Incident Narrative
ON 10/30/22 AT APPROXIMATELY 02:50, AN INDIVIDUAL NOT AFFILIATED WITH THE COMPANY AND NOT A CONTRACTOR WORKING FOR THE COMPANY DROVE A VEHICLE OFF THE ROAD AT THE INTERSECTION OF WEST LUCAS STREET (HIGHWAY 52) AND PISGAH ROAD IN FLORENCE, SOUTH CAROLINA. AFTER LEAVING THE ROAD, THE VEHICLE COLLIDED WITH AND DAMAGED THE FLORENCE TEC TOWN BORDER STATION (13-110) THAT IS SITUATED APPROXIMATELY 40 FT FROM THE SIDE OF THE INTERSECTION. THE VEHICLE CAME TO A STOP ON TOP OF THE STATION. THE FUNCTION OF THE STATION IS METERING AND REGULATING THE NATURAL GAS BEING SUPPLIED TO THE CITY OF FLORENCE. THE STATION WAS IN NORMAL OPERATION WITH AN OPERATING PRESSURE OF APPROXIMATELY 610 PSIG WHEN THE INCIDENT OCCURRED. THE COLLISION RESULTED IN THE ENTIRE STATION BEING SHEARED OFF AT THE RISERS AND NATURAL GAS BEGAN BLOWING TO THE ATMOSPHERE. THE INDIVIDUAL DRIVING THE VEHICLE WAS NOT INJURED AND SAFELY EXITED THE VEHICLE. THE VEHICLE DID NOT IGNITE THE BLOWING NATURAL GAS. AT APPROXIMATELY 03:07, GAS CONTROL RECEIVED A SCADA LOW LOW PRESSURE ALARM ON THE STATION. AT APPROXIMATELY 03:14, GAS CONTROL DISPATCHED OPERATIONS PERSONNEL TO INVESTIGATE THE SCADA LOW LOW PRESSURE ALARM AT THE STATION. AT APPROXIMATELY 03:32, GAS CONTROL RECEIVED NOTIFICATION FROM THE FLORENCE COUNTY 911 DISPATCHER. AT APPROXIMATELY 04:04, OPERATIONS PERSONNEL ARRIVED AT THE STATION AND ASSESSED THE SITUATION. AT APPROXIMATELY 04:07 OPERATIONS PERSONNEL AT THE SITE REQUESTED ADDITIONAL SUPPORT TO HELP ISOLATE THE STATION. IT WAS DETERMINED AT 07:41 THAT THE INCIDENT LIKELY INVOLVED AN ESTIMATED PROPERTY DAMAGE OF $129,300 OR MORE WHICH WOULD CLASSIFY IT AS REPORTABLE INCIDENT. INITIAL TELEPHONIC NOTIFICATION WAS MADE TO THE NATIONAL RESPONSE CENTER AT 08:13. THE NATIONAL RESPONSE CENTER ASSIGNED INCIDENT REPORT #1351225 TO THE INITIAL NOTIFICATION OF THE INCIDENT. OPERATIONS PERSONNEL STARTED TO ISOLATE THE STATION AT APPROXIMATELY 08:05. OPERATIONS PERSONNEL COMPLETED THE ISOLATION OF THE STATION AT APPROXIMATELY 08:21. THE ISOLATION OF THE STATION WAS ACCOMPLISHED BY HYDRAULICALLY PINCHING THE INLET RISER WHERE IT HAD BEEN SHEARED OFF BY THE VEHICLE AND WELDING ON A CAP WITH A NEEDLE VALVE AS A PRECAUTIONARY MEASURE TO RELIEVE ANY POTENTIAL OVER PRESSURIZATION. THE TAP VALVE TO THE STATION WAS ALSO CLOSED AND THE ENTIRE STATION WAS REMOVED FROM THE SITE. A FOLLOW UP TELEPHONIC NOTIFICATION WAS MADE TO THE NATIONAL RESPONSE CENTER ON 10/31/22 AT 16:16 TO CONFIRM THE INITIAL TELEPHONIC NOTIFICATION AND PROVIDE AN UPDATE ON THE CURRENT STATUS OF THE STATION AND THE ESTIMATED VOLUME OF UNINTENTIONAL GAS LOSS. THE NATIONAL RESPONSE CENTER ASSIGNED INCIDENT REPORT #1351347 TO THE FOLLOW UP TELEPHONIC NOTIFICATION. FOLLOWING INCIDENT REPORT #1351347, THE ESTIMATED VOLUME OF UNINTENTIONAL GAS LOSS WAS LATER CONFIRMED TO BE 1757 MCF. THE FOLLOWING REMEDIATION ACTIONS WERE COMPLETED: THE ENTIRE STATION WAS REBUILT, THE REBUILT STATION WAS INSTALLED AT THE SITE, AND THE REBUILT STATION WAS RESTARTED AND RETURNED TO SERVICE ON 12/20/22 AT 15:00. ENHANCED PROTECTION MEASURES FOR THE STATION ARE PLANNED TO BE INSTALLED.
