On June 9, 2025, I was involved in a motor vehicle accident while driving my 2016 Honda Accord. During the crash, the seatbelt system failed in a way that caused severe force to my body rather than restraining me properly. The seatbelt detached and violently pulled my body, resulting in significant injuries. The force from the seatbelt failure ruptured my right breast implant, which required surg
... ical replacement. I was transported by ambulance to Loyola Hospital, where CT scans and ultrasound confirmed the rupture. I have since undergone surgery and continue to experience ongoing medical issues including nerve symptoms and physical therapy. In addition, the airbags did not deploy during the collision, which raised additional concerns about the vehicle’s safety systems. Because of the unusual behavior of the restraint system during the crash, I am reporting this as a potential safety defect. After researching online, I discovered other drivers reporting seatbelt and safety-related issues with similar Honda Accord model years. Because the seatbelt system is a critical safety component designed to protect occupants during a crash, I believe this incident should be documented and reviewed for possible defects. This report is being submitted so that the issue can be investigated and documented for public safety.Read more

I would like to submit a formal complaint regarding the inadequate rental reimbursement being provided while a 2022 Chrysler Pacifica is undergoing recall repairs (Recall No. 226 – Ejection Mitigation). I took my vehicle to the dealership for recall repairs on March 12, 2026. The dealership does not provide loaner or rental vehicles. Chrysler Customer Care representatives advised that rental rei
... mbursement would be limited to $60 per day. However, the actual cost of obtaining transportation exceeds this amount. Even a significantly smaller compact vehicle currently costs approximately $69 per day to rent. A comparable vehicle to the Chrysler Pacifica, such as a minivan or similar family-sized vehicle, is substantially more expensive than the reimbursement amount being offered. As a result, a compact vehicle that is significantly smaller and not comparable to the Chrysler Pacifica had to be rented. This vehicle does not meet the same functional needs, comfort level, or standard of transportation normally relied upon. Additionally, the difference between the rental cost and the reimbursement limit must be paid out-of-pocket. This situation creates an unfair financial burden resulting solely from a manufacturer recall that is outside of the consumer’s control. Customer Care has been contacted multiple times regarding this matter; however, no resolution or adjustment to the reimbursement limit has been offered. The current policy places additional financial strain on the consumer during the recall repair process. Assistance in reviewing this matter and encouraging a fair resolution would be greatly appreciated.Read more

My 2017 Subaru Forester [Touring] (VIN: [VIN]) has a recurring front passenger airbag deactivation problem even when an adult is seated and belted. In many cases, the airbag will only enable if the passenger sits after engine start—classic Occupant Detection
... System (ODS) misclassification behavior. The ODS recall WUM‑98 (NHTSA 19V‑701) for 2015–2018 Forester was completed before I purchased the vehicle in April 2020, but the same safety condition has returned. On 02/25/2025 at Subaru of Sterling (VA), the dealer wrote: “Seats: occupant detection sensor mat circuit code. passenger side seat sensor mat has failed causing air bag light. recommend replacing passenger side seat bottom in the amount of $2,276.51.” I was also told the recall was “voided” because it had been completed before my ownership, and I must pay $2,276.51 to restore airbag function. I opened a case with Subaru of America: 25‑982548‑811. This is the same hazard identified in NHTSA 19V‑701 (Subaru WUM‑98)—the ODS may misclassify an occupied seat and deactivate the passenger airbag, increasing the risk of injury in a crash. I am requesting NHTSA to record this as a post‑remedy recurrence and to follow up with Subaru to ensure a permanent corrective action (e.g., seat cushion/ODS sensor replacement) without shifting cost to owners. I can provide the dealer RO/estimate and photos/video upon request. I am reporting a failure of the Steering Column Control Module (SCCM) in my 2021 Jeep Grand Cherokee L Overland. The failure is manifesting as an error message saying "service transmission" and the code that it is throwing says that the paddle shifters are overheating. This component is identical to the modules currently under Safety Recall 36B (NHTSA 24V-199) and Equipment Recall 15B (NHTSA 24E-0
... 25). The manufacturer has admitted that these modules possess an internal weld defect that prevents the driver’s side airbag from deploying in the event of a crash. Despite the clear safety risk and the fact that this model year/body style (WL) is the primary population for this defect, the manufacturer (Stellantis/Jeep) refuses to cover the repair, stating my specific VIN is not "flagged" in their internal database. I am concerned that the scope of the existing recalls is too narrow and that my vehicle is currently being operated with a non-functional or unreliable driver's airbag system due to this known hardware defect. I am requesting an investigation into why 2021 Grand Cherokee L models with these exact symptoms are being excluded from the safety remedy.Read more

I reached out to Honda several times because they were the last ones to work on my vehicle for the fuel pump recall, but they noted VSA\ABS for the last couple of weeks I finally talked with Service Manager Marvin Cabrera and he directed me to American Honda Motors and after reaching out to them since the 3rd of February, they keep telling me about appointing an investigation inspection with the FTS (field technical specialist) and I have not received any updates about my vehicle.
