The 2015 Chevy Traverse LTZ that I own is presenting with the same problems as many of its body style and year. The water intrusion has caused wiring to corrode in the side detection system & the drivers door wiring harness is presenting with open circuits. The LH side object module is throwing code U0233:00. They tested circuits at RH module. Found water intrusion in module, & corrosion on termi
... nals & harness. Due to nature of location of modules, they recommend replacing LH & RH modules & harness. DTC B0085:05 current. Tested circuits & found open circuit internal to drivers door harness. Recommend replacing harness. The air bag code threw 2 days after I bought the veh 04/01/25, I attempted to contact the dealership I bought the vehicle from, AutoCredit in Hayden, ID, who admitted to the dangerous code being present, and that they were going to do nothing about it. Some time late April, my veh threw the code for side detection. On 05/27/25 I took my veh into Taylor & Sons Chevy in Ponderay, who dx my codes, and advised the above was needed. They also confirmed the fact that when that airbag code is present, that none of my airbags would deploy at time of impact if a collision occurred. This is a huge safety risk as much of my daily commute is on the hwy, and the influx of drivers in my area not familiar with the roads is increasing daily. My side detection system is a safety hazard as well due to the Traverse’s blind spots. Thank you for your time. I did not know what to do until I contacted Chevy directly who advised me to contact the NHTSA.Read more

The 2016 Nissan Maxima (VIN: [XXX] ) I purchased in January 2025 has been confirmed by a certified mechanic to have no driver airbag installed and no passenger side curtain airbag installed, despite a previous airbag deployment shown on the vehicle history report. There is no record of any airbag replacement or repair. The vehicle was sold to me in this unsafe condition without disclosure. Driving
... the vehicle presents a serious safety risk because no airbags will deploy in a collision. Nissan and the selling dealer did not repair or disclose this federally required safety equipment failure (49 U.S.C. §30122). Safety Risk: This defect puts me at severe risk of injury or death in a crash. The airbag system is nonfunctional, and the vehicle is being operated on public roads without any airbags installed. This vehicle should not have been sold or financed in this condition. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

When right front pax sits in seat and fastens seatbelt, the airbag indicator correctly says "ON." But after a few minutes, the airbag turns "OFF" and the driver's dash gets an "unfastened seatbelt" warning and a "faulty airbag" warning. All of these occur while the front right pax has remain seated and buckled. If the pax unbuckles and exits the seat, these incorrect warnings all remain the sam
... e. The right front pax seat belt buckle has been replaced with no effect. The seat has been recalibrated (seat sensors) with no effect. The wires under the seat have been disconnected, cleaned, and re-connected with no effect. This is a safety issue in that the airbag on that front right side de-activates when a passenger is in that seat. There is a Toyota Recall Campaign for this identical problem, but it does not include my model year (2015). Requesting NHTSA get Toyota to extend their recall to include 2015 models, please.Read more

Component/System: Passenger Occupant Detection System (PODS) / Airbag system — Recall 74E3 / NHTSA 21V874. What Happened: The passenger airbag may switch off even when the seat is occupied due to a faulty PODS wiring connection. The warning message “Passenger Airbag Off” appears with a chime. This is an active safety recall that was issued before I purchased the vehicle, but the recall was
... not disclosed at the time of sale. Safety Risk: If the system shuts off the airbag incorrectly, the passenger may have no airbag protection in a crash, creating a serious risk of injury or death. Inspection Availability: Vehicle is available for inspection. I live on the Big Island of Hawaii, where there is no Audi-authorized repair facility. Reproduction/Confirmation: Symptoms match the known recall defect. Audi has confirmed the recall applies to my VIN, but no local dealer exists to inspect or repair it. Manufacturer/Dealer Inspection: Audi has refused to cover shipping to the nearest authorized dealership on Oahu, which is the only location where the recall can be performed. Therefore, the defect has not been inspected due to lack of accessible remedy. Warnings/Symptoms: Airbag warning light illuminates and a message appears indicating the passenger airbag is off, along with a chime. Additional Notes: I cannot safely use or sell the car because the required safety repair is inaccessible. Audi’s refusal to provide transportation makes it impossible to complete the mandated recall.Read more

My 2022 Tesla Model X has two recurring safety-critical defects that remain unresolved despite multiple repair attempts. 1. Airbag / Restraint System Failure (RCM2 / OCS Defect): The vehicle frequently misclassifies the front passenger seat. It shows a passenger when the seat is empty and activates passenger seatbelt warnings incorrectly. Recently the vehicle displayed: “Front passenger safety
... restraint system issue: Service required (RCM2_a064).” Tesla attempted to repair an electrical harness in the seat, but the problem returned. A Tesla technician documented that the seat may still need a new occupancy sensor. Because this affects the Occupant Classification System and airbag deployment logic, it raises potential FMVSS 208 compliance concerns. 2. FSD / GPS Safety Hazard (5 Unsuccessful Repair Attempts): Across five service visits, Tesla attempted to fix GPS drift and unsafe FSD routing. The vehicle still attempts incorrect turns, performs premature lane changes, and misinterprets intersections. One recent incident involved FSD attempting a left turn one intersection early, nearly causing a collision. Tesla replaced the GPS antenna, recalibrated cameras, and replaced the computer, but the unsafe behavior continues. These two defects—incorrect airbag/OCS behavior and hazardous automated-driving/navigation behavior—create a significant safety risk to occupants and other drivers. Both defects persist after repair attempts. I can provide service records, videos, and documentation upon request.Read more

I am reporting a serious safety defect involving a vehicle purchased from [XXX] on [XXX]. The vehicle was later involved in a collision that resulted in a total loss. According to the police report, I was not at fault. During this crash, none of the airbags deployed, which indicates a possible defect or prior undisclosed issue with the airbag system. The vehicle was sold with a warranty and was r
... epresented as safe and road-ready. The dealership also provided and managed the full-coverage insurance policy. However, the total-loss claim has been delayed and obstructed, and the dealership has not taken responsibility for ensuring the vehicle’s safety or functionality. There is significant concern regarding the condition of the vehicle prior to sale. Records show that Byrider cleared the vehicle’s computer on July 16, 2025, less than a month before purchase. This may have erased important diagnostic information, including any prior airbag-system faults. Clearing the computer in this manner raises questions about whether a known issue was hidden or whether mandatory diagnostic data was removed before the sale. At the time of purchase, I signed an airbag disclosure form in which Byrider stated: •They had “no knowledge of any reason why the airbag system would not operate properly,” and •They “cannot guarantee that the airbag system will deploy in the event of an accident.” Despite this disclosure, the fact that the airbags did not deploy in a crash severe enough to total the vehicle suggests a significant safety defect, potential misrepresentation, or a failure to comply with federal safety requirements. Airbag non-deployment in a total-loss collision indicates a system failure that poses a serious risk to consumers. VIN: [XXX] Owner: [XXX] Purchase Date: December 2022 I am filing an urgent safety complaint regarding my 2023 Kia Telluride. ... ft engagement defect (vehicle may roll out of Park) Kia never properly notified me of these recalls and never repaired them, despite my family using the vehicle daily. I drove this SUV with my autistic child and infant grandchild, believing it was safe. I have now discovered that Kia allowed me to operate a vehicle that could: •Fail to protect us in a crash •Catch fire •Roll away unexpectedly •Malfunction in ways that could cause severe injury or death This is a serious safety negligence issue. Kia did not offer a loaner vehicle, did not notify me promptly, and has not provided any remedy despite these recalls being federally regulated. I am requesting NHTSA intervention to: 1.Investigate Kia’s failure to properly notify and protect consumers. 2.Require Kia to provide a full replacement vehicle (2025/2026 Kia Telluride) or a full buyback due to prolonged safety risk. 3.Ensure Kia corrects VIN-level recall handling and compliance failures. 4.Document that Kia exposed vulnerable passengers (disabled child and infant grandchild) to unsafe conditions for almost two years. My family has been placed in danger this entire time, and Kia has not provided a timely or adequate remedy. During snowy but manageable conditions, the brake system on my 2021 RAV4 Prime SE failed to respond, and the vehicle did not decelerate when I pressed the brake pedal. When I applied the brakes, my RAV4 Prime did not slow down at all—there was no deceleration, no ABS pulsing, and no traction control intervention. The vehicle continued moving forward as if the brakes were not engaging. At the same time, I saw a dashboard warning indicating a Pre-Collision System (PCS) malfunction. I attempted to turn the wheel to avoid the collision, but the vehicle did not respond to steering input quickly enough to prevent impact. Despite the collision, no airbags deployed. The vehicle has been regularly maintained (oil changes every 5,000 miles, new tires last year, all service intervals followed), and there were no warning lights or issues beforehand. Safety Concerns: •Complete or partial brake failure •Electronic braking or stability control malfunction •Pre-Collision System malfunction •Airbag non-deployment in a front impact •Potential systemic issue affecting Toyota hybrid/Prime models This incident caused significant fear and distress for both me and my child who was in the vehicle, and raises serious concerns about whether similar failures could occur for other owners.Read more

In June 2025, I brought my vehicle to the dealership (Hendricks Chrysler Dodge Birmingham, AL) for a full diagnostic inspection, and at that time I was told everything was fine. There were no warnings, no notes about electrical issues, airbags, coolant leaks, engine leaks, or any of the major concerns now being reported. On my most recent visit (October 2025), the dealership suddenly reported mult
... iple “Immediate Attention” failures, including: • Airbag system issues • Electrical system failure • Coolant system leak • Water pump leak • Engine oil separator leak These items were never mentioned during my previous full diagnostic despite being safety-related components that typically show symptoms or codes. Because these new issues involve critical components like the airbags, engine, and electrical system, my safety was put at risk. I have been driving the vehicle daily under the belief that it had passed a full diagnostic months earlier. If these problems really existed at that time, then I was unknowingly operating an unsafe vehicle. If they did NOT exist, then the sudden appearance of multiple major failures raises concerns about incorrect diagnostics or improper prior inspection. I have not received any warning lights except the airbag light. The dealership has not provided a clear explanation as to how these issues appeared only months after a full inspection. This has caused significant concern regarding vehicle safety, diagnostic reliability, and whether the previous inspection was performed correctly. I am reporting a potential safety defect involving the seatbelt, airbag, and occupant detection system in my 2019 BMW 430i. At the time of purchase, the vehicle’s software had not been updated since 2018. Because the software was so outdated, multiple safety-related fault codes did not appear during the dealer’s pre-sale inspection or during initial diagnostic checks. During a later service a
... ppointment, the dealership informed me that they were unable to perform accurate diagnostics because the software was old and could interfere with sensor readings. After I paid for the software update, several safety-related codes appeared, including faults connected to the seatbelt/occupant classification system. None of these safety defects were disclosed to me prior to the sale. This raises concern that the vehicle had an existing safety defect at the time of sale, but the outdated software prevented the detection and reporting of these faults. If other vehicles are being sold without current software, this may result in safety-critical issues being hidden from consumers and inspectors. A malfunctioning seatbelt or occupant detection system significantly increases injury risk in a crash. If outdated vehicle software suppresses or fails to display safety-critical faults, consumers may unknowingly be operating unsafe vehicles.Read more

On October 17, 2025, I was driving my 2014 Volkswagen Jetta at approximately 25–30 mph on the bypass in Muscatine, Iowa. Another vehicle cut into my lane and struck the front driver-side of my car. My vehicle suffered significant front-end impact, including wheel/tire damage, bumper and fender damage, and visible suspension impact. During the crash, the driver-side airbag did NOT deploy, even t
... hough the impact was directly to the front driver-side area where the crash sensors are located. The airbag warning light did not come on before or after the crash. Because the force of the impact was strong enough to damage the front wheel, suspension components, and bumper structure, I believe the airbag system (airbag module, front crash sensor, or related wiring) failed to detect the crash properly. A collision at this speed and angle should have triggered airbag deployment. This failure put me at risk of serious injury. I am reporting this because the airbag system did not function as intended in a front-end collision, and I am concerned there is a defect affecting the airbag deployment sensors or modules in this model.Read more
