ON 5/3/22 AT 12:00 LOCAL OPERATIONS PERSONNEL WERE NOTIFIED BY A 3RD PARTY OF A CRUDE OIL ODOR IN THE BESSIE HEIGHTS MARSH AREA ALONG THE 30-INCH BAYOU BRIDGE PIPELINE. LOCAL OPERATIONS MANAGEMENT WAS NOTIFIED ALONG WITH THE CONTROL CENTER AND THE LINE WAS SHUTDOWN AND PERSONNEL WERE DISPATCHED TO THE AREA TO INVESTIGATE. AERIAL PATROL WAS ALSO CONTACTED AND REROUTED TO THE AREA. PRESENCE OF CRUDE OIL WAS CONFIRMED IN THE AREA HOWEVER THE SOURCE WAS UNABLE TO BE DETERMINED AT THE TIME. THE COMPANY NOTIFIED NRC AT 13:42 AND REPORT NO. 1335046. OSROS WERE ALSO NOTIFIED AND DISPATCHED TO ASSIST WITH CLEANUP. OVER THE FOLLOWING TWO DAYS ADDITIONAL RESOURCES WERE DISPATCHED TO THE LOCATION TO ASSIST WITH IDENTIFYING THE LEAK LOCATION AND PERFORM EXCAVATION. THE PIPELINE WAS PURGED AND FILLED WITH NITROGEN BETWEEN PT. NECHES AND MLV 1 TO ASSIST IN DETERMINING LEAK LOCATION. FAILURE LOCATION WAS POSITIVELY IDENTIFIED AT APPROXIMATELY 14:50 ON 5/5/22. A TOTAL OF 11BBLS OF CRUDE OIL WAS RELEASED AND RECOVERED. AT 15:01 ON 5/5/22 A 48HR NRC UPDATE REPORT WAS FILED (NO. 1335246). THE FAILED SECTION OF PIPE WAS EXCAVATED, CUT OUT AND REPLACED AND THE PIPELINE WAS RETURNED TO SERVICE ON 5/11/22. THE FAILED SECTION OF PIPE WAS PRESERVED AND SENT TO A 3RD PARTY LABORATORY FOR ANALYSIS TO DETERMINE FAILURE MECHANISM. UPON RECEIPT OF LABORATORY REPORT A SUPPLEMENTAL 7000.1 REPORT WILL BE FILED. SUPPLEMENTAL - METALLURGICAL ANALYSIS DETERMINED THAT THE FAILURE COINCIDED WITH A CRACK-LIKE INDICATION ACROSS THE GIRTH WELD COINCIDING WITH AN AREA OF REDUCED WELD REINFORCEMENT (SUCH AS A START/STOP). AN EXTERNAL SURFACE BREAKING ORIGIN FEATURE WAS IDENTIFIED WHICH THEN GREW THROUGH WALL AND INTO THE ADJACENT UPSTREAM AND DOWNSTREAM PIPE VIA FATIGUE. THE PRESENCE OF STEPWISE CRACKING ADJACENT TO THE EXTERNAL BREAKING INDICATION AND HIGHER HARDNESS AT THE EXTERNAL SURFACE SUPPORT THE LIKELIHOOD THAT THE ORIGIN FEATURE WAS THE RESULT OF HYDROGEN CRACKING. CIRCUMFERENTIAL ULTRASONIC CRACK ILI TOOL WAS RUN POST FAILURE TO ENSURE THE CRACKING ASSOCIATED WITH THE FAILURE WAS NOT A SYSTEMIC ISSUE. NO ADDITIONAL SIMILAR FEATURES WERE DISCOVERED. SUPPLEMENTAL REPORT: UPDATED LOCAL TIME OPERATOR IDENTIFIED FAILURE.
