ON 3/7/2024 AT APPROXIMATELY 09:47 AM, LOCAL BUCKEYE PERSONNEL OBSERVED A SHEEN ON A GRAVEL PARKING AREA INSIDE THE STATION AND NEARBY DISCOLORED VEGETATION NEAR THE STATION SUMP AREA. INTERNAL NOTIFICATIONS WERE MADE FOR AWARENESS AS A RELEASE WAS UNCONFIRMED DUE EXTREME AMOUNTS OF ONGOING RAINFALL. IT WAS UNSURE IF THE SHEEN WAS DUE TO HISTORICAL PRODUCT RUNOFF. LOCAL OPERATIONS SUSPENDED USE OF THE STATION SUMP. FURTHER INVESTIGATION INCLUDED EXCAVATING THE AREA WITH A SOFT DIG TRUCK AND HAND DIGGING AROUND THE SUMP AREA. LOCAL OSRO WAS ON SITE AND RESPONDED TO REMOVE ALL AFFECTED EXCAVATED SOIL AND WATER/PRODUCT MIX. THE NRC AND OTHER EXTERNAL NOTIFICATIONS WERE MADE ON 3/7/2024 AT APPROXIMATELY 12:30 PM OUT OF AN ABUNDANCE OF CAUTION DUE TO A POTENTIAL UNCONFIRMED RELEASE. EMAIL UPDATES WERE MADE TO PHMSA ACCIDENT INVESTIGATOR B. PIERZINA OVER THE NEXT TWO DAYS AS THE INVESTIGATION INTO LOCATING A POSSIBLE SOURCE CONTINUED. AFTER FULLY EXCAVATING AND EXPOSING THE SUMP TANK AND APPURTENANCES A VISUAL INSPECTION REVEALED A TOTAL OF THREE SMALL HOLES IN THE SIDE WALL OF THE STRAINER VESSEL. APPROXIMATELY 208.5 BARRELS OF WATER/TRANSMIX WAS RECOVERED AND SEPARATED. THE ESTIMATED AMOUNT OF TRANSMIX RECOVERED AFTER SEPARATION WAS APPROXIMATELY 67.5 BARRELS. THE STRAINER VESSEL WAS REMOVED, AND A TEMPORARY CONNECTION WAS INSTALLED FROM THE STATION DRAIN SYSTEM TO THE SUMP TANK. STRAINER VESSEL WILL BE SENT TO 3RD PARTY FOR METALLURGICAL ANALYSIS. UPDATES TO THIS REPORT WILL BE PROVIDED. 4/15/2024 ADJUSTED PART G TO REFLECT EXTERNAL CORROSION. SUMP STRAINER WAS SENT OUT TO 3RD PARTY FOR FAILURE ANALYSIS HOWEVER POSSIBLE CAUSE IS EXTERNAL CORROSION. ANY CORRECTIONS/CHANGES WILL BE MADE WHEN ANALYSIS IS COMPLETE. ADDITIONAL UPDATES TO THIS REPORT WILL BE SUBMITTED. 8/20/2024 THE 3RD PARTY FAILURE ANALYSIS REPORT WAS COMPLETED AND REVEALED INTERNAL CORROSION ON THE STRAINER. THE CORROSION OCCURED DUE TO MICROBIOLOGICALLY INFLUENCED CORROSION (MIC) WITH SOME CONTRIBUTION FROM THE PRESENCE OF OXYGEN. THE ANALYSIS ALSO REPORTED THE LEAK LOCATIONS ON THE STRAINER WERE DISCRETE AREAS ON THE LEDGE BETWEEN THE STRAINER BODY AND THE STRAINER BOTTOM. PART G OF THIS REPORT WAS ADJUSTED TO REFLECT THE 3RD PARTY FINDINGS. QUESTION A14 WAS ALSO CORRECTED TO ""ONSHORE PUMP/MET STATION EQUIPMENT AND PIPING"".
