BETWEEN THE HOURS OF 8:30PM ON 7/10/24 AND 8:46AM ON 7/11/24 APPROXIMATELY 2,800 BARRELS OF CRUDE OIL RELEASED FROM THE PIPING MANIFOLD INTO THE PRIMARY CONTAINMENT SURROUNDING TANK 202. THE RELEASE WAS DISCOVERED AT 8:45 AM ON 7/11/24 BY THE MAINTENANCE MANAGER (MM) WHO SMELLED THE CRUDE OIL WHILE DRIVING BY THE TANK FARM. THE MM IMMEDIATELY INVESTIGATED THE SPILL AND DISCOVERED THAT THE 2"" DIXON ANDREWS CAM FITTING HAD FAILED, AND THE MANIFOLD VALVE WAS OPEN ON THE LOW POINT OF TANK 202. THE MM SHUT THE MANIFOLD VALVE AND OPERATIONS LEADERSHIP WAS NOTIFIED. WHILE THE VALVE WAS REQUIRED TO BE OPEN DURING THE MAINTENANCE ACTIVITY, IT WAS NOT CLOSED AT THE COMPLETION OF THE DAY'S ACTIVITY AS A SECONDARY LAYER OF PROTECTION. BUT FOR THE CRACK AND SUBSEQUENT FAILURE OF THE BODY OF THE 2"" DIXON ANDREWS CAM FITTING, THERE WOULD NOT HAVE BEEN A RELEASE. RECOVERY AND REMEDIATION EFFORTS BEGAN THE SAME DAY. SPILL CLEAN UP CONTRACTORS ARRIVED AT THE SITE WITHIN 30 MINUTES OF THE SPILL BEING DISCOVERED. THE INVESTIGATION DETERMINED THAT THE BODY OF THE 2"" DIXON ANDREWS CAM FITTING CONNECTED TO THE VALVE FAILED, AND THE VALVE TO THE MANIFOLD WAS OPEN ALLOWING PRODUCT TO FLOW FREELY FROM TANK 201 AND 205, UNTIL THE VALVE WAS CLOSED. THE PROCEDURES WERE DETERMINED TO BE INADEQUATE. THIS WAS A ONE-OFF MAINTENANCE ACTIVITY THAT HAS NOW BEEN ADEQUATELY DOCUMENTED IN THE REVISED PROCEDURES. ALSO, TRAINING HAS COMMENCED WITH OPERATIONS PERSONNEL. PLEASE NOTE IN PART E (2, 2.A, 2.B, 2.C, 3 & 8) THESE ARE ANSWERED REGARDING THE 2"" DIXON ANDREWS CAM FITTING MOP/MAOP. THE DATE FOR E2.B WAS LISTED AS 7/11/24 AS THE ESTABLISHED MOP DATE FOR THE 2"" DIXON ANDREWS CAM FITTING BY THE MANUFACTURER IS UNKNOWN AS OF NOW. THE 2"" DIXON ANDREWS CAM FITTING WAS PROPERLY INSTALLED APPROXIMATELY ONE WEEK BEFORE THE ACCIDENT AND WAS INTERMITTENTLY IN SERVICE DURING THAT TIME FRAME AND THERE WERE NO LEAKS THAT OCCURRED PRIOR TO THE 7/11/2024 DATE. *NOTE FROM SECTION G6: THE FAILED SECTION OF THE SUBMITTED ENGINE CAM SAMPLE SHOWED PRIMARY BRITTLE INTERGRANULAR FRACTURE MODE DUE TO MECHANICAL OVERSTRESS OF THE PART WITH MATERIAL DEGRADATION DUE TO CORROSIVE CHLORIDE ATTACK. MAINTENANCE ACTIVITY: REPLACE MIXER MOTOR ON TANK 202. INADEQUACY OF PROCEDURES: THE PROCEDURE DID NOT EMPHASIZE THE INSPECTION OF THE EXTERNAL VALVE FITTING. PROCEDURE REVISIONS: THEY HAVE BEEN UPDATED TO INCLUDE THE INSPECTION OF THE EXTERNAL VALVE FITTING. WE HAVE ALSO ADOPTED AN INSPECTION PROCESS THAT HAS IN-SERVICE DATES, END DATES, AND TIME SCHEDULED INSPECTIONS FOR CRITICAL COMPONENTS. WE ARE IN THE PROCESS OF CHANGING OUR CAM FITTINGS TO STAINLESS STEEL.
