SAFETY ASSESSMENT: 2019 Porsche Cayenne S Dashboard Material Failure Vehicle Identification: VIN [XXX] , Current Odometer: 46639 Assessment Date: 12/11/2025 Evaluator: [XXX] , Registered Owner/Operator ________________________________________ EXECUTIVE SUMMARY The dashboard upper assembly in this vehicle exhibits progressive adhesive delamination and synthetic leather separation (colloquially “b
... ubbling”) across the instrument cluster housing and passenger airbag deployment zone. This defect, while initially cosmetic, presents measurable safety risks under Federal Motor Vehicle Safety Standards (FMVSS) by compromising passive restraint systems, driver visibility, and interior structural integrity during collision events. The failure pattern is consistent with documented manufacturing defects affecting 2018-2022 Cayenne production units. ________________________________________ TECHNICAL ANALYSIS Defect Manifestation: •Multi-point adhesive failure between dashboard substrate and synthetic leather wrap •Bubbles ranging 2-8cm diameter directly above passenger-side airbag module •Progression rate: 15% surface area increase over 4-month observation period •Material: Thermoplastic olefin (TPO) substrate with polyurethane coating exhibiting UV-degradation and outgassing Root Cause: Inadequate adhesive curing during manufacturing combined with incompatible material expansion coefficients, resulting in progressive de-lamination under thermal cycling. ________________________________________ SAFETY CONCERNS 1. PASSENGER AIRBAG DEPLOYMENT (FMVSS 208 Violation Risk) 2. DRIVER VISIBILITY IMPAIRMENT (FMVSS 101 Violation Risk) 3. INTERIOR STRUCTURAL INTEGRITY (FMVSS 201/214) 4. SECONDARY PROJECTILE HAZARD 5. OCCUPANT HEALTH & COGNITIVE LOAD This defect constitutes a latent safety hazard that degrades over time, not a static cosmetic issue. The combination of airbag deployment trajectory interference, driver visibility impairment, structural integrity compromise. The contact took the vehicle to the dealer. The manufacturer was notified of the failure, and a field inspector from Genesis stopped by the Hyundai dealer to retrieve the Event Data Recorder (EDR). The inspector informed the contact that the Event Data Recorder (EDR) showed that the contact's vehicle was a GV80. The contact stated that her vehicle is a G80. The field inspector then requested permission from the contact to retrieve the information from the Event Data Recorder (EDR) to change the vehicle information to a G80. The contact refused. The contact stated that the Genesis application on the contact's phone provided the following codes: B1378, which meant that the "Side Airbag Front-Driver Resistance is too High," and B1649, which meant "Crash Recorded - Rollover Event." The dealer replaced the control arm. The air bags were still deflated. The vehicle is at the contact's residence. The insurance company was informed. There was a recall on this vehicle from 2022, and we did not own this vehicle at that time. Recall passenger side airbag contact under seat can cause fire or electrical issues. I contacted Subaru and they said the recall was satisfied in 2022. If that's the case, the repair piece must have been faulty as the issue has come back. The dealership I took the car to said they have seen about 4 of this

I've asked more questions of Subaru since their answer and have not received a response. I would like to know why their website says this was complete July 15, 2025, not 2022. It was not complete in 2025, or we would have had the repair done. Any help in getting someone to see if this is in fact another recall and how to tell if the recall was truly completed in 2022, possible faulty recall parts now. I am submitting this complaint regarding failure to complete a federally mandated safety recall and the subsequent release of my vehicle in an unsafe condition after prolonged dealer custody. Vehicle Information provided above Safety Recall Information Recall Type: Federal airbag safety recall Recall Notice Received: 2023 Summary of Safety Concern I delivered my vehicle to authorized Kia dealershi
... ps for completion of a federally mandated airbag safety recall. The vehicle remained in dealership custody for approximately seven months without completion of the recall. During this period, the dealerships demanded payment for diagnostics and asserted that recall-related issues were “warranty-related” or “customer responsibility,” which effectively blocked completion of the safety recall. Despite my continued communication and willingness to proceed with recall repair, the recall was not completed. Kia declined to authorize repairs necessary to complete the recall. Vehicle Released Unrepaired and Unsafe The dealership ultimately demanded that I remove the vehicle without completing the safety recall. Upon retrieval of the vehicle, I discovered: Missing and/or altered vehicle components Extensive interior water damage These conditions were not present when the vehicle was delivered and occurred while the vehicle was in the dealership’s exclusive custody. The vehicle was released in a materially worse and potentially unsafe condition. Ongoing Safety Risk The federal safety recall remains unresolved. Electrical systems and safety components may be compromised. The vehicle is currently unsafe to operate, and repair is no longer a reasonable remedy. Basis for NHTSA Review This complaint concerns: Obstruction and noncompletion of a federal safety recall Improper conditioning of recall repairs on payment Release of a recalled vehicle without repair Potential compromise of safety systems while in dealer custodyRead more

Rear Belt Monitor Fault continues to be an issue. We received this warning light and supposedly had this repaired in October of 2024. Additionally, we are receiving an airbag warning related to the rear belt monitor. When driving even short distances the warning light appear

On Thursday, January 8, 2026, I was operating my vehicle when an oncoming single vehicle traveling in the opposite direction entered a curve at an excessive speed, lost control, and crossed into my lane. I attempted to steer away to avoid the collision; however, the oncoming vehicle struck my vehicle along the driver’s side. The collision caused significant damage to the driver's side and result
... ed in a failure of the passenger-side rear wheel/tire assembly, complete detachment of the tire from the vehicle. During the collision, none of my vehicle’s airbags deployed, despite the side-impact nature of the crash and the force of the collision. 1. What component or system failed or malfunctioned, and is it available for inspection upon request? The airbag restraint system failed to deploy during a significant collision involving the driver. The vehicle and airbag system remain intact and are available for inspection upon request. 2. How was your safety or the safety of others put at risk? The lack of airbag deployment during a driver-side impact increased the risk of injury to me as the vehicle operator. Airbags are designed to provide supplemental protection in collisions of this type, and the absence of deployment reduced occupant protection. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? No diagnostic inspection or reproduction testing has yet been performed. Evaluation by a dealer or manufacturer representative is pending. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? Police responded to the accident scene. The incident has been reported to insurance representatives. Manufacturer inspection has not yet occurred, but the vehicle remains available for review. 5. I am writing to formally report a serious safety defect in my Genesis vehicle and to request immediate investigation, documentation, and corrective action. Following a recent crash event, my vehicle’s diagnostic scan revealed DTC B1378, which is associated with a fault in the airbag system circuitry. This code is widely recognized as an indicator of an airbag defect and is consistent with condi
... tions that warrant manufacturer recall. During the crash: - Multiple airbags deployed, - The steering‑wheel airbag did NOT deploy, - The SRS system recorded a crash severe enough to trigger other airbags, - The vehicle now displays B1378, confirming an internal fault. The failure of the primary driver airbag to deploy during a crash is a critical safety malfunction. Combined with the presence of B1378, this raises substantial concerns regarding a defective airbag module, wiring harness, or sensor — all of which fall under manufacturer responsibility and federal safety standards. I am requesting the following: Immediate corrective action, including repair or replacement of all defective components. Documentation of this issue in your internal safety‑defect tracking system. Given the severity of this failure and its implications for driver safety, I am also submitting this information to the National Highway Traffic Safety Administration (NHTSA) for review. To Whom It May Concern: The purpose of my complaint is to bring to your attention reprehensible policies and misconduct displayed by the automaker, Acura, the premium subsidiary brand of the American Honda Motor Company. On 11/18/2025, I brought in my 2021 Acura TLX Tech w/SH-AWD in for a problem related to the brakes. The front brakes were and continue to make a jarring, very loud, high-pitched s
... queaking noise at low speeds. After 4 hours at Radley Acura of Falls Church, VA, the technician stated "could not duplicate" the concern. The problem persisted and I again returned on 12/18/2025 for the same issue. My vehicle has been at the dealership awaiting a response from Acura Client Relations on a claim that I submitted on 12/19/2025. Radley Acura diagnosed the issue as a well-known defect of 2021 TLX models, as evidenced by Acura's own service bulletin describing the very issue I continue to experience (and yet were unable to diagnose on 11/18). Radley Acura is stating that although my vehicle is 28,000+ miles UNDER the manufacturer's warranty, that by time, since purchase, it is outside the manufacturer's warranty by approximately 6 months. However, on 1/27/2025 (when my car was within manufacturer warranty and only had 17,215 miles on the odometer), I completed a lease buyout of my car. In addition, I paid to have it Acura Precision Certified. As part of this paid certification, Radley had my car for over a day to allegedly perform a 182-point inspection, and address a recall open for over a year (with a remedy from Acura - but allegedly no available repair parts to fix my car, until I requested to CPO my at the time of lease buyout). During their CPO inspection, the dealership failed to address the open service bulletin on the front brakes--a documented issue Acura has been aware of since before 2/15/2022, as established by their service bulletin. Now the dealership is stating the manufacturer's defect that they failed to address is $500 out of pocket expense.Read more

-Airbag warning light on -Horn not working -Steering wheel scroll buttons not working -Clicking noise when turning wheel This failure occurred during normal driving. Airbags will not deploy and horn is inoperable, creating a safety risk myself and my family. I have never been into any accidents and have only driven normal. These safety systems should not fail. This is a manufacturing issue.

Our Dash board cracked very badly from the top next to the speaker to the passenger side air bag. We purposely use a dash board cover and never place heavy things on it. This is a safety issue as if getting in an accident wouldn’t be bad enough flying shrapnel would be worse. We just had it fixed at a dealership to the tune of $2500. This is not okay as I see this is a constant problem.

I have a 2024 Kia EV6 I took my car in for an unrelated repair. Tech found an airbag code that I asked to be investigated. The airbag wiring harness was damaged and needed replacement. I had a service airbag warning go off days prior which was then followed by my parking brake warning and a constant seatbelt warning chime that happened concurrently. The vehicle had these issues cleared by my mechanic but hasn't been able to be resolved. From what i understand, the seatbelt sensor is faulty and should I have an accident in the near future the airbags will not deploy.
