On January 12, 2026, I was involved in a multi-vehicle collision on I-4 West near US-27 Off Ramp in Polk County, Florida. I purchased a used 2015 Jeep Wrangler from a dealership in New York. At the time of sale, the vehicle had an open urgent safety recall (19V-018) for a defective Takata passenger airbag inflator. This recall was never disclosed to me by the dealership prior to or at the time of purchase. The recall advisory was noted on the NYS inspection report conducted the same morning the vehicle was sold to me
... , but was not communicated to me in any way. I discovered the open recall after experiencing repeated mechanical failures following purchase. Four days after purchase the vehicle overheated and had to be towed — the coolant system was found to be completely empty. An independent licensed mechanic subsequently inspected the vehicle and stated on record that the mechanical defects appeared to be pre-existing conditions that existed prior to the sale. The vehicle currently has an unrepaired Takata airbag inflator which the manufacturer has identified as capable of exploding and sending metal fragments into the vehicle interior at high speed, risking serious injury or death to occupants. The vehicle is available for inspection upon request. The recall remains unrepaired. The dealership was contacted and a manager verbally acknowledged they did not disclose the known defect prior to sale.Read more

I am filing this complaint regarding serious safety and consumer protection concerns involving my 2022 Jeep Wrangler (VIN: [XXX] ), manufactured by Stellantis, as well as issues involving the towing company E&R Towing following my accident. Prior to my accident on November 8, 2025, my vehicle experienced multiple ongoing mechanical issues, including overheating, shaking while driving, and abnormal
... loud noises. These issues raised serious safety concerns and were not properly resolved. On November 8, 2025, I was involved in a rollover accident. Following the accident, Stellantis initiated an investigation and retained Bosch/Engineering Analysis Associates to extract Event Data Recorder (EDR) data from my vehicle. While presented as an independent inspection, this analysis was conducted by a vendor hired by Stellantis, raising concerns about the objectivity and scope of the findings. Stellantis ultimately denied responsibility, stating that the conditions of the crash did not meet the threshold for airbag deployment. However, this conclusion fails to address the ongoing mechanical and safety issues I experienced prior to the accident, including the possibility of system or sensor malfunction. Additionally, my claim for relief under Illinois Lemon Law was denied without fully considering the pattern of defects and safety concerns. I have also filed a complaint with the National Highway Traffic Safety Administration (NHTSA), Complaint #XXX, regarding safety concerns with this vehicle. Following the accident, my vehicle was handled by E&R Towing. I have concerns regarding how my vehicle was managed after the crash, including the handling process and lack of transparency regarding procedures and fees. I am requesting that this matter be reviewed for potential violations of Illinois consumer protection laws, including unfair or deceptive practices, failure to properly address known vehicle safety issues, and improper handling of my vehicle following acciden INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACTRead more

I am reporting a critical safety failure involving an improperly installed (wrong year) passenger-side airbag in my 2022 Subaru Outback Wilderness, which was negligently missed for nearly two years by multiple Subaru service departments. This is a pervasive life-safety defect and an extensive failure in manufacturer inspection protocols. Chain of Negligence: 1. Initial Misleading Certification:
... Shortly after my purchase, the vehicle underwent a "certified" multi-point safety inspection at a Subaru retailer in Ohio, which negligently certified the vehicle as safe with a critical safety defect present. 2. Systemic Dealer Misdiagnosis: Over the subsequent nearly two years, I took my vehicle to multiple other Subaru dealership service departments, actively showing them a persistent visual misalignment of the passenger-side trim. Despite active complaints, no dealership took the proper time to diagnose the underlying cause, and they missed the critical safety defect. 3. Discovery by Chance: Critically, there were no warning lights, messages, or other electronic indicators. The only symptom was the visual misalignment. The true cause was only traced back to the wrong part after I personally spoke with a Subaru parts department team member who was resourceful enough to figure it out. This finding was subsequently officially certified as correct by the service team at Mike Shaw Subaru, which finally provided documentation that the component was incorrect. Safety Risk: The incorrect airbag component poses an immediate and incalculable life-safety risk, as its deployment behavior is unknown and potentially compromised. This unaddressed safety risk was allowed to continue for nearly two years due to repeated inspection failures. Manufacturer Response Failure: Subaru of America has been formally notified of this systemic safety defect and the complete failure of their retailer network’s inspection protocols. While driving on the highway in light traffic at approximately 70 mph and changing lanes, the side airbags in my vehicle suddenly deployed without warning. There was no collision, no contact with another vehicle, no road debris, and no apparent external cause for deployment. I did not strike anything before, during, or after the deployment. There were no vehicles within several car lengths of my v
... ehicle at the time. Highway Patrol was dispatched to the scene. Officer TJ Carter with Guilford County responded. Upon inspection of the vehicle at the scene, we found no evidence of impact or contact with another vehicle, object, or debris. The vehicle was covered in yellow pollen at the time, and even light contact would have left visible marks or disturbance in the pollen. After examining the vehicle, no signs of contact were identified. The vehicle does have unrelated prior body damage from over a year ago on one side, but that damage predates this incident and was not caused by this event. The responding officer can confirm there was no collision or new impact associated with the airbag deployment. There was no accident associated with this airbag deployment. The airbags deployed while I was traveling at highway speed, which caused a sudden and dangerous driving event and required me to quickly maneuver to the side of the roadway. This created a serious safety risk to me and to surrounding traffic. Prior to this incident, there were no warning lights, safety alerts, or service notifications indicating an issue with the airbag system or vehicle safety systems. However, a notification did appear in the My Chevrolet app at or around the time of the incident. The vehicle has not yet been fully inspected following the incident. A safety claim has been submitted to General Motors. I also have video footage of the Highway Patrol Officer cutting down the airbags on-site but cannot add that below so I have added a screenshot.Read more

My complaint is that I was sold a dangerously defective vehicle with a known, unrepaired “stop driving” airbag safety recall, was not warned about the deadly risk, and am now being told to move and tow the car in direct contradiction to official safety instructions. ### Factual background - The vehicle I purchased has a Takata‑type airbag safety recall that carries a formal “do not drive
... ” or “stop driving” warning because the airbag can explode and kill or severely injure occupants, even in a minor crash.[1][2][3][4] - Federal safety authorities have confirmed that at least 28–29 people in the United States have been killed and more than 400 injured by exploding Takata airbag inflators.[3][4][5][1] - I received the recall notice about two months ago, but I was told the repair parts were back‑ordered, which delayed fixing the deadly defect through no fault of my own.[1][3] - During this delay, the vehicle’s battery became completely dead, and I am now being told by the recall and towing company that I must make sure the car is taken out of the garage, which would require entering and moving a vehicle I have been legally and explicitly told not to drive. ### Legal and safety concerns - Official guidance from the National Highway Traffic Safety Administration (NHTSA) and the manufacturer is that vehicles under this Takata “do not drive” recall must not be driven at all until the defective airbag is replaced, because even a minor collision can cause the inflator to explode and send metal shrapnel into the cabin.[2][4][3][1] - The instruction to move or drive the car out of the garage conflicts with this clear safety warning and effectively asks me to assume a risk that has already caused multiple deaths and serious injuries nationwide.[4][5][2][3][1] - Federal law prohibits franchised dealers from selling new vehicles with unrepaired safety recalls, and legal experts warn that selling used vehicles with undisclosed safety defects can expose sellers and dealers toRead more

The SRS spiral cable assembly (clock spring), located directly behind the driver's instrument display in the steering column, is producing an intermittent crackling noise. The component is available for inspection upon request. Safety risk: The clock spring is a safety-critical component responsible for airbag deployment, horn operation, and steering wheel controls. A malfunctioning clock spring
... can cause airbag non-deployment in a collision or inadvertent airbag deployment while driving. The intermittent nature of the malfunction suggests internal ribbon cable contact or thermal expansion failure within the SRS circuit, which may worsen over time. The problem has not been confirmed by the dealer. The vehicle has been brought to Lexus of Jacksonville, a Lexus authorized service center, on 3 occasion(s). On each visit the dealer was unable to reproduce the noise and closed the repair order without repair. The noise is intermittent, occurring after approximately 60–90 minutes of continuous highway driving, lasting 10–15 seconds before stopping. It occurs with the steering wheel straight and during turns of varying degrees. The thermal onset pattern is consistent with internal SRS spiral cable component failure under heat soak conditions. The vehicle has been inspected by the Lexus authorized dealer only. Two audio recordings of the crackling sound were provided to the dealer with confirmation from the service foreman the crackling noise is heard. No independent inspection has been performed to date. There are no warning lamps or dashboard messages associated with this malfunction. The only symptom is the audible crackling noise originating from the steering column clock spring location. The noise first appeared at approximately 700 miles and has occurred on multiple occasions since.Read more

The wiring for the air bag system under the driver's seat is exposed and unprotected which has led to disconnection and or damage to the wiring harness which then results in a disabled air bag system. See attached photos for evidence of how exposed this wiring harness is and suscebtible to damage as well as an example of damage which can occur which other owners have reported. Damage to this unit due to poor design results in compromised driver and passenger safety and extremely costly repairs ($10,000) which it appears Hyundai is refusing to cover under warranty. The first problem part is the airbag. The clockspring was fixed twice under warranty. Then the dealership said they wont keep doing it and blamed me wrongly. The car burns through 20 quarts of oil in between changes, it has no leaks. It has so much carbon buildup. They replaced a gasket and thats another part that has continually needed replaced but they did not fix the issue that has caused it to go bad. When the small mechanics hear about the issue they are confused because the type of motor I have was recalled in other cars for the same issues. RE: Failed Radar Braking on a 2024 Hyundai Palisade What component or system failed or malfunctioned .. . . On Feb 8th, 2026 I was driving in to Lakeland FL on I-75 N, in the right hand lane, near a green stoplight following a car 2 lengths ahead. The radar cruise control was slowing me down, keeping up with the car in front. Another driver suddenly cut in front of me from the far left lane. T
... he cruise slammed on the brakes. Within seconds he took a hard right to catch an exit ramp. The radar cruise then accelerated at an extremely high rate, and no Braking, causing me to hit the car in front of me, which was stopped about a car length away at a red light. Our air bags, 3 on driver's side, 1 on the passenger side deployed and my wife and I appeared to have no injury. The Hyundai front end crumpled in, trapping us in the car. The police pried open the doors to get us out. The car was totaled. We rented a car through our insurance and continued our 4 day journey home. On the 3rd day my right leg started turning black and blue. By the 4th day my left leg started doing the same. On returning to Stevens Point I went to the ER. The Dr. said the bruises would take several weeks to clear. And it is not unusual for the bruising to appear several days later. On March 9th both my legs developed large hematomas. The right one just above the ankle, the left just below the knee. A week after that the right hematoma developed 2 deep open wounds and I had to go for wound care involving twice daily bandage changes and then a wound vac. The Dr. says it will be a long recovery. I would have been better off without the leg airbags. Two people in the front car claimed whiplash and were taken to the hospital for observation. The police said they did not appear to be injured, but better safe than sorry. We were trapped in the car and couldn't get out till the Police got there 20 minutes later. By then the ones in the front car had been taken away.Read more

Vehicle experienced multiple safety-related failures at approximately 500 miles. The failures included complete instrument cluster loss (no speedometer or gauges), unpredictable windshield wiper operation, headlight flickering, complete loss of brake lights and turn signals, rear liftgate malfunction, and airbag/seatbelt warning indicators. These issues created unsafe operating conditions, includ

This presented a risk to both the driver and surrounding traffic. The failures occurred intermittently and in combination, indicating a potential systemic electrical issue rather than a single isolated component failure. The vehicle has been taken to an authorized Mazda dealership and is currently under inspection and repair. The dealership has indicated the instrument cluster requires replacement and is currently on backorder. Full root cause has not been confirmed. Warning indicators, including airbag and seatbelt system alerts, were observed prior to and during the failures. I am attempting to complete a manufacturer-issued safety recall involving my vehicle’s airbag system, which includes a “Do Not Drive” advisory due to the risk of airbag inflator failure. Despite multiple good-faith attempts, I have encountered repeated barriers at the dealership level that have prevented timely completion of the repair. I scheduled a service appointment with Culver City To
... yota and clearly indicated I required a rental vehicle to leave my car. My confirmation stated “Arrangement: Rental Vehicle,” and the appointment was scheduled accordingly. When I arrived, I was told no rentals were available and that they are first-come, first-served. I was turned away and told to return another day. When I returned, I was able to leave my vehicle, but was asked how long I would need the rental without being given a repair timeline. Only after asking was I told the repair would take approximately two days. This was not communicated in advance, and given the prior lack of rental availability, it is unclear how I would have been expected to proceed without transportation. This has occurred across multiple attempts: lack of rental availability and requests to leave my vehicle for extended periods without transportation have made it difficult or impossible to complete the recall. As a result, I have been placed in the position of continuing to operate a vehicle with a known safety defect under a “Do Not Drive” advisory due to the inability to access timely and practical repair. I am concerned that dealership-level processes are creating barriers that delay or prevent consumers from completing urgent safety recalls. As of today, I have left my car at this dealership for the recall repair, however the inconsistent communication and handling of this process have created uncertainty about whether the repair will be completed in a timely and reliable manner. The airbag warning light came on, so we took it in to see about a recall on the airbag control unit issue that required a wire filter kit as out lined in a recall in 2011-2013 Sonatas and Sonata Hybrids. In fact, upon reading the invoice filling out this form, I think that is exactly the issue, and Potamkin Hyundai is ignoring the recall. The invoice states that "AIRBAG MODULE NEED FIXING caused b
... y the DTC for the driver airbag resistance is too high, and has liquid leaking out of it through the connector into the clock spring, and need to replace the air bag incongruent with clock spring to perform repair correctly" The technician explained to my son that driving the car in it's current state, the air bag could go off at any moment, possibly killing him. Their airbag could kill my child. The repair is quoted at $1588 and the solution was to turn off the airbag until a decision could be made if my son wanted to drive the car home, which he did, so not it is disabled. It is a Hyundai manufactured airbag, a safety feature installed into this vehicle that should not degrade over time. It is actually the reason my aunt bought the car for her child and why my son bought the car from her. The car has never been wrecked, the airbag had never been deployed or otherwise molested, has been dealer maintained, and had an extended warranty. I spoke to a Hyundai representative and am awaiting a call back, I do not think that he should be responsible for repair of a defective, degrading safety feature built and manufactured by Hyundai, installed a vehicle built and manufactured by Hyundai. I believe other people should be warned that their Sonata airbags might also have this dangerous issue and it should be immediately investigated. Attached is the invoice given to my son.Read more

The passenger occupancy sensor has failed on my 2016 ELR after 14k miles. The part is no longer available (23504893). I've inquired with General Motors and they said they could not help me. This seems to be a 1year only part used only on the Cadillac ELR. I believe this is an integral part of the Air Bag safety system in my vehicle and that the passenger seat is no longer safe for passengers.
Incident date: Mar 21, 2026
The vehicle developed a leak in the AC condensate drain. The leak is directly above the safety restraint control module. The module got wet deactivating the vehicle and causing havoc with the airbag system. Many electrical systems turned off such as the power to the steering wheel buttons( the left blinker, the right blinker, and the horn). Luckily this happened while my vehicle was parked but i
... f this happened while someone was accelerating or driving on the highway they would lose control of the vehicle. I spoke to a Tesla Service Tech about the concern and he informed me that this AC leak happens very often in the 2021 and newer Tesla Model S vehicles and that Tesla is aware that the AC drain leaks onto the Restraint Contol Module but has not created a technical service bulletin or a recall for the safety issue.Read more

Vehicle: MINI Cooper Clubman (Model R55) VIN: [XXX] Recall: NHTSA 23V-337 Dealer: Flow MINI of Raleigh, NC 1. COMPONENT FAILED: The Junction Box Electronics (JBE) failed due to moisture/water intrusion, producing electrical fault codes across all core systems including airbags, brakes, and a JBE communication fault. The vehicle is located at Flow MINI of Raleigh and is available for inspection.
... 2. SAFETY RISK: Recall 23V-337 identifies that moisture intrusion into the JBE can lead to corrosion, short circuit, and in rare cases a thermal event (fire). My vehicle's carpet is wet, confirming active water intrusion into the footwell where the JBE is installed. Airbag and brake faults appear exclusively when it rains or when the vehicle is washed, directly and unambiguously linking all symptoms to water intrusion. The vehicle is currently unsafe to operate. 3. DEALER CONFIRMATION: Flow MINI of Raleigh confirmed moisture/water intrusion is causing the JBE faults — directly reproducing the failure during a dealer-performed wash. The dealer declined the recall repair solely because [XXX] is not listed among covered states. 4. MANUFACTURER RESPONSE: MINI USA denied coverage exclusively on the basis that North Carolina is not included in the recall's geographic region. The defect itself has not been disputed. 5. BASIS FOR COMPLAINT: The recall's geographic restriction was established based on road salt usage. North Carolina roads do receive road salt treatment during winter weather events. The recall documentation (RCRIT-23V337-7125.pdf, Page 4, Item 12) explicitly names the JBE as a covered component. I request that NHTSA: (1) treat this as urgent given confirmed airbag and brake faults, (2) review whether NC should be included in the recall's geographic scope, and (3) compel MINI USA to perform the recall repair on VIN [XXX] at no cost. I have had numerous safety warning messages display upon startup and also while driving. The messages are usually combined with a cold ambient temperature and do not display during warm temperatures. The FCA dealership has attempted to fix this issue on at least six different occasions so far without any change. To date, FCA has replaced the original battery, they have updated, flashed the ECU mod
... ule, performed NHTSA Recall B4C for flashing the cluster, and most recently they replaced the ORC module. FCA indicated that NHTSA-25V882 was not applicable to my VIN despite my VIN production date in April '25; within your suspect period. Original issue remains, FCA STAR case generated however cannot replicate error condition for FCA engineers. The next scheduled visit to FCA is to replace a telematics module for possible link to issue with the Uconnect5 system. This safety issue was first noted before 800 miles and continues through 2768 miles thus far. As this issue pertains directly to a vital Air Bag sensor issue and the inability to call for a SOS assistance, I expect the NHTSA to raise this concern with FCA USA and to determine the cause of this safety defect and issue a new Recall Notification for repair and/or replacement of affected parts.Read more
