the manufacturer. The manufacturer advised the contact that the vehicle insurance company would need to file a claim with the manufacturer; however, the insurance company had already deemed the vehicle a total loss and had sold the parts of the vehicle. The failure mileage was approximately 5,300.Read more

The vehicle was sold with a “minor accident” reported, but the condition suggests the damage may have been more significant. After purchase, I discovered the rear seatbelt did not work and required interior panels to be removed and replaced. A non-working seatbelt points to possible prior impact or structural damage affecting the restraint system. The vehicle also has an active safety recall f
... or the side curtain airbags. The combination of a recall and a failed seatbelt raises concern that the vehicle may have been in a more severe collision than reported or repaired incorrectly. A failed seatbelt is a major safety hazard and should not be present at sale. The defect was confirmed by the dealer’s service department, and the vehicle has been in their possession since November 10, 2025. No warning lights appeared; the issue was found only when trying to use the seatbelt.Read more

I am reporting a safety-related violation. The dealership told me the vehicle had only a minor accident with no airbag deployment. This was false. Carfax shows the vehicle actually had a major accident with airbag deployment, which they failed to disclose. This misrepresentation affects the safety system, airbag sensors, and the ability of the airbags to function properly in future collisions. Th

On 09/16/25, I was exiting the Florida turnpike onto Sawgrass Highway via the exit ramp on exit 71. As I decelerated from 60 MPH to 35 MPH to match the speed of the exit ramp, I was in the left most lane in the two-lane exit ramp. My car spun out and I lost control of the vehicle, which caused the vehicle to spin through the two lanes of the ramp several times and ended up drifting onto the edge o
... f the retention pond on the left of the exit ramp. I probably had spun around over three times and my car hung off the edge of the retention pond with the only thing preventing my car from falling into the pond being the rocks to prevent erosion on the side of the pond. The airbags on the drivers side and the curtain airbags both had deployed upon the car coming to a stop. The seatbelts were also locked upon trying to exit the car. The car was fully serviced prior to the incident and in good standing with BMW. The brakes were recently replaced a month ago and regular milestone inspections were conducted on the car. The road conditions on the day in question were great with no water or puddles on the road.Read more

Component/System That Failed: The airbag system, forward-collision sensors, and collision-warning/braking system failed. Despite a severe frontal crash, no airbags deployed and no warning or automatic braking occurred. The Bosch Crash Data Report shows abnormal sensor spikes (up to +52g), inconsistent Delta-V readings, and failures in object detection (“No Object” recorded before impact). The
... components remain available for inspection through the body shop and insurer. Safety Risk: My family and I were injured because the vehicle provided no warning, no crash avoidance, and no airbag deployment during a major impact requiring a full frame replacement. This failure places all Jeep 4XE drivers at risk of severe injury or death in similar conditions. Has the Problem Been Reproduced? The problem occurred unexpectedly during the crash and cannot be safely reproduced. However, Stellantis’s inspection did not explain the sensor irregularities or system failures. Vehicle Inspections: The vehicle has been inspected by Stellantis (File 94277361), my insurance company, and the collision repair center. Stellantis claimed “insufficient deceleration,” which contradicts the severity of damage, sensor data irregularities, and the fact that the other vehicle’s airbags deployed. Warning Lamps/Symptoms Before Failure: For weeks before the crash, the Jeep produced false crash warnings, beeping, and forward-collision alerts with no obstacles present. During the actual impact, all warnings stopped, and the system failed completely. This sudden shift from false alarms to total silence suggests a sensor or module defect. Urgency: Multiple active recalls already affect this vehicle line. I fear this Jeep is unsafe to drive even after repairs. I am requesting NHTSA’s urgent assistance and investigation into this failure to prevent further injuries.Read more

I own a 2019 Model X which I have been driving for six years without any driving issues. I did have a few incidents in which the screen went black while driving. That was scary - driving without controls. These issues reported to Tesla and were addressed by Tesla. Today I was pulling into a parking lot when all of a sudden my Tesla automatically accelerated to high speed. I could not control or
... stop the vehicle despite applying hard brakes. The car crashed through a metal fence and continued to accelerate through the wall of a building. The only reason it finally stopped was because the back tires got stuck on the ledge of the wall. The Tesla went through the metal gates and walls of the building, but the safety airbag never deployed. The car has been towed away and reported to the insurance immediately after the police report. There was no warning messages or indication prior to the incident.Read more
