My vehicle experienced a catastrophic engine failure requiring full engine replacement after a recall remedy (oil change with thicker oil) was performed and the vehicle passed the pico test. Engine symptoms became more pronounced after the oil change. In addition, the vehicle has experienced repeated steering system abnormalities requiring multiple repairs, including replacement of airbag-related
... components and the entire steering column, with abnormal steering noise persisting afterward. The dealership confirmed the engine failure after finding metal debris in the oil and diagnosing a spun thrust bearing, which resulted in a complete engine replacement under warranty. The steering issue was confirmed by dealership technicians, including a lead technician who acknowledged during a ride-along that the steering noise was not normal, despite multiple repair attempts and replacement of the steering column.Read more

Driver side Headrest Airbag Yes vehicle is available for inspection. No accident Yes injury The driver side headrest airbag deployed while driving without an accident happening. I suffered whiplash and headaches after the incident that happened on January 18th, 2026. The headrest airbag deploying could have caused an accident but lucky for me I was able to pull my vehicle over safely. There is
... a possibility of going to the doctors if neck pain and headaches continue. I will be taking it to the dealership on Tuesday January 20th, 2026 to find out what caused the airbag to deploy. It has not been inspected yet but will be tomorrow January 20th, 2026. No warning lights came on before or during the airbag deploying.Read more

Warranty Alignment: This failure occurred at 47.7k miles, well under the 60,000-mile threshold of the Supplemental Restraint System Limited Warranty. Known Defect: This repair is the subject of a specific Service Bulletin (RCM_a078), confirming it is a known manufacturing issue and not standard wear-and-tear. Safety Criticality: The SRS is a primary safety system. Forcing a customer to pay for a
... known defect on a life-saving system at such low mileage is inconsistent with Tesla’s safety mission. I have already paid the high invoice to ensure the vehicle was safe to drive. Dash Delamination: The leather dashboard is experiencing severe delamination and pulling away from the substrate, specifically in the area surrounding the passenger-side airbag. I believe this condition poses two significant safety risks: Airbag Interference: The detached and warped material may obstruct, delay, or alter the deployment path of the passenger-side airbag during a collision. Visibi
... lity and Distraction: This is a material defect, not wear and tear, and it poses a potential safety concern due to the reflection/glare on the windshield, as well as the distraction it causes while driving. Audi has acknowledged this "process weakness" and issued a Limited Warranty Extension (AWA-23-11) for 2019-2020 Q8 models. Despite this, my request for a safety-related repair was denied due to over 80K. The issue should be considered under mileage. I am requesting that the NHTSA investigate this defect to determine if it constitutes a safety non-compliance with FMVSS No. 208, as the structural integrity of the dashboard is vital for proper airbag performance. Thank you for your assistance in ensuring the safety of this vehicle.Read more

I purchased my 2025 Legacy from Brunswick Subaru (Brunswick,OH) on 9/22/25. A few days later, while riding in the passenger seat, the airbag light/icon stayed off. I am a petite female, weighing 100 pounds. I immediately called Brunswick Subaru's service department. The service manager drove me around, and he saw that what I was reporting was true. He said he would order sensors to fix the prob
... lem. I was told that the airbags should deploy at 35 pounds from my salesperson and 80 pounds from the service director. The sensors arrived in December, and I made an appointment on 12/30/25 to have them installed. I waited two hours only to learn that they could not be installed since this was a safety issue and Subaru is all about safety. I had to leave my car, and I was provided with a loaner, which was also a 2025 Legacy. The passenger seat's airbag light/icon stayed off when I sat in the passenger seat of the loaner too. I reported this to Mark Harris, a Subaru Advocacy Rep. He told me to sit in the back seat from now on. He also said to put my purse on my lap to add weight, which is highly discouraged in the Legacy's manual. As of today, 1/19/25, my Legacy is still in the service garage at Brunswick, and no one is giving me any answers-not the service manager, John Cross, nor the Subaru Advocate, Mark Harris. I have emailed and called every day since 12/31/25. Because this behavior occurred in two separate 2025 Legacies, it appears to be a system design defect rather than an isolated incident. The owner's manual states that the airbag may not activate for a "small adult", but this term is NOT defined by weight or any objective criteria. As a consumer, I had no way of knowing that an adult my size would not be protected by the front passenger airbag. This lack of disclosure prevented an informed purchase decision. I believe this presents a safety risk because adult occupants may be riding without frontal airbag protection. I request NHTSA review my concernsRead more

A few months ago, June 18, 2025 my 2018 BMW 430i convertible, which has 57,202 miles on the odometer, alerted me that the passenger side restraint system was malfunctioning. BMW declined to address the issue. Now, I am receiving the same notification for the driver side. Once again, BMW refuses to provide repairs. This poses a significant safety risk, as my airbags could potentially deploy unexp

During my online search, I found a recall for the same issues, the same year and model; however, my VIN was not included in that recall. I am apprehensive about driving my vehicle, as it may be a potential death trap. I believe BMW needs to acknowledge and take responsibility for this serious dangerous defect. 2014 Chrysler Town & Country – VIN [XXX] Both front active head restraints (driver and passenger) have self-deployed without any crash or impact, on two separate occasions about 3–4 years apart. The first occurred around 2019. Each time we were driving normally with occupants seated. We heard a sudden “pop” and the head restraint opened, exposing the internal mechanism. No one was struck
... or injured, but the unexpected deployment was distracting and occurred close to an occupant. After each event, the head restraint could not be reset or closed and remains deployed. The vehicle now has an airbag/SRS warning light that stays on, suggesting the restraint system may not function properly in a crash. There were no warning lights or symptoms before deployment. The deployed head restraints are still installed and available for inspection. A Chrysler dealer previously acknowledged the issue and ordered a replacement head restraint, but the repair was not completed due to relocation during the COVID period. 1. What component or system failed or malfunctioned, and is it available for inspection upon request? The airbag system, specifically the curtain airbags and related airbag control components, malfunctioned by deploying spontaneously without any collision or impact. The vehicle and deployed airbags are still available and can be inspected upon request. 2. How was your safety or the safety of other
... s put at risk? The spontaneous deployment of all four curtain airbags created an immediate safety hazard. Airbag deployment can cause injury, disorientation, or loss of control, especially if it occurs unexpectedly. Although the vehicle was stationary at the time, deployment without warning posed a serious risk of injury to occupants and could have resulted in a crash if it had occurred while driving. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? The problem has not been reproduced, as the airbags have already deployed. However, the spontaneous deployment itself is clear evidence of a malfunction. The issue has not been resolved or repaired, and responsibility for repair has not been accepted. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? The incident was reported to Progressive Insurance, which denied coverage because there was no accident or collision. The vehicle has not yet been inspected by the manufacturer for defect analysis. No police inspection occurred because there was no crash or injury. 5. Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear? Yes. The airbag warning light was illuminated prior to the incident, indicating a potential system fault. Additionally, a strong chemical odor was noticed inside the vehicle immediately before deployment, originating from the front passenger side, moments before all four curtain airbags deployed.Read more

The vehicle’s "Passenger Airbag Off" light and dashboard SRS warning lamp are illuminated. A diagnostic scan using a BMW-specific tool confirms manufacturer Fault Code 93C3 (Passenger Seat Occupancy Detector). This indicates a hardware failure of the OC3 internal seat sensor mat. Safety Risk: The failure of this sensor prevents the vehicle from correctly identifying if an adult is in the passen
... ger seat. This compromises the Supplemental Restraint System (SRS) and may result in the passenger airbag failing to deploy during a collision, posing a significant risk of injury or death to the occupant. Confirmation of Problem: The fault was confirmed via a digital diagnostic system (Foxwell NT510) which identified the specific "Open Circuit" within the seat mat. The component is currently installed in the vehicle and is available for inspection. Connection to Existing Recalls: This failure is identical in symptoms and fault code to those described in NHTSA Safety Recall 17V-428. Although this vehicle is the same year and model (2008 BMW 3-Series) as those covered in the recall, the VIN was excluded from the initial recall range. This appears to be a recall omission of a known manufacturing defect, as the failure occurred despite minimal use of the passenger seat.Read more

The front passenger airbag warning light came on with a dash warning flashing light. I isolated the fault to the passenger seat weight sensor, which prevents the airbag from deploying when a passenger of less than 90 pounds is sensed in the front seat. The time from the onset of the warning light to discovery of the actual fault was less than a week and this was facilitated by a mechanic with a so
... phisticated enough OBD reader to flesh out Infiniti codes. I called my local Infiniti dealer in Tacoma, WA and they were able to supply a part number but it has been discontinued for the 2011-2013 Infiniti M37 models. Without this sensor working, the passenger airbag may not deploy in the event of a collision, and that is unsatisfactory/downright unacceptable. These cars will operate for 20 plus years and are otherwise engineered to last 200/000 miles. I have 90,000 miles on the odometer and the car is otherwise in great shape, but I may be forced to scrap it if I can't resolve the weight sensor malfunction.Read more

I own a 2018 Land Rover Range Rover where the airbag warning light remains illuminated at all times on the dashboard, even when the front passenger seat is empty and no cargo is present on the seat. The warning light for airbag indicatesthat the air bag is not activated and will not work in a collision. The passenger air bag has been inoperable for over 2 years. This issue has persisted for several
... years, with documented dealer service records dating back to before my warranty expired. The dealer (Land Rover of Palm Beach) diagnosed it as resulting from the front passenger seat movement causing detachment or strain on the wiring harness/connector for the occupant classification system (OCS), which detects passenger presence and controls airbag deployment. The Passenger seat air bag sensor or wiring harness is defective and needs replacement for the passenger side air bag to function. They said that all 2018 Range Rover air bag harnesses are too short and are prone to being detached by seat movements / adjustments. They have repeatedly reset the light and reconnected the harness, but the warning recurs shortly after, indicating an unresolved design or manufacturing defect in the under-seat wiring that is prone to failure from normal seat adjustments. This prevents reliable operation of the passenger frontal airbag, posing a serious safety risk as it may fail to deploy (or deploy improperly) in a crash, endangering occupants. The dealer refuses to install a new larger harness or provide a permanent fix, despite this being a known recurring problem reported by other owners of similar L405 Range Rover models. They say that Land Rover corporate is not providing a newer replacement harness and is not paying for the repairs despite this being a serious safety issue. I request NHTSA investigation into this potential widespread defect to determine if a recall or safety action is warranted. This may be the same problem as reported in NHTSA Campaign # 11500093 & 11693335.Read more

I was heading to work on a stretch of road. I was traveling at about 45-50mph. A car pulled out in front of me and we had an accident. The airbags in the older Subaru of the person who collided with me went off and mine did not. Surprisingly. My head was slammed against the B pillar and a small bruise on my leg. Insurance is saying my car is totaled. With an accident of that magnitude, my air bag

Incident details: While driving under winter road conditions, the vehicle experienced a brief slide. During this slide, the vehicle lightly scrapped a curb, leaving a minor mark on the wheel. Despite this, the airbags deployed unexpectedly. There was no collision, no rollover, and no other object was struck, and there is no visible exterior damage to the vehicle. _________________________________
... _______ What component or system failed or malfunctioned, and is it available for inspection upon request? The suspected malfunction involves the airbag system, potentially including the airbag control module (ACM), crash sensors, or the electronic stability control (ESC) system. The airbags deployed without a qualifying collision or impact. The vehicle is being preserved and has not been repaired, and it is available for inspection upon request, with event data recorder (EDR) data preserved and available for retrieval.________________________________________ ________________________________________ Has the problem been reproduced or confirmed by a dealer or independent service center? No. A Mazda-authorized dealer (Mazda of Salem, Oregon) declined to accept the vehicle, stating that they do not perform post–airbag deployment investigations. As a result, the issue has not yet been reproduced or confirmed by a dealer or independent service center. I contacted Mazda of Hillsboro (Oregon), and the service center manager contacted the Mazda Experience Center on my behalf under case number 00404588. I was informed that the Mazda Experience Center advised them that the case would be closed with no further investigation. I was advised to take the vehicle to a body shop and to file an insurance claim. ________________________________________ Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? As I was pulling into a parking space in a parking garage, the vehicle's engine revved, tires squealed and then the vehicle propelled forward on its own, without my foot being on the accelerator. The vehicle only stopped when it hit a support column and the wall of the parking structure. There was extensive damage to my vehicle and to the parking structure wall. My safety and the safety of other
... s was put at risk due to the car accelerating on its own. The problem has not been confirmed or reproduced by the dealer, but we did take the vehicle to the dealer for inspection and they said they could not find anything wrong with the vehicle that would have caused the unintended acceleration. The vehicle has been inspected by the manufacturer, the insurance company and an investigator for Hyundai. Hyundai infomed us that they are not taking any responsibility for anything that occurred and that they will not share their investigator's report with us. There were no warming lamps or messages related to the problem prior to the failure. However, there have been other occasions while driving on the open road when the engine has revved on its own and I told Hyundai about that. In doing our own research, we have found that there are other 2023 Hyundai Tucson drivers who have had this exact same problem (unintended acceleration causing a crash) while pulling into parking spaces. Additionally, the air bags did not deploy during the collision and therefore the incident was not recorded by the EDR.Read more
