Incident Narrative
AT APPROXIMATELY 15:50 ON 2/9/2022, EMPLOYEE FROM ANOTHER OPERATOR CONTACTED ENERGY TRANSFER COMPANY (ETC) PIPELINE TECHNICIAN TO REPORT A LEAK ON THE JC NOLAN RECEIVER LOCATED AT THE DELEON STATION (~MP212). THIS IS A SHARED STATION BETWEEN THE TWO OPERATORS. THE EMPLOYEE FROM THE OTHER OPERATOR PLACED A TROUGH UNDER THE VALVE TO PREVENT FURTHER RELEASE AND THE ETC PIPELINE TECHNICIAN ADVISED HE WAS ON HIS WAY TO THE SITE. ETC PIPELINE TECHNICIAN ARRIVED AT THE DELEON STATION AT APPROXIMATELY 17:20 AND DETERMINED THAT A 4-INCH DRAIN VALVE LOCATED ON THE JC NOLAN RECEIVER AT DELEON STATION WAS LEAKING DIESEL FROM THE VALVE BODY SEAL / GASKET. ETC PIPELINE TECHNICIAN CLOSED THE VALVES ON THE RECEIVER TO ISOLATE THE LEAKING VALVE. THE PIPELINE SYSTEM WAS NOT SHUTDOWN BECAUSE THE LEAKING VALVE WAS ISOLATED ON THE RECEIVER AND DID NOT AFFECT THE OPERATIONAL CAPABILITY OF THE STATION OR PIPELINE. INITIAL ASSESSMENT WAS A RELEASE OF APPROXIMATELY 1000 GALLONS (23.809 BBLS) OF DIESEL WHICH WAS CONTAINED ON COMPANY PROPERTY. THE BALON VALVE WAS REMOVED AND SENT TO BE ANALYZED FOR THE CAUSE OF FAILURE. A BLIND FLANGE WAS INSTALLED AS A TEMPORARY REPAIR. REMEDIATION BEGAN AS SOON AS POSSIBLE ON 2/10/22 AND IS STILL IN PROGRESS, WITH ENVIRONMENTAL TESTING BEING AN ONGOING PROCESS. THE MANUFACTURERS ANALYSIS OF THE VALVE INVOLVED IN THE FAILURE INDICATES THAT THE BODY O-RING HAD BEEN EXTRUDED. WATER DROPOUT IN THE DRAIN LINE VALVE FROZE IN THE VALVE BODY JOINT AND SEPARATED THE VALVE AT THE O-RING. WHEN THE WATER FROZE THE VALVE WAS OVER PRESSURED AND STUDS WERE STRETCHED. THE SEATS WERE SCRATCHED AND ABRAIDED, SEAT POCKET SHOWED SIGNS OF CORROSION WHICH INDICATES THE PRESENCE OF WATER. ON 2/10/2022 IT WAS DETERMINED THE COST OF REMEDIATION WOULD EXCEED THE $50,000 THRESHOLD FOR IMMEDIATE NOTIFICATION AND NRC NOTIFICATION WAS MADE AT 13:14 ON 2/10/2022 (NRC# INITIAL1328614, 48 HOUR FOLLOW-UP 1328793). SOIL IS BEING REMOVED VIA COMPRESSED AIR EXCAVATION AND DISPOSED OF AT AN APPROVED FACILITY, AS REMEDIATION IS ONGOING.
